트립토판과 리보플라빈을 이용한 광역동 치료의 여드름 치료 효과 (Topical Photodynamic Therapy with Triptophan and Riboflavin for the Treatment of Acne vulgaris)

  • Objective : Photodynamic therapy (PDT) using topical aminolevulinic acid (ALA) has increasingly been used for the treatment of acne vulgaris and several studies have shown its clinical efficacy. However, ALA-PDT needs a relatively long incubation period and is frequently associated with adverse effects. Triptophan and Riboflavin has been introduced as a new photosensitizer for the treatment of acne recently. Method : 11 patients with acne vulgaris were received PDT with tryptophan and riboflavin four to six times over two to three months. The photos of each patient were taken before and after treatment. Result and conclusion : PDT with tryptophan and riboflavin may be an effective and safe treatment for the Acne vulgaris patients.

  • Onychomycosis is the most common disease of the nail, with a worldwide prevalence of 5.5%. It causes local pain, paresthesia, and cosmetic problems and decreases the patients' quality of life. Conventional treatments include the administration of topical and systemic agents. However, factors such as subungual hyperkeratosis and biofilm formation may affect the efficacy of these treatments. Moreover, side effects associated with systemic agents are a major concern for patients. Therefore, various novel treatments are being developed; among them, photodynamic therapy (PDT) and Nd:YAG laser are promising, and several studies have demonstrated the efficacy and safety of PDT and laser therapy. Here, we report the efficacy and tolerability of PDT combined with 1064-nm Nd:YAG laser therapy and topical efinaconazole for the treatment of three patients with refractory onychomycosis.

Streptococcus mutans에 대한 생강과 천연추출물의 광역학 항균효과 (Antibacterial effect of Zingiberaceae extracts mediated photodynamic therapy on Streptococcus mutans)

  • Purpose: This study evaluated the antibacterial effects of curcuma, ginger, and finger root extracts in water-soluble powder on planktonic Streptococcus mutans(S.mutans), which is known to cause dental caries, in order to confirm whether these extracts could perform as photosensitizers for the effects of photodynamic therapy (PDT). Methods: This study used the strain of streptococcus mutans ATCC 25175 distributed by the Korean Collection for Type Cultures of the Korea Research Institute of Bioscience & Biotechnology. Commercial edible curcuma, ginger and finger root were used as the natural extracts for the use of photosensitizer. To extract organic solvent, 3 g of each powder was mixed in $30m{\ell}$ of dimethyl sulfoxide (DMSO, VWR, Germany) before extraction. $1.8m{\ell}$ of the photosensitizer solution, manufactured in the concentrations of 5, 0.5, and $0.05mg/m{\ell}$, was mixed with $0.2m{\ell}$ of the S. mutans culture medium that had been cultured for 2 days. To induce the photodynamic reaction, Qraycam (AIOBIO, Seoul, Korea) equipped with 405 nm LED was used to expose light for 5 minutes to irradiate 59 nW energy for 300 seconds. Results: Compared with the case with no light, a higher photodynamic therapeutic effect was confirmed with $0.05mg/m{\ell}$ curcuma powder extract, the concentration of $0.5mg/m{\ell}$ and LED light of 405 nm wavelength (p=0.000, p=0.003). $0.05mg/m{\ell}$ of curcuma powder extract and the concentration of $0.5mg/m{\ell}$ showed 100% antibacterial effect when exposed to light, whereas the concentration of $5mg/m{\ell}$ showed 11.95% antibacterial effect. When exposed to light, $0.05mg/m{\ell}$ of ginger powder extract showed an antibacterial effect which didn't statistically decrease. The concentrations of $0.5mg/m{\ell}$ and $5mg/m{\ell}$ did not show any antibacterial effects. As a result of examining any photodynamic therapeutic effects of finger root powder extract on S. mutans, no statistically significant effect was found. Conclusion: The curcuma powder extract is expected to perform as a photosensitizer. Even though belonging to the same ginger family, ginger powder and finger root powder seem difficult to perform as photosensitizer.

치면착색제를 이용한 광역동 치료의 항균 효과 (Antimicrobial Effect of Photodynamic Therapy Using Plaque Disclosing Agent)

  • 이 연구의 목적은 치면착색제인 10 - 20 mM erythrosine을 광감각제로 사용한 광역동 치료의 항균 효과를 평가하는 것이다. Streptococcus mutans, Lactobacillus casei, Candida albicans를 포함하는 다종 우식원성 세균막을 hydroxyapatite disc에 형성하였다. 광감각제로 20 μM, 10 mM, 20 mM erythrosine을 3분간 적용 후 24초 동안 광조사를 시행하였다. 각 실험군의 Colony-forming unit (CFU)을 측정하고 세균막을 Confocal laser scanning microscopy (CLSM)을 이용하여 관찰하였다. 10 - 20 mM erythrosine을 광감각제로 사용한 광역동 치료 실험군에서 CFU의 현저한 감소가 관찰되었고, 그 결과는 CLSM 관찰에서도 확인되었다. 이번 연구를 통해 치과 임상에서 사용되는 치면착색제를 광감각제로 이용한 광역동 치료의 높은 항균효과를 확인할 수 있었다.

Photochemical Property and Photodynamic Activity of Tetrakis(2-naphthyl) Porphyrin Phosphorus(V) Complex

  • To examine the photosensitized biomolecules damaging activity, dimethoxyP(V)tetrakis(2-naphthyl)porphyrin (NP) and dimethoxyP(V)tetraphenylporphyrin (PP) were synthesized. The naphthyl moiety of NP hardly deactivated the photoexcited P(V)porphyrin ring in ethanol. In aqueous solution, the naphthyl moiety showed the quenching effect on the photoexcited porphyrin ring, possibly through electron transfer and self-quenching by a molecular association. Binding interaction between human serum albumin (HSA), a water soluble protein, and these porphyrins could be confirmed by the absorption spectral change. The apparent association constant of NP was larger than that of PP. It is explained by that more hydrophobic NP can easily bind into the hydrophobic pockets of HSA. The photoexcited PP effectively induced damage of the tryptophan residue of HSA, through electron transfer-mediated oxidation and singlet oxygen generation. NP also induced HSA damage during photo-irradiation and the contributions of the electron transfer and singlet oxygen mechanisms were speculated. The electron transfer-mediated mechanism to the photosensitized protein damage should be advantageous for photodynamic therapy in hypoxic condition. The quantum yield of the HSA photodamage by PP was significantly larger than that of NP. The quenching effect of the naphthyl moiety is considered to suppress the photosensitized protein damage. In conclusion, the naphthalene substitution to the P(V)porphyrins can enhance the binding interaction with hydrophobic biomacromolecules such as protein, however, this substitution may reduce the photodynamic effect of P(V)porphyrin ring in aqueous media.

Moraxella catarrhalis의 광역학적 비활성화 (Photodynamic Inactivation of Moraxella catarrhalis)

  • The aim of this study was to evaluate the bacterial effects of Moraxella catarrhalis in otitis media with effusion (OME) by photodynamic therapy (PDT). Bacterial suspensions (10000 CFU/mL) were prepared. The colony forming units (CFU) of Moraxella catarrhalis have been measured after an application of photogem plus 632 nm diode laser irradiation. One ml of the bacterial suspensions have been incubated in the dark for 3h with various concentrations of photogem ($0.625{\sim}5.0_{\mu}g/mL$) and then irradiated with 632 nm diode laser ($15J/cm^2$). After, the PDT Moraxella catarrhalis suspensions ($50{\mu}L$) were inoculated on chocolate agar plate and cultured in the dark at $37^{\circ}C$, 5% $CO_2$ condition for 18h. The colony forming units off the bacteria were measured. Also transmission electron microscopy (TEM) was employed to evaluate the effect of otitis media pathogens by PDT. The nucleus of Moraxella catarrhalis was stained using green fluorescent nucleic acid dye thiazole orange and the fluorescence intensity of the nucleus was measured by flow cytometry. The PDT was effective in killing Moraxella catarrhalis at the photogem dose of $5.0_{\mu}g/mL$, respectively, As assessed by flow cytometry analysis the fluorescence intensity of the nucleus got lower after PDT. TEM result appeared to able to cause damage to the bacterial membranes. On the basis of these findings, bacterial photodynamic therapy with photogem can be considered to be a promising new therapeutic approach for OME.

Enhancement of Anticancer Effect through Photodynamic Therapy with High Oxygen Concentration

  • In photodynamic therapy (PDT), oxygen plays important role. Because of singlet oxygen which is produced by activated photosensitizer after laser irradiation of specific wavelength. The aim of this study is to find how oxygen concentration affects anticancer effect in PDT. Groups were divided into PDT with oxygen applied group and only PDT applied group. PDT with oxygen applied group supplied oxygen for 15 minute before laser irradiation. In vitro, CT-26 cell was incubated with various concentration of photofrin $(50.0{\sim}0.05{\mu}g/ml)$ and was irradiated with 632nm diode laser 6hr after application of photofrin. The cell viability of two groups was assessed by MTT assay. In vivo, CT-26 cell line was transplanted into the subcutaneous tissue of BALB/c mouse. The anticancer effect of two groups was measured by tumor volume change. In vitro study, the cell viability was significantly decreased at $1.56{\sim}3.13{\mu}g/ml$ in PDT with oxygen applied group. In vivo study, the PDT with oxygen applied group significantly higher reduction rate of tumor volume 7 days after PDT compared to PDT only group. The high oxygen concentration might enhance the anticancer effect of the photodynamic therapy.

Photodynamic Therapy Using Topically Applied 5-ALA, MAL and CLC for Canine Otitis Externa

  • Canine otitis externa is a common disorder in small animal practice with prevalence up to 20%. In a large percentage of cases, canine otitis externa is a chronic and recurrent disease also associated with drug-resistant bacteria that is difficult to treat with traditional antibiotics. Photodynamic therapy (PDT) is a new strategy to exterminate pathogenic microorganisms such as bacteria and fungi. The purpose of this study was to investigate the effectiveness of photodynamic therapy against canine otitis externa using three photosensitizer (PS); 5-Aminolevulinic acid (5-ALA) and Methyl aminolevulinic acid (MAL) with semiconductor laser diode (SLD, 635nm of wave length), Chlorophyll-lipoid complex (CLC) with light-emitting diode (LED, 660nm of wave length). After PDT, dogs showed improved Otitis Index Score (OTIS) in swelling, exudate, odor, and pain. A result of the cytology test revealed decrease of bacteria and malassezia count in the oil immersion field and colony forming units count. PDT was effective as a bacteriocide of methicillin-resistant Staphylococcus pseudintermedius (MRSP) and a fungicide of Malassezia pachydermatis. MAL and 5-ALA were more effective PS against canine otitis externa than CLC. These results suggest that PDT is a new strategy to exterminate pathogenic microorganisms such as bacteria and fungi. PDT can be considered as a new therapeutic approach for canine recurrent otitis externa and a countermeasure to drug resistance that is a disadvantage of traditional antibiotic and antifungal therapy.
