Carcinoid tumor of gallbladder in a dog (개의 담낭에서 발생한 carcinoid tumor 증례 보고)

  • A 7-year-old female mixed-breed dog was presented to a veterinary clinic for pyometra. During abdominal ultrasonography, an abnormal finding was noted in the gallbladder. A cholecystectomy was performed. Grossly, the gallbladder wall was thickened with a polypoid nodular projection into the lumen. Microscopically, the polyp consisted of a solid sheet of round to polygonal-shaped cells that formed small packets with fibrovascular septa. The neoplastic cells contained abundant cytoplasmic eosinophilic granules. Immunohistochemically, the neoplastic cells were positive to chromogranin A and neuron-specific enolase, whereas they were negative to vimentin and cytokeratin. This case was diagnosed as a primary gallbladder carcinoid tumor.

  • 캡술내시경은 식도부터 항문까지 소화기관 전체를 한번에 촬영할 수 있는 의료기기로, 한번의 검사에 평균 8~12 시간 정도의 길이와 5만장 이상의 프레임으로 구성된 영상을 생성한다. 그러나 생성된 영상에 대한 분석은 수작업으로 진행되고 있어, 캡술내시경 영상 분석 자동화에 대한 기술적인 수요가 높아지고 있는 추세이다. 이를 위해, 캡슐내시경 영상 분석에 대한 많은 연구가 진행되고 있는데, 본 연구에서는 그 중에서도 폴립 영상에 대한 검출 자동화 연구에 주목하였다. 폴립이란 위장관 내에서 발견될 수 있는 융기성 병변으로, 많은 연구에서 기계학습 혹은 딥러닝 방식을 적용하여 이를 검출하기 위한 연구를 수행하였다. 그러나 캡슐내시경 영상의 특성상, 병번이 있는 영상이 굉장히 적기 때문에 일반적인 딥러닝 방식의 적용으로 좋은 성능을 내기 어렵다. 따라서 본 논문에서는 폴립 검출 컨볼루션 신경망 설계를 위한 멀티 스케일에 대한 원형 검출기법을 결합하여 폴립이 의심되는 영역을 추출해주는 특징 추출 기법으로, 수집한 데이터 150장에 대한 실험한 결과 약 82%의 성능을 보였다.

  • The solitary fibrous tumor (SFT) is a mesenchymal neoplasm that is described as spindle-shaped tumor cells on a collagenous background originating from pleural tissues. Recently, extrapleural SFT has been reported in nearly all sites, including the sublingual gland, parotid gland, nasal cavity and paranasal sinuses. Complete surgical excision is primary treatment for SFT, but diagnosing SFT is not often made until immunohistochemical evaluation after surgical resection. We report that the patient, 45-year-old male, was considered as a case of inflammation polyp arising from left nasal cavity with initial biopsy, however, it has turned out to be SFT after surgical treatment.

Clinical Applications of Office-Based Laryngeal Surgery With KTP Laser (KTP 레이저를 이용한 외래 기반 후두 수술의 임상적 적용)

  • The pulsed photoangiolytic 532-nm potassium-titanyl-phosphate (KTP) laser has emerged in recent years as an efficacious treatment modality for vocal fold lesions. It also has broadened the indications for other laryngeal laser surgery. Features of KTP laser that it is a fiber-based delivery system and its energy is selectively absorbed by oxyhemoglobin make it suitable for office-based laryngeal procedures. An office-based KTP laser surgery provides an alternative management option for benign laryngeal diseases and can be performed comfortably under flexible endoscopic guidance which is placed through the nose of a fully awake patient. Office-based laryngeal surgery with a KTP laser can alleviate the need for general anesthesia. However, there are some limitations to apply due to reduced visual precision and the fact that the vocal folds are moving during procedures. Clinicians should carefully weigh the advantages and disadvantages of office-based procedures before a treatment option is selected. Patient selection and standardized laser energy parameters may help in decreasing complications and improving the treatment results.

Cholelithiasis as a Risk Factor for Gallbladder Cancer (담낭암 발생의 위험인자로서의 담석증)

  • Although the incidence of gallbladder cancer is relatively low, Korea is one of the countries with the highest incidences of gallbladder cancer in the world. Gallbladder cancer is also often diagnosed in an advanced state, and has poor prognosis. A gallstone is one of the major risk factors for gallbladder cancer and a majority of epidemiological studies support correlation between cholelithiasis and gallbladder cancer. Clear mechanism, however, regarding how gallstones cause gallbladder cancer is not known. In this article, results of studies about the relationship between gallstone and gallbladder cancer were thoroughly reviewed. Also, it was discussed whether prophylactic cholecystectomy is necessary to prevent gallbladder cancer in asymptomatic gallstone patients without other risk factors such as a gallbladder polyp or porcelain gallbladder.

Premalignant Lesions of the Small Intestine (소장의 전암성 병변)

  • Tumors of the small intestine are rare and generally asymptomatic or with nonspecific symptoms. The small intestine is difficult to approach using conventional endoscopy, and early diagnosis of the small intestinal tumors is difficult. Therefore, many of the small intestinal tumors are diagnosed at an advanced stage, which makes the prognosis poor. Premalignant lesions of the small intestine or known risk factors of small bowel cancer are sporadic adenoma, adenoma associated with familial adenomatous polyposis, hamartomatous polyp associated with Peutz-Jeghers syndrome, Crohn's disease, and celiac disease. Therefore, it is necessary to recognize that the small bowel cancer can occur in these patients with premalignant lesions or risk factors of small bowel cancer. To reduce the possibility of small bowel cancer or to detect at an earlier stage, attention should be paid to screening and surveillance of these patients with premalignant lesions or risk factors of the small bowel cancer.

Endoscopic management of giant colonic polyps: a retrospective Italian study

  • Background/Aims: Polyps greater than 30 mm are classified as "giants". Their endoscopic removal represents a technical challenge. The choice of the endoscopic removal technique is important because it provides a resection sample for precise histopathological staging. This is pivotal for diagnostic, prognostic, and management purposes. Methods: From a retrospective analysis, we obtained a sample of 38 giant polyps. Eighteen polypectomies were performed using the epinephrine volume reduction (EVR) method, nine polypectomies utilized endo-looping or clipping methods, and 11 patients underwent surgery. Results: We obtained en bloc resection with the EVR method in all cases; histology confirmed the correct indication for endoscopic resection in all cases. Moreover, no early or delayed complications were observed, and no patient required hospitalization. Using endo-looping or clipping methods, we observed advanced histology in 1/9 (11.1%) cases, while another patient (1/9, 11.1%) had delayed bleeding. Among patients who underwent surgery, 5/11 (45.5%) were deemed overtreated and three had post-surgical complications. Conclusions: We propose EVR as an alternative technique for giant polyp resection due to its safety, effectiveness, cost-efficiency, and the advantage of avoiding the need to postpone polypectomy to a later time. Further prospective studies might help improve this experience and enhance the technique.

Mansonian schistosomiasis in rectum - Report of a case - (직장에 발생한 만손주혈흡충증 일례 보고)

  • Schistosomiasis is a snail-transmitted trematodiasis acquired by immersion in water which contains the cercariae. In Korea, six imported cases of urinary schistosomiasis by Schistosomc haematobium and one case of imported cerebral schistosomiasis by S. mansoni were reported. Herein we report a case of S. monsoni infecting rectum of a 46 year-old Korean male, who had been to Saudi Arabia for two years. On colonoscopy for routine physical check up, a 0.4 cm polyp in the rectum was detected and biopsy was done. Microscopically, rectal mucosa showed several granulomas which were composed of macrophages, Iymphocytes, neutrophils and eosinophils. The center of each granuloma showed an ovoid egg often containing miracidium. The eggs measured 130 × 60 ㎛ in average size. They had yellowish-brown transparent shell loth the characteristic lateral spille. This is the 8th imported case of schistosomiasis in Korea and the second one infected by S. mansoni. The Study for Voice Onset Types in Benign Vocal Fold Lesions (양성성대질환에서의 발성시작유형에 관한 연구)

  • Background and Objectives: Benign vocal fold lesions have shown various voice onset types on phonation, however, they have not been documented yet. We studied to know the relationships between benign vocal fold lesions and voice onset types. Materials and Method: 114 subjects were evaluated by using videokymographic examinations. The subjects were classified into three types: normal, contact, and open types according to the patterns of voice onset types on phonation. Benign vocal fold lesions were investigated and voice onset types were compared between normal and disease groups. Voice parameters were obtained from and compared in all subjects to assess acoustic and aerodynamic factors. Results: The normal type among onset types were more than contact type or open type in both normal and disease groups. Disease group showed many contact and open types when. compared with normal group. Vocal nodule and vocal polyp were showed many normal and contact types, however, sulcus vocalis was almost showed open type among voice onset groups. The values of mean flow rate (MFR) of contact type were significantly higher compared to normal type in disease group (p<0.05). Shimmer of contact type was higher than normal type in diseasegroup, but the difference was not significant (p=0.057). Conclusion: Benign vocal fold lesions were related to the various types of voice onset. The various types of voice onset should be considered when benign vocal fold lesions were examined.

Clinical evaluation of sinus bone graft in patients with mucous retention cyst

  • Background: Mucous retention cyst refers to a cyst made by expansion due to the blockage of the salivary gland near the maxillary sinus, and it is surrounded by epithelial cells. Most of them are small; therefore, they cannot be found well and are frequently with antral polyp. The aim of this study was to evaluate the clinical prognosis of sinus bone graft in patients with mucous retention cyst. Methods: This study was performed retrospectively on 23 patients who had sinus bone graft. Group 1 was 8 patients (10 sinuses) who had a mucous retention cyst, and group 2 was 15 patients (17 sinuses) who had no pathologic history about the maxillary sinus. For these patients, sinus bone graft was performed using the lateral approach technique. The total 51 implants were placed 6.22 weeks on the average after sinus bone graft. Sinus membrane perforation during operation, postoperative complications, marginal bone loss after restorative function, implant success rate, and survival rate were analyzed. Results: There was no complication in group 1, and there were three complications in group 2. In group 2, two cases of implants failed. The types of postoperative complications consisted of two minor infections and one wound dehiscence. Two implants of total 51 implants were removed, and the survival rate of implants was 96.08 % (group 1 100 %, group 2 93.5 %). The total success rate of implants was 92.2 % (group 1 95 %, group 2 90.3 %). Conclusions: The clinical prognosis was not affected by the presence of mucous retention cyst.
