주정중독환자에서 초기 비특이적 면역력의 주된 반응인 백혈구 탐식작용을 이용하여 이들 환자들의 건강관리를 용이하게 할 수 있는지를 알아 보기위하여 백혈구 탐식 작용력을 측정하고자 실시하였고, 연구대상은 주정중독 의존형으로 진단된 남자 95명, 여자 3명과 대조군으로서는 건강한 헌혈자 32명을 대상으로 실시하였다. 또한 주정중독환자의 건강상태를 확인하기 위하여 백혈구수, 혈색소, 적혈구 평균용적, 혈청 단백질 전기영동, 림프구 아형 분리 및 림파구 유약화 시험도 실시하였따. 분석결과에서 혈색소와 적혈구 평균용적은 대조군에 비하여 신, 장기 입원 환자군에서 유의할만한 차이를 보였으며(P<0.05), 총 T와 B 림프구는 감소하였고, $T_{Helper}/T_{suppressor}$ 비율은 $1.6{\pm}0.8%$로 증가하였다. 특히 신, 장기입원 주정중독자의 Staphylococcus aureus Cowan I을 사용한 백혈구의 phagocytic plaque 형성력에서도 대조군에 비하여 현저하게 저하되었고, 이제까지 보고된 바 없는 특이한 strange body, 사슬상 탐식현상을 관찰 할 수 있었다. 따라서 Staphylococcus aureus Cowan I을 사용한 백혈구의 phagocytic plaque 형성 시험은 특별한 기구가 없이 간단하게 이용 가능하여 주정중독환자의 초기 면역력 측정에 매우 유용한 방법임을 알 수 있었다.
Objectives : Since the program about sodium lauryl sulfate that might cause oral dryness and taste change including oral tissue allergy was on the air, the ingredients of D.I.Y dentifrices without sodium lauryl sulfate have been sold in online shopping mall and ordinary people can make the dentifrices easily. But there have not been any reports about the effect of dental plaque removal and preference about D.I.Y dentifrices. Therefore, this study was designed as a pilot study which aimed to investigate the effect of dental plaque removal and prefernece about D.I.Y dentifrices. Methods : 6 subjects were collected to test the effect of dental plaque removal with D.I.Y dentifrices with written consent. They didn't brushed the teeth during 12 hours until the check-up time on the next day. The O'leary index was calculated in baseline, 1 minute, 2 minute, 3 minute after brusing with D.I.Y dentifrices and market dentifrices. The preference about D.I.Y dentifrices through self-administered questionnaire was surveyed in 51 subjects after using D.I.Y dentifrices. Results : 1. The market dentifrices could remove larger amount of dental plaque than D.I.Y dentifrices, but, it wasn't significant. 2. In feeling refreshment, the market dentifrices had more positive answers significantly. 3. In feeling taste change, the market dentifries had more duration of taste change significantly. 4. In feeling oral dryness, the market dentifrices had more duration of oral dryness after toothbrushing. but, it wasn't significant. 5. As a result about reviewing the dental journals about ingredients of D.I.Y dentirices, green tea, sodium carbonate, bamboo salt, propolis had each evidence. But, We could not find out the evidences of calculus adhesion by corn starch, preservative by napri, disinfectant of peppermint. Conclusions : Although we cannot find the difference of the effect of dental plaque removal between D.I.Y dentifrices and market dentifrices, and D.I.Y dentifrices have the merits of decrease of oral dryness and taste change, it was suggested to have another test about stability and safety of D.I.Y dentifrices for safety of the user of D.I.Y dentifrices.
Objective : The extensive vasa vasorum network functions as a conduit for the entry of inflammatory cells or factors that promote the progression of angiogenesis and plaque formation. Therefore, we investigated the correlation between the carotid vasa vasorum activities and carotid plaque vulnerability using indocyanine green video angiography (ICG-VA) during carotid endarterectomy (CEA). Methods : Sixty-nine patients who underwent CEA were enrolled prospectively from September 2015 to December 2017. During CEA, a bolus of ICG was injected intravenously before and after resecting the atheroma. Additionally, we performed immunohistochemistry using CD68 (a surface marker of macrophages), CD117 (a surface marker of mast cells), and CD4 and CD8 (surface markers of T-cells) antibodies to analyze the resected plaque specimens. Results : The density of active vasa vasorum was observed in all patients using ICG-VA. The vasa vasorum externa (VVE) and interna (VVI) were seen in 11 (16%) and 57 patients (82.6%), respectively. Macroscopically, the VVE-type patterns were strongly associated with preoperative angiographic instability (81.8%, p=0.005) and carotid plaque vulnerability (90.9%, p=0.017). In contrast, the VVI-type patterns were weakly associated with angiographic instability (31.6%) and plaque vulnerability (49.1%). CD68-stained macrophages and CD117-stained mast cells were observed more frequently in unstable plaques than in stable plaques (p<0.0001, p=0.002, respectively). Conclusion : The early appearance of VVE, along with the presence of many microvessel channels that provided nutrients to the developing and expanding atheroma during ICG-VA, was strongly associated with unstable carotid plaques. The degree of infiltration of macrophages and mast cells is possibly related to the formation of unstable plaques.
The purpose of this study was to investigate and compare the frequence of 4 periodontal pathogens in the supra- and subgingival plaque in periodontally healthy subjects. Twenty adult individuals aged 22 to 28 years (mean age 23.65 years) participated in this study. All subjects had no pocket sites more than 3 mm deep, and the sites selected for sampling were all negative for bleeding. After drying and isolation of the sites with cotton rolls, supragingival plaque was sampled using sterile periodontal curette. Each plaque sample was placed in individual tubes containing 500 ml of 1X PBS. After removal of the supragingival sample and any remaining supragingival plaque, subgingival plaque samples were taken from the same sites using sterile curette and placed in similar individual tubes. Identification of 4 putative periodontal pathogens from the samples was performed by polymerase chain reaction based on 16S rDNA. Chi-square test was employed to identify significant explanatory variables for the presence of the 4 periodontal pathogens. The data show that Actinobacillus actinmycetemcomitans, Porphyromonanas gingivalis, Bacteroides forsythus, and Fusobacterium nucleatum occurred in 16.9%, 14.4%, 52.5%, and 80.6%, respectively. No significant differences were noted in the periodontal pathogens between supra- and subgingival plaques according to the kind of teeth. However, the incisors were at higher risk for harboring F. nucleatum (p <0.05). Conclusion: These results reveal that anaerobic periodontal pathogens can be detected in supragingival plaques. Supragingival plaque may function as a reservoir of peri-odotopathogens.
이 연구의 목적은 고정식 교정장치 부착환자에서 음파잇솔질과 수동잇솔질의 치면세균막 제거능력을 비교, 평가하여 교정치료시 지속적이고 효과적인 구강위생 관리 방법을 제시하고자 하는데 있다. 비발치 환자로 상, 하악 제 2대구치까지 고정식 교정장치를 부착한 청소년기 아동(남자 23명, 여자 37명)과 성인(남자 15명, 여자 45명)을 대상으로 하여 각각을 수동잇솔질과 음파잇솔질 군으로 분류하였다. 음파칫솔은 Philips oral healthcare HX-4401을 사용하였으며 잇솔질시간은 음파, 수동 모두 2분으로 표준화시켰다. 잇솔질 후 치면세균막 지수는 Wilkins의 PHP를 변형시켜 측정하였다. 각 군의 치면세균막 지수를 측정하여 비교한 결과 다음과 같은 결과를 얻었다. 1. 청소년, 성인 모두에서 음파잇솔군의 치면세균막지수 감소효과가 수동잇솔군에 비해 유의하게 크게 나타났다(청소년 : p<0.01 성인 : p<0.05). 2. 청소년과 성인을 구분하여 치면세균막지수 감소양상을 비교하였을 때 청소년에서 더 큰 감소 효과를 보였다.(p<0.01). 3. 같은 칫솔종류를 사용할 경우 청소년, 성인간에 유의한 차이를 보이지 않았다.
본 연구는 1990년부터 2015년까지의 기간 동안 국내에서 PMPR과 구강위생교습을 시행한 문헌 22편을 대상으로 아동과 성인의 구강위생관리 효과를 체계적으로 고찰함으로써 연구의 효과를 파악하기 위하여 실시되었다. 자료의 결과를 종합해 보면 PMPR을 하는 데 있어서 1~2주 간격으로 4~5번의 방문이 가장 효과적이고, 구두교육이나 모형교육보다는 치면착색제를 사용하고 OHE와 전문가 칫솔질, 치면세마, SRP 등을 병행하여 환자의 특성에 맞춰 반복적인 교육을 하는 것이 효과적임을 보여주었다. 실제 임상적으로 치주와 부착수준의 개선됨을 확인함으로써 환자 구강의 특성에 맞는 규칙적인 프로그램과 정기적인 일정으로 반복적 PMPR이 필요하다. 분석에 포함된 PMPR, 칫솔질 교습의 중재효과에 대하여 구강위생관리 효과가 큰 것으로 밝혀짐에 따라, 구강건강을 증진하고 유지하는 데 가장 필수적인 요소로 OHE와 함께 시행되면 구강건강 증진과 향상에 도움이 된다. 최근 시행되는 성인대상 치석제거술의 건강보험 급여제도와 더불어 교육수행에 관련해서도 급여범위에 포함이 된다면 구강병 발생이 감소되고 건강보험 재정에도 도움이 되어 국민구강증진에 더욱 시너지 역할을 할 것으로 생각된다. 지속적으로 복지정책에 필요한 근거중심의 체계적인 연구가 시행될 수 있도록 기초적인 자료를 마련해야 할 것이다.
The purpose of present study was to evaluate the relationship between the early change of gingival condition and methyl mercaptan concentration during experimental gingivitis. Ten men(23-25 years old) whose gingiva were clinically healthy were selected. The participants have ceased to perform all forms of oral hygiene during 14 days and then did thorough plaque control for 7 days. For each subject, the methyl mercaptan concentration was measured by $B.B.Checker^{(R)}$ (Bad Breath Checker with printer, Tokuyama Soda Co.,LTD., Japan)before experiment and 1,4,7,14,21 days during experiment. Plaque index(Silness & $L\ddot{o}e$), gingival sulcus depth and sulcus bleeding index($M\ddot{u}hlemann$ & Son)score were recorded. The results were as follows. 1. Methyl mercaptan concentration increased continuously from the first day to the 14th day, decreased on the 21th day but it was still higher(P<0.001). 2. Plaque index score and sulcus bleeding index score tended to increase on the 4th day, markedly increased on the 14th day and returned to baseline level on the 21th day. 3. There was parallel relationhsip among methyl mercaptan concentration, plaque index score and sulcus bleeding index score. This result suggests that methyl mercaptan concentration increased with deterioration in gingival health, but decreased during recovery of normal health condition.
Introduction : Inadequate oral health control is a major risk of oral diseases. Regular home-based care is essential to maintain good oral hygiene. In particular, mouthrinses can support conventional tooth brushing in reducing accumulation of oral plaque. Effect : Antimicrobial mouthrinses are used as part of daily oral care to reduce plaque and gingivitis. Mouthrinses contains fluoride could help remineralization of enamel and dentin. The most common molecules contained in mouthrinses are chlorhexidine, essential oils, cetyl pyridinium chloride, triclosan, hyaluronic acid. Currently, chlorhexidine is the most efficacious compound, with both antiplaque and antibacterial activities. Similar results are reported for essential oils and cetyl pyridinium chloride, although with a somewhat reduced efficacy. Considering the adverse effects of chlorhexidine and its time-related characteristics, this molecule may best be indicated for acute/short term use, while essential oils and cetyl pyridinium chloride may be appropriate for long-term, maintenance treatment. Conclusion and suggestion : Antimicrobial mouthrinses are safe and effective, and when used in conjunction with brushing and flossing, they are an important method of reducing plaque and gingivitis. To improve compliance, dental health care professionals should adapt oral health care recommendations to fit patients' specific needs.
