소아 및 청소년 비만증에서 인슐린 저항성에 미치는 요인 (Affecting Factors of Insulin Resistance in Obese Children and Adolescents)

  • 목 적 : 인슐린 저항성은 소아 및 청소년 비만증에서 내당능 장애, 당뇨병증, 심혈관계질환을 일으키는 중요한 위험요인 중 하나로 알려져 있다. 따라서 이 연구의 목적은 비만증 청소년이 소아에 비해 인슐린저항성이 증가하는가 그리고 성별에 따른 차이점을 규명하며 인슐린저항성에 영향을 미치는 요인들을 분석하고자 한다. 방 법 : 5-16세의 학생 9,837명을 신체검사하여 92명의 비만증 소아와 187명의 비만증 청소년을 선별하였고, 이들을 대상으로 당부하검사, 공복상태의 혈당, 인슐린농도, 총콜레스테롤, LDL-콜레스테롤, HDL-콜레스테롤, 중성지방의 농도, leptin, hs-CRP 및 adiponectin 농도를 측정하였다. 결 과 : 당부하검사를 통한 비만증 소아 및 청소년에서 성별에 따른 인슐린의 농도는 여자에서 인슐린의 농도가 높았다(P<0.05). 비만증 소아에서 4명(4.3%), 비만증 청소년에서 25명(13.3%)에서 내당능 장애로 나타났으며, 당뇨병으로 진단된 경우는 1례도 없었다. 인슐린 저항성은 여자, leptin, adiponectin, 중성지방의 농도와 밀접한 상관관계가 있었다(P<0.05). 결 론 : 비만증에서 인슐린 저항성은 소아보다 청소년에서 컸으며, 남자보다 여자에서 컸다. 또한 leptin 및 중성지방의 농도가 높을수록, adiponectin의 농도가 낮을수록 인슐린 저항성이 컸다. 소아 및 청소년 비만증에서 인슐린 저항성에 미치는 요인들에 관하여 좀 더 많은 연구가 필요하다.

Efficacy of evogliptin and cenicriviroc against nonalcoholic steatohepatitis in mice: a comparative study

  • Dipeptidyl peptidase (DPP)-4 inhibitors, or gliptins, are a class of oral hypoglycemic drugs that have been widely used as a second-line treatment for type 2 diabetes. Gliptins, which were introduced for clinical use a decade ago, have been shown to be beneficial against nonalcoholic fatty liver disease/nonalcoholic steatohepatitis (NASH) in animals and humans. Cenicriviroc (CVC), a dual antagonist of C-C chemokine receptor type 2 and 5, is currently under investigation against NASH and fibrosis. It was previously discovered that evogliptin (EVO) reduces hepatic steatosis in diet-induced obese animals but the effectiveness of EVO on NASH remains unexplored. Here, we compared the effectiveness of EVO and CVC against NASH and fibrosis in mice fed a high-fat and high-fructose diet (HFHF). Biochemical and histological analyses showed that mice fed a HFHF for 20 weeks developed severe hepatic steatosis and inflammation with mild fibrosis. Administration of EVO (0.2% wt/wt) for the last 8 weeks of HFHF feeding significantly reduced hepatic triglyceride accumulation, inflammation, and fibrosis as well as restored insulin sensitivity, as evidenced by lowered plasma insulin levels and the improvement in insulin tolerance test curves. Treatment of mice with CVC (0.1% wt/wt) inhibited hepatic inflammation and fibrogenesis with similar efficacy to that of EVO, without affecting hepatic steatosis. CVC treatment also reduced plasma insulin concentrations, despite no improvement in insulin tolerance. In conclusion, EVO administration efficiently ameliorated the development of NASH and fibrosis in HFHF-fed mice, corroborating its therapeutic potential.

The Plasma Level of Insulin-like Growth Factor-I (IGF-I) in Relation to Mammary Circulation and Milk Yield in Two Different Types of Crossbred Holstein Cattle

  • The objective of the present study was to determine the plasma level of insulin-like growth factor-I (IGF-I) in relation to mammary blood flow and milk yield including biological variables of relevance to milk synthesis in two different types of crossbred Holstein cattle at 3 different stages of lactation. Eight heifers were 87.5% HF and eight 50% HF animals were selected for the experiments. The three stages of lactation tested were: early lactation (30 days postpartum), mid-lactation (120 days postpartum) and late lactation (210 days postpartum). Animals in each group were fed a concentrate and rice straw treated with 5% urea as the source of roughage throughout the experiments. In early lactation, mammary blood flow and milk yield of 87.5% HF animals were significantly higher than those of 50% HF animals. In mid- and late lactation, both mammary blood flow and milk yield showed a proportional decrease from the early lactating period of 87.5% HF animals. The trends for persistency were observed in 50% HF animals as for udder blood flow and milk yield throughout the experimental periods. The plasma glucose level of the 50% HF animals was significantly higher than those of 87.5% HF animals in both early and mid-lactation. The concentrations of arterial plasma free fatty acids ($C_{16}\;to\;C_{18}$) were higher in 50% HF animals as compared with 87.5% HF animals in all periods of study. In early lactation, the concentration of plasma growth hormone (GH) of 87.5% HF animals was higher than those of the 50% HF animals, thereafter the mean level of plasma growth hormone declined in both mid- and late lactation. The concentration of plasma IGF-I of 50% HF animals was significantly higher than those of 87.5% HF animals in all stages of lactation. There were no differences among stages of lactation for the levels of plasma IGF-I, insulin and growth hormone in 50% HF animals. In 87.5% HF animals, the plasma levels of both IGF-I and insulin were lower in early lactating period while it showed an increase during mid- and late lactation. The present results indicated that the regulatory role for the higher mammary blood flow and milk yield during lactation in 87.5% HF are not mediated via the higher level of circulating IGF-I. Differences in mammary blood flow and milk yield between 50% HF and 87.5% HF animals are in part due to a higher concentration of circulating growth hormone. The lower level of circulating growth hormone in 50% HF animals would be regulated by higher levels of IGF-I, free fatty acid and glucose in plasma.

Effects of Refeeding with a Protein-Free Diets Supplemented with Various Essential Amino Acids on the Plasma Insulin-Like Growth Factor-I Concentration in Fasting Young Chickens

  • The effect of refeeding with various single essential amino acids on the recovery of plasma insulin-like growth factor-I (IGF-I) concentration in fasted young chickens was examined. Young chickens (29 days of age) were divided into 15 experimental groups. Chickens in one group were fed on the commercial diet ad libitum for 4 days. The remaining 56 chickens in 14 experimental groups were fasted. After 2 days of fasting, 52 chicks in 13 fasted groups were refed with one of the following experimental diets for 2 days. Eleven experimental diets were protein-free diets supplemented with one of 11 essential amino acids (Arg, Gly, His, Ileu, Leu, Met, Phe, Lys, Thr, Trp, Val). The remaining 2 experimental diets were a protein-free diet containing 11 essential amino acids and a protein-free diet not supplemented with amino acids. Birds in the remaining fasted group continued to be fasted for 2 days. Fasting for 2 days markedly reduced plasma IGF-I concentration. When fasted chickens were refed the protein-free diet containing either Gly alone or all essential amino acids, plasma IGF-I concentration was recovered to the level similar to that of fed chickens. Protein-free diet alone, however, failed to restore the reduced IGF-I concentration in plasma. Body weight loss modulated by feeding with protein-free diets supplemented with various single essential amino acids was associated with changes in plasma IGF-I concentrations. We concluded that body weight loss by feeding with a protein-free diet was lower than that of fasted chickens and that body weight loss associated with the decrease in plasma IGF-I concentration was modulated by feeding with protein-free diets containing various single essential amino acids.

이첨판 패쇄부전증에 이환된 개에서 NT-proBNP농도, 비만, 당 관련 인자 및 지방 관련인자간에 상관관계에 대한 연구 (Relationships between NT-proBNP and Obesity, Glucose and Lipid Profiles in Dogs with Chronic Mitral Valve Insufficiency)

  • 사람에 연구에 따르면 심부전 진단에 널리 사용되는 NT-proBNP농도는 비만, 비만 관련인자들과 밀접한 상관 관계가 있는 것으로 알려져 있다. 그러나 이러한 상관관계는 아직까지 수의분야에서는 입증된 바가 없다. 따라서 본 연구에서는 다양한 정도의 이첨판 패쇄부전증(CMVI)에 이환된 73마리 개 집단에서 NT-proBNP와 비만 및 비만 관련인자들간의 상호관계를 조사하였다. 절식시 혈당 농도, 지방관련 인자(i.e., total triglycerides [TG], total cholesterol [TC], high-density lipoprotein cholesterol [HDL-C], low-density lipoprotein cholesterol [LDL-C]), fructosamine, insulin 및 NT-proBNP 농도를 측정하였다. Insulin/glucose ratio 역시 계산하였다. NT-proBNP 농도는 CMVI에 의한 심부전의 정도에 따라 농도변화가 관찰되었으며, body condition scores (BCS), insulin 및 fructosamine 농도와도 관련이 있었다. 또한 본 연구를 통해 비만과 혈중 NT-proBNP농도 간에 상관관계 역시 확인되었다. 또한 NT-proBNP농도는 비만과 관련 있는 인자인 fructosamine과 insulin농도와도 관련이 있었다. 본 연구는 개에서 NT-proBNP농도와 비만 정도와 상관관계를 입증한 첫 보고이다.

Fibroin Enhances Insulin Sensitivity and Reverses Insulin Resistance in 3T3-L1 Adipocytes

  • Type 2 diabetes is characterized by hyperglycemia and hyperinsulinemia, features of insulin resistance. In vivo treatment of ob/ob mice with hydrolyzed fibroin reverses these pathological attributes (6). To explore the mechanism underlying this effect, we have used the 3T3-Ll adipocytes as a cell type which would represent the periphery, in vivo. Exposure of 3T3-Ll adipocytes to chronic insulin leads to the a 50% loss of insulin-stimulated glucose uptake. Chronic exposure to fibroin blocked, in part, the response to chronic insulin but also increased the sensitivity of control cells to the acute action of insulin. The later effect was most robust at physiological concentrations of insulin. Fibroin did not prevent the insulin-induced down-regulation of the insulin receptor or the tyrosine kinase activity associated with the receptor. Further, fibroin had no affect on the loss in activity of the insulin-sensitive down-stream kinase, Akt. Interestingly, fibroin accelerated glucose metabolism and glycogen turnover independent of insulin action. In addition, fibroin up-regulated GLUT1 which increased its expression at the cell surface and caused the redistribution of GLUT4 to the plasma membrane. Together, these later effects would lead to an improvement in hyperglycemia in vivo which would in turn reduce the need for insulin.

홍경천 섭취와 운동수행이 비만 쥐의 인슐린 민감도와 골격근내 당수송 관련 단백질 발현에 미치는 영향 (Effect of Rhodiola Sachalinensis Administration and Endurance Exercise on Insulin Sensitivity and Expression of Proteins Related with Glucose Transport in Skeletal Muscle of Obese Bucker Rat)

  • Peripheral insulin resistance in obese/type II diabetes animals results from an impairment of insulin-stimulated glucose uptake into skeletal muscle. Insulin stimulate the translocation of GLUT4 from intracellular location to the plasma membrane. Soluble N-ethylmaleimide-sensitive factor attachment protein receptors (SNAREs) is implicated in mediation of fusion of GLUT4-containing vesicle with the plasma membrane. Present study investigated regulatory effects of Rhodiola sachalinensis administration and exercise training on the expression of GLUT4 protein and SNAREs protein in skeletal muscles of obese Zucker rats. Experimental animals were randomly assigned into one of five groups ; lean control(LN), obese control(OB), exercise-treated(EXE), Rhodiola sachalinensis-treated(Rho), combine of Rho & EXE (Rho-EXE). All animals of exercise training (EXE, Rho-EXE) performed treadmill running for 8 weeks, and animals of Rho groups (Rho, Rho-EXE) were dosed daily by gastric gavage during the same period. After experiment, blood were taken for analyses of glucose, insulin, and lipids levels. Mitochondrial oxidative enzyme (citrate synthase, CS ; $\beta$-hydroxyacyl-CoA dehydrogenase, $\beta$-HAD) activity were analysed. Skeletal muscles were dissected out for analyses of proteins (GLUT4, VAMP2, syntaxin4, SNAP23). Results are as follows. Exercise and/or Rhodiola sachalinensis administration significantly reduced body weight and improved blood lipids (TG, FFA), and increased insulin sensitivity. Endurance exercise significantly increased the activity of mitochondrial enzymes and the expression of GLUT4 protein, however, administration of Rhodiola sachalinensis did not affect them. The effect of exercise and/or Rhodiola sachalinensis administration on the expression of SNARE proteins was unclear. Our study suggested that improvement insulin sensitivity by exercise and/or Rhodiola sachalinensis administration in obese Zucker rats is independent of expression of SNARE proteins.

  • Antidiabetic effects of the acid hydrolysate of silk fibroin were investigated by oral administration to animal models for diabetes mellitus, Fibroin protein was extracted from cocoon and digested to peptides of low-molecular weight range (mainly below 3,000) and amino acids by acid hydrolysis, Feeding of the fibroin hydrolysate resulted in a significant recovering effect on reduction of body weight gain and a lowering effect on blood glucose gain in streptozotocin-induced diabetic Sprague Dawley rats (STZ rats) which were used as an insulin-dependent diabetic animal model. But the body weight and blood glucose level in C57BL/KsJ-db/db mice (db/db mice), an non-insulin-dependent diabetic animal model, were not changed significantly by the feeding, On the other hand, plasma leptin levels increased according to increased feeding amount of the hydrolysate in STZ rats and db/db mice in common, It was concluded from the results that the fibroin hydrolysate might stimulate the insulin secretion by recovering or activating pancreatic ${\beta}$ cells and result in the increased plasma leptin level. It was also deduced that the antidiabetic improvements in body weight and blood glucose gain in STZ were thought to be due to the increased insulin secretion, but in db/db mice of which the diabetic symptoms were caused by insulin resistance, the stimulated secretion of insulin was unlikely to be able to change body weight and blood glucose level significantly.

  • Liver is the major organ to regulate the systemic glucose homeostasis and insulin resistance. Excess energy intake leads to triglyceride accumulation in adipose tissue first and subsequent accumulation in liver, resulting in obesity and type 2 diabetes. The representative pathological animal model for obesity associated insulin resistance is a high fat diet (HFD) fed mice model. Given the essential role of liver fat accumulation in developing systemic insulin resistance in obesity, I measured the liver triglyceride contents in HFD fed mice as a function of time. As such, in this report, I show the cause and effect relationship with regard to time during a HFD feeding between a variety of factors that are related to systemic insulin resistance including glucose intolerance, plasma insulin level and inflammatory gene expression in liver and adipose tissue.

Altered Secretory Pattern of Pancreatic Enzymes and Gastrointestinal Hormones in Streptozotocin-induced Diabetic Rats

  • This study was performed to investigate the pancreatic exocrine dysfunction in streptozotocin- induced diabetic rats. Changes in pancreatic enzymes secretion and in pancreatic enzymes content were observed. The output and the tissue content of amylase were significantly reduced in diabetic rats, while the output and the content of lipase were increased. Plasma secretin and cholecystokinin (CCK) concentrations of diabetic rats were significantly increased compared to those of normal rats. The altered pancreatic exocrine function was abolished by the exogenous insulin administration. The exogenous insulin also restored the increased plasma secretin and CCK concentrations. From the above results, it is suggested that, in streptozotocin-induced diabetic rats, anticoordinated changes in pancreatic enzymes secretion as well as pancreatic enzymes content are attributable to insulin deficiency and that the insulin deficiency is responsible for the increased plasma concentrations of both secretin and CCK. However, it is not clear whether the elevated plasma secretin and CCK concentrations played a direct role in changes of pancreatic exocrine function.
