A Case of Pleomorphic Liposarcoma of the Lateral Neck (측경부에 발생한 다형성 지방육종 1예)

  • Pleomorphic liposarcoma is extremely rare in the head and neck region. It can be diagnosed by identifying multivacuolated lipoblast with pleomorphism. Most studies have reported that pleomorphic liposarcoma shows an aggressive behavior with poor prognosis. The treatment of choice is wide resection to ensure negative margin. The authors report a case of pleomorphic liposarcoma in a 67-year-old female, who was treated with primary resection of tumor and postoperative radiation therapy. The patient is well without any recurrence or metastasis after 1 year of follow-up.

GLYCOSAMINOGLYCAN EXPRESSION IN PLEOMORPHIC ADENOMAS OF THE SALIVARY GLAND (타액선 다형성 선종에서 Glycosaminoglycan의 발현)

  • Pleomorphic adenoma is the most common benign tumor in salivary glands, and occurred in frequency of 60% in parotid gland tumors, and 50% in submandibular gland tumors, and 25% in sublingual gland tumors. Histopathologically, pleomorphic adenoma is composed of epithelial cells and mesenchymal tissues, and called 'mixed tumor' because of morphological divergency. The cell structures of luminal area are composed of polyhedral and cuboidal secretory epithelial cells and modified myoepithelial cells around it, and mesenchymal tissue is composed of some myoepithelial cells and stromal tissue. In stromal tissue, myxoid change, chondroid change, or hyalinization can be seen even if bone tissue. In many studies, tumor cells of pleomorphic adenoma containing modified myoepithelial cell participate in synthesis of glycosaminoglycans. In this study, tissue sample of pleomorphic adenoma of human salivary gland were obtained from 20 surgical specimens, and all specimens were routinely fixed in 10% formalin and embedded. Serial 4-8${\mu}m$ thick sections were cut from paraffin blocks. The histopathologic evaluation was done with light microscopy. And, with immunohistochemical staining, characteristics of glycosaminoglycan were observed. And, for biochemical analysis of glycosaminoglycan, isolation of crude glycosaminoglycan from tumor tissue and immuno-blot analysis were carried out. With transmission electromicroscopy, tumor cells and biologic behavior of pleomorphic adenoma were observed with distribution and expression of glycosaminoglycan in tumor cells, The results were obtained as follows: 1. In immunohistochemical study, chondroitin 4-sulfate is highly postively stained in myxoid stromal tissue, and chondroitin 6-sulfate is highly positively stained in chondroid mesenchymal tissue, both glycosaminoglycans are positively stained in non-luminal cell of ductal area. 2. Dermatan sulfate and keratan sulfate is positively stained in periductal non-luminal tumor cells. 3. In immunohistochemical study, heparan sulfate is weakly stained in luminal cells and non-luminal cells around duct, and chondroid mesenchymal tissue. 4. In transmission electromicroscopic view, the tumor cells are composed of modified myoepithelial cells, and contain many microfilaments and well developed rough endoplasmic reticulum. 5. In Immuno-Blot analysis, the expression of glycosaminoglycans is expressed mostly in chondroitin 6-sulfate and chondroitin 4-sulfate. From the results obtained in this study, tumor cells of pleomorphic adenoma are composed of modified myoepithelial cells, and glycosaminoglycans of chondroitin 4-sulfate and chondroitin 6-sulfate mostly participate in the development of pleomorphic adenoma, but dermatan sulfate, keratan sulfate and heparan sulfate glycosaminoglycans were expressed variably.

  • Pleomorphic adenoma is also called a mixed tumor and it most commonly occurs in the salivary gland. This neoplasm has a low grade malignant potential, but it may also show aggressive clinical behavior like recurrence or metastasis. We report here on a case of a tumor that was confirmed to be primary pulmonary carcinoma ex-pleomorphic adenoma by the pathologic examination after complete resection, and it had the characteristics of malignant neoplasms, such as multiple metastases.

Long-term Survival following Surgical Resection for Recurrence of Pulmonary Pleomorphic Carcinoma (폐에 발생한 다형태 암종 환자에서 전이부위 절제술 후 장기 생존)

  • Pulmonary pleomorphic carcinomas are rare malignant tumors that account for 0.1 to 0.4% of all lung cancers. They are notable for their aggressive clinical behavior and poor prognosis. We report here on a patient who is alive and disease-free 12 years after receiving surgical treatment for the rib recurrence of pulmonary pleomorphic carcinoma.

Clinical Investigations of Major Salivary Gland Tumors (주타액선 종양의 임상적 고찰)

  • Objectives: Salivary gland neoplasms are a diverse group of benign and malignant tumors with a wide range of biologic behaviors. The surgeon must understand the pathologic behavior of each tumor type to develop an appropriate treatment plan. The authors planned this study to evaluate our clinical experiences and establish a new treatment strategy. Materials and Methods: From Sep. 1997 to June 2001, 25 cases of major salivary gland tumors which were underwent surgery were evaluated retrospectively. Results: 20 cases were benign and 5 were malignant. Most(17) of benign cases were pleomorphic adenoma and they showed wide distribution in age. Also we experienced other benign such as warthin's tumor, oncocytoma. In malignant, there were 2 cases of carcinoma ex-pleomorphic adenoma, 2 cases of adenoid cystic carcinoma, and one lymphoma. In carcinoma ex-pleomorphic adenoma, one showed dismal prognosis in spite of multimodality and the other were recurrent to be salvaged. Conclusion: We concluded that salivary gland neoplasms are challenging because of their relative infrequency, inconsistent classification, and highly variable biologic behavior. We need to establish new effective strategies with the regard of factors influencing survival.

Characteristics of a Novel Acinetobacter sp. and Its Kinetics in Hexavalent Chromium Bioreduction

  • Cr-B2, a Gram-negative hexavalent chromium [Cr(VI)] reducing bacteria, was isolated from the aerator water of an activated sludge process in the wastewater treatment facility of a dye and pigment based chemical industry. Cr-B2 exhibited a resistance for 1,100 mg/l Cr(VI) and, similarly, resistance against other heavy metal ions such as $Ni^{2+}$ (800 mg/l), $Cu^{2+}$ (600 mg/l), $Pb^{2+}$ (1,100 mg/l), $Cd^{2+}$ (350 mg/l), $ZN^{2+}$ (700 mg/l), and $Fe^{3+}$ (1,000 mg/l), and against selected antibiotics. Cr-B2 was observed to efficiently reduce 200 mg/l Cr(VI) completely in both nutrient and LB media, and could convert Cr(VI) to Cr(III) aerobically. Cr(VI) reduction kinetics followed allosteric enzyme kinetics. The $K_m$ values were found to be 43.11 mg/l for nutrient media and 38.05 mg/l for LB media. $V_{max}$ values of 13.17 mg/l/h and 12.53 mg/l/h were obtained for nutrient media and LB media, respectively, and the cooperativity coefficients (n) were found to be 8.47 and 3.49, respectively, indicating positive cooperativity in both cases. SEM analysis showed the formation of wrinkles and depressions in the cells when exposed to 800 mg/l Cr(VI) concentration. The organism was seen to exhibit pleomorphic behavior. Cr-B2 was identified on the basis of morphological, biochemical, and partial 16S rRNA gene sequencing chracterizations and found to be Acinetobacter sp.

Histopathologic Classification of Salivary Gland Neoplasm (타액선 종양의 병리조직학적 분류)

  • Background and Objectives : Salivary gland neoplasms are unique because of their infrequency, inconsistent classification, and highly variable biologic behavior. The aim of this study is to analysis the histopathologic classification of salivary glnad neoplasm and to suggest a guideline of management. Materials and Methods : The medical records of 310 patients with salivary gland neoplasm who treated at Asan medical center between 1992 and 2001 were analyzed retrospectively. Among the 310 patients, 138 patients were male and 172 patients were female. Mean age was 50.5 years. Results : Benign salivary neoplasms were 213 cases. They consisted of 153 cases (71.8%) of parotid tumor, 41 cases (19.2%) of submandibular gland tumor and 19 cases (8.9%) of minor salivary gland tumor. Pleomorphic adenoma was the most common benign neoplasm. Malignant salivary neoplasms were 97 cases. They consisted of 45 cases (46.4%) of parotid tumor, 26 cases(26.8%) of minor salivary gland tumor, 24 cases(24.7%) of submandibular gland tumor and 2 cases(2.1%) of sublingual gland tumor. Adenoid cystic carcinoma was the most common malignant neoplasm. Conclusions : The most commonly involved gland was parotid (64%) and the most frequent tumor was pleomorphic adenoma (52%). Although the majority of minor salivary gland neoplasms are malignant, three of parotid tumors are benign.

Clinical Analysis of Benign Salivary Gland Tumors (타액선 양성 종양에 대한 임상적 분석)

  • Benign salivary gland tumors have relatively lower incidence, but it have various histopathologic diagnosis and biological behavior. Authors analyzed retrospectively 77 patients with benign salivary gland tumor who were treated surgically and had the following results. The most frequent age group was 5th decade, and sex distribution was not specific. The most common location was parotid gland(75.3%) and submandibular gland(20.8%) was next. Histopathologically, the most common salivary gland tumor was pleomorphic adenoma(82.7%) and Warthin's tumor(8.6%) was next. An asymptomatic mass was the most common presentation. Duration of symptoms and signs were mostly under the 5years(90.9%). Diameter of tumors was mostly under 4cm(76.7%). Parotid gland tumors were treated mostly with superficial parotidectomy and submandibular gland tumors were treated mostly with submandibular gland resection. The most common complication was facial nerve palsy(9 cases).

  • Adenoid cystic carcinoma of the uterine cervix is a rare tumor accounting for less than 1% of all cervical adenocarcinoma. This tumor is characterized by aggressive biological behavior with frequent local recurrence or metastatic spread, postmenopausal onset, and occasional association with conventional squamous cell carcinoma. The cytologic diagnosis of adenoid cystic carcinoma in the uterine cervix is often difficult because of negative smear due to intact overlying mucosa, cytologic findings mimicking endometrial cells, and masquerade as squamous ceil carcinoma. Recently we have experienced a case of adenoid cystic carcinoma arising in the uterine cervix, which was identified on the routine Papanicolaou smear and was histologically confirmed by the consequent biopsy. The smear showed abundant cellularity composed of relatively uniform cells. The tumor cells were arranged in small clusters, acini, naked cells, and loose sheets with abortive cribriform pattern. There were scattered globoid basement membrane-like materials and tumor diathesis. The nuclei were pleomorphic and showed hyperchromatic and coarsely granular choromatin with inconspicuous nucleoli. The punch biopsy of the uterine cervix showed typical histologic findings of adenoid cystic carcinoma characterized by tumor nests composed of hyperchromatic uniform basaloid cells, cribriform pattern, and cylindrical hyaline bodies.

A Clinical Analysis of Minor Salivary Gland Tumors (소타액선 종양에 관한 임상적 고찰)

  • Minor salivary gland tumors vary in their primary sites, histopathology and bilogical behavior. Therefore various factors are considered in selecting the treatment modality and predicting the prognosis. The prognosis of milignant tumors of minor salivary glands are worse than that of such lesions of major salivary glands. Authors experienced 26 cases of minor salivary gland tumors(10 benign tumors and 16 malignant tumors) during the past 7 years and analyzed their clinical characteristics. 1) The palate was the most common site of origin of minor salivary gland tumors (38.5%). 2) The most common benign tumor was pleomorphic adenoma and majority of them occur red in the palate(60.0%) and the remaining occured in the nasal cavity or the paranasal sinuses (30.0%), and the lip ( 10.0%). 3) In the case of malignant tumors the most frequent sites were the nasal cavity or the paranasal sinuses (31.3%) with the following histopathologic frequencies: adenoid cystic carcinoma(56.3%), malignant pleomorphic adenoma(12.5%), mucoepidermoid carcinoma(12.5%), polymorphic adenocarcinoma (12.5%), epthelial-myoepithelial carcinoma (6.25%). 4) Minor salivary gland tumors have high probability of malignancy and tumor extension is important to their treatment.
