Microbial detection on pill types of herbal medicine in South Korea (국내 유통 중인 환약의 미생물 검출수준)

  • Objectives : This study has aimed to monitor microbial detection on pill types of herbal medicine which are circulating in markets including Oriental Medical Clinics(O.M.C.s) and Pharmacy in Korea. Methods : 10 different samples of O.M.Cs and 10 different samples of Pharmacy were investigated by culture method and non-culture method based on the 9th edition of microbial examination released by Korea Food & Drug Administration. Results : The total microbial count among each 10 samples were detected within the limit suggested by WHO. 2 samples of O.M.C.s and 1 samples of Pharmacy exceeded WHO's limit in fungi count. No samples exceeded WHO's limit in bacteria count. Most bacteria founded in samples were the phylum of Firmicutes and Proteobacteria which are common in soil by non-culture method. Conclusion : Further study should be followed to set up proper microbial limit of herbal materials including pill types.

  • Objective : This article is based on a formula taken from Essential Prescriptions of the Golden Chamber(金匮要略). Shen Qi Pill(肾气丸) is generally used for such deficiency cases as: consumptive diseases, xiaoke(消渴) (waisting, thirst), beriberi, urinary problems during pregnancy, phlegm and so forth, and its main purpose is to treat Kidney Qi depletion, Qi transformation disorders, without obvious hot/cold appearance patterns. Method : Mainly through the literature study of related treatise on IInner Canon of the Yellow Emperor's internal classic(黃帝内经) and The Essential Prescriptions of the Golden Chamber. Result : According to the Yellow Emperor's internal classic, the Kidney has three main functions, storing essence, controlling the waters and holding Qi. Shen Qi Pill's(肾气丸) source formula is intended to supplement even though its constituent herbs aren't the warming type. To reinforce the Qi's transformation ability is considered to be a main function of the kidney organ, and in clinic, diagnosed kidney related problems and its treatment process are mainly associated with warming to help the Qi transforming. Conclusion : Thus recovering the kidneys Qi's transformation function is part of the process to eliminate diseases. Just cause the Kidney is deficient we shouldn't think immediately of using warming supplement herbs.

A Case Report on Central Post-stroke Pain in Medullary Infarction Treated with BackJun-pill (백중환이 유효했던 연수 경색 후 발생한 중추성 통증 환자 1례)

  • Objective: This study addressed a case of central post-stroke pain described as right arm painful with coldness and left lateral medullary infarction. Methods: A patient, 71 years old female with medullary infarction, was treated with a Korean herbal medication (BackJun-pill). The improvement of symptoms was evaluated using the Numeral Rating Scale and reports of coldness and pain in the affected areas. We also measured the body temperature difference between the left and right arms using digital infrared thermographic imaging (DITI). Results: After six weeks of treatment with Korean medicine, the patient's pain and sensation of coldness decreased. Conclusions: This clinical case study suggests that BackJun-pill may be effective for alleviating pain and coldness due to central post-stroke pain.

A Research on Cylindrical Pill Bottle Recognition with YOLOv8 and ORB

  • This paper introduces a method for generating model images that can identify specific cylindrical medicine containers in videos and investigates data collection techniques. Previous research had separated object detection from specific object recognition, making it challenging to apply automated image stitching. A significant issue was that the coordinate-based object detection method included extraneous information from outside the object area during the image stitching process. To overcome these challenges, this study applies the newly released YOLOv8 (You Only Look Once) segmentation technique to vertically rotating pill bottles video and employs the ORB (Oriented FAST and Rotated BRIEF) feature matching algorithm to automate model image generation. The research findings demonstrate that applying segmentation techniques improves recognition accuracy when identifying specific pill bottles. The model images created with the feature matching algorithm could accurately identify the specific pill bottles.

The Study on the Consumption and the Preference of Chungkukjang Products among Middle School Students in Seoul (서울 지역 일부 중학생들의 청국장 제품의 이용 실태 및 기호도 조사)

  • This study was performed to investigate the consumption and the preference of chungkukjang products among the middle school students. The subject were 538 middle school students(male 283, female 255) in $3^{th}$ grade, and were asked to fill out the questionnaire. The collected data were analyzed by SAS package. The result showed that most students knew about traditional chungkukjang(88.5%) and chungkukjang powder(68.2%), but only 43.5% and 29.4% of the respondents knew about raw chungkukjang and chungkukjang pill, respectively. When asked how often they eat chungkukjang products, traditional chungkukjang were taken often, but chungkukjang powder, raw chungkukjang and chungkukjang pill were shown to be taken relatively less. Over 60% of the students responded that they liked chungkukjang products, because of 'encourage by parents' and 'good for health', but there was no statistical difference by gender. When asked about the reasons for the dislike of the chungkukjang products, male students responded 'bad smell'of traditional chungkukjang, while female students 'eplied' not eating at home'. The students disliked the chungkukjang powder, chungkukjang pill and raw chungkukjang because of 'not knowing it.' These results showed that the social interest and support for the chungkukjang products were necessary and there was a need to advertise the chungkukjang products.

  • An experimental study was performed to investigate the characteristics of refrigerant flow rate control and superheat in an evaporator with an electronic expansion valve(EEV). The EEV used in this study was devised using a needle valve coupled with a stepping motor controlled by a personal computer. A Pill control equation was used to control the superheat of the evaporator and to set the superheat to $5^{\circ}C$. In order to determine an optimum running condition for the system, Pill parameters were varied for the wide range of values. The running condition of an air conditioning system with a PI control was reasonably stable compared with that of the Pill control. Experimental results for the PI control using parameter values, $K_p=1.5$, $T_i=400(sec)$ and $T_s=6(sec)$ show that the superheat reached its target value. When external disturbances were introduced to the system, the superheat target value was reached within about 3 minutes. When the EEV was applied to the air conditioning system driven by an inverter, room temperature control was excellent.

An Investigation of Medications Returned to the Community Pharmacies through "Drug-Take Back" Program ('폐의약품 수거사업'을 통해 지역약국으로 회수된 폐의약품의 분석)

  • Unused medication disposal is a problem due to the cost of disposing as well as potential risk of inadvertent dosing. Investigating medication returns is expected to suggest areas for targeting interventions to reduce medication waste. Therefore, the aim of this study was to examine types of medications and identify the expiration date of the medications returned to the community pharmacies through "Drug-Take Back" program. Method: From October 10, 2012 to November 14, 2012, the medications returned to the 58 community pharmacies in Korea were examined. Results: A total of 22,160 g of pill medications were collected; 52.8% for prescription drugs and 47.2% for non-prescription drugs, respectively. The weight of the expired pill medications was more than 5 times that of the non-expired pill medications. On the other hand, 6,168 ml of liquid medications were returned; 80.0% for prescription medication and 20.0% for non-prescription medications, respectively. Of the total oral liquid medications, the volume of the expired medications was more than 5 times that of the non-expired medications. Conclusion: The majority of medications returned to the community pharmacies were prescription drugs rather than non-prescription drugs. In addition, most of the drugs were expired when they returned.

Pill Identification Algorithm Based on Deep Learning Using Imprinted Text Feature (음각 정보를 이용한 딥러닝 기반의 알약 식별 알고리즘 연구)

  • In this paper, we propose a pill identification model using engraved text feature and image feature such as shape and color, and compare it with an identification model that does not use engraved text feature to verify the possibility of improving identification performance by improving recognition rate of the engraved text. The data consisted of 100 classes and used 10 images per class. The engraved text feature was acquired through Keras OCR based on deep learning and 1D CNN, and the image feature was acquired through 2D CNN. According to the identification results, the accuracy of the text recognition model was 90%. The accuracy of the comparative model and the proposed model was 91.9% and 97.6%. The accuracy, precision, recall, and F1-score of the proposed model were better than those of the comparative model in terms of statistical significance. As a result, we confirmed that the expansion of the range of feature improved the performance of the identification model.

Novel upper gastrointestinal bleeding sensor capsule: a first human feasibility and safety trial

  • Background/Aims: Upper gastrointestinal bleeding (UGIB) is the most common GI condition requiring hospitalization. The present study aimed to evaluate the safety and feasibility of using the PillSense system (EnteraSense Ltd.), a novel diagnostic tool designed for the rapid in vivo detection of UGIB, in human volunteers. Methods: In the present study, 10 volunteers swallowed a PillSense capsule, followed by 2 servings of an autologous blood preparation. Participants were monitored for capsule passage, overall tolerability of the procedure, and adverse events. Results: The procedure was completed per the protocol established in the present study in 9/10 cases. In 9 of the subjects, after capsule ingestion, the device indicated the absence of blood with sensor output values of 1. After the ingestion of the first blood mixture, the sensor outputs of all devices increased to a range from 2.8 to 4, indicating that each sensor capsule detected blood. The sensor output remained within that range after the ingestion of the second mixture; however, in one case, the baseline capsule signal was positive, because of a preexisting condition. The passage of the capsule was verified in all patients, and no adverse events were reported. Conclusions: The first trial of the PillSense system in human subjects demonstrated the feasibility, safety, and tolerability of utilizing this product as a novel, noninvasive, and easy-to-use triage tool for the diagnosis of patients suspected of having UGIB.

A Study of the Use of Oral Contraceptive Among Women Rescinding in the Yonhee Dong Blum Area (연희 영세지역 주민의 먹는 피임약 복용상태에 관한 조사연구)

  • This study is an attempt to evaluate the use of an oral contraceptive by: 1. Deforming the acceptance rate of the oral contraceptive according to socio-demographic characteristics 5. Estimating the duration of the use of the contraceptive f. Discovering reasons why the contraceptive was discontinued 4. Computing the cumulative continuation and discontinuation rate according to socio-demographic characteristics, history of pregnancies, and attitude of husband at the time of selection of the contraceptive. One hundred sixty-eight women of child bearing age (14-49 years), residing in Yonhee Dong, Seoul, Korea and registered as oral contraceptive accepters at Yonsei Community Health Center, were interviewed during a three week period in 1973. The questionnaire was designed to determine the socio-demographic characteristics of the oral contraceptive accepters, the current status of their family planning practice, medical reactions if any, discontinuation practice, and the duration of the use of the contraceptive. The major findings of the study were as follows; 1. The acceptance rate was highest for women between 30 and 34 years of age, the mean age being 34.4 years. 2. Fifty six point five percent of all respondents said that thirty years of age was the ideal age to stop having children. 3. The average number of living children was 3.5 and fifty four point eight percent of respondents felt 2 boys and I girl was the ideal number of children. 4. The average number of pregnancies was 5.2 and the average number of induced abortion was I. 4. 5. Fifty eight point eight percent of the respondents had experience in the use of contraceptive methods in the past. 6. Forty one point seven percent of the respondents replied that they chose the oral contraceptive because, "it was not complicated to use." 7. Sixty four point eight percent of the women said their husband approved of their taking the pill. 8. The cumulative discontinuation rate was sixty point five percent of the total respondents for one year in first segment. 9. Sixty six point nine percent of the respondents had experienced side-effect while using the pill. The side effects for 68.9 percent of these women were castro-intestinal upsets. Twenty point eight percent of the women who had side effects consulted with medical personnel about them. Women who had more education had more side effects. 10. Seventy three point two percent of the women who discontinued the pill did so because of medical reasons. Women who were younger discontinued the pill for personal reasons more-often than older women. Among personal reasons listed for discontinuing the pill was the attitude of the husband. 11. The average duration of continuance of the contraceptive was 5.3 cycles under 29 years of age; 7.4 cycles between 30 and 34 years of age; and 8.4 cycles over 40 years of age. 12. The discontinuation rate was seem to increase sharply in the early cycle and increase more slowly in later cycle. Conclusions and suggestions: Since the attitude of the husband was shown to be important, the current family planning program should be expended to include approach to husband. For women who must use the oral contraceptive, education and support must be enhanced so that the discontinuation rate due to side effects will be decreased.
