Subacute Osteomyelitis on Phalangeal Bone Resulting from Multiple Kirschner Pin Fixation: Case Report (원위지 골절에 K-강선을 이용한 비관혈적 정복술 후 생긴 아급성 골수염 1례)

  • Purpose: Percutaneous Kirschner wire fixation is common method for hand fracture. It is simple but has risk of ascending infection through the pin and bony injury by multiple drilling. Ascending infection through pin tract is mostly superficial and can be treated with antibiotics and aseptic dressing. This is a case review of subacute osteomyelitis on phalangeal bones after Kirschner wire fixation with literature review. Methods: A 40-years-old man with distal phalangeal fracture on right second finger is presented. He went to a local clinic and had percutaneous Kirschner wire fixation under local anesthesia. He was transferred to our hospital for ulcerative wound on DIP joint at 4 weeks after operation. Radiography showed osteolytic change around medulla of middle and distal phalanges, leading to diagnosis of a subacute osteomyelitis. We treated it with amputation at the level of shaft of middle phalanx. Results: The postoperative course was uneventful. We thought several possible reasons for osteomyelitis in our case. First, it could resulted from ascending infection through the wire. Second, it could be resulted from a bony burn by repeated drilling. And bony necrosis could be a consequence of arterial insufficiency caused by 2 pin insertion. Conclusion: We suggest that a precise pinning based on accurate anatomical understanding is required for a percutaneous Kirschner wire fixation. The frequency of drilling should be minimized. Careful observation and patient education for pin site care are essential.

Application of 1mm fictitious notch radius approach to the fatigue strength assessment of welded joint (1mm 가상 노치 반경을 이용한 용접부 피로강도 평가에 관한 연구)

  • Fictitious notch radius approach is based on the Neuter's microstructural support hypothesis which assumes that fatigue crack is governed by highly stressed volume of the material right on the weld toe area rather than the surface stress at a pin point of weld toe area. Variety of successes have been achieved in applying this methodology to the fatigue of welded joint, hence, it became one of recommended design procedure in IIW's recommendation as well as many ship classification societies. 1mm fictitious notch radius approach was applied to the various fatigue problems of welded joints in this study covering the effect of weld size, notch stress calculation for 3D geometry and low cycle fatigue problem. It was found that fictitious notch radius approach fumed out to be very effective and accurate in dealing with fatigue strength of welded joint.

  • In many structures a common method of construction is to make use of bolted or riveted joints. With this type of joint the load is transmitted through a pin from one section of the structure to another. Fatigue cracks often start from the edges of holes, due to local stress concentration. In order to predict the fatigue crack growth, the stress intensity factor K for hole-edge cracks should be available. In this paper the stress intensity factors are computed for cracks in bolt-joint region considering the contact condition.

  • The structure of the variable liquid column oscillator(VLCO) is analogous to that of the tuned liquid column damper used to suppress oscillatory motion in large structures like tall buildings and cargo ships. VLCO is using the technology which absorbs high potential energy made by process of accelerated motions effect of air spring by installation of inner air chamber. So, the application of VLCO can improve the efficiency of energy than that of wave energy converters made in Pelamis Company. In this research, experiments were performed for the models which have two different liquid column sizes. In order to find out the biggest motion response, two major conditions are taken into account. Two conditions are to open(or close) the valves and to differentiate the height of the liquid column.

  • A probabilistic model and analysis methods to determine the means and variances of the velocity and acceleration in stochastically-defined planar pin jointed kinematic chains are presented. The presented model considers the effect of tolerances on link length and radial clearance and uncertainty of pin location as a net effect on the link's effective length. The determination of the mean values and variances of the output variables requires the calculation of sensitivities of secondary variables with respect to the random variables. It is shown that this computation is straightforward and can be accomplished by a conventional kinematic analysis package with minor modification. Thus, the concepts of tolerance and clearance have been captured by the model and analysis. The only input data are the nominal linkage model and statistical information. The "effective link length" model is shown to be applicable to both analytical solution and Monte Carlo simulation. The results from both methods are compared. This paper Ksolves the higher-order kinematic problems for the probabilistic design analysis of stochastically-defined mechanisms.echanisms.

  • In this paper a new concept on z-pinning technology named by authors as 'z-pinning patch' will be introduced, which advantage is easy application at manufacturing site of composite structures. Using the trial manufactured z-pinning patch, the z-pinned composite single-lap shear joint specimens were successfully manufactured and tested to check the improvement of joint strengths. The z-pin's material is stainless steel and its surface was machined as zagged shape and chemically corroded to improve joint force with composite materials.

  • This study is to compare the weldability of Zircaloy-4 end cap of nuclear fuel pin using by GTA and Nd :YAG LB. The welding parameters which affect bead width and penetration depth have been investigated. The effect of joint geometry of end cap for GTAW and LBW has been studied and optimum conditions of Zircaloy-4 endcap welding have been found. Microstructures and microhardness of GTA and LB welded zones have been also compared.

  • The roof grid of single-layer space frame structure, for Energy Core of Incheon Airport Second Terminal, is very simple and aesthetic, but it is apt to buckle under external force because of mild curvature and complex shape. The object of this study is to estimate the stability of single-layer space frame structures for Energy Core of Incheon Airport Second Terminal with the analytical conditions of structural design. The results show that the buckling load of model(pin-pin, uniform load, rigid joint), that is, the most similar model to the analytical conditions of structural design. was $10.7kN/m^2$.

  • The purpose of this study is to compute the stress intensity factors of various conditions in the cracked p! ate. The stress intensity factor of pin-loaded cracked plate is investigated using the finite element method. This paper is divided into the two parts. The first part is the contact analysis, and the second is to compute the stress intensity factors. In the contact analysis, the iterative method is used, and convergence of this method is presented. In the computation of the stress intensity factors of plate, the length of crack, clearance, and angle are considered

  • The structural adhesives have the advantage of improving automobile performances and are being applied to joining light weight materials like aluminium and composite. In order to characterize the impact behavior of structural adhesive, instrumented impact tests were performed with respect to pin-ring adhesively bonded joint specimens. Also dynamic FE analysis was carried out using LS-DYNA to compare the results with experiments.
