A Case of Mutational Falsetto with Marked Contraction of Suprahyoid Muscles Treated with Botulinum Toxin (설골상근의 과도수축을 동반한 변성발성장애환자에 대한 보툴리눔 독소 주입 치료 1례)

  • The mutational falsetto is failure to change from the higher pitched voice of preadolescence to the lower pitched voice of adolescence and adulthood. The one of characteristic findings is contraction of suprahyoid muscles. The large majority of young men with inappropriately high voice have excellent voice therapy prognosis. We have experienced one case of mutational falsetto treated with botulinum toxin injection on suprahyoid muscles. His suprahyoid muscles are contracted markedly simultaneously with each phonation. fundamental frequency$(F_0)$ of his vowel phonation was 332Hz. Extensive voice therapy including manual compression of thyroid notch was ineffective. Forty units of Botox$^{\circledR}$ was injected under the EMG-guidance(20U bilaterally). At seven days post-injection, his voice changed lower than before and at 40 days after procedure, his $F_0$ was 126Hz.

  • Measuring phonatory stability using MDVP(Multi-dimensional voice program, Kay Elemetrics Corp., NJ, USA) are becoming popular in many Korean clinics and laboratories, yet questions about standardization and reference values have remained. The purpose of present study was to examine the effects of frequency and intensity variation on the results of acoustic analysis related to phonatory stability. Twenty young adults(ten females and ten males) were asked to sustain vowel /a/ for more than 3 seconds under 9 different pitch and loudness conditions. Using MDVP, nine voice samples were analyzed, and jitter percent, fundamental frequency variation, shimmer percent, peak amplitude variation, noise to harmonic ratio, amplitude tremor intensity index, and degree of subharmonics were compared. The results showed that intensity changes can significantly affect various phonatory stability measures, and the lowest perturbation values can be obtained from slightly louder(10dB) phonatory condition than comfortable level phonation.

  • Sasang Constitutional Medicine, a branch of traditional Korean medicine, believes that the health of human beings can be promoted by taking advantage of the fact that people have different constitutions. It utilizes the characteristics in human voice to diagnose the constitution of the patients. This study aims at establishing the relationship between Sasang constitutions and their corresponding voice characteristics by investigating source-related variables. Voice recordings of 23 patients from three different constitutions were obtained whose constitutions had been already diagnosed by the experts in the fields. Fundamental frequency related variables (average pitch, maximum/minimum pitch, pitch range), phonation type, speaking tempo were measured and analyzed for each group. The phonation type seemed to be a possible candidate for a successful variable to determine constitution. No statistically significant relationship was manifested between other variables and constitutions. Despite its failure to firmly establish the relationship between voice and constitutions, the current study suggests that future research should include not only source-related variables

  • Unilateral vocal cord palsy which is associated with laryngeal trauma is not uncommon event. In a 42-year-old male, a cricoid cartilage fracure had been developed after blunt trauma. The endoscopic findings showed contusion and diffuse swelling around the left arytenoid and false cord. During phonation, the mobility of left side true vocal cord was decreased. There were no level difference and displacement of the left side arytenoid. We used the laryngeal electromyography (LEMG) to make a differential diagnosis between the cricoarytenoid joint dislocation and the injury of recurrent laryngeal nerve. At the right thyroarytenoid muscle and cricothyroid muscle, the findings of LEMG were normal. But the amplitude and frequency during phonation were decreased (partial denervation) at the left thyroarytenoid muscle. LEMG is a very useful method to predict the diagnosis of vocal cord palsy.

Aerodynamic Characteristics of Whispered and Normal Speech during Reading Paragraph Tasks (문단낭독 시 속삭임 발화와 정상 발화의 공기역학적 특성)

  • The present study was performed to investigate and discuss the aerodynamic characteristics of whispered and normal speech during reading paragraph tasks. 39 normal females(18-23 yrs.) read 'Autumn' paragraph with whispered and normal phonation. Their readings were recorded and analyzed by 'Running Speech' in Phonatory Aerodynamic System(PAS) instrument. As results, during whispered speech, the total duration was longer and the numbers of inspiration were more frequently shown than normal speech. The Peak expiratory and inspiratory rate were higher in normal speech, but the expiratory and inspiratory volume were higher in whispered speech. By correlation analysis, both whispered and normal speech showed significantly high correlation between total duration and expiratory/inspiratory airflow duration; numbers of inspiration and inspiratory airflow duration; expiratory and inspiratory volume. These results show that whispered speech needs more respiratory effort but shows poorer aerodynamic efficacy during phonation than normal speech.

  • Dysarthrias are a sort of neuromotor disorders because of the weakness of neuromotor controls. They are classified in six types on the basis of Mayo clinic research: flaccid, spastic, ataxic, hypokinetic, hypekinetic and mixed types. Five dysarthria types are investigated in this study. MRI, EMG, neuropathological tests are essential diagnostic processing. Visi-Pitch and Spectrgraphy, CSL are used for assessing dysarthria speech. Maximum phonation time, diadochokinetic rate, Voice Onset Time and substitution rate are the speech evaluation parameters. Maximum phonation time and diadochokinetic rates are the lowest in case of spastic and ataxic dysarthrias. Spastic dysarthria shows the substituted glottalized consonants. However, flaccid, ataxic and hypokinetic dysarthrias show the substituted aspirated consonants. VOT is the longest for hypokinetic dysarthria and the shortest for ataxic dysarthria. Jitter shows higher percentage in comparison with control group. Speech evaluation using experimental phonetic instruments help create on international standardization of speech evaluation for speech disorders.

Intelligibility and Aerodynamic Study of Tracheopharyngeal and Tracheoesophageal Speechs (기관인두발성과 기관식도발성에 대한 이해도 및 공기역학적 검사)

  • Selected characteristics were compared in the speech of three tracheoesophageal, five tracheopharyngeal and ten normal laryngeal adult speakers. Tracheoesophageal speakers use Blom-Singer voice prosthesis after total laryngectomy and tracheopharyngeal speakers use tracheopharyngeal myomucosal shunt after near-total laryngectomy. Intelligibility judgement was based on standard Korean monosyllabic and bisyllabic word lists of 50 items. Aerodynamic study was composed of maximum phonation time, phonaton quotient. phonation pressure and mean air flow rate. Results indicate that intelligibility of tracheopharygeal speech is more similar to normal laryngeal speech than tracheoesophageal speech using Blom-Singer voice prosthesis.

A Clinical Study of Polypoid Vocal Fords (폴립양 성대의 임상적 연구)

  • A retrospective review of clinical records of 34 cases with polypoid vocal folds who underwent endolaryngeal microsurgery was carried out. The results were followings; 1) The incidence was high in elder age group(mean age : 53.7 years old). 2) Bilateral lesions were much more frequent than unilateral ones(67.6% vs. 32.4%). 3) Nine out of 34 patients had associated local pathologies and nine out of 34 patients had associated systemic pathologies. 4) Smoking appeared to be the most dominant predisposing factor(76.5%). 5) Maximal phonation time was decreased in 10 cases out of 14 cases(71.4%) and phonation quotient was increased in 9 cases out of 14 cases(64.3%). 6) All the cases were treated with sucking technique and the voice quality was improved in 32 cases(94.1%) The average duration for voice improvement was 2.8 months.

  • 최근 들어 음성에 대한 일반인들의 관심이 높아지면서 음성이상을 호소하며 이비인후과 외래를 방문하는 환자가 점차적으로 증가하는 추세에 있다. 그러나 이러한 음성이상을 객관적으로 평가할 수 있는 기준이 국내에는 거의 없는 실정이다. 이에 저자들은 본 연구에서 기왕력 상 폐질환이나 신경계질환, 후두질환을 앓은 적이 없으며 음성조율 기관에 이상이 없고 정상적인 청력을 가진 10세에서 69세까지의 무작위 추출된 정상 한국인 남자 112명과 여자 122명을 대상으로 Aerophone II voice function analyzer를 이용하여 공기역동학적 검사 중 최대발성지속시간(maximum phonation time), 평균호기류율(mean airflow rate), 발성율(phonation quotient), 발성시 기류량(phonatory flow volume) 및 성문하압(subglottal pressure)의 정상 한국인의 남녀 년령별 평균치를 측정하고자 하였다.   • Pop singers frequently use breathy voice to express diverse emotion. Such method is the major factor that divides vocalization in pop and classical music. Using breathy voice too much for a long time could make singer short-winded, which might cause the voice injured. In order to prevent the problem, singers need to find resonance balance between high notes and low notes and among vowels. Moreover, they should take more attention when producing resonance in consonants with much air flow. Along with finding resonance balance, if skilled at controlling the volume of voice, singers can use the breathy voice selectively at the desired part, adjusting close quotient freely. Through this kind of approach, pop singers can develope their unique style of vocalization and avoid the vocal damage.
