보건진료원 직무교육 교과과정 개선을 위한 일 연구 (A Study on Curriculum Development For Community Health Practitioners)

  • This study was designed to develop a conceptual framework for the curriculum and develop the details of the learning content for the education of Community Health Practitioners (CHPs). Since education programs for CHPs started 10 years ago, concepts related to CHP services have changed because of changes in society. The objectives of the study were as follows : 1) to analyse the usefulness of the present education program for CHPs, 2) to analyse the Job performance and self -confidence of the CHPs, 3) to identify the health needs of the clients served by the CHPs and the community problems related to health. 4) to develop a conceptual framework for the curriculum, for the education of CHPs, 5) to develops details for the learning content of the education program for CHPs. Phase I of the study was conducted by questionnaires to 150 CHPs who have worked in remote rural areas for more than 2 years. Among them, 147 responded. Data was collected from August 16, to August 25, 1990. In order to identify the health needs of the community people, research within the last five years was reviewed and analyzed. The data on 1, 842 communities gathered by the WHO Nursing Collaborations Center of the College of Nursing, Yonsei University was utilized to identify community problems related to health and the self - confidence in job performance of the CHPs. Psase II of the study consisted of a workshop with 13 professionals including Community Health Practitioners to evaluate the existing education program and a conceptual framework of the curriculum for the job education of CHPs. The results of the study are Summariged below : 1. The only 26 among 45 content items of the education program related to job skills was used by 80% of the responding CHPs. The knowledge of $\ulcorner$Networking community organization$\lrcorner$ was used by only 53.7% of the respondents. Educational content about $\ulcorner$Mental disease$\lrcorner$ was used by less than 50% of CHPs because of a knowledge deficit. 2. The CHPs reported that their activities concentrated on clinical services during the last six months. The survey showed that they seemed to neglect the activities for health promotion and disease prevention. Thus, $\ulcorner$Education for community loaders$\lrcorner$(15.9%), $\ulcorner$Activity for eavironmental health$\lrcorner$(16.3%) and $\ulcorner$Social work for needey people$\lrcorner$(23.3%) were done by less than 30% of CHPs. 3. More than 90% of CHPs reported being self - confident for the activities of $\ulcorner$Health education and counselling$\lrcorner$, $\ulcorner$Medicine prescription$\lrcorner$ and $\ulcorner$Immunization$\lrcorner$. But 50% of CHPs reported that they were not have self - confident in $\ulcorner$Management of water and environmental health$\lrcorner$ and only 25.6% of CHPs could insert an IUD independently. 4. It was identified that respiratory diseases and the gastrointestinal diseases were most common problems for the community people, followed by musculoskeletal and skin problems. 5. The community problems were classified into eight categories : physical environmental problems, environmental hygiene, health problems, health behavior, social problem, lack of resources, financial problem and the problems of the cultural and value system. 6. The conceptual framework consisted of the target population and their health status, nursing process working site and primary health care services such as health promotion, disease prevention, treatment and rehabilitation. 7. The contents of curriculum of education program for CHPs were formulated from the results of this study.

사이버 안보에 대한 국가정보기구의 책무와 방향성에 대한 고찰 (A Study about the Direction and Responsibility of the National Intelligence Agency to the Cyber Security Issues)

  • 2001년 9/11 테러공격 이후에 미국은 사이버 안보를 가장 위중한 국가안보 문제로 인식한다. 미국 국방부는 2013년 처음으로 사이버 전쟁이 물리적인 테러보다 더 큰 국가안보 위협임을 확인했다. 단적으로 윌리암 린(William J. Lynn) 국방부 차관의 지적처럼 오늘날 사이버 공간은 육지, 바다, 하늘, 우주 다음의 '제5의 전장(the fifth domain of warfare)'이라고 함에 의문이 없다. 인터넷의 활용과 급속한 보급은 사이버 공간에서의 상상하지 못했던 역기능을 창출한 것이다. 이에 사이버 정보와 사이버 네트워크 보호까지를 포괄하지 않으면 국가안보 수호의 목표를 달성할 수 없게 되었다. 그런데 이러한 위험성에도 불구하고 각국은 운영상의 효율성과 편리성, 국제교류 등 외부세계와의 교류확대를 위해 국가기간망의 네트워크화를 더욱 확대해 가고 있고 인터넷에의 의존도는 심화되고 있다. 하지만 그 실천적인 위험성에도 불구하고 우리의 법제도적 장치와 사이버 안전에 대한 인식수준은 현실을 제대로 반영하지 못하고 있는 것으로 판단된다. 오늘날 가장 실천적이고 현실적인 위협을 제기하는 사이버 안보의 핵심은 하나도 둘도 계획의 구체성과 실천력의 배양이다. 대책회의나 교육 등은 부차적이다. 실전적인 사이버 사령부와 사이버 정보기구 그리고 사이버 전사의 창설과 육성에 더 커다란 노력을 경주해야 하고, 우리의 경우에는 가장 많은 경험을 가지고 인력과 장비를 가진 국가정보원의 사이버 수호 역량을 고양하고 더 많은 책무를 부담시키고 합리적인 업무 감독을 다하는 것에 있다고 할 것이다. 이에 본고는 법규범적으로 치안질서와 별개 개념으로서의 국가안보에 대한 무한책임기구인 국가정보기구의 사이버 안보에 대한 책무와 그에 더하여 필요한 사이버 정보활동과 유관활동의 범위를 검토하고자 한다. 사이버 테러와 사이버 공격을 포괄한 사이버 공격(Cyber Attack)에 대한 이해와 전자기장을 물리적으로 장악하는 전자전에 대한 연구도 포함한다.

본태성 고혈압 환자의 자기실현 및 욕구구조에 관한 연구 (Manifest Weeds and Self-Actualization of Patients with Essential Hypertension)

  • Much of a person's energy is spent in the effort of becoming a productive member of to-day's complex society. This activity may cause tension, and chronic unrelieved tension is an influential factor in blood pressure elevation. The problem of this study was to identify manifest needs and self-actualization of patients with essential hypertension, and to analyse and compare their manifest needs and selt-actualization with the selected general characteristics of We, sex, religion, occupation and level of education with a control group of patients with normal blood pressure readings. The purpose was to contribute to the planning of nursing interventions toward reducing the impact of complex psycho-somatic factors on the anxiety of patients with essential hypertension. The instruments used included selected items from the Edwards (1959) Personal Preference Schedule (EPPS) as adapted by Hwang (1965) and from the Personal Orientation Inventory (POI) (Shostrom 1964, 1974) adapted by Kim and Lee (1977) to measure manifest needs and self-actualization. The convenience sample was chosen from 149 persons who presented themselves for general physical examinations at Ewha University Medical Centre and 41 patients diagnosed with essential hypertension at three general hospitals in Seoul during June 1 and August 31, 1977. Forty-nine persons from the Ewha group with blood-pressure readings exceeding 150/90 were added to the experimental group. Data were analysed by the S.P.S.S. computer programme using t-test and tests for statistical significance. Statistically significant findings were as follows: A. Blood Pressure and Manifest Needs. 1. with the exception of Autonomy, patients with hypertension had significantly high scores on all variables Abasement, Achievement, Affiliation, Aggression, Dominance, Emotionality, Exhibitionism and Sex. 2. When mean scores of normal persons were compared by age groups, normal persons had higher scores in the following order on Abasement (50's, 40's, 20's, 30's), Achievement (50's, 30's, 40's, 20's), Affiliation (50's, 40's, 30's, 20's), Dominance (50's, 40's, 40's, 20's) and Exhibitionism (30's, 50's, 40's, 20's). In each case, there was a significant difference between the first and last age group scores. 3. When the mean scores of normal persons were compared by sex, normal men had higher scores than women on Achievement, Affiliation, Aggression, Dominance, Exhibitionism and Sex. Male patients had higher scores than female patients on Achievement, Dominance, Exhibitionism and Sex, but female patients scored higher in Emotionality. 4. Normal persons had higher scores related to religion in the following order on Achievement (Buddhism, no religion, Christianity). Hyper tensive patients had higher scores on. Exhibitionism (no religion, Christianity, Buddhism). 5. Normal persons had higher scores related to occupation in the following order on Achievement and Exhibitionism (unemployed, office workers, teachless, businessmen), Emotionality (office workers, unemployed, businessmen, teacher) and Sex (office workers, unemployed, teachers, businessmen). Hypertensive patients had higher scores on Achievement and Aggression (teachers, businessmen, office worker, unemployed), Dominance and Exhibitionism (businessmen, teacher, of ace workers, unemployed) and Sex (teachers, office worker, businessmen, unemployed). 6. Normal persons had higher scores related to level of edification in the following order on Abasement, Emotionality and Autonomy (secondary school graduation, university). Hypertensive patients had higher scores on Abasement (no education, primary, university, secondary), Achievement (no education, secondary, university, primary) , Dominance (university, no education, secondary, primary), Exhibitionism (university, secondary, no education, primary), and Sex (university, secondary, primary, no education). B. Blood Pressure and Self_Actualization 1, Patients with hypertension had significantly lower scores on all variables. 2. Normal persons had higher scores related to age groups in the following order on Existentiality (20's, 30's, 40's, 50's). Hypertensive patients showed no significantly different scores. 3. Normal women had higher scores than men on Time Competence. Normal men had higher scores on Feeling Reactivity. Male patients had higher scores than women on Self-Actualizing Value and Self-Regard. 4. Normal persons ha 1 higher scores related to religion on spontaneity (Buddhism, no religion, Christianity). Hypertensive patients had higher scores on Time Competence and Nature of Man (Buddhism, Christianity, no religion). 5. Normal persons had higher scores related to occupation in the following order on Existentiality (teachers, office workers, businessmen, unemployed) and Self-Regard (unemployed, office workers, teachers, businessmen). Hypertensive patients showed no significantly different scores. 6. Normal persons had higher scores related to level of education in the following order on Existentiality and Self-Acceptance (university, secondary). Hypertensive patients had higher scores on inner-Director (university, secondary, no education, primary) and Existentiality (university, secondary, primary, no education). Recommendations for nursing interventions with hypertensive patients with emotional problems or low self-actualization were made. 1. The nurse should encourage the patient through her interactions with other members of the medical team to accept counselling and health education. 2. Through her therapeutic interpersonal relationships with the patient, the nurse should help him discover the causes of his emotional tension. 3. Through her health teaching with the family, the nurse should encourage them to participate with the medical team in the patient's therapeutic plan and in providing him with the minimum possible emotional support. 4. Through frequent counselling with the obsessive-thinking and inflexible patient, the nurse should reevaluate the patient's behaviour and her interventions. 5. Seriously ill patients should be given needed reeducation by members of the professional medical team.

인공위성 원격탐사를 이용한 백두산 화산 감시 연구 리뷰 (A Review on Monitoring Mt. Baekdu Volcano Using Space-based Remote Sensing Observations)

  • 백두산은 중국과 북한의 국경 경계에 위치하고 있는 성층화산으로 신생대 올리고세 이후 주요 분화 단계를 거쳐 형성된 것으로 알려져 있다. 2010년 이후 마그마 재활동으로 인한 백두산 화산활동 여부에 대한 관심이 증대되고 있다. 백두산 화산 활동을 감시하기 위한 연구는 기상청, 한국지질자원연구원 등 국가기관 중심으로 활발히 수행되고 있다. 2018년에는 한-중 백두산 공동 관측 장기연구 과제가 선정되었으며 이로부터 화산특화연구센터가 설립되기도 하였다. 그러나 백두산은 우리나라로부터 지리적으로 멀리 떨어져 있어 접근에 대한 제약이 있을 뿐만 아니라 백두산 화산 주변에 설치되어 있는 현장 관측 장비로부터 수집된 현장 자료의 공유 혹은 접근이 쉽지 않은 상황이다. 원격탐사는 직접적인 물리적 접촉 없이 대상 물체에 대한 특성을 원격으로 측정하는 수단으로서, 대상물의 관측을 위해 자동차, 무인기, 항공기, 인공위성 등 여러 형태의 플랫폼이 사용된다. 지난 수십 년 간, 다양한 파장 대역에서의 전자기파를 이용한 원격탐사 자료를 활용하여 화산 감시 연구가 수행되어 왔다. 특히 레이더 원격탐사는 주야조건, 기상조건에 관계없이 자료를 획득할 수 있을 뿐만아니라 위상정보를 이용한 레이더 위상간섭기법을 통한 미세 지표 변위 관측이 가능하여 매우 널리 이용되고 있는 화산 감시 기술이다. 본 논문의 목적은 백두산 화산 관측을 위해 수행된 기존 원격탐사 연구 문헌을 수집하고 동향을 파악하는 것이다. 또한 지속적인 화산 감시를 위한 가용 영상레이더 위성정보를 조사하여 향후 이를 바탕으로 백두산 화산 지표 변위의 주기적 탐지 연구를 수행하는데 활용할 예정이다.

내면화된 수치심과 자기통제력이 뇌졸중 환자의 대인관계에 미치는 영향 (The Effect of Internalized Shame and Self-Control on Interpersonal Relationships in Stroke Patients)

  • 목적 : 본 연구의 목적은 뇌졸중 환자를 대상으로 내적 수치심과 자기통제력이 대인관계에 어떠한 영향을 미치는가를 조사하고, 그 관련성을 분석함으로써 임상에 필요한 근거와 정보를 제공하고자 한다. 연구방법 : 본 연구를 위해 전라남도 및 충청남도 지역 소재에 작업치료사가 근무하고 있는 기관에서 작업치료 서비스를 받고 있는 뇌졸중 환자 150명을 대상으로 2019년 3월 1일부터 2019년 4월 30일까지 일반적 특성, 대인관계(RCS), 자기통제력(SCS), 내면화된 수치심 척도(ISS) 설문지를 사용하여 설문을 실시하였다. 연구 대상자의 일반적인 특성은 빈도분석과 기술통계를 실시하였고, 일반적 특성에 따른 대인관계를 비교하기 위해 독립표본 t검정(t-test)과 일원배치분산분석(ANOVA)을 사용하였다. 내면화된 수치심과 자기통제력 그리고 대인관계의 관련성은 피어슨 상관계수로 분석하였고, 뇌졸중 환자의 대인관계에 미치는 영향요인을 파악하기 위하여 다중 회귀분석을 실시하였다. 결과 : 일반적 특성에 따른 대인관계를 비교한 결과, 연령과 교육수준 항목에서 유의한 차이가 나타났다. 내면화된 수치심과 대인관계, 자기통제력과 내면화된 수치심은 유의한 역상관 관계를 나타내었고, 자기통제력과 대인관계는 정적인 상관관계가 나타났지만, 대인관계 하위요인인 개방성, 민감성, 친밀감, 의사소통 항목과 자기통제력은 통계적으로 유의하지 않았다. 또한, 내면화된 수치심의 하위 영역인 부적절함(β =-0.32)과 모험 추구(β =-0.23) 항목은 역방향으로 영향을 미치고, 자기통제력의 하위 영역인 신체활동(β =-0.22)과 자기중심(β =0.24) 항목은 정방향으로 영향을 미치는 것으로 나타났다. 결론 : 따라서 뇌졸중 환자의 대인관계 형성을 위해서는 다양한 심리적인 요인이 적용된 재활치료 프로그램이 운영될 수 있는 추가적인 연구가 필요하다.

비접촉 데이터 사회와 아카이브 재영토화 (Contactless Data Society and Reterritorialization of the Archive )

  • 한국 정부가 UN의 2022년 전자정부 발전 지수에서 UN가입 193개국 중 3위에 랭크됐다. 그동안 꾸준히 상위국으로 평가된 한국은 분명 세계 전자정부의 선도국이라 할 수 있다. 전자정부의 윤활유는 데이터다. 데이터는 그 자체로 정보가 아니고 기록도 아니지만 정보와 기록의 원천이며 지식의 자원이다. 전자적 시스템을 통한 행정 행위가 보편화된 이후 당연히 데이터에 기반한 기록의 생산과 기술이 확대되고 진화하고 있다. 기술은 가치중립적인 듯 보이지만 사실 그 자체로 특정 세계관을 반영하고 있다. 더구나 비물질적 유통을 기반으로 하는 디지털 세계, 온라인 네트워크의 또 다른 아이러니는 반드시 물리적 도구를 통해서만 접속하고 접촉할 수 있다는 점이다. 디지털 정보는 논리적 대상이지만 반드시 어떤 유형이든 그것을 중계할 장치 없이는 디지털 자원을 읽어 내거나 활용할 수 없다. 초연결, 초지능을 무기로 하는 새로운 기술의 디지털 질서는 전통적인 권력 구조에 깊은 영향력을 끼칠 뿐만 아니라 기존의 정보 및 지식 전달 매개체에도 마찬가지의 영향을 미치고 있다. 더구나 데이터에 기반한 생성형 인공지능을 비롯해 새로운 기술과 매개가 단연 화두다. 디지털 기술의 전방위적 성장과 확산이 인간 역능의 증강과 사유의 외주화 상황까지 왔다고 볼 수 있을 것이다. 여기에는 딥 페이크를 비롯한 가짜 이미지, 오토 프로파일링, 사실처럼 생성해 내는 AI 거짓말(hallucination), 기계 학습데이터의 저작권 침해에 이르기까지 다양한 문제점 또한 내포하고 있다. 더구나 급진적 연결 능력은 방대한 데이터의 즉각적 공유를 가능하게 하고 인지 없이 행위를 발생시키는 기술적 무의식에 의존하게 된다. 그런 점에서 지금의 기술 사회의 기계는 단순 보조의 수준을 넘어서고 있으며 기계의 인간 사회 진입은 고도의 기술 발전에 따른 자연적인 변화 양상이라고 하기에는 간단하지 않은 지점이 존재한다. 시간이 지나며 기계에 대한 관점이 변화하게 될 것이기 때문이다. 따라서 중요한 것은 기계를 통한 커뮤니케이션, 행위의 결과로서의 기록이 생산되고 사용되는 방식의 변화가 의미하는 사회문화적 함의에 있다. 아카이브 영역에서도 초지능, 초연결사회를 향한 기술의 변화로 인해 데이터 기반 아카이브 사회는 어떤 문제에 직면하게 될 것인지, 그리고 그 속에서 누가 어떻게 기록과 데이터의 지속적 활동성을 입증하고 매체 변화의 주요 동인이 될 것인가에 대한 연구가 필요한 시점이다. 본 연구는 아카이브가 행위의 결과인 기록뿐만 아니라 데이터를 전략적 자산으로 인식할 필요성에서 시작했다. 이를 통해 전통적 경계를 확장하고 데이터 중심 사회에서 어떻게 재영토화를 이룰 수 있을지를 알아보았다.

간호원의 환자교육 활동에 관한 연구 (Study of Patient Teaching in The Clinical Area)

  • Nursing of today has as one of its objectives the solving of problems related to human needs arising from the demands of a rapidly changing society. This nursing objective, I believe, can he attained by the appropriate application of scientific principles in the giving of comprehensive nursing care. Comprehensive nursing care may be defined as nursing care which meets all of the patient's needs. the needs of patients are said to fall into five broad categories: physical needs, psychological needs, environmental needs, socio-economic needs, and teaching needs. Most people who become ill have adjustment problems related to their new situation. Because patient teaching is one of the most important functions of professional nursing, the success of this teaching may be used as a gauge for evaluating comprehensive nursing care. This represents a challenge foe the future. A questionnaire consisting of 67 items was distributed to 200 professional nurses working ill direct patient care at Yonsei University Medical Center in Seoul, Korea. 160 (80,0%) nurses of the total sample returned completed questionnaires 81 (50.6%) nurses were graduates of 3 fear diploma courser 79 (49.4%) nurses were graduates of 4 year collegiate nursing schools in Korea 141 (88,1%) nurses had under 5 years of clinical experience in a medical center, while 19 (11.9%) nurses had more than 5years of clinical experience. Three hypotheses were tested: 1. “Nurses had high levels of concept and knowledge toward patient teaching”-This was demonstrated by the use of a statistical method, the mean average. 2. “Nurses graduating from collegiate programs and diploma school programs of nursing show differences in concepts and knowledge toward patient teaching”-This was demonstrated by a statistical method, the mean average, although the results showed little difference between the two groups. 3. “Nurses having different amounts of clinical experience showed differences in concepts and knowledge toward patient teaching”-This was demonstrated by the use of a statistical method, the mean average. 2. “Nurses graduating from collegiate programs and diploma school programs of nursing show differences in concepts and knowledge toward patient teaching”-This was demonstrated by a statistical method, the mean average, although the results showed little difference between the two groups. 3. “Nurses having different amounts of clinical experience showed differences in concepts and knowledge toward patient teaching”-This was demonstrated by the use of the T-test. Conclusions of this study are as follow: Before attempting the explanation, of the results, the questionnaire will he explained. The questionnaire contained 67 questions divided into 9 sections. These sections were: concept, content, time, prior preparation, method, purpose, condition, evaluation, and recommendations for patient teaching. 1. The nurse's concept of patient teaching: Most of the nurses had high levels of concepts and knowledge toward patient teaching. Though nursing service was task-centered at the turn of the century, the emphasis today is put on patient-centered nursing. But we find some of the nurses (39.4%) still are task-centered. After, patient teaching, only a few of the nurses (14.4%) checked this as “normal teaching.”It seems therefore that patient teaching is often done unconsciously. Accordingly it would he desirable to have correct concepts and knowledge of teaching taught in schools of nursing. 2. Contents of patient teaching: Most nurses (97.5%) had good information about content of patient teaching. They teach their patients during admission about their diseases, tests, treatments, and before discharge give nurses instruction about simple nursing care, personal hygiene, special diets, rest and sleep, elimination etc. 3. Time of patient teaching: Teaching can be accomplished even if there is no time set aside specifically for it. -a large part of the nurse's teaching can be done while she is giving nursing care. If she believes she has to wait for time free from other activities, she may miss many teaching opportunities. But generally proper time for patient teaching is in the midmorning or midafternoon since one and a half or two hours required. Nurses meet their patients in all stages of health: often tile patient is in a condition in which learning is impossible-pain, mental confusion, debilitation, loss of sensory perception, fear and anxiety-any of these conditions may preclude the possibility of successful teaching. 4. Prior preparation for patient teaching: The teaching aids, nurses use are charts (53.1%), periodicals (23.8%), and books (7.0%) Some of the respondents (28.1%) reported that they had had good preparation for the teaching which they were doing, others (27.5%) reported adequate preparation, and others (43.8%) reported that their preparation for teaching was inadequate. If nurses have advance preparation for normal teaching and are aware of their objectives in teaching patients, they can do effective teaching. 5. Method of patient teaching: The methods of individual patient teaching, the nurses in this study used, were conversation (55.6%) and individual discussion (19.2%) . And the methods of group patient teaching they used were demonstration (42.3%) and lecture (26.2%) They should also he prepared to use pamphlet and simple audio-visual aids for their teaching. 6. Purposes of patient teaching: The purposes of patient teaching is to help the patient recover completely, but the majority of the respondents (40.6%) don't know this. So it is necessary for them to understand correctly the purpose of patient teaching and nursing care. 7. Condition of patient teaching: The majority of respondents (75.0%) reported there were some troubles in teaching uncooperative patients. It would seem that the nurse's leaching would be improved if, in her preparation, she was given a better understanding of the patient and communication skills. The majority of respondents in the total group, felt teaching is their responsibility and they should teach their patient's family as well as the patient. The place for teaching is most often at the patient's bedside (95.6%) but the conference room (3.1%) is also used. It is important that privacy be provided in learning situations with involve personal matters. 8. Evaluation of patient teaching: The majority of respondents (76.3%,) felt leaching is a highly systematic and organized function requiring special preparation in a college or university, they have the idea that teaching is a continuous and ever-present activity of all people throughout their lives.
