A Case of a Patient with Pontine Infarction Complaining of Discomfort in Both Legs (양 하지 불편감을 호소하는 교뇌 경색 환자에 대한 한의 치험 1례)

  • Objective: This study examined the effectiveness of Korean medical treatment for an 81-year-old female patient with pontine infarction complaining of discomfort in both legs. Methods: The patient was hospitalized for 45 days and treated with herbal medicine (mainly Bohyulanshin-tang [補血安神湯], pharmacopuncture, acupuncture, electroacupuncture, and moxibustion). We used the International Restless Legs Scale (IRLS) and Numerical Rating Scale (NRS) and checked the duration of the symptoms to evaluate the clinical effects of the treatment. Results: After 45 days of hospitalization, the IRLS score decreased from 33 to 19, with the lowest score of 8 recorded on the twenty-seventh day after discharge. The NRS score and the duration of symptoms also improved, and the treatment effect continued following discharge. Conclusions: This study suggests that Korean medical treatment may be effective for the treatment of restless legs.

  • One and a half syndrome is an ipsilateral gaze paresis or palsy combined with an internuclear opthalmoplegia on controlateral gaze. The lesion site is at the paramedian pontine reticular formation and the adjacent MLF. The common causes are unilateral pontine infarction and multiple sclerosis. We experienced a case of one and a half syndrome which has a suspected small pontine infarct.

A Clinical Study on One Case of Exotropia Caused by Unilateral-internuclear- ophthalmoplegia due to Pontine Infarction (뇌교 경색에 의한 일측성핵간마비로 유발된 외사시 환자에 대한 증례보고)

  • Objectives : This study is to report one case of the patient who has exotropia caused by unilateral internuclear ophthalmoplegia due to pontine infarction, which is a rare disease so there has few research about the effect of acupuncture therapy on it, treated with the SAAM(舍巖) Acupuncture Sojangjeonggyeok(小湯正洛) Methods & Results : In the point of differentiation of Syndrome, these subjects were diagnosed as deficiency of Yin and blood(陰血不足). We treated her with SAAM Acupuncture Sojangjeonggyeok(小湯正洛), herb medication(淸肝湯, 六味地黃湯), and the symptoms were resolved clearly by above treatment during about a week. Conclusion : This case shows proper oriental medical treatment based on the exact differentiation of symptoms has good effect on this disease as one of conservative therapies.

A Case of a Soyangin Patient with Aggravated Voiding Dysfunction after Pontine Infarction (교뇌경색 이후 배뇨장애가 악화된 소양인 환자의 한의 치료 1례 보고)

  • ■Objective The purpose of this case report is to report the effect of Korean medicine treatment on a patient with pontine infarction complaining of discomfort due to aggravated voiding dysfunction after the onset of the stroke. ■Methods The patient was hospitalized for 44 days and treated with herbal medicine, acupuncture, electroacupuncture, moxibustion, and interferential current therapy(ICT). We checked the amount of self-voided volume and postvoid residual urine volume(PVR), and asked the patient to subjectively evaluate the degree of discomfort due to voiding dysfunction. ■Results After 44 days of hospitalization, the patient responded that the subjective discomfort due to residual urine remained 30% and delayed urine remained 50% compared to the day of admission. The maximum amount of PVR was 234 ml on the 3rd day, and the minimum amount of PVR was checked on 25th and 35th day as 0 ml. ■Conclusions This study suggests that Korean medicine treatment may be effective in the treatment of voiding dysfunction after stroke.

  • Persistent left superior vena cava (PLSVC) is a common venous anomaly of the thorax and usually drains into the right atrium. Less often it drains into the left atrium and has previously been related to ischemic stroke. We report a case of PLSCV that founded during ischemic stroke evaluation in a 77-year-old woman which was detected on transesophageal echocardiography (TEE) and transcranial Doppler ultrasonography (TCD) with saline agitated test and computed tomography.

  • Objectives : To report three cases of eye movement impairment: internuclear ophthalmoplegia (INO) due to pontine infarction, traumatic abducens nerve palsy, and abducens nerve involvement in Miller-Fisher syndrome. Cases Summary : There were three cases. First, a 64-year-old woman, who was given a diagnosis of INO due to pontine infarction, had left gaze palsy of her right eye while the convergence was intact. Second, a 34-year-old man had abduction impairment of his right eye after a traffic accident. Third, a 66-year-old man, who was diagnosed with Miller-Fisher syndrome, had left gaze palsy of his left eye. Their symptoms improved substantially and their anxieties were relieved after treatment with herbal medicine and acupuncture. Conclusions : There has been no certain cure for eye movement impairment yet. In this report, we present three successful cases of patients with eye movement impairment and show that Korean medical treatment could be a solution for this incurable disease.

Clinical Report of Baliao Electroacupuncture Treatment on a Stroke Patient Suffering from a Neurogenic Overactive Bladder (뇌경색을 동반한 신경인성 과민성 방광 환자의 배뇨장애 팔료혈 전침 치료 1례)

  • Objective: The purpose of this case study is to report the effectiveness of Baliao acupoints electro-acupuncture treatment on a neurogenic overactive bladder induced by stroke. Method: A patient suffering from middle cerebral artery infarction was diagnosed with a neurogenic overactive bladder and treated by Korean medicine, including electro-acupuncture on Baliao acupoints six times per week. The evaluation was performed by monitoring urinary frequency and using the Korean versions of the Overactive Bladder Symptom Score (OABSS) and Urinary Incontinence Quality of Life (I-QoL). Results: After 27 days of electro-acupuncture treatment, the OABSS score improved from 6 to 3. The I-QoL total score improved from 60 to 87. The patient also showed a decrease in nocturia frequency. Conclusion: This case suggests that electro-acupuncture on Baliao acupoints can be effective for overactive bladders due to supra-pontine lesions.

Case Study of Facial Anhidrosis in a Pontine Infarction Patient Treated by Traditional Korean Medical Treatments (교뇌경색으로 인한 안면무한증을 호소하는 환자 치험 1례)

  • Objective: This study presents a case of a 64-year-old Korean male with left-side hemifacial anhidrosis due to stroke who was treated with traditional Korean medical treatments.Method: The patient was treated with acupuncture, electroacupuncture, and moxibustion around the eyes, and was administered Chungpyesagan-tang and Yuldahanso-tang. We performed a sweat percentage test and compared the results with those of the other side of the face to evaluate sweat dysfunction.Results: Sweat function appeared to improve after the administration of traditional Korean medical treatments.Conclusion: Traditional Korean medical treatments may be effective in treating hemifacial anhidrosis due to stroke.

  • Clinical observation was made on 210 cases of CVA that were confirmed through brain CT, MRI scan and clinical observation. They were hospitalized in the oriental medical hospital of Kyungwon University from January to December in 1998. Objective : To evaluate the current status of cerebrovascular disease in Korea, we performed observation on stroke patients. Methods : We performed a retrospective study on 210 cases of stroke who had been admitted to the Dept. of Internal Medicine, College of Oriental Medicine, Kyungwon University from Jan. 1, 1998 to Dec. 31, 1998. Results : The incidence was highest in the patients with the age of above fifty. The frequency of strokes was similar between male and female cases. Ischemic stroke(70.0%) was more common than hemorrhagic stroke(20.5%), with transient ischemic attacks(9.5%) occuring less frequently. Middle cerebra! arterial territory was the most commonly involved site in ischemic strokes. Among intracerebral hemorrhages, putaminal hemorrhages were found in 69.8%, and was followed by IVH(9.3%), IVH with ICH(7.0%), subcortical hemorrhages (4.7%) and pontine hemorrhages(4.7%). The most ordinary preceding disease was hypertension. The next was diabetes mellitus. Most cases were given simultaneous treatment in both western and oriental medical methods. Conclusions : Generally it is thought that CVA occurs frequently in winter. But our observations reveal no such relationship. The predisposing factors of cerebral infarction were usually initiated during the time of resting and sleeping and those of cerebral hemorrhage chiefly during the time of exercising. I Patients whose condition of consciousness was bad at the time of admission tended to have a bad prognosis. This study suggests changing trends of stroke in Korea, and that a multicenter prospective study using stroke registry is required for confirmation.
