Purpose: This study was designed to evaluate the clinical utility of the Four square step test (FSST) for predicting falls in stroke patients, to compare the ability of the FSST test to discriminate between subgroups of fallers, and to determine if the test has any predictive value in identifying stroke patients who will fall. Methods: Stroke patients (N=37) who could walk at least 50 m with minimal assistance were recruited consecutively when attending a physical therapy session during their rehabilitation. Dynamic standing balance was measured using the FSST. The main outcome measures were FSST time and fall number. Numbers of falls were compared with FSST scores. Differences between the groups in FSST scores were examined using a t-test and 1-way analysis of variance. Post hoc analysis using the Tukey B procedure was used to identify specific group differences. Alpha was set at 0.05. Results: A total of 15 participants (40.5%) reported falls 6 had recurrent falls (2 falls) and 9 fell once. The mean FSST time differed significantly between groups with zero and multiple falls. A cutoff score of greater than 17 seconds on the FSST was associated with a sensitivity of 83% and a specificity of 84% for the identification of subjects with multiple risk factors for falls. Conclusion: The FSST is a feasible and valid clinical test of dynamic standing balance and can predict falls in post-stroke patients.
The objective of this study was to investigate the physical work capacity and rest allowance for Korean males and females. Eleven males and 13 females participated in the study as subjects. An ergometer exercise test and a treadmill exercise test were conducted to measure oxygen consumption rates. The PWC values of Korean males and females were 2605.62ml/min and 1853.01ml/min in the cycle ergometer test, respectively, and 2872.10ml/min and 2057.91ml/min in the treadmill test. The PWC values of Korean females were about 70% of Korean males. The PWC values of Korean females were74.36%-78.79% of western males in the treadmill test. and 88.66%-89.32% of western males in the ergometer test. The PWC values of Korean females were 81.34%-82.98% of western females in the treadmill test, and 87.11% of western femaleles in the ergometer test. And, the results of this study showed that the rest allowance should be determined separately for male and female because of the difference in physical work capacity.
Background: The hyoid bone is the only non-jointed structure among the skeletal tissues of the head and neck region, and its movement and posture depend on the attached muscle, ligament, and fascia. The location of the hyoid bone is important for airway maintenance, vocalization, chewing, swallowing, breathing, and head and scapular position. In general, the location of the hyoid bone is measured using radiographs and 3D computed tomography, and no studies have reported on clinical measurement methods. Objects: This study was performed to suggest clinical measurement methods for lateral deviation of the hyoid bone and to evaluate their reliability. Methods: In this study, 24 healthy volunteers (12 males, 12 females) in Cheongju-si participated. Two examiners performed the center point test and lateral motion test twice each to measure the lateral displacement of the hyoid bone. The reliability of the center point test was analyzed using intra-class correlation coefficients (ICC), and the reliability of the lateral motion test was analyzed using Cohen's kappa coefficient. Results: The intra-rater reliability of the center point test was good, and the inter-rater reliability was moderate. The intra- and inter-rater reliability of the lateral motion test showed substantial reliability. Conclusion: Based on these results, the center point test and the lateral motion test can be used as an alternative methods of the measurement of lateral deviation of the hyoid bone for people who have musculoskeletal disorders of the head, neck, and scapula.
In this study, a horticultural therapy program was performed by employing the anger experienced by people during daily life activities in the emotion regulation strategy-based program to identify the influence of the horticultural program on anger control in the caregivers for patients with dementia. To measure this influence, two tools were used: a self-administered questionnaire survey, which is a subjective test, and an instrumental test, which can measure the physical index that detects the physical changes through anger. For the preliminary test, depression, stress, self-esteem, anxiety, and anger state-characteristic were tested using the self-administered questionnaire. For the follow-up test, the self-administered survey and the test using the physical index were performed in a manner similar to that of the preliminary test. The self-administered questionnaire comprised questions suitable to this study and considered the age, education level, and economic aspect of the subjects. The test results indicated that the experimental group subjected to the emotion regulation strategy-based horticultural therapy showed very significant reduction in depression and anger and significant reduction in stress, self-esteem, and anxiety. In the physical index test, the systolic and diastolic blood pressures were reduced, and the pulse rate and respiration rate reduced before and after the therapy, thus, proving the effectiveness of the therapy in anger control.
The purpose of this study was to call attention to the mental, physical and occupational hazards of the anticancer-drug-handling nurses by examining the possible urinary mutagenicity and measuring physical symptoms and stress level of the nurses exposed to anticancer drugs. The experimental group of the urinary mutagenicity assay was 14 nurses handling anticancer drugs at the medical wards of a hospital located in J city ; the control group was 12 psychiatric nurses of the same hospital. The test material was the nurses' 24hrs urine, which was concentrated by XAD-2 column chromatography. Tester strains were TA98(±S9 mix), TA100(±S9 mix), TA1535(±S9 mix) and TA1537(±S9 mix) ; Salmonella mammalian-microsomal test(Ames test) was employed for the urinary mutagenicity assay. The physical symptoms of which the nurses experienced were investigated through self-reports on open-questionnaires. The stress levels of the experimental group were measured by a stress measuring instrument developed by this author. Reliability of this instrument was found to be adequate (Cronbach's Alpha=0.9079). To ascertain the urinary mutagenicity of the experimental group, the mean and the standard deviation of the colonies of Tester strains appearing on the minimal plates were taken and compared differences between two groups. T-test was employed for the significance test of two groups. The physical symptoms were compared between the two groups through the analysis of the nurse' self-reports. The mean and standard deviation of the stress levels of the experimental group were also calculated and were examined through t-test. The results were summarized as follows : 1. The experimental group revealed significantly higher urinary mutagenicity both in the activation method test and the non-activation method test of the tester strains TA98, TA100 and TA1535. In the case of TA1537, two groups showed no difference in the non-activation method test, but the activation method revealed difference. 2. The physical symptoms were also much more frequently reported in the experimental group. 79.3% of the experimental group reported more than 1 kind of physical symptoms. On the other hand, 33.2% of the control group complained of 1 kind of physical symptom. The items with high symptom frequency were 'headache', 'itching sensation', 'corneal congestion', 'skin allergy' 3. The mean score of stress in the experimental group was 2.41(range 1-4). The experimental group showed the stress level above 2.0 in the 14 of 15 items in all. The highest stress level were recorded in the following items in the order quoted, 'I fear that anticancer drug may touch any part of body while handling it.', 'I feel concerned there is no protective countermeasure against anticancer drug handling.', 'I am afraid the anticancer drug handling may produce a fetal loss in the future'.
The purpose of this study was to investigate the effects of 12-week training on changes in physical fitness and cardiovascular factors for firefighters. For this purpose, 40 men in their 20s and 30s who agreed to participate voluntarily were recruited. They were divided into four groups: the firefighters' physical fitness test training group (hereinafter referred to as PT group), firefighters' physical fitness test and aerobic training group (hereinafter referred to as PT+AR group), firefighters' physical fitness test and both aerobic and anaerobic training group (hereinafter referred to as PT+CO group). Physical fitness factors (grip strength, back muscle strength, seated forward bend, standing long jump, sit-ups, 20-meter shuttle run), cardiovascular factors (total cholesterol, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, glucose, waist circumference, systolic blood pressure, diastolic blood pressure) and the relationship between Framingham Heart Risk Score and physical/cardiovascular factors were compared and analyzed, and the following conclusions were obtained. Aerobic training, anaerobic training, and combined training, including 12 weeks of firefighter physical examinations, all had positive effects on fitness and cardiovascular factors, which would be an appropriate way for firefighter examinees to improve physical strength and reduce the risk of cardiovascular disease.
The purpose of this paper is to provide an overview of the clinical physical therapy program used at the University of Yonsei Rehabilitation Hospital, for the practicing university trained physical therapists who may be unfamiliar with patients who have suffered a myocardial infarction. The four primary phases of the cardiac physical therapy graded exercise program are: 1) coronary care unit program (phase I), 2) general ward program (phase II), 3) convalescence program (phase III), 4) maintenance program (phase IV). The exercise prescription defines the exercise intensity, duration, frequency, and mode of exercise a after pre- discharge low level graded exercise test(LL-GXT) or symptom limited maximum graded exercise test. A typical exercise routine consists of preparation warm-up exercise, therapeutic exercise, cool-down exercise. Physical therapy is involved in the acute care and rehabilitation of the patient after a myocardial infarction. Therefore, the physical therapist must throughly comprehened the cardiac anatomy, cycle, performance, conduction system, pathogenesis, risk factors, and exercise benefits.
This study examined the effects of physical activities using Korean dance upon the motor abilities of young children. The subjects of this study were divided into 2 groups. The first group was an experimental group (boy=12, girl=13) who participated in the Korean dance class and the other group was a comparative group (boy=11, girl=14). Experiments were performed during 18 sessions for 9 weeks. The study findings are as follows. First, physical activities using Korean dance had positive effects upon the development of basic motor abilities in young children. In terms of these basic motor abilities, movement motor ability and whole muscle operation motor ability showed significant differences between the pre-test and post-test stage in the experiment group and between the experimental and comparative groups in the post-test stage. On the other hand, in terms of non-movement motor abilities, there was a significant difference between the pre- and post-test stages for the experimental group, but no significant difference in the post-test stage for the experimental or comparative groups. Second, physical activities using Korean dance had positive effects upon the development of perceptive motor abilities in young children. In other words, in the sub-variables of perceptive motor abilities, there were significant differences between the pre- and post-test stages of the experimental group which experienced physical activities using Korean dance. In conclusion, the study findings showed that physical activities using Korean dance had positive effects upon the increase of both basic motor abilities and perceptive motor abilities in young children. Additionally, the experimental group exhibited more positive effects in the various areas of motor abilities than the comparative group.
Purpose: This study aimed to investigate the immediate effects of posture correction and real-time visual feedback using a video display on muscle activity and change of head position during overhead arm lift test in individuals with forward head posture. Methods: Fifteen subjects with forward head posture and fifteen normal subjects who volunteered were included in this study. During both groups performed the overhead arm lift test, the muscle activity of the upper trapezius, serratus anterior, sternocleidomastoid, and lower trapezius muscle were measured using electromyography, and head position change was measured using photographs. Then, forward head posture group was asked to perform overhead arm lift test again after posture correction and real-time visual feedback using a video display respectively. One-way analysis of variance (ANOVA) was used to analyze four conditions: pre-test, posture correction, real-time visual feedback, and the control group. Results: The upper trapezius and lower trapezius muscle activity significantly decreased posture correction, real-time visual feedback, and control group than pre-test of forward head posture group (p<0.05). The sternocleidomastoid muscle significantly decreased real-time visual feedback and control group than pre-test of forward head posture group. Head position change significantly decreased three conditions than pre-test of forward head posture group and real-time visual feedback and control group significantly decreased than posture correction. Conclusion: This study recommend for maintaining cervical stability during the overhead arm lift test, postural control using real-time visual feedback is more effective in subjects with forward head posture.
Purpose: The purpose of this study was to compare the effects of Tai-Chi exercise (TCE) and resistance exercise (RE) when used as part of a community-based exercise program on improvement of physical function in elderly women with knee arthritis. Methods: Forty-seven women with knee arthritis participated in this study. They were assigned to one of two groups: the TCE group (n=22) or the RE group (n=25). Tai-Chi exercise and resistance exercise sessions were held for 1 hour per session, twice per week, for 8 consecutive weeks. At pre-treatment and post-treatment, subjects were tested using the following measurements: one-legged stand test (sec), a functional reach test (cm), a test of the strength of the knee extensor and flexor muscles, determination of the pathway of center of foot pressure and vertical ground reaction force for stance phase at pre and post treatment time points. An independent t-test and a ${\chi}^2$ were used to determine the significance of differences between group means using SPSS 12.0. Results: After 8 weeks of participation in the exercise programs, there were significant improvements for both groups in joint pain, difficulty of performing activity, muscle strength of knee extensor and flexor. Also, vertical ground reaction force increased at the loading response phase for both groups. The RE group was significantly different from TC group on the eyes-closed one-legged stand test (sec). Conclusion: Tai-Chi exercise and resistance exercise programs improve physical functioning and reduce pain and locomotion difficulties.
