간호사 및 간호조무사 활용에 관한 개업의사의 의견조사 (A survey of private physician's opinion on utilization of nursing manpower)

  • The medical insurance system has been adopted in 1989. Since then, the utilization of medical care services has increased rapidly. It appears that the medical institution needs more nursing manpower such as nurses and nurse aides to meet an increased demand for medical care for the residents. However, the medical care clinics run by a medical practitioners has a more shortage of nursing manpower than hospitals. The purpose of this survey was to analyze the current employment status and to obtain an opinion on the improvement of utilization such as recruitment and retention of nursing manpower. The questionnaire sent to the private physician and the response rate was 28.7% ; 87 out of 300 physicians. 82.6% of the respondents employed nurses aides rather than nurses and an average number of employees per clinic was 2.5 persons. Most physicians had difficulties in recruiting nursing manpower. It took more than one month for replacement on average, therefore, they were suffering from giving good continual care for the patient and also high resignation of nursing personnel. The low wage, long working hours, hard work and lack of incentives are the biggest reasons for the difficulty in recruiting and retaining of the nursing personnel obtained for the clinics survey. The above mentioned problems would be solved in the near future by changing those difficulties. In the meantime, the private physicians are marking an effort to retain the nursing personnel by keeping a relationship of the private physician's cooperatives.

의사-환자 관계에서 '환자가 의사를 신뢰한다'의 의미 고찰 (An Exploratory Study on the Meaning of 'the Patient Trusts the Doctor')

  • 환자가 의사를 신뢰한다는 의미를 살펴보는 것은 의료서비스를 바탕으로 이루어지는 환자-의사 관계관리 뿐만 아니라 진료만족도, 병원재방문 등 마케팅 측면에서도 중요하다. 이 연구에서는 의사-환자 관계에서 신뢰의 의미가 무엇인지를 병원방문경험이 6개월 이내인 20대 이상 성인들을 대상으로 설문조사한 328명의 응답결과를 분석자료로 활용하여 살펴보았다. 연구결과, 환자가 의사를 신뢰한다는 의미는 한마디로 정의하기는 힘들지만 적어도 네 가지 의미를 담고 있음을 알 수 있었다. 환자가 신뢰하는 의사는 첫째, 환자의 이야기를 주의깊게 경청하며 관심을 표현하는 대화 분위기를 형성하는 커뮤니케이션 행위를 보여야 한다. 둘째, 환자의 건강을 먼저 생각하고 환자를 이해하려고 하며 공감할 수 있도록 해야 한다. 셋째, 기술적인 능력뿐만 아니라, 치료와 관련한 검사와 절차에 있어서 솔직하여야 한다. 마지막으로 환자와 협력을 자아내고자 노력해야 한다.

의사들의 의료정보 시스템 수용도에 영향을 미치는 요인에 관한 연구 (A Study on factors affecting physician's acceptance of Electronic Health Record(EHR) System)

  • 성공적 EHR 구현을 위해서는 의사들의 EHR에 대한 수용도(acceptance)와 태도(attitude)를 이해하는 것이 중요하다. 본 연구에서는 사우디 아라비아에 수출된 한국 EHR에 대한 사우디아라비아 의사들의 수용도(acceptance)를 조사하여 한국 EHR의 해외 확산을 위한 기초자료를 제공하고자 하였다. 의사가 EHR을 수용하는 주요 요인인 의사의 참여도, 적절한 훈련, 의사의 자율성, 의사와 환자와의 관계, 사용편리성, 유용성, 태도 중 자율성과 의사와 환자와의 관계 요인을 제외하고는 모두 Likert 척도 3.5점으로 높게 조사되었다. 의사들의 한국 EHR을 실제 사용한 경험이 EHR 수용태도에 영향을 미쳤을 것이며, 긍정적인 연구결과로 비추어 볼 때 한국산 EHR이 해외 시장에서도 충분히 경쟁력이 있다고 판단할 수 있다.

부산시 개원 의사와 개국 약사의 의약분업 정책목적 달성도와 손익에 대한 인식도 (Physicians and Pharmacists' Perceptions about the Goal Achievement of the Separation Policy of Drug Prescribing and Dispensing, and Benefit and Loss Caused by the Policy in Busan)

  • The purpose of this study was to identify health care providers' perceptions about the goal achievement and benefit/loss caused by the separation policy of drug prescribing and dispensing after the policy implemented on July 1, 2001. Uslng stratified sampling method based on the administration area, Ku, 315 physicians and pharmacists were sampled from the rosters of physician and pharmacist association in the city of Busan on 2001. There were 122 and 115 responses from physician and pharmacist sample, respectively. 78.3% of physicians and 50.4% of pharmacists evaluated that the goal of the policy was not achieved. Moreover, 75.3% of physicians and 40.7% of pharmacists did not support the policy. Most physicians and pharmacists considered preventing the citizens with drug abuse and misuses as the most important benefit derived from the policy. However, physicians and pharmacists concerned over raising health care cost that could be patients' burden. The most important physicians' benefit derived from the policy was free choice of all possible medicine that might result in effectiveness of medication. In physicians' the most important loss, most physicians worried about that breaking traditional patient and physician relationship might cause physicians' authority in treating diseases to be damaged. Pharmacists considered the most important policy benefit as hiked social status resulted from enforcement of profession due to the policy whereas they considered the most significant loss as expected financial problems of small pharmacies compared to that of large pharmacies or pharmacies adjacent to hospitals. In the current problems of the policy, physician and pharmacists blamed the government for inadequate preparations of the policy implementation. Physicians and pharmacists also considered citizens' mature attitudes toward the policy as a crucial success factor.

장기재원환자의 특성 및 전원 인지도와 전원 의향과의 관계 - 장기재원환자의 효율적 전원을 위한 전략 제시 - (Relationship between Characteristics of Lengthy Hospital Stay Patients, Knowledge of Transfer Needs and Their Willingness to Transfer - Strategies for the Effective Transfer of Lengthy Hospital Stay Patients -)

  • Background : It is very common in Korea to take care of non-acute patients in an acute setting, due to the lack of long-term facilities. Long term hospitalization increase medical expenses and decreases the bed utilization, which can affect the urgent and emergent admissions, and eventually jeopardize the hospital financially. In this study, strategies for effective transfers to the lower levels of care, and to decrease the length of stay were presented by surveying and analyzing the patient's knowledge of the transfer needs, and the willingness to transfer those whose hospital length of stay was more than 30days. Method : The survey is subject to a group of 251 patients who have been hospitalized over 30 days in a general hospital in Seoul. Excluding those that were in the Intensive Care Unit and psychiatric ward, 214 in-patients were used as participants. They were surveyed from April 9, 2002 to April 17, 2002. One hundred and thirty seven out of 214 were responded which made the response rate 64%. Data were analyzed by SAS and SPSS. Result : Multi-variable Logistic Regression Analysis showed a significant effect in medical expenses, knowledge of referral system and the information of the receiving hospital. The financial burden in medical expenses made the patient 10.7 times more willing to be transferred, knowledge of the referral system made them 5 times more willing to be transferred, and the information of receiving hospital makes 6.5 times more willing to be transferred. Reasons for willing to be transferred to a lower level of care were the phase of physical therapy, the distance from home, the attending physician's advice and being unable to be treated as an out patient. Reasons for refusing to be transferred were the following. The attending physician's competency, not being ready to be discharged, not trusting the receiving hospital's competency due to the lack of information, or never hearing about the referring system by the attending physician. Conclusion : Based on this, strategies for the effective transfer to the lower levels of care were suggested. It is desirable for the attending physician to be actively involved by making an effort to explain the transfer need, and referring to the Healthcare Coordinating Center, which can help the patient make the right decision. Nationwide networking for the referral system is the another key factor that may need to be suggested as an alternative to decrease the medical expenses. Collaborating with the Home Health Agency for the early discharge planning and the Social Service Department for financial aid are also needed. It is recommended that the hospital should expedite the transfer process by prioritizing the cost and the information as medical expenses, knowledge of referring system and the information of the receiving hospital, are the most important factors to the willingness to transfer to a lower level of care.

표준화 화병환자를 활용한 한의대생의 진료 및 의사소통 수준연구 (A Study about the Medical Communication Proficiency of Korean Traditional Medical Students Using Standardized Patients with Hwa-Byoung)

  • Objectives : After analyzing the proficiency of medical communication of the students in College of Korean Traditional Medicine using standardized patients, we suggests ways to improve clinical practice in the future class and medical communication curriculum development. Methods : 20 students before clinical practice class (3rd grade) and 20 students after 1 year clinical practice class (4th grade) participated and did their medical interview on Standardized patient. They were evaluated on patient-physician communication skills by standardized patients and professor evaluator. In addition to be evaluated on patient-physician relationship, medical interview skills by professor evaluator. Results : As follows in the evaluation of clinical practice with standardized patients 1. More than half of the participated students regardless of their grade received poor score in their medical communication evaluated by SP(Standardized patient) and PE(Professor evaluator). 2. Greeting, History taking parts were higher in the 4th students who received 1 year clinical practice class, but verbal-nonverbal response, voice tone parts were higher in the 3rd students who do not received clinical practice lesson. 3. Pronunciation&Voice tone parts were higher in the male students but, gathering information part was higher in the female students. Conclusions : We think that the current clinical practice lessons are insufficient as a way to learn and improve medical knowledge and medical communication skills, and it is necessary a new form of clinical practice class. Participatory lesson using standardized patient could be a good alternative of that in the future class.

정신질환자의 타해(他害)사고와 의료과오책임 (Psychotherapist's Liability for Failure to Protect Third Person)

  • Psychiatrists who treat violent or potentially violent patients may be sue for failure to control aggressive outpatients and for the discharge of violent inpatients. Psychiatrists may be sued for failing to protect society from the violent acts of their patients if it was reasonable for the psychiatrists to have known or should have known about the patient's violent tendencies and if the psychiatrists could have done something that could have safeguarded in public. The courts of a number of jurisdictions have imposed a duty to protect the potential victims of a third party on persons or institutions with a special relationship to that party. In the landmark case of Tarasoff v Regents of University of California, the California Supreme Court held that the special relationship between a psychotherapist and a patient imposes on the therapist a duty to act reasonably to protect the foreseeable victims of the patient. Under Tarasoff, when a therapist has determined, or under applicable professional standards should determine, that a patient poses a serious threat of violence to another, he incurs an obligation to use reasonable care to protect the intended victim against such danger. In addition to a Tarasoff type of action based on a duty to warn or protect foreseeable victims of psychiatric outpatients, courts have also imposed liability on mental health care providers based on their custody of patients known to have violent propensities. The legal duty in such a case has been stated to be that where the course of treatment of a mental patient involves an exercise of "control" over him by a physician who knows or should know that the patient is likely to cause bodily harm to others, an independent duty arises from that relationship and falls on the physician to exercise that control with such reasonable care as to prevent harm to others at the hands of the patient. After going through a period of transition, from McIntosh, Thompson and Brady case, finally, the narrow rule of requiring a specific or foreseeable threat of violence against a specific or identifiable victim is the standard threshold or trigger element in the majority of states. Judgements on these kinds of cases are not enough yet in Korea, so that it may be too early to try find principles in these cases, however it is hardly wrong to read the same reasons of Tarasoff in the judgements of Korea district courts. To specific, whether a psychiatric institute was liable for violent behavior toward others depends upon the patients conditions, circumstances and the extent of the danger the patients poses to others; in short, the foreseeability of a specific or identifiable victim. In this context if a patient exhibit strong violent behavior toward others, constant observation should be required. Negligence has been found not exist, however, when a patient abruptly and unexpectedly attack others or unidentifiable victim. And the standard of conduct that is required to meet the obligation of "due care" is based on what the "reasonable practitioner" would do in like circumstances. The standard is not one of excellence or superior practice; it only requires that the physician exercise that degree of skill and care that would be expected of the average qualified practitioner practicing under like circumstances. All these principles have been established in cases of the U.S.A and Japan. 환자-의사 커뮤니케이션 개선을 위한 의사코칭 모델 개발 (Development of Physician Coaching Model for Improvement of Patient-Doctor Communication)

  • 최근 의과대학과 의료서비스 산업에서 환자-의사 커뮤니케이션에 관한 논의가 활발히 이루어지고 있다. 환자-의사 커뮤니케이션이란 '진료면담'에서 환자와 의사간의 쌍방향 의사소통으로써, 환자와 의사 양자에게 모두 긍정적인 효과를 가져다주는 것으로 알려져 있다. 이 연구에서는 의사의 커뮤니케이션 스킬을 향상시켜 환자와 의사간의 의사소통을 원활히 할 수 있는 방안으로 코칭기법을 접목한 의사코칭 모델을 개발하였다. 모델 개발 방법은 문헌검토를 통해 초안을 개발하였고, 전문가 자문과 의사평가를 통해 수정 보완하였다. 연구 결과, 의사코칭 모델을 총 5단계로 구성하였다. 첫째, '관계 형성'으로 의사가 코칭에 대한 기대나 관심을 가지도록 한다. 둘째, '변화 주제 인식'으로 환자와의 커뮤니케이션에서 의사의 문제점과 장점을 파악하여 코칭의 방향을 설정한다. 셋째, '관점의 획득'으로 의사가 환자의 관점에서 생각할 수 있도록 유도한다. 넷째, '문제 해결 및 의사소통 역량 강화'로서 의사의 커뮤니케이션 스킬 개선사항을 세부항목으로 제시한다. 다섯째, '목표 설정 및 지원'으로 개선 사항 목표를 설정하고 장점을 유지 강화할 수 있는 실천 방안을 합의한다. 개발된 의사코칭 모델은 환자-의사 커뮤니케이션 개선을 위해 의사들을 대상으로 코칭기법을 처음으로 적용했다는데 가장 큰 의의가 있다. 향후 의료서비스 현장에서 활용될 경우, 의사들의 커뮤니케이션 스킬 향상과 환자 공감적 능력 향상에 효과가 있을 것으로 판단된다. 이를 통해 환자들의 진료만족도를 높이는데 기여할 것이다.

A little-known relationship between immune recovery syndrome and herpes zoster

  • Following anti-retroviral therapy (ART) or highly active antiretroviral therapy, there is an increased response to latent infections such as herpes zoster, which may lead to their reactivation. This is a result of improved immunity brought about by ART, also termed immune recovery syndrome. A 75-yearold male patient arrived at our institute with widespread vesicles and scabs on the right half of his face and oral cavity, suggesting the involvement of the trigeminal nerve. The patient had a history of being on ART two months earlier and a history of tooth extraction eight days prior to his arrival at our institute. The incidence of human immunodeficiency virus (HIV)-positive cases amongst herpes zoster cases is high, and these patients become susceptible to infections following ART. Therefore, regardless of the presence of risk factors, every herpes zoster patient should be tested for HIV infection, and high anti-retroviral therapy should be commenced/reinstituted as soon as possible. In addition, the treating physician should maintain a high level of vigilance for the patient during the first few months of ART, the peak incidence of immune recovery inflammatory disease.

퇴원후 뇌졸중환자의 재적응과 사회적 지지와의 관계분석 (Readjustment and Social Support of the Post Hospitalized Stroke Patients)

  • An explanatory design was employed to identify the relationship of physical, emotional & social readjustment and social support of post hospitalized stroke patients and their caregivers. A convenient sample of 254 patients who given follow-up care at the outpatient department after discharge and 225 caregivers were recruited. Mental Status Questionnaire (MSQ), Social Support Inventory Stroke Survivors (SSISS), Illness intrusiveness(II), Instument Activity of Daily Living(IADL), Center of Epidemilogic Studies-Depression(CES-D), social activity and caregiver burden were used for measurement in this study. Results showed patient's physical level measured by IADL and psychological level measured by depression were high. But social activity was low. Cognitive function, depression & social activity were not significantly different by the posthospitalized period, but IADL was. The source of professional support was mostly the physician at the outpatient department. The family support was found significantly related to patient's depression & social activity and caregiver's subjective burden. Professional support was found significantly related to patient's IADL & depression. Illness intrusiveness as a mediating variable was a sig nificantly predicting power on patient's IADL & depression. The path analysis was used to identify the variables to predict the physical, emotional, and social status of patients. As a result, patient's age, cognitive function, illness intursiveness and professional support significantly predicted the level of IADL ; patient's cognitive function, illness intrusiveness and family support significantly predicted the level of depression ; and patient's age and family support significantly predicted the level of social activity of posthospitalized stroke patients. Based upon these results, the rehabilitation programs to reduce the illness intrusiveness and improve cognitive funtion were recommended for the readjustment of the stroke patients. This model of the readjustment of the posthospitalized stroke patients is recommended as the framework for care of the stroke patients.
