A study on the grief and coping pattern of mothers who have a chronically ill child (만성질환아를 가진 어머니의 비통감과 이에 대응하는 정도에 관한 연구)

  • This study was done in order to identify symptoms of grief and patterns of coping in mothers of chronically ill children. Nurses, as health professionals, must understand helpful coping methods for mothers who have chronically ill children. Based on this knowledge, nurses can develop appropriate nursing intervention strategies, and so help the mothers to develop effective ways of coping and give support to mothers with chronically ill children in process of coping with this grief. The research questions guiding this research were: 1. what type of grief does the mother have at the time of child's diagnosis and at present\ulcorner 2. what are the problems confronting the mother 3. what are the coping patterns of the mother\ulcorner 4. is there a difference according to child's diagnosis in the mother's grief and coping pattern\ulcorner The subject for this study were obtained by taking a convenience sample of 120 mothers with a chronically ill child. Interviews took place in four medical school hospitals and one medical center in Seoul from March 19th to May 16th 1990. The tools used in this study were Mcfarlan's(1983), Grief contents, Folkman & Lazarus(1983), Ways of Coping and Hymovich's (1983) the Chronicity Impact and Coping Instrument. The findings were as follows; 1. Grief items had a maximum score of three points. The highest item scrores at the time of diagnosis, were in order, heart broken, talkative, and could not do anything, at the present, 'talkative', 'heart broken' and 'busy myself with other things'. 2. The problems confronting the mothers were 'worry about ill child's future', 'her responsibilities in taking care of the ill child.' 3. Most of the mothers used similar ways of coping, that is 'problem focused coping', 'detachment', 'wishful thinking', 'seeking social support" and 'focusing on the positive and hardly any of them used 'tension reduction'. 4. There was a sinificant difference in coping methods according to the childs diagnosis, leukemia, cancer hemophillia and nephrotic syndrome. The most frequently used coping method was detachment, especially for the mother of the child with. 5. At the time of diagnosis there was a positive correlation between the level of grief and the coping method of detachment and seeking social support.

  • The hospital is not the place for functional isolationalism, that is, for each department to do their own activities independently. Rather it is an organization in which each part is joined together and concentrating on the same objectives. The hospital must maintain a one-system-function, so that all the resources of the hospital may be used efficiently. Although we do not yet completely understand how the nursing image as perceived by the medical team and the other members of the interdisciplinary team affects nursing practice. It is important to identify how the client recognizes the nursing image as this will influence the growth of the nursing profession directly and indirectly. So we must reevaluate the nursing image as presented to the client and find the ways to present them with a better image of nursing. The study procedures were as follow; First, the image of nurses was analyzed. The subjects for this study were 270 nurses, 172 doctors, 237 hospital employees, and 240 patients and their families from 5 local branches of the Yonsei Medical Center. The four negative responses shown in this study are as follows; 1) Nurses don't listen to patients. 2) Nurses are self-centered. 3) Nurses and careless. 4) Nurses are mechanical. Second, the concrete causes of the four negative responses were analyzed. Third, the contribution of the nurses for clinical practice, research and education was analyzed. Data were collected by questionnaires all seps of the research. Data were analyzed by use of SPSS/PC. In conclusion, the image of nurses differed according to which group, the respondents belonged. Generally, the public have a positive image of nurses, but hospital employees have a negative images of nurses. Further the nurses gave themselves a low grade. They showed negative as to the value of their job, professionality and their contribution to clinical practice when it was compared to that of the doctors.

A Study on Urinary Tract Infections in Intensive Care Unit Patients with an Indwelling Urinary Catheter

  • This study was performed in order to evaluate the risk factors for nosocomial urinary tract infection and the frequencies of organisms isolated, and to provide the epidemiologic and basic data of hospital acquired urinary tract infection in intensive care unit. A prospective analysis was performed with 1,235 urine samples following urinary bladder catheterization in 569 patients, who had no evidence of UTI at the time of catheter insertion, admitted to intensive care unit in Pusan P hospital between June 1997 and May 1998. To identify risk factors for UTI, clinical characteristics of infected patients were analyzed. We analyzed these data by percentage, chi-square and odd ratio. Obtained results were as follows: A total of 569 patients (male 341 and female 228) were an average age of 50.8 years and catheterization of 8.04 days. Incidence of UTI was 16.1% (199/1,235) and The risk factors of UTI were duration of catheterization over 7 days, no use of systemic antibiotics, summer and female, and During the first 7 days these risk factors were no use of systemic antibiotics, summer, place of first catheter insertion (ICU) and type of intensive care unit (NSICU). A total of 220 the isolated strains were Gram negative rod 83 (37.7%), yeast like fungi 74 (33.6%) and Gram positive cocci 63 (28.6%). The common organisms isolated were Enterococcus faecalis 23 (10.5%), Serratia marcescens 19 (8.6%), Pseudomonu spp.17 (7.7%), E. ooh 16 (7.3%), Staphylococcus epidemidis 11 (5.0%) mdklebsiellapneumoniae 8 (3.6%). Therefore, in these results 199 of 569 (35%) patients in ICU with indwelling urinary catheter developed UTI. The risk factors for UTI are prolonged duration of catheterization, no use of systemic antibiotics, summer, and female.

Technical Approaches for Assessment of Ground Water Contamination with TCE in an Industrial Area

  • Despite its usability, TCE has been managed as a hazardous material due to the toxicity and many contamination cases were surveyed in some developed countries. U.S.EPA(Kram et al., 2001) suggested DNAPL characterization methods and approaches based on survey experiences at several sites. However, Korea has not the least assessment of contamination and trial of remediation, although there are a lot of doubtable areas where ground water would be contaminated with TCE. In this study, we try to assess the volume and extent of ground water contamination with TCE and delineate the contamination source zones in an industrial area. Ground water in this area flows through fractures and the contaminant TCE has the properties of high volatility, high density and low partitioning to soil material. Thus, we applied a variety of technical approaches to identify the contamination status; documentary, hydrogeochemical, hydrogeological and geological surveys. In addition, additional survey was performed based on the interim findings, which showed that ground water contamination was limited to the relatively small area with high concentrations to the deep place. The contamination source zone is estimated to be the asphalt test institute where a great deal of TCE has been used to analyze the amount of asphalt soluble in TCE since 1984. Based on the contamination characterization and a myriad of documents about ground water remediation, appropriate site remediation management options will be recommended later. This study is now under way and this paper was focused on describing the technical approaches used to achieve the goals of this study.

Treadmill-induced Hand Injury in Children (뜀틀기틀에 의한 소아 수부외상)

  • Recently, the usage of an exercising treadmill has increased in Korean homes. Along with increasing utilization of these machines, injuries relating to the inadvertent usage of the machine have gradually increased for the last several years. The purpose of this study is to identify the feature of this type of injury, thereby to help prevent and to raise an awareness of the potential injury. 25 patients who had been admitted to Hangang Sacred Heart Hospital from January 2002 through March 2004 were investigated retrospectively. Age, sex, place of injury, the area and the extent of injury, treatment and prognosis were analyzed for each case. All injuries involved in the children with an average age of 3.5 years. This new type of injury was at least a deep second-degree burn of the friction-burn type in all cases. Most of them primarily involved hands. Inattention of the guardian was the main cause of such injuries. 68% of the cases needed an operation such as a split-thickness skin graft or a full-thickness skin graft. In this study, authors reached a conclusion that such injuries may be prevented by an education of the potential risks of using a treadmill and by developing additional safety devices.

Ozone Oxidation of Trans-3-hexene with/without Pyridine (Pyridine 존재여부에 따른 Trans-3-hexene의 오존 산화 반응)

  • It was explored, whether the usual course of the ozonolysis of olefins can be modified with the help of pyridine. In the First step, the ozone oxidation of trans-3-hexene was performed with and without pyridine in the inert solvents n-pentane and dichloromethane. In addition, base catalyzed decompositions of monomeric and polymeric ozonides were also examined to identify the reaction mechanism. The reaction products were identified by modern analytical tools. The results of this work showed that reactions of ozone with olefins in the absence of pyridine in aprotic solvents gave, one hand, dominantly peroxidic products, namely monomeric and polymeric ozonides. The other hand, they in the presence of pyridine gave only the non-peroxidic products, namely propionaldehyde and rearranged propionic acid without peroxidic products. It seems, also, that the pyridine-catalyzed isomerization of the Criegee zwitterion of trans-3-hexene to give propionic acid takes place in the ozone oxidation of trans-3-hexene.

  • When the fire takes place in Nuclear Powr Plants(NPPs), the reactor should achieve and maintain safe shut-down conditions and minimize the radioactive material released to the environment. The U.S. Nuclear Regulatory Commission (NRC) has issued numerous generic communications related to fire protection over the past 20 years, after it issued its requirements in the Fire Protection Rule set forth in Title 10, Section 50.48 of the Code of Federal Regulations (10 CFR 50.48) and Appendix R to the 10 CFR 50. The and Nuclear Energy Institute (NEI) has developed a Methodology for Risk Informed Fire Safe-Shutdown Analysis, which is related to the Deterministic Method for Multiple Spurious Operations solutions. The aim of this study was to identify, achieve, and maintain Post-Fire Safe-Shutdown of the Main Control Room (MCR) of the CANDU reactor, even though one train of the multiple Safety Structures, Systems, and Components (SCCs) fail by the technical specification and analysis method.

Selection of a Fire Detector for Wood Cultural Property (목조문화재 건축물 구조에 따른 화재감지기 종류 선정에 관한 연구)

  • A fire detector installed in wood cultural properties has not have selected the detector type appropriate for the features of cultural properties and the structure of wood fire after the fire in Sungnyemun-Gate since 2008. Applying wooden cultural properties different from the general architecture of the structure and fire characteristics is difficult. Therefore, buildings were classified into four shape types and field survey and wooden architecture structure characteristics to identify the problems of the detectors installed on wooden cultural property buildings. The problems appeared to lack the adaptability to external fire detection sensor selection and missing fire detectors installed in accordance with the place. To solve the problem, the closed and open space of the rooms used a smoke detector, outdoor select flame or fixed temperature linear detector to solve the problem.

  • Because the treatment of a physician generally pertains to the intrusion into body of a patient, his/her consent is a must in order for such conduct to be justifiable. To ensure effective consent of a patient, the physician should fully inform him/her of kind and details of the disease and way of treatment and risks associated with it. The patient can, then, make a decision whether he/she should accept any treatment or operation, if necessary, on the basis of such information. The obligation of physicians to explain has since long been recognized as important in view of guaranteeing the rights of patients for self-decision and protecting them from arbitrary assessment of physicians for treatment. Progress has been made in this respect even to the extent that physicians treat patients on equal terms and think first of all much of establishing trustworthy relationships with patients. Lots of studies in Korea and foreign countries have tried to explore the issues concerning the obligation of physicians to explain in the meantime but seem to have failed to make concrete and versatile approaches from the standpoint of protecting the rights of patients. Wouldn't it be really possible for patients to perceive their own rights and cope actively with the medical treatments? If physicians have full understanding to the rights of patients, they will be put in a better situation to protect themselves and patients, in turn, can identify their own responsibility correctly, which will eventually contribute to fulfilling the goal of treatment. With this background, the present paper examines briefly the obligations of physicians for explanation based mainly on the preceding theories and judicial precedents in the first place and then deals with the status quo and contents of the German medical laws, with a focus on the treaty of European Law 1997 and its working document on the applications of genetics for health purposes that stipulate the detailed criteria on the medical treatment and rights of patients and Germany's $\ulcorner$Charter of Rights for Patients$\lrcorner$ promulgated in 2003.

A Study on the Relation Between Self Care Agency and Mental Health in Vocational High School Students (일 실업계 고등학생의 자가간호역량과 정신건강과의 관계 연구)

  • This study was designed to identify the relation between Self-care Agency and mental health in vocational high school students of a rural area. The subjects for this study were 210 students living in Chonnam province: the breakdown of first, second and third grade students were 74, 78 and 58 respectively. The data were collected from April 1 to 4, 1997. The instruments used in this study were the Self-Care Agency Questionnaire developed by Deneys (1981) and the simplified SCL-90 developed by Lee, H. K. (1986) for measuring Mental Health. The data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation using the SAS $PC^+$ Program. The results were as follows: 1. The mean score of the Self-care Agency was 2.43. 2. The mean score of Mental Health was 2.11. 3. There were significant differences in sex (t=6.13, p= .014), religion (t=4.65, p= .032), the educational level of the father (F=3.62, p= .032), economic status (F=5.90, p= .003), and school performance (F=5.94, p= .003) with respect to selfeare agenecy. 4. There were significant differences in dwelling place (t=3.18, p= .025), economic status (F=4.55, p= .012), school performance (F=6.72, p= .002) and health problems (t= 5.41, p= .021) with respect to mental health. 5. The relationship between self care agency and mental health revealed a significant correlation (r=- .24, p= .005). The results showed that a low level of self care agency has an effect on mental health. We need an effective health education program to promote mental health.
