제2형 당뇨병 환자의 신체활동이 혈당조절에 미치는 영향 (Effects of Physical Activity on Glycemic Control in Type 2 Diabetics)

  • Purpose: The purpose of this study was to examine the effects of physical activity on glycemic control among Koreans with type 2 diabetes. Methods: A total of 215 patients with type 2 diabetes (82 male and 133 female) were recruited (mean age = $59.0{\pm}9.7$ years). The amounts of physical activity was assessed using the physical activity scale for elderly (PASE). Fasting blood glucose (FBG), Hemoglobin A1c ($HbA_1c$), and 2 hours post-prandial glucose (2hrPG) were measured. Results: The highest PASE score was housework-related physical activity. However, the amounts of walking was significantly higher in good FBG level (Z = 1.39, p=.041) and the amounts of leisure-time physical activity was higher in good HbA1 and 2hrPG level than in the poor glycemic control group (Z = 2.29, p<.001; Z = 1.99, p=.001). A logistic regression analysis showed that patients with leisure-time physical activity in the top quartile more likely presented with good glycemic control in $HbA_1c$, OR=3.84 (95% Confidence Interval (CI) = 1.65~8.95) and in 2hrPG, OR=4.06 (95%CI = 1.77~9.27), compared to patients in the lowest quartile. Conclusion: Leisure-time physical activity is effective for controlling the glucose levels, especially $HbA_1c$ and 2hrPG among type 2 diabetic patients. It is suggested that health providers need to more focus on providing aggressive recommendations on physical activity considering physical activity patterns by individuals.

  • Purpose: The purpose of the study was to identify factors influencing physical activity among community-dwelling older adults with type 2 diabetes. The study design was based on the Theory of Triadic Influence. Methods: A total of 242 older adults with type 2 diabetes participated in this study. Six variables related to physical activity in older adults, including self-efficacy, social normative belief, attitudes, intention, experience, and level of physical activity, were measured using reliable instruments. Data were analyzed using descriptive statistics, Pearson's correlation analyses, and a path analysis. Results: The mean physical activity score was 104.2, range from zero to 381.21. The path analysis showed that self-efficacy had the greatest total effect on physical activity. Also, experience had direct and total effects on physical activity as well as mediated the paths of social normative beliefs to attitudes and intention to physical activity. These factors accounted for 10% of the total variance, and the fit indices of the model satisfied the criteria of fitness. Conclusion: The findings of the study reveal the important role of self-efficacy and past experience in physical activity in older adults with type 2 diabetes.

한국 성인의 신체활동 유형에 따른 식생활 및 건강행태 - 2016~2018년 국민건강영양조사 자료를 이용하여 - (The Study of Dietary Habits and Health Behaviors according to Physical Activity Type in Korean Adults -Based on the 2016~2018 Korea National Health and Nutrition Examination Survey-)

  • Objectives: This study evaluated the dietary habits and health behaviors of Korean adults according to their physical activity. Methods: Adults aged 19~64 years, who participated in the 2016~2018 National Health and Nutrition Examination Survey, were enrolled in this study. The subjects were classified into the physical inactivity group, aerobic physical activity group, strength exercise group, and combined exercise group. Results: Significant differences in skipping breakfast, frequency of eating out, dietary supplements, and alcohol drinking status were observed among physical activity groups (P < 0.001). The combined exercise group had the highest % KDRI of protein, vitamin A, thiamin, riboflavin, niacin, calcium, potassium, and iron (P < 0.001). The physical inactivity group had the highest obesity rate (35.1%), and they perceived their body image type to be obese. In the combined exercise group, 47.8% of respondents said they were in good health (P < 0.001). The health-related quality of life score of the physical inactivity group was the lowest, with a score of 0.94. The metabolic syndrome risk rate of the combined exercise group was lower at 0.62 times (95% CI, 0.51-0.75) than the physical inactivity group. Conclusions: The physical activity type was associated with metabolic syndrome. These results can be useful for supporting dietary education and physical activity programs for adults.

뇌졸중 환자와 정상인의 근 수축 유형에 따른 피질 신경원의 활성도 비교 (Comparison of the Activity of Cortical Neurons According to Muscle Contraction Type between Post Stroke Hemiplegic Subjects and Healthy Subjects)

  • Purpose: In order to examine difference in the activity of potential of spinal neurons and cortical neurons according to muscle contraction type in post stroke hemiplegic subjects and healthy subjects, the present study conducted an experiment as follows. Methods: The subjects in the experimental group were 17 left-side hemiplegic subjects (9 female, 8 male; mean age, 63.41$\pm$9.86 years) with the right hand as the dominant hand selected among post stroke hemiplegic subjects, and 17 age matched healthy control subjects (10 female, 7 male; mean age, 64.12$\pm$12.07 years). Movement-related cortical potentials (MRCPs) were measured using surface electromyography and electroencephalography while concentric and eccentric movements were made alternately. Results: As to the pattern of the activity of MRCP, which indicates the activity of motor cortical neurons, we found that the amplitude is high (p<0.01), the length of excitement is short (p<0.01) and the ascending gradient of amplitude to the peak increases (p<0.05) in post stroke hemiplegic subjects' lesion sites different from healthy subjects. Conclusion: The activity of cortical neurons was no difference in activity according to contraction type was observed in post stroke hemiplegic subjects' lesion sites. This suggests that there is no distinction in the activity of cortical neurons between concentric contraction and eccentric contraction. Accordingly, if post stroke hemiplegic subjects' activity of motor cortex is analyzed by measuring MRCPs, it is considered useful in research on neural plasticity or as a ground of clinical effects in the area of physical therapy for the central nervous system.

경사로 보행에서 신발의 형태가 발목 근육의 근활성도에 미치는 영향 (Effects of Footwear Type on Ankle Muscle Activity during Sloped Walking)

  • Purpose: This study aims to identify the differences in the muscle activity of the ankle joint muscle depending on the type of footwear (sneakers, mule sneakers, slippers) worn while walking on a slope. Methods: The subjects wore each shoe (sneakers, mule sneakers, and slippers) and walked on an 18-degree slope at a speed of 110 beats/min. While walking, the muscle activities of the tibialis anterior and medial gastrocnemius were measured. Of the three walking cycles, the second walking cycle was measured except for the first and third walking cycles, which are the beginning and end, and a three-minute break was taken to prevent muscle fatigue when the type of footwear was changed. Results: When walking on a slope, there was no significant difference in the muscle activity of the tibialis anterior according to the type of footwear. However, when walking on a slope, the difference in muscle medial gastrocnemius muscle activity was significant between sneakers and mule sneakers. There was also a significant difference between sneakers and slippers, but there was no significant difference between slippers and mule sneakers. Conclusion: There was no significant statistical difference between sneakers and mule sneakers, but there was a numerical difference. Therefore, the presence or absence of the shoe collar may affect the muscle activity of the medial gastrocnemius when walking on a slope. This can lead to patellofemoral pain syndrome caused by the excessive use of the quadriceps, so it must be considered that caution is needed.

제2형 당뇨병 환자의 심혈관질환 위험요인 예측인자 (Predictors of Cardiovascular Risk Factors among Type 2 Diabetic Patients)

  • Purpose: The purpose of this study was to explore the predictors of cardiovascular risk factors among type 2 diabetic patients. Method: Data were collected from November, 2003 to June, 2004 using a physiological index and questionnaires. Patients(N=159) aged 40 and above were conveniently recruited from health care centers in B city. Data were analyzed with descriptive statistics, Pearson correlation and stepwise multiple regression using SPSS WIN 10.0 program. Results: The cardiovascular risk factors were negatively related with female gender, household monthly income, educational experience about diabetes, physical activity, self-care, self-efficacy and problem oriented coping, while positively related with the duration of diabetes, diabetic family history and depression. Self-care, diabetic family history, female gender, monthly household income, self-efficacy, affective-oriented coping and physical activity predicted 41.5% of the variance in cardiovascular risk factors of diabetic patients. Conclusion: According to the findings of this study, we concluded that cardiovascular risk factors of type 2 diabetic patients are related to the modifiable and non-modifiable variables. Self-care, self-efficacy, affective oriented coping, and physical activity were identified as modifiable variables. Intervention programs to increase those variables are warranted to reduce cardiovascular risk factors among type 2 diabetic patients.

노년층여성(老年層女性)의 활동량(活動量)에 따른 신체적(身體的) 특성(特性) (The Physical Characteristics of Elderly Women Resulting from activity Amoumt)

  • In the order to grasp their physical characteristics stemming from activity amount, this paper has divided elderly women into the following group ; those with high activity in the past, those with low activity in the present, those with high activity in the present and those with low activity in the present. The analyses of the body measurements and the pie chart, and the classification of back shapes by taking photographs have led to the following results. 1. The items of depth and girth among the body measurements were significant in the past and the present activity. Those with high activity in the past had smaller sizes in depth and girth than those with low activity. 2. From the pie chart, it was shown that those with high activity in the past had smaller sizes in every index as well as in girth than those with low activity in the past. Both those with low activity in the present and those with medium activity in the present had somewhat large sizes in waist girth, bust girth, hip girth and abdominal girth and in the indices of these items. 3. The classification of back shapes by taking photograpes has shown that those with high activity in the past had the less bent body type-42 percent of Type A and 6.5 percent of Type D. Among those with medium activity in the present, Type A was most outstanding and Type C and Type D were less, This fact shows that those with medium activity in the present keep the most normal body type. This proves that the medium activity of elderly women is desirable for keeping the normal body type.

뇌졸중 환자의 무릎관절 신전 등척성 운동 시 의자 종류와 골반통제가 넙다리네갈래근 근력과 근활성도에 미치는 영향 (The Influence of Different Chair Type and Pelvic Control on Quadriceps Muscle Activity and Strength During Knee Joint Extension Isometric Exercise in Hemiplegic Patients)

  • The purpose of this study was to suppose basis data the influence of different chair type and pelvic control on quadriceps muscle activity and strength during knee joint extension isometric exercise in hemiplegic patients. This research were investigated in ten healthy adults and tens hemipelgic patients. Surface electromyography (EMG) and Biodex system were used to collect kinematic data and muscle activity, respectively. Independent t-test, paired t-test and one-way repeated ANOVA were used to determine a statistical significance. The results showed as follows: (1) Posterior pelvic angles in healthy group and hemiplegic group were significantly different on isokinetic equipment (p<.05). (2) Different chair type and pelvic control on quadriceps muscle activity and strength were significantly different in hemiplegic patients (p<.05). From the result of this research, posterior pelvic angle control during knee joint extension isometric exercise in hemiplegic patients on isokinetic equipment is necessary to increase quadriceps muscle strength in hemiplegic patients.

Zinc status and dietary quality of type 2 diabetic patients: implication of physical activity level

  • The purpose of this study was to analyze the relationships among zinc status, diet quality, glycemic control and self-rated physical activity level of type 2 diabetic patients. Dietary intakes for two non-consecutive days were measured by 24-hour recall method for seventy-six diabetic patients. Fasting blood glucose and HbA1c were measured for the assessment of glycemic control. We evaluated the extent of dietary adequacy by the percentage of subjects with a dietary intake of a nutrient less than the estimated average requirement(EAR), the dietary diversity score(DDS) and the dietary variety score(DVS). Zinc status was assessed from serum levels and urinary excretion. Dietary inadequacy was serious for five nutrients: riboflavin, calcium, thiamin, zinc and vitamin C. Dietary intakes from the meat, fish, and egg food groups and the milk food group were below the recommended level.   • Purpose. Physical health status according to the presence or absence of physical activity showed a significant difference in daily life activities and limited mental health differences were not significant. Factors affecting the physical activity of the elderly with chronic diseases appeared to limit the presence and activities of daily living. Conclusions. As well there is no activity restrictions can be more physically active everyday life well. It is suggested that it is important to approach everyday life, I can help eliminate the factors that independently giving limits on the activities of the elderly with chronic diseases.
