The Surgical Impact of E-Cigarettes: A Case Report and Review of the Current Literature

  • We report a case of a 51 years old female with a 25 pack year smoking history who underwent bilateral mastectomy and immediate tissue expander reconstruction for newly diagnosed right breast cancer. The patient reported herself as a non-smoker despite significant e-cigarette use, with resulting significant mastectomy skin flap necrosis and breast reconstruction failure. Little is known about the physiologic effect of e-cigarettes on wound healing and tissue perfusion. To this end, we provide an updated review of the impact of e-cigarettes on surgical outcomes. PubMed, Ovid MEDLINE, and PRS GO were searched for the terms "e-cigarette", "electronic cigarette", "e-cig", "electronic nicotine delivery system", "vaping", "surgery", "surgical", "peri-operative", "operate", "operative", and "wound healing". Abstract review of all articles was performed. 123 articles returned that contained both variants of e-cigarettes and surgery as keywords. Of those, manual assessment returned three articles which were found to be relevant to e-cigarette use in the surgical patient. No articles were found that compared perioperative complications in e-cigarette versus traditional cigarette users in humans. In conclusion, our case report depicts the potential dangers associated with e-cigarette use in the surgical patient. There is a public misconception that e-cigarettes are healthier than traditional cigarettes and as such their use may go unreported by patients. Early evidence suggests e-cigarettes may induce some of the same physiologic changes as traditional cigarettes, and may have a significant deleterious effect on wound healing.

연조직 증강을 위한 세포치료 시 비계로서 키토산의 온도 감응성 및 섬유모세포의 증식에 미치는 영향에 관한 연구 (STUDY ON THERMOSENSITIVITY OF CHITOSAN SCAFFOLD AND ON ITS EFFECTS ON FIBROBLAST PROLIFERATION IN CELL THERAPY FOR SOFT TISSUE AUGMENTATION)

  • Traditional surgical method or injection using filler is performed for soft tissue augmentation. Surgical methods have disadvantage of surgical morbidity. Commercially available injectable materials have the disadvantages such as resorption, short-term effect. repeated application and hypersensitivity. Significant shortcoming of cell therapy using autologous fibroblasts is delay of treatment effect. Chitosan/${\beta}$-glycerol phosphate (GP) solution has thermosensitive property and allows sol-gel transition at physiologic pH and temperature. These properties may resolve the delay of treatment effect. The purposes of this study are to evaluate the viscosity and pH changes of chitosan/${\beta}$-GP solutions and to evaluate the effect of chitosan/${\beta}$-GP solution on fibroblast proliferation and production of collagen. We measured the viscosity and pH as function of temperature, of the solution containing 1:0.7, 1:0.75, 1:0.8 chitosan (1, 10, 100, 700 kDa) /${\beta}$-GP. Fibroblasts from ears of 5 rats were cultured in chitosan/${\beta}$-GP solutions for 3 weeks. Cell proliferation and collagen contents were measured every week with WST (water-soluble tetrazolium salt) assay and Collagen assay respectively. The Results are 1) Chitosan(100 kDa<)/${\beta}$-GP solution (1:0.75) showed sol-gel transition at physiologic pH and body temperature and injectable properties. It will enable to resolve the delay in treatment effect 2) Cell proliferation and total collagen contents of the control group were increased with time. However, these decreased after the 1st week in experimental group 3) Collagen contents in the experimental group are higher than that of control group. 인도메타신이 개의 유치 치근 흡수에 미치는 영향 (Effects of Indomethacin on the physiologic root resorption of deciduous teeth in dogs)

  • 이 연구는 흡수중인 유치를 대상으로 인도메타신이 치근흡수에 미치는 영향을 조사하고. 치근흡수에 관련된 주위 조직의 변화를 관찰하기 위하여 시행되었다. 인도메타신은 파골세포의 수를 감소시키고 골흡수를 감소시키며, 골형성도 감소시키는 것으로 보고되어 왔으나 형태와 기능이 유사하다는 파치세포에 미치는 영향에 관한 연구는 희소하다. 생후 12-13주된 잡견 6마리를 통상적 복용량인 인도메타신 2 mg/kg/day를 14일간 투여한 군과 과량의 8mg/kg/day를 14외간 투여한 군과 대조군으로 구분하였으며 흡수중인 하악 절치를 연구대상으로 하였다 연구대상 치아는 $5{\mu}m$ 두께의 절편을 만들고, H&E 중염색, Masson의 trichrome 염색을 시행하고 광학현미경으로 검경하였으며, 파치세포의 수와 핵의 수를 비교하였다. 관찰 결과 유치 치근 흡수 조직은 골개조 소견과 함께 염증소견과 유사한 소견을 보였다. 흡수중인 유치의 치수는 치근흡수부위에 가까운 조상아세포층은 변성의 소견을 보이나 멀리 떨어진 치수는 종상인 소견을 보였으며 인도메타신이 투여된 실험군에서는 파치세포의 수적 감소와 핵의 수적 감소를 미약하게 나타냈다 그러나 인도메타신이 흡수중인 치아의 치수에 미치는 영향은 관찰되지 않았다. 이상의 결과에 의하면 인도메타신은 파치세포의 수적 감소를 미약하게 일으키며 장기간의 인도메타신 투여는 유치 치근흡수의 지연을 초래할 가능성이 있음을 시사한다.

체표 온도 변화에 대한 문헌 고찰 (A literature study on change of surperficial temperature)

  • Object: A study on the changes of superficial temperature as the ambient temperature varies. Method: After performing research on the physiologic mechanism of heat loss from skin, heat transfer to skin and heat productions of body, the conclusions would be drawn from experiments on temperature changes in every parts of the body as ambient temperature varies . Result and conclusion: Superficial temperature is in equilibrium with ambient temperature after a certain period. Part of the body with big change in superficial temperature tends to have slower temperature change and longer time for adaptation than the part with small change in superficial temperature. The temperature difference between left and right side of the body needs no attention. If adequate adaptation time, a short-period living supervision prior to measurements and appropriate indoor environment management are provided, meaningful conclusion would be attained for infrared thermal diagnosis.

수면의 생리 (Physiology of sleep)

  • Sleep is a vital, highly organized process regulated by complex systems of neuronal networks and neurotransmitters. Normal sleep comprises non-rapid eye movement (NREM) and REM periods that alternate through the night. Sleep usually begins in NREM and progresses through deeper NREM stages (2, 3, and 4 stages), but newborns enter REM sleep (active sleep) first before NREM (quiet sleep). A period of NREM and REM sleep cycle is approximately 90 minutes, but newborn have a shorter sleep cycle (50 minutes). As children mature, sleep changes as an adult pattern: shorter sleep duration, longer sleep cycles and less daytime sleep. REM sleep is approximately 50% of total sleep in newborn and dramatically decreases over the first 2 years into adulthood (20% to 25%). An initial predominant of slow wave sleep (stage 3 and 4) that peaks in early childhood, drops off abruptly after adolescence by 40% from preteen years, and then declines over the life span. The hypothalamus is recognized as a key area of brain involved in regulation of sleep and wakefulness. The basic function of sleep largely remains elusive, but it is clear that sleep plays an important role in the regulation of CNS and body physiologic processes. Understanding of the architecture of sleep and basic mechanisms that regulate sleep and wake cycle are essential to evaluate normal or abnormal development of sleep pattern changes with age. Reduction or disruption of sleep can have a significant impact on daytime functioning and development, including learning, growth, behavior, and emotional regulation.

슬관절의 운동학적 분석 (Arthrokinetic Analysis of Knee Joint)

  • Purpose : To describes the important aspects of knee joint movement and function used when applying PNF technique to the lower limb. Method : The knee was a very important roles in the lower limb movement and ambulation. This study summarizes the physiologic movement of knee to the PNF lower extremity patterns. Result : The tibiofemoral joint is usually described as a modified hinge joint with flexion-extension and axial rotation by two degrees of freedom movement. These arthrokinematics are a result of the geometry of the joints and the tension produced in the ligamentous structures. The patellofemoral joint is a sellar joint between the patella and the femur. Stability of the patellofemoral joint is dependent on the passive and dynamic restraints around the knee. In a normal knee the ligaments are inelastic and maintain a constant length as the knee flexes and extends, helping to control rolling, gliding and translation of the joint motions. Conclusions : It is important to remember that small alterations in joint alignment can result in significant alterations in patellofemoral joint stresses and that changes in the mechanics of the patellofemoral joint can also result in changes in the tibiofemoral compartments. Successful treatment requires the physical therapist to understand and apply these arthrokinematic concepts. When applied to PNF low extremity patterns, understanding of these mechanical concepts can maximize patient function while minimizing the risk for further symptoms or injury.

Therapeutic potential of eccentric exercises for age-related muscle atrophy

  • Recent studies have focused on evidence-based interventions to prevent mobility decline and enhance physical performance in older adults. Several modalities, in addition to traditional strengthening programs, have been designed to manage age-related functional decline more effectively. In this study, we reviewed the current relevant literatures to assess the therapeutic potential of eccentric exercises for age-related muscle atrophy (sarcopenia). Age-related changes in human skeletal muscle, and their relationship with physical performance, are discussed with reference to in vitro physiologic and human biomechanics studies. An overview of issues relevant to sarcopenia is provided in the context of the recent consensus on the diagnosis and management of the condition. A decline in mobility among the aging population is closely linked with changes in the muscle force-velocity relationship. Interventions based specifically on increasing velocity and eccentric strength can improve function more effectively compared with traditional strengthening programs. Eccentric strengthening programs are introduced as a specific method for improving both muscle force and velocity. To be more effective, exercise interventions for older adults should focus on enhancing the muscle force-velocity relationship. Exercises that can be performed easily, and that utilize eccentric strength (which is relatively spared during the aging process), are needed to improve both muscle force and velocity.

The Signaling of UV-induced Apoptosis in Melanocytes

  • Ultraviolet B (UVB) radiation may activate or deteriorate cultured human epidermal melanocytes, depending on the doses and culture conditions. In this study, we examined whether apoptosis of melanocytes can be induced by physiologic doses of UVB irradiation. PI staining for DNA condensation and flow cytometric analyses demonstrated the apoptotic cell death of melanocytes after UVB irradiation. The level of p53 and Bax revealed a dose-dependent increase with increasing dose of UVB, but the level of Bcl-2 remained unchanged. Confocal microscopic examination showed that Bax moved trom a diffuse to a punctate distribution after UVB irradiation. However, there were no changes in the pattern of Bcl-2. We next examined the downstream targets of apoptosis. Our results showed that a precursor form of caspase-3 disappeared with increasing doses of UVB. We also observed cleavage of poly(ADP-ribose) polymerase (PARP) after UVB irradiation. In addition, UVB irradiation resulted in a remarkable activation of c-Jun N-terminal kinase (JNK). These results indicate that UVB may induce apoptosis via JNK activation in human melanocytes.

  • Traumatic occlusion is a morbid condition that is caused by repeated occlusal stresses exerted on the periodontium that exceed the physiologic limits of tissue tolerance and contribute to the breakdown of the supporting tissue of the teeth. In reviewing these cases, the authors obtained the following results. 1. Roentgenographic examination revealed the typical signs of traumatic occlusion: severe resorption of roots, destruction of alveolar bone. 2. The major factor of traumatic occlusion is abnormal occlusal force. 3. The rsorptions of roots and periodontal changes which are caused by traumatic occlusion depend on the individual variation of the physical potential. 4. The pathologic features of traumatism are basically different from those of inflammation.

기능성 위장관 질환에서 핵의학 검사의 역할 (Scintigraphic Evaluation of Gastrointestinal Motility Disorders)

  • Current scintigraphic tests of gastrointestinal motor function provides relevant pathophysiologic information, but their clinical utility is controversial. Many scintigraphic methods are developed to investigate gastrointestinal motility from oral cavity to colon. These are esophageal transit scintigraphy, oropharyngeal transit study, gastric emptying test, small bowel transit time measurement, colon transit study and gastroesopahgeal reflux scintigraphy. Scintigraphy of gastrointestinal tract is the most physiologic and noninvasive method to evaluate gastrointestinal motility disorders. Stomach emptying test is regarded as a gold standard in motility study. Gastrointestinal transit scintigraphy also has a certain role in assessment of drug effect to GI motility and changes alter therapy of motility disorders. Scintigraphy provides noninvasive and quantitative assessment of physiological transit throughout the gastrointestinal tract, and it is extremely useful for diagnosing gastrointestinal motor dysfunction. This article reviews the current procedures, indications, significance and guidelines for gastrointestinal motility measurements by scintigraphy.
