Studies on the Hemodilution Perfusion with Rygg-Kyvsgaard Oxygenator (혈액희석 체외순환법에 관한 임상적 관찰 -상온하 Rygg-Kyvsgaard 산화기 및 Sigmamotor pump 사용예를 중심으로-)

  • Clinical perfusion data on 16 cases of cardiopulmonary bypass using Sigmamotor pump and RyggKyvsgaard Oxygenator which performed at Seoul National University Hospital during the period of Aug. 1968 to Aug. 1970 was analized. AIl cases were hemodiluted and the perfusion was carried out under the normothermic condition. The age of the patients ranged between 6 and 43 years. The b:dy weight varied between 18.3 and 54.0 kg and the body surface area between 0.78 and 1. 59$M^2$. The priming solution was consiste:I with fresh ACD blood. Hartmann solution and Mannitol. The average amount of priming was approximately 2242 ml. The average hemodilution rate was 17%. The flow rate ranged from 1.7L to 3.5L/Min/$M^2$ and averaged 2.4L/Min/$M^2$ or 78mI/Min/kg. The duration of perfusion varied from 22 to 110 min with average of 56.9 minutes. Some hemodynamic responses were observed. The arterial pressure dropped immediately after the initiation of partial perfusion and was more marked after the total perfusion foIlowed by gradual increase to the safety level. The central venous pressure reflected the reduced blood volume especially in the cases of prolonged perfusion which lasted over 60 min. In most of the cases, red blood cell count decreased and white blood ceIl count increased after the perfusion. Hemoglobin level was decreased, averaging of 12.5mg%, Hct 3.3% and platelets count of 18% postoperatively. Plasma hemoglobin increased mildly, from pre-perfusion average value of 4. 06mg% to postperfusion value of 22.5mg%. Serum potassium was 4.4mEq/L pre-operatively and was decreased to 3.7mEq/L postoperatively. Five cases showed definite hypopotassemia immediately after the operation. Sodium and chloride decreased mildly. These electrolyte changes are thought to be related with hemodilution. diuretics and reduced blood volume during and after the perfusion. Arterial blood pH value revealed minimal to moderate elevation from preperfusion average value of 7.376 to 7.461 during perfusion and then 7.365 after perfusion. The pC02 and hicarbonate showed minimal to moderately lowered values. The total CO2 was decreased. Buffer base decreased during perfusion (Av. 42.6mEq/L) and further decreased after the perfusion (Av. 40.8mEq/L). These arterial blood acid base changes suggested that the metabolic acidosis was accompanied by respiratory alkalosis during and immediately after the perfusion. Authors belived that the acidosis could more effectively be corrected with the more additional dose of bicarbonate than we used by this study. The chest tune drainage during the first 24 hours following operation was 1158 ml in average. One case (Case No. 15) showd definite bleeding tendency and it was believed that the cause might be due to the defect of heparin and protamine titration. The average urinary out put during 24 hours post-perfusion was 1291ml. One case (Case No. ]) showed definite post perfusion oliguria. As conclusion hemodilution using fresh ACD blood. Hartmann and Mannitol solution added with Bivon and high flow rate unler normothermia. was thought to amelioratc the severity of mctabolic acidosis during and after perfusion with relatively satisfactory effect on the diuresis and bleeding tendency.

Scintigraphy with Indium-111-oxine Labeled Leukocytes for Localization of Abscesses (복부농양진단을 위한 Indium-111 표지백혈구스캔)

  • Detection of deep-seated abscesses is sometimes difficult with ultrasonogrpahy or computed tome graphy alone. Indium-111-labeled leukocyte has widely used in the localization of abscesses after introduction by Segal and Thakur in 1976. But there are some difficulties in using indium-111-oxine in our country because of hardness to get the radiopharmaceutical timely and long time for labeling leukocytes. So we peformed the indium-111-labeled leukocyte scan for establishment of the labeling procedure and clinical application. We labeled the mixed leukocytes from 36 ml of patient's blood using 4 ml of ACD solution, 7 ml of 6% hydroxyethyl starch solution $(HESPAN^{(R)})$, 1 mCi of indium-111 oxine, 5 ml of normal saline and centrifuge. It took about 2 hours for the preparation of radiolabeled leukocytes and attention for contamination was needed. The average injected dose of labeled mixed leukocytes was 465 uCi. The average number of injected leukocytes was $2.5\times10^8$ and the labeling ratio was $57{\pm}13%$ (Table 2, Fig. 5). These number and ratio were sufficient for the localization of abscess. About twenty per cent of indium was labeled to red blood cells and platelets (Fig. 6) and the half-life of injected radiolabeled leukocytes was 8.3 hours. Scan was performed in 9 patients who were suspected to have abscesses clinically or radiologically. Three patients were positive, in one patient who had abscess close to lower lumbar vertebrae was surgically drained and another 2 positive cases did not show abscess clearly on computed tomography, so only antibiotics were administrated and treated successfully. The negative 6 patients were improved without specific treatment. In conclusion, the use of indium-111 oxine labeled leukocytes for localization of abscesses were very specific and helpful in the decision of treatment considering its relatively simple labeling method, and could be easily performed providing timely supply of the radiopharmaceutical.

Dabigatran Toxicity Secondary to Acute Kidney Injury (급성신손상으로 인해 발생한 dabigatran 독성)

  • Dabigatran is the first oral direct thrombin inhibitor approved by the US Food and Drug Administration (FDA) for prevention of stroke and systemic embolism in patients with nonvalvular atrial fibrillation. Because dabigatran is excreted mainly by the kidneys, serum levels of dabigatran can be elevated to a supratherapeutic range in patients with renal failure, predisposing to emergent bleeding. We describe the case of a 66-year-old man taking dabigatran 150 mg twice daily for atrial fibrillation and cerebral infarction who presented with hematochezia and disseminated intravascular coagulation. Laboratory evaluation showed a hemoglobin level of 6.3 g/dL, platelets of $138,000/mm^3$, activated partial thromboplastin time (aPTT) of 10 s, and an international normalized ratio (INR) of 8.17. Colonoscopy showed a bleeding anal fissure. Hemostasis was provided by hemoclips and packed red blood cells and fresh frozen plasma were transfused. Since then, there was no further hematochezia, however, bleeding including oral mucosal bleeding, hematuria, and intravenous site bleeding persisted. At presentation, his serum creatinine was 4.96 mg/dL (baseline creatinine, 0.9 mg/dL). Dabigatran toxicity secondary to acute kidney injury was presumed. Because acute kidney injury of unknown cause was progressing after admission, he was treated with hemodialysis. Fresh frozen plasma transfusion was provided with hemodialysis. At 15 days from admission, there was no further bleeding, and laboratory values, including hemoglobin, partial thromboplastin time, and prothrombin time were normalized. He was discharged without bleeding. After 2 months, he undergoes dialysis three times per week and no recurrence of bleeding has been observed.

Difference in Characteristics in the Formation of Anti-E and Anti-E/-c in Patients with the CDe Phenotype (CDe 표현형의 환자에서 항-E와 항-E/-c 항체 생성 특이성의 차이)

  • Background: Anti-E or paired anti-E/-c antibodies can develop in patients with the Rh CDe phenotype. This study examined the differences in transfusion in patients with the CDe phenotype according to formation of anti-E or anti-E/-c antibodies. Methods: Retrospective reviews were carried out on the results of antibody identification tests performed in 2014. The Rh phenotype and antibody specificity were investigated. The transfusion and medical records of patients with the CDe phenotype were examined. Results: In total, 76 patients were included in the review. Of these 76 patients, 38 (50.0%) were of the CDe phenotype. Anti-E antibodies were the most frequent (60.5%), followed by anti-E/-c antibodies (23.7%). The total transfusion units and platelet transfusion units were significantly higher in patients with anti-E/-c antibodies (P=0.028 and P=0.01, respectively). The distribution of categorized diseases was similar in the patients with the anti-E and anti-E/-c antibodies. A frequency of transfusion episodes greater than or equal to four was higher in patients with hepatobiliary diseases (85.7%). Conclusion: In CDe phenotype patients, platelet transfusion was significantly higher in the anti-E/-c positive group than the anti-E positive group, indicating that platelets play a role in red blood cell alloimmunization. Because E is the most immunogenic antigen in Korea, it is important to define the disease group, in which patients with CDe phenotype require a transfusion of E and c-negative blood.

Inhibitory Effects of Scopoletin in Collagen-induced Human Platelet Aggregation (콜라겐으로 유도한 사람 혈소판 응집에 미치는 Scopoletin의 억제 효과)

  • Platelet aggregation is essential for the formation of a hemostatic plug in the case of blood vessel damage. On the other hand, excessive platelet aggregation may cause cardiovascular disorders, such as thrombosis, atherosclerosis, and myocardial infarction. Scopoletin, which found in the root of plants in the genus Scopolia or Artemisia, has anti-coagulation and anti-malaria effects. This study examined the effects of scopoletin on human platelet aggregation induced by collagen. Scopoletin had anti-platelet effects via the down-regulation of thromboxane $A_2$ ($TXA_2$) production and intracellular $Ca^{2+}$ mobilization ($[Ca^{2+}]_i$), which are aggregation-inducing molecules produced in activated platelets. On the other hand, scopoletin increased both the cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP) levels, which are known as intracellular $Ca^{2+}$-antagonists and aggregation-inhibiting molecules. In particular, scopoletin increased the potently cAMP level more than cGMP, which led to suppressed fibrinogen binding to ${\alpha}IIb/{\beta}_3$ in collagen-induced human platelet aggregation. In addition, scopoletin inhibited collagen-elevated adenosine triphosphate (ATP) release in a dose-dependent manner. The results suggest that aggregation amplification through granule secretion is inhibited by scopoletin. Therefore, scopoletin has potent anti-platelet effects and may have potential for the prevention of platelet-derived vascular diseases.

Radiation Protection Effects of Schizonepeta Tenuifolia Water Extracts by Gender in SD Rats (SD rats의 성별에 따른 형개 열수추출물의 방사선 방호효과)

  • This study evaluates the radiation protection effects of Schizonepeta tenuifolia water extract on white rat by gender. Schizonepeta tenuifolia contains polyphenol and flavonoid, which are typical substances that remove free radical. Thus, to determine the effectiveness of radiation protection, the Schizonepeta tenuifolia water extract was administered to Sparaqu-Dawely (SD) rat males and females for two weeks, followed by hematological analysis, analysis of changes in the length of the small intestine villi length, and SOD activity evaluation. The male rat in ST+IR group showed a slightly greater recovery of platelets than the IR group. The ST+IR group also showed a higher platelet recovery capability than the IR group. lymphocyte showed that male and female rat ST+IR groups have higher resilience than IR groups likewise platelet. After irradiation, the villi length of the male rat and female rat decreased less in ST+IR group than in the IR group, indicating that the villi length of all genders was less damaged. This result confirmed that the Schizonepeta tenuifolia water extract had a radiation protection effect.

Numerical Study on Impact Resistance of Nonuniform Nacre-patterned Multi-layer Structures (비균일 진주층 모사 다층형 복합재료의 내충격성에 관한 수치해석)

  • Significant efforts have been devoted to developing high-performance composite materials by emulating the structure of biological creatures with superior mechanical characteristics. Nacre has been one of the most sought-after natural structures due to its exceptional fracture toughness compared with the constituent materials. However, the effect of manipulating the nacre-like geometry on the impact performance has not been fully investigated thus far. In this study, composites of randomly manipulated nacreous geometry are numerically developed and the impact performance is analyzed. We develop an algorithm by which the planar area of platelets in the nacre-like design is randomly resized. Thereafter, the numerical models of nonuniform nacre-patterned multi-layer structures are developed and the drop-weight impact simulation is performed. The impact behaviors of the model are evaluated by using the ratio of absorbed energy, the von Mises stress distribution, and the impact force-time curve. Therefore, the effect of the geometric irregularity on the nacre-patterned design is elucidated. This insight can be efficiently utilized in establishing the optimum design of the nacre-patterned structure.

Effects of Blood Indicators and Stress on Obesity by Gender in Korean Adults (성별에 따른 혈액 지표 및 스트레스가 비만에 미치는 영향)

  • This study aimed to investigate the effect of subjective factors (figure recognition and stress) and objective factors (blood biochemical parameters) on body mass index by gender difference in Korean adults. This study analyzed 3,343 subjects older than 20 years according to gender using the Sixth Korean National Health and Nutrition Examination Survey database. Specifically, male subjects had higher body mass index than female subjects (p < .001) and were more frequent in the obese group. Female subjects had higher stress and platelet score than male subjects (p < .001). Male subjects had significantly higher white blood cell counts (p < .001). The factors affecting the occurrence of obesity by gender difference was highly associated with subjective figure recognition (95% CI = 0.026~0.045, p < .001), stress (95% CI = 0.487~0.925, p = .015) and white blood count (95% CI = 1.232~1.392, p < .001) in females. These findings demonstrate a gender-specific difference in recognizing subjective factors and especially the objective factors of white blood cells and platelets affecting obesity. These results can be used for obesity education or fundamental data for establishing a nursing arrangement program for Korean adults.

Inhibitory effect of ethanol extract of Gryllus bimaculatus on platelet aggregation and glycoprotein IIb/IIIa activation (쌍별귀뚜라미 에탄올 추출물의 혈소판응집반응과 당단백질 IIb/IIIa 활성화 억제 효과)

  • Platelets act a fundamental role in primary- and secondary-hemostasis, however, platelet activation may cause thrombosis simultaneously. Therefore, control of platelet aggregation is crucial in preventing thrombosis-mediated diseases. Recently, the development of insect materials is attracting attention. Among the highly nutritious functional food sources, insects such as two-spotted cricket (Gryllus bimaculatus). Gryllus bimaculatus (G. bimaculatus) contains high protein and unsaturated fatty acids and has been registered as a food material September 2015 by the Ministry of Food and Drug Safety of Korea. In this study, we examined whether G. bimaculatus extract (GBE) inhibits platelet aggregation, intracellular calcium mobilization, thromboxane A2 production and glycoprotein IIb/IIIa (integrin αIIb/β3) activation. We investigated whether GBE can regulate signaling molecules, such as 1, 4, 5-triphosphate receptor type I, extracellular signal-regulated kinase, cytosolic phospholipase A2, mitogen-activated protein kinases p38, vasodilator-stimulated phosphoprotein, phosphatidylinositol-3 kinase, Akt, glycogen synthase kinase-3α/β, and SYK. Taken together, GBE is a potential therapeutic drug candidate to prevent platelet-related thrombosis and cardiovascular disease.

Comparison of Antiplatelet Activities of Green Tea Catechins

  • We have previously reported that green tea catechins(GTC) displayed potent antithrombotic effect, which was due to the antiplatelet activity. In the present study, the antiplatelet activity of each green tea catechin components was compared in vitro. Galloylated catechins including (-)-epigallocatechin gallate (EGCG), (-)-gallocatechin gallate (GCG), (-)-epicatechin gallate (ECG) and (-)-catechin gallate (CG), significantly inhibited collagen $(5{\mu}g/mL)-induced$ rabbit platelet aggregation with $IC_{50}$ values of 79.8, 63.0, 168.2 and $67.3{\mu}M$, respectively. EGCC GCG and CG also significantly inhibited arachidonic acid (AA, $100{\mu}M$)-induced rabbit platelet aggregation with $IC_{50}$ values of 98.9, 200.0 and $174.3{\mu}M$, respectively. However catechins without gallate moiety showed little inhibitory effects against rabbit platelet aggregation induced by collagen or AA compared with galloylated catechins. These observations suggest that the presence of gallate moiety at C-3 position may be essential to the antiplatelet activity of catechins and the presence of B ring galloyl structure may also contribute to the antiplatelet activity of GTC. In line with the inhibition of collagen-induced platelet aggregation, EGCG caused concentration-dependent decreases of cytosolic calcium mobilization, AA liberation and serotonin secretion. In contrast, epigallocatechin (EGC), a structural analogue of EGCG lacking a galloyl group in the 3' position, although slightly inhibited collagen-stimulated cytosolic calcium mobilization, failed to affect other signal transductions as EGCG in activated platelets. Taken together, these observations suggest that the antiplatelet activity of EGCG may be due to inhibition of arachidonic acid liberation and inhibition of $Ca^{2+}$ mobilization and that the antiplatelet of EGCG is enhanced by the presence of a gallate moiety esterified at carbon 3 on the C ring.
