Change in nostril ratio after cleft rhinoplasty: correction of nostril stenosis with full-thickness skin graft

  • Background: Patients with secondary deformities associated with unilateral cleft lip and nose might also suffer from nostril stenosis due to a lack of tissue volume in the nostril on the cleft side. Here, we used full-thickness skin grafts (FTSGs) to reduce nostril stenosis and various methods for skin volume augmentation. We compared the changes in the symmetry of both nostrils before and after surgery. Methods: From February 2016 to January 2020, 34 patients underwent secondary cheiloplasty and open rhinoplasty for secondary deformities of the unilateral cleft lip and nose with nostril stenosis. FTSG was used on the nostril floor, nasal columella, and alar inner lining. The measured nasal profile included the nostril surface, nostril circumference, width of the nostril floor, and distance from the alar-facial groove to the nasal tip. The "overlap area," which was defined as the largest overlapping area when the image of the cleft nostril was flipped to the left and right and overlaid on the image of the normal side nostril, was also calculated. The degree of symmetry was evaluated by dividing the value of the cleft side by that of the normal side of each measured profile and expressed as "ratios." Results: The results of all profile ratios, except for the nostril floor width, became significantly close to 1, which represents full symmetry. The overlap area ratio improved from 62.7% to 77.3%, meaning that the length and width of the nostril as well as the overall shape became similar (p< 0.05). Conclusion: When performing cleft rhinoplasty with nostril stenosis, FTSG is useful to achieve symmetry in the nostril size and shape. Skin grafting is simpler to perform than the other types of local flap, and the results are generally satisfactory.

턱관절균형의학에서 편차발생현상의 신경과학 및 재료역학적 해석과 일반화 (Interpretation and Generalization by Neuroscience and Material Mechanics on Deviation in Temporomandibular Joint Balancing Medicine)

  • Objectives: For the deviation phenomenon occurring during the treatment process in temporo-mandibular balancing medicine (TBM), hypotheses were established regarding the cause and mechanism of formation from the perspective of neuro-science and material mechanics, and a verification method was proposed. Methods: The deviation phenomenon was theoretically analyzed based on the structure theories of material mechanics of the joint and the neurological pain mechanism. Results: Deviation occurs due to temporary yield by the accumulation of heterogeneous stress in the temporo-mandibular joint and the affected joint. Because the joint structures are corresponding with material mechanics showing compressive and tensile properties. The size of the deviation is expressed in terms of strain. The occlusal surface of the teeth is level with the axial joint. Since the magnitude of the deviation has a proportional relationship with the degree of abnormality of the temporo-mandibular joint, the magnitude of the deviation calculated by the balance measurement can be replaced by the strain. The major variables involved in the occurrence of deviations are the strength of joint structures and neurological conditions. Therefore plastic deformation and adaptation occur as a long-term depression of neural circuits is strengthened in different ways at different locations each time in various clinical situations. This is the reason why the sequence of the restoration process while correcting deviations is following reverse order of the accumulation in many layers in the muscular nervous system. Conclusions: From the above results, it can be inferred that the occurrence and correction of the deviations are corresponding with the plastic deformation and neuro-plasticity.

  • 순수변위 비선형 4절점 쉘요소의 정식화를 제안하여 철근 콘크리트, 강재및 복합재료등 범용 목적의 구조물의 해석에 적합하도록 하였다. 기하강성의 정식은 2차 운동역학적 관계를 이용하여 쉘이 중립면에서 정의되었고 이러한 기하강성은 면내응력, 휨 모멘트와 수직 전단력의 형태로 구성되어 두꺼운 판 및 쉘의 해석에 효과적이다. 가정된 자연 변형률 방법을 사용하여 전단잠김 문제를 제거한 복합 쉘 요소는 얇은 판및 쉘의 경우에도 정확한 해를 구할 수 있다. 콘크리트 경우 소성이론 및 탄소성 파괴역학에 근거한 비탄성 해석이 가능하며 강재경우 폰미스의 항복이론과 이바노브의 항복이론을 이용한 소성해석이 가능하다. 복합 재료의 수직전단 강성 행렬은 평형방정식으로부터 유도하여 구성하였다. 본 연구에서 제안한 쉘 요소는 해석 예제들이 참고문헌과 잘 일치하여 정확성이 입증되었으며 범용목적의 박판구조 해석에 적합한 것으로 사료 되었다.

  • PET/MRI에서는 MRI의 진단적 가치를 높이기 위해 T1 조영제를 사용하고 있다. PET의 감쇠 보정을 위해 T1 시컨스 계열인 VIBE DIXON은 조영제에 직접적으로 영향을 미치지만, 실제 ${\mu}-map$과 감쇠 보정된 PET 영상에는 큰 변화가 없었다. 그러므로 PET/MRI 검사시 조영제 사용은 PET 데이터 얻기 전 후 언제든 사용할 수 있을 것이다.

영상정보디스플레이용 비대칭 비구면 삼각 프리즘 광학계 연구 (Investigation of Asymmetric Aspherical Triangular Prism Optical System for Video Information Display)

  • We have investigated anamorphic prism lenses with distortions of 0.3-0.5%. We designed the plastic triangular lens and confirmed the minimum resolution using MTF graphs. Also we confirmed that the SVGA optical system can realize a resolution of $864{\times}648$ 56 megapixels. A distortion of about 0.5% aberration appears in the maximum field, and a finite beam aberration of about $15{\mu}m$ is confirmed. We made a mold based on the design data and completed the prism lens through exodus molding. We confirmed the shape error (< $30{\mu}m$) and surface roughness (> 40 nm) of the three sides. We made the video-information-display prototype glasses using prism lens by measuring the performance, we determined the distortion aberration (0.3%) and SVGA resolution. Our approach will enable fabrication of a portable large-screen display device for glasses and sunglasses for the domestic market and, after 2015, for the world market.

빙해선박용 강재의 저온특성에 관한 연구 (On the Mechanical Properties at Low Temperatures for Steels of Ice-Class Vessels)

  • Tensile tests were conducted at low temperatures for the steel materials which are used for outer shell of the vessels making transit through the polar regions. The selected steel materials were GL-DH32, GL-DH36 and GL-EH36. In comparison with the results at room temperature, the yield stress increases approximately by 10 to 13 percent at $-30^{\circ}C$ and by 13 to 19 percent at $-50^{\circ}C$ while the tensile strength increases about by 9 percent at $-30^{\circ}C$ and 11 to 14 percent at $-50^{\circ}C$. To obtain true stress-true strain, i.e. correct plastic hardening characteristics, Bridgman's(1952) necking correction formula was introduced taking triaxial state of stresses after onset of diffuse necking into consideration. Photographs of fractured surfaces were taken by using Scanning Electron Microscope immedately after tensile tests completed and one for GL-EH36 has been presented in this paper.

유공판의 좌굴강도에 관한 연구 (A Study on the Buckling Strength of Plate Panels with Opening)

  • The aim of the present study is to investigate the buckling strength of plates and stiffened panels with opening under transverse thrust and shear actions. It is observed that the existing design formulation for critical-buckling strength of plates are not valid for perforated plates, because the current design formulation trends can significantly overestimate or underestimate the load-carrying capacity of plates when plates have large opening and/or are thick. A series of eigen value and elastic.plastic large deflection finite element analyses are carried out with varying the aspect ratio of plate, the opening size and location on plate until and after the ultimate strength is reached. Based on the results obtained from the present study, closed-form design formulations for the elastic buckling strength of plates and stiffened panels with opening are derived. The derived design formulations are considered plasticity correction of the material and verified by experimental tests and results of nonlinear finite element computations.

Automatical Cranial Suture Detection based on Thresholding Method

  • Purpose The head of infants under 24 months old who has Craniosynostosis grows extraordinarily that makes head shape unusual. To diagnose the Craniosynostosis, surgeon has to inspect computed tomography(CT) images of the patient in person. It's very time consuming process. Moreover, without a surgeon, it's difficult to diagnose the Craniosynostosis. Therefore, we developed technique which detects Craniosynostosis automatically from the CT volume. Materials and Methods At first, rotation correction is performed to the 3D CT volume for detection of the Craniosynostosis. Then, cranial area is extracted using the iterative thresholding method we proposed. Lastly, we diagnose Craniosynostosis by analyzing centroid relationships of clusters of cranial bone which was divided by cranial suture. Results Using this automatical cranial detection technique, we can diagnose Craniosynostosis correctly. The proposed method resulted in 100% sensitivity and 90% specificity. The method perfectly diagnosed abnormal patients. Conclusion By plugging-in the software on CT machine, it will be able to warn the possibility of Craniosynostosis. It is expected that early treatment of Craniosynostosis would be possible with our proposed algorithm.

Influences of seepage force and out-of-plane stress on cavity contracting and tunnel opening

  • The effects of seepage force and out-of-plane stress on cavity contracting and tunnel opening was investigated in this study. The generalized Hoek-Brown (H-B) failure criterion and non-associated flow rule were adopted. Because of the complex solution of pore pressure in an arbitrary direction, only the pore pressure through the radial direction was assumed in this paper. In order to investigate the effect of out-of-plane stress and seepage force on the cavity contraction and circular tunnel opening, three cases of the out-of-plane stress being the minor, intermediate, or major principal stress are assumed separately. A method of plane strain problem is adopted to obtain the stress and strain for cavity contracting and circular tunnel opening for three cases, respectively, that incorporated the effects of seepage force. The proposed solutions were validated by the published results and the correction is verified. Several cases were analyzed, and parameter studies were conducted to highlight the effects of seepage force, H-B constants, and out-of-plane stress on stress, displacement, and plastic radius with the numerical method. The proposed method may be used to address the complex problems of cavity contraction and tunnel opening in rock mass.
