For the evaluation of physical assessment by school nurses, this study was designed in two steps physical assessment. The first step is the primary health screening by school health nurses. The second step is the Physicians' physical examination of ill health students selected by school nurses. This study was conducted on a total of 3,525 students of three primary schools located in urban, township, and rural area during the period from May to June, 1987, all data were collected through direct observation & survey method. The main findings are as follows 1. The number of ill health students selected by two step method was more than that of one step method by the physician. 2. The types of ill health students selected by two step method had more diversity than that of one step method by the physician. 3. Budgets and time consumption for two step method were more reductive than that of one step method by the physician.

PURPOSE: The purpose of this study was to propose clinical criteria to differentiate patients who are able to perform the step-through-step gait pattern in chronic stroke patients. METHODS: Sixty patients with chronic stroke patients participated this study. To differentiate patients who could perform the step-through-step gait pattern, age, gender, and causes of stroke were noted, a Chedoke-McMaster (CM) damage list, Fugl-Meyer (FM) assessment scales and the Berg Balance Scale (BBS) were determined. A 10 meter gait test and Timed Up and Go (TUG) test were conducted to determine the differences in gait speed and dynamic balance between patients walking with or without canes in the step-through-step gait pattern group. RESULTS: There was no significant statistical difference in age, gender, and stroke type between all subjects. There were significant differences in the CM scale for postural and lower extremities, and FM scale for lower extremities and BBS. The dynamic balance ability and gait speed showed significant differences between the subjects in the step-through-step gait pattern with or without a cane during gait. CONCLUSION: CM and FM scales for the lower extremities and postural control, as well as BBS scales, can be used as criteria to differentiate patients who are able to perform the step-through-step gait pattern. These results can also be used to provide beneficial information to patients that are walking with canes.

This study researched into the left-right inclination of the rear foot at the lower limb joints, knee joint angle, angular velocity of the knee joint, angular acceleration and the max. Based on the analysis of kinematics according to the changes in the height of step box (6, 8, 10 inches) during step aerobics of female college students majoring in physical education. The findings of this study are as follows: Then angle of the knee joint decreased as the height of the step box increased the min. Angle was measured right before the right foot was on the step box, and the angle tended to decrease as the step box get heightened. The left-right inclination of the rearfoot angle according to the height of step box increased as the height increased. In the 'pull-up' stage during which the weight was loaded on the right foot the angle increased, while in the right foot stepping stage during which the right foot was on the ground, the left-right inclination of the rearfoot angle increased as the height of the step box increased. The angular velocity of the knee joint according to the height of step box started increasing when the right foot initially stepped on the step box and during the initial stepping section, the angular velocity decreased as the height of step box increased. The changes in angular acceleration of the knee joint according to the height of step box increased as the height of step box increased.

Purpose : The purpose of this study was to identify the influence of Bobath and conventional method has an effect on gait of adult hemiplegic patients. Methods : The data were collected by each 15 adult stroke patients. The treatment was based on Bobath and conventional approach. Temporal and spatial parameters of gait were analysed for using the computerized GAITRite system. Results : The gait step (p<.05), gait velocity (p<.05), cadence (p<.05) and step length (p<.05) was significantly different with the Bobath method. The Gait step (p<.05) and gait velocity (p<.05) was significantly different with the conventional method. But the cadence and step length was not significantly increased in the conventional method. Conclusion : The Bobath method is more useful to improve the gait in hemiplegic patients than conventional method.

PURPOSE: Gait disturbances in patients with hemiplegic stroke involve asymmetry of stance time. Step box training is used to supplement the limitations of stair walking training and increasing the torque value of the paralyzed lower leg's strength. This study aimed to investigate whether step-up training on unstable support could change walking ability in patients with chronic stroke. METHODS: Thirty stroke patients were randomly assigned to the step-up training group (experimental group), that performed training on an unstable surface, and the control group, that performed training on a stable surface. Walking speed, step length, and cadence were measured before and after training. Paired t-tests were used to compare pre- and post-intervention data, while the independent samples t-test was used to determine intergroup differences. Values of p < .05 were considered statistically significant. RESULTS: There was a significant difference in walking ability before versus after the intervention in both groups, although the experimental group showed greater differences than the control group (walking velocity by 8.1%; step length of the non-paralyzed side by 6.9%, respectively; p<.05). CONCLUSION: Step-up training might be more effective on an unstable surface than on a stable surface for increasing walking speed and step length of the non-paralyzed side.

Certified reference materials (CRMs) have been used to calibrate surface profilers for reliable measurements. In this paper, we present a newly designed step height CRM which has a step height pattern with two different widths and various special patterns for checking radial magnification, distortion of optical viewing systems, etc. Especially, it could be useful for multi-probe inspection instruments in the manufacturing lines. The fabrication was done by conventional optical lithography and dry etching process with optimized conditions. To verify the step height values, a white-light scanning interferometer was used with objective lenses having magnification of $10{\times}$ and $100{\times}$. CRMs with nominal step heights of $0.5\;{\mu}m$, $1\;{\mu}m$, $3\;{\mu}m$, $5\;{\mu}m$, $7\;{\mu}m$, and $10\;{\mu}m$ were fabricated and the uniformity of these CRMs was evaluated to be less than 3 nm ($1{\sigma}$).
Background: Community ambulation has been recently recognized as one of the most essential factors of activities of daily living in patients with post-stroke hemiparesis. This study aimed to compare walking velocity and step number in 5 community situations in patients with post-stroke hemiparesis. Methods: Ten chronic stroke patients volunteered for this study. The main variables analyzed were walking speed and step number, and these were measured in 5 different community situations: a physical therapy room, a parking lot, a bank, a crosswalk, and a hospital lobby. The measurements obtained for walking in the physical therapy room were measured using a 10m walk test and were used as baseline data for comparison with each option. The ambulation distance was set at 300m for the parking lot and the bank and 150m for the crosswalk and hospital lobby. For data analysis, walking speed and step number were standardized with the distance options of each ambulation. Results: Compared to the walking speed in the physical therapy room, those in the other situations, except for the parking lot, were significantly different (p<.05). Moreover, there were significant differences in the speeds between the bank and the parking lot and between the parking lot and the crosswalk (p<.05). Compared to the step number in the physical therapy room, those in all situations except for the crosswalk were significantly different (p<.05). Further, there was a significant difference in the step number between the bank and the crosswalk (p<.05). Conclusion: The walking ability of patients with hemiparesis in real environments within a community could be different from that in a physical therapy room. Therefore, the evaluation of walking should be performed in a variety of community situations.
The purpose of this study was to compare spatio-temporal parameters during walking between patients with idiopathic Parkinson's disease and a control group matched for age, height, and weight. Thirty-three subjects were included in this study. Fifteen normal subjects (age, $63.3{\pm}5.8$ yrs; height, $164.1{\pm}8.7$ cm; weight, $60.7{\pm}17.5$ kg) and eighteen patients (age, $64.0{\pm}7.7$ yrs; height, $164.7{\pm}7.3$ cm; weight, $63.6{\pm}7.7$ kg) participated in the study. The Vicon 512 Motion analysis system was used for gait analysis in each group during walking, with and without an obstacle. The measured spatio-temporal parameters were cadence, walking speed, stride time, step time, single limb support time, double limb support time, stride length, and step length. Results in stride length and step length, when walking without an obstacle, showed a significantly greater decrease in the patient group compared to the control group. During walking with an obstacle, the patient group showed a significantly greater decrease in the step length as compared to the control group. For the control group, there were significant decreases in parameters of cadence and walking speed and increases in parameters of stride time, step time, and single limb support time when walking with an obstacle. The patient group had lower cadence and walking speed and higher stride time, step time, and single limb support time during walking with an obstacle than in walking without an obstacle. These results suggest that patients with Parkinson's disease who walk over an obstacle can decrease cadence, stride length, and step length. Further study is needed, performed with more obstacles and combined with other external cues, such as visual or acoustic guides.
Objectives : The purpose of this study was to describe and compare the temporo-spatial gait characteristics of healthy young people with those of healthy elderly people. Methods: The data were collected by 40 volunteers. 20 subjects were between 20 and 31 years of age, and 20 subjects were between 65 and 84 years of age. Temporal and spatial parameters of gait were analysed for using the computerized GAITRite system. Results : The system integrates specific components of locomotion to provide a single, numerical representation of gait, the Functional Ambulation Performance score. Differences in gait characteristics between the two groups were examined using a correlated t-test(p<.05). Significant differences were observed between the groups for step length, step/extremity ratio and velocity. Young people demonstrated a significantly larger velocity, step length and step/extremity ratio than the elderly people. Conclusions: These results indicate that the GAITRite system can be useful in detecting footfall patterns and selected time and distance measurements of young and older persons. Additionaly, differences in walking velocity, step length and step/extremity ratio between old and young people may have influenced the gait characteristics measured.
To evaluate training effect, the step exercise was loaded to three mem for nine weeks. Step score, cardiopulmonary functions and blood constituents were measured before, during and after the test exercise (50 cm-step exercise and treadmill running), and were compared with the pre-tranining values. The results were as follows: 1) By the training, Harvard step score increased remarkably, expecially in the early stage of training. 2) The post-training values of maximal oxygen uptake increased very significantly and it seemed to be due to increases of stroke volume and tissue oxygen extraction. 3) After the training, the degree of increase in expired volume was small during the treadmill exercise. 4) By the training, increasing rate of respiratory quotient lessened during the exercise and it was considered to be caused by the decreases of carbohydrate consumption and anaerobic metabolism. 5) The blood cholesterol concentrations were harldy changed with this degree of training. 6) The blood lactate level decreased during the recovery periods and the values of the recovery 0 and 5 minutes decreased remarkably, in comparison with the pre-trained values. The above results suggest that the 9 week-training of the step exercise brings about the enhancement of circulatory functions and tissue oxygen utilization, and changes of food-stuffs used during the exercise.
