통증(痛症)의 한방치료(韓方治療)에 관(關)한 소고(小考) (A Study on the treatment of pain in oriental medicine)

  • There are many factors to evoke pain, for example cold, heat, seven emotion, digestion, fatigue, venemous worms, deficiency of blood etc., and its pathogenesis is complicated. Treatments for pain in oriental medicine are as follows. 1. Pharmacotherapy is a method to treat pain by different effects of herbal medicine, for example, regulating vital energy, activating blood circulation, eliminating phlegm, and dispelling the cold evil etc. 2. Acupuncture and moxibustion therapy is a kind of external application, it has a distinct effect in relieving pain. Recently its operation method has been more developed, so new acupuncture therapies are used in clinics. There are electroacupuncture, auricular acupuncture and injection of drugs at an acupuncture points etc. 3. Oriental physiotherapy is a treatment method to stimulate acupuncture points and meridians. (1) Hot pack, Paraffin bath, Microwave, Ultrasound, Short wave, Infra-red etc. are used to treat cold syndrome by dispersing cold by warming the channels. (2) E.S.T., I.C.T., T.E.N.S., S.S.P., cupping treatment etc. are used to treat disorders of vital energy. (3) Health ion, Carbon, T.D.P., etc are used to treat conic and deficiency syndrome.

The Effect of Group Exercise Program on Cognitive Function of Elderly people

  • The goal of this study was to assess the effect of a group exercise program on cognitive function of elderly people. Subjects were chosen to be elders with dementia having minor to moderate degrees of cognitive function. Study was started out by randomly dividing the 16 subjects into two groups, each with 8 people; the group exercise group and the control group. The group exercise group performed 8 weeks of group exercise program and general physiotherapy while the control group only performed general physiotherapy. Cognitive function was measured by Korean version mini-mental state examination. The study group's attention and calculation statistically significantly improved but the control group saw no statistically significant change. The group exercise program affected improvement in cognitive function of elderly people with dementia and in particular, was effective for enhancing their attention and calculation.

4주간 복합 운동치료가 유방암 림프부종 중년여성의 통증, 견관절 가동범위에 미치는 영향 (The Influence of 4 wks Complex Therapeutic Exercises on Visual Analog Scale of Pain and Range of Motion for Middle-Aged Women with Breast Cancer-Related Lymphedema)

  • PURPOSE: This study aimed to investigate the effect of complex theraputic exercise on visual analog scale of pain, shoulder' range of motion for middle-aged women with breast cancer related lymphedema. METHODS: The 14 middle-aged women involved voluntary in this study and then they were divided into two groups(n =7 per group). The complex exercise program was implemented over 4 weeks, 60 minutes per day, with 3 types of exercise for stimulation whereas the control group was performed a classical decongestive physiotherapy in a same day. For data analysis, the mean and standard deviation were estimated; 2 way repeated measures ANOVA was carried out. RESULTS: First, The level of VAS was significantly reduced on time, interaction effect in the group. Second, most factors of ROM were significantly increased on time, interaction effect whereas extension was not significantly increased. CONCLUSION: In conclusion, Our results showed that complex therapeutic exercise could improve or maintain VAS and ROM of shoulder joint for middle-aged women with breast cancer related lymphedema.

  • This article discusses the diagnosis, anatomic consideration, and surgical management of masseter muscle hypertrophy. Surgical correction is advised for patients who have esthetic complaints. Esthetic improvement can be achieved by surgery and not by conservative treatment. Recently, the intraoral method, which leaves no scar on the face and minimizes the possibility of injury to the marginal branch of the facial nerve, has been supported by many surgeons. Patients who complained of marked swelling of unilateral or bilateral mandibular angle area and showed abnormal bony growth at the mandibular angle area and enlarged masseter muscle received mandibular angle shaving and excision of the inner layer of masseter muscle with intraoral approach. After operation, physiotherapy was done with EAST(eletrical acupuncture stimulation therapy) for encouraging the mouth opening and reducing the swelling. They showed early maximum mouth opening and reduction of swelling.

아로마 마사지가 림프부종에 미치는 영향 (Effects of Aromatherapy Massage on Edema Reduction in the Treatment of Lymphedema)

  • The purpose of this study was to determine whether aromatherapy massages with manual lymph drainage (MLD) are significantly more effective than sham massages with MLD in reducing edematous limb volume in lymphedema. This study was performed on 46 patients who had developed unilateral upper or lower lymphedema. Twenty-three patients served as the experimental group and were treated with complex decongestive physiotherapy (CDP) applications including aroma massages with MLD, multi layered compression bandages, remedial exercises and skin care. Twenty-three patients in the control group were treated with CDP applications including sham massages with MLD, bandages, exercises and skin care. Patients undergo a therapy program once a day; 5 days a week for 2 weeks. Percentage excess volume (PEV) and body mass index (BMI) were recorded before and after treatment. PEV and BMI were significantly decreased before and after treatment in the experimental as well as in the control groups (p<.05). However, the percentage excess volume and BMI were not significantly improved after treatment between the two groups (p>.05). This study there suggests that aroma massages is not effective in the edema of patients with lymphedema.

슬관절 전치환술 후 수동 및 능동 보조 관절가동범위운동이 슬관절 가동범위 증가에 미치는 영향 (The Effect of PROM and AAROM Exercise After TKA on Increasing the Knee Range of Motion)

  • Background: The purpose of this study was to find out the real truth of the effect of PROM (Passive range of motion) and AAROM (Active assistive range of motion) exercise on increasing the knee flexion. This randomized, controlled trial examined whether or not the incorporation of PROME(passive range of motion exercise) to a postoperative rehabilitation protocol would offer a better clinical outcome after TKA (Total knee arthroplasty) Method: The subject (n=36) measured range of motion (ROM).18 consecutive patients who underwent TKAs at SNUH dept. OS received PROME for knee by a physical therapist during the physiotherapy sessions and not for the other 18 patients who underwent TKAs received No-PROME (AAROME) for knee by herself. Result: There were significant differences in the PROME group, No-PROME group ROM progress width And there were significant differences in the PROME group between No-PROME group ROM improvement width. Conclusion: This study demonstrates that the incorporation of PROME does offer additional clinical benefits to the patients after TKA. Our findings may suggest that encouraging patients to perform PROM exercises would be a better option and that physiotherapy session by a physical therapist holds good even now.

  • Purpose : The purpose of this study was to record, analyze and describe in detail, content of PNF intervention for recovery of function after stroke using SPIRIT intervention recording tools. Methods : A convenience sample of 21 physical therapist from a rehabilitation hospital recorded the intervention for 69 patients with stroke who treated during 30 minutes. The content of the treatment sessions was recorded using the Stroke Physiotherapy Intervention Recording Tool(SPIRIT). Descriptive statistics assessed for describe the intervention. Results : A convenience sample of 21 physical therapist from a rehabilitation recorded the interventions for 64 patients with stroke who treated during 30 minutes. The content of the treatment sessions was recorded using the Stroke Physiotherapy Intervention Recording Tool(SPIRIT). Descriptive statistics assessed for describe the interventions accurately and investigate the difference according to time since stroke. Conclusion : Facilitation techniques were the most frequently used intervention(n=1226,35.6%), practice(n=967, 28.0%) and exercise(n-670, 19.4%) in clinical practice of physical therapist. In contrast, Teaching careers and health care professionals to do or assist category is not frequently used intervention(n=5, 0.1%). Therefore, We need to be careful again with category of teaching careers and health care professionals.

Effects of Action Observation Training and Mirror Therapy on the Electroencephalograms of Stroke Patients

  • Purpose: This study examined the effectiveness of action observation training (AOT) and mirror therapy in improving the electroencephalograms (EEG) of stroke patients. Methods: Patients were allocated randomly to three groups: an action observation training with activity (AOTA) group (n=12), a mirror therapy with activity (MTA) group (n=11), and an AOT-only group (n=12). All groups received conventional physiotherapy in five 60-minute sessions over six weeks. The AOTA, MTA, and AOT groups practiced AOTA, MTA, and AOT, respectively, in three 30-minute sessions over six weeks. The differences between the pre- and post-treatment EEGs were assessed using a paired t-test. Comparisons between the groups were performed using one-way ANOVA. Results: The participants in the AOTA and MTA groups showed significant improvement in the EEG. AOTA improved the alpha waves of the prefrontal, temporal, and parietal lobes significantly (p<0.05). MTA improved the alpha waves of the temporal lobe significantly (p<0.05). AOT did not result in significant improvement Conclusion: AOTA and MTA improve stroke patients' EEGs. Mirror neuron activation combined with conventional stroke physiotherapy promotes motor recovery and functioning. The effect is enhanced when the actions are executed after observation. Further research into mirror neuron activation will be needed to develop methods to improve the EEGs of stroke patients.

자궁평활근종의 수술적 치료 후유증에 대한 보허탕가감방 한방 치험 5례 (Clinical Study of Korean Medicine with Boheo-tang-gagambang for Recovery after Gynecological Surgery for Myoma of the Uterus: 5 Cases Report)

  • Objectives: The purpose of this case study is to report clinical effectiveness of the Boheo-tang-gagambang with acupuncture, moxa and physiotherapy treatments on the after-effects of myomectomy or hysterectomy. Methods: The subjects of this study were five women who were diagnosed with uterine myoma and underwent myomectomy or hysterectomy. They mainly complained of lower abdominal pain, back pain, and pelvic pain. They received combination treatment of Korean medicine and physiotherapy. The Korean medicine included Boheo-tang-gagambang, acupuncture and moxa. The effects were evaluated through Numeric Rating Scale (NRS). Results: In this study, patient's clinical symptoms, including lower abdominal pain, low back pain, and pelvic pain, improved after hospitalization treatment. Conclusions: This case report shows that the combination treatment of Korean medicine with Boheo-tang-gagambang, acupuncture and moxa may be effective for treating after-effects of myomectomy or hysterectomy.

Effects of Progressive Scapular Stabilization Exercise on Neck, Muscle Strength, Upper Extremity Function in Patients with Acute Whiplash Injury

  • Objective: The purpose of this study was to investigate the effects of a combination of progressive scapular stabilization exercises, neck and thoracic spine mobilization, and thermoelectric physical therapy on pain, range of motion, muscle strength, and function in patients with acute whiplash injury. Design: A randomized controlled trial design. Methods: A total of 24 subjects were included in this study. They were randomly assigned to an experimental group (n=12) that performed scapular stabilization exercises, neck and thoracic spine mobilization, and physiotherapy, and a control group (n=12) that only performed neck and thoracic spine mobilization and physiotherapy. The pre-test was conducted before the intervention and consisted of a total of 12 treatment sessions of 60 minutes each, three times a week. A posttest was conducted 4 weeks later. Pain, range of motion, muscle strength, and function were assessed before and after intervention. Results: The results of the study showed that there was a significant difference in pain reduction and range of motion and muscle strength improvement in the experimental group, and a significant difference was also found between the experimental group and the control group in terms of functional evaluation. Conclusions: The combination of exercise therapy and joint mobilization technique and physical therapy resulted in greater improvements in pain, range of motion, strength, and function assessment, contributing to improved overall function.
