This study aims 1) To explicate the multidimensional structure of a widely used measure of subjective well-being, the Philadelphia Geriatric Center(PGC) Morale Scale is used to measure health in elderly populations and 2) To examine the relationship between the socioeconomic status and related variables, health and physical disability and subjective well-being in elderly populations. The selection of subjects was determined through a survey of 672 rural dwelling elderly persons(269 males and 403 females) aged 60 years and over. The respondents were interviewed by 18 trained health workers using the questionnaire from July 4 to July 9 in 1994. The subjects were surveyed again during the period from August 1 to August 6 in 1994 to conform the questionnaire's reliability. Subjective well-being was evaluated using the Revised Questionnaire of the Philadelphia Geriatric Center(PGC) Moral Scale(17 items, Lawton, 1975). The results are as follows : 1) The item scores were intercorrelated and subjected to a principal component analysis. A rotated three-factor solution was done, accounting for 40.9% of the total variance. Thus, the PGC morale Scale can be derived from three stable factors : Factor 1 was explained "Lonely Dissatisfaction(7 items)", Factor 2 was explained "Agitation (5 items)", Factor 3 was explained "Attitude Towards Own Aging (5 items)". Further, these factors have a high degree of internal consistency, as determined by Cronbach's alpha : 0.7852. 2) The total mean score of PGC Morale Scale was 8.73. Sex, Age, Education, Current disease, Family type, Economic status, ADL, IADL were significantly difference in mean scores of PGC morale scale. 3) In the results of stepwise multiple regression analysis of subjective well-being. the most contributing factors were economic status, IADL, current disease, family type, sex and the R square was 0.23.
Objectives: The objective of this study was to build COD regression models for the Han River and evaluate water quality. Methods: Water quality data sets for the dry season (as of January) during a four-year period (2012-2015) were collected from the database of the Han River automatic water quality monitoring stations. Statistical techniques, including combined genetic algorithm-multiple linear regression (GA-MLR) were used to build five-descriptor COD models. Multivariate statistical techniques such as principal component analysis (PCA) and cluster analysis (CA) are useful tools for extracting meaningful information. Results: The $r^2$ of the best COD models provided significant high values (> 0.8) between 2012 and 2015. Total organic carbon (TOC) was a surrogate indicator for COD (as COD/TOC) with high reliability ($r^2=0.63$ in 2012, $r^2=0.75$ for 2013, $r^2=0.79$ for 2014 and $r^2=0.85$ for 2015). The ratios of COD/TOC were calculated as 2.08 in 2012, 1.79 in 2013, 1.52 and 1.45 in 2015, indicating that biodegradability in the water body of the Han River was being sustained, thereby further improving water quality. The BOD/COD ratio supported these findings. The cluster analysis revealed higher annual levels of microorganisms and phosphorous at stations along the Hangang-Seoul and Hantangang areas. Nevertheless, the overall water quality over the last four years showed an observable trend toward continuous improvement. These findings also suggest that non-point pollution control strategies should consider the influence of upstreams and downstreams to protect water quality in the Han River. Conclusion: This data analysis procedure provided an efficient and comprehensive tool to interpret complex water quality data matrices. Results from a trend analysis provided much important information about sources and parameters for Han River water quality management.
Objective: The aim of this research was to investigate the effects of functional electrical stimulation (FES) applied to the gluteus medius (Gm) and tibialis anterior (TA) during stair climbing in persons with stroke compared to FES applied to the TA only during stair climbing, and during stair climbing without FES in persons with stroke. Design: Cross-sectional study. Methods: Twenty subjects with stroke participated in this study. Subjects were included if: 1) they were diagnosed as stroke at least 6 months before; 2) had Mini Mental State Examination- Korean score of 24 or higher; 3) were able to climb a flight of 10 stairs independently (with or without walking aid). The patients walked 10 stairs 3 times with FES applied to the Gm and TA, only TA, or no FES. There was a 1-minute rest period between each bout. The assessments were made using the Timed Up & Down Test and the Wii Balance Board. Results: Stair climbing with FES applied to the Gm and TA was significantly faster than stair climbing with FES applied to the TA only and without FES (p<0.05). Stair climbing with FES applied to the Gm and TA exhibited significantly greater sway velocity than stair climbing without FES (p<0.05). However, maximal sway distances were not significantly different between groups. Conclusions: Stair climbing with FES applied to the Gm and TA can be an important component of a rehabilitation program for improving stair climbing ability in persons with stroke.

Efforts are presently underway for developing an optimal design methodology for GEN-IV nuclear reactors based on target failure probabilities. A typical example is the system-based code, in which the results are represented in the form of partial safety factors (PSFs). Thus, a PSF is one of the crucial elements in either component design or integrity assessment based on target failure probabilities during the operation period. In the present study, a procedure for calculating the PSF of a circumferential through-wall cracked pipe based on the elastic-plastic crack initiation criterion is established, in which the importance of each input variable is assessed. Elastic-plastic J-integrals are calculated using the GE/EPRI and reference stress methods, and the PSF values are calculated using both first- and second-order reliability methods. Moreover, the effect of statistical distributions of assessment variables on the PSF is also evaluated.
This paper presents the flow and the suspended sediment movement over ripples for oscillatory flows. A new numerical model system is developed, and applied to a laboratory experimental condition of regular waves and a fictitious condition of irregular waves. The flow field is obtained from a programme proposed by Kim et. al.(1994), which is a modified version of SOLA based on SMAC scheme. The sub-model solves the continuity and Reynolds momentum equations in the x-z plane. The wave orbital velocities, shear stresses, and pressure are all reasonably reproduced by the model. The model results on the vertical velocity component show good agreement with the measurements. The suspended sediment transport sub-model is newly set up to solve the advection-diffusion equation of suspended sediment using a split method, and involving a special shear entrainment from the whole ripple surface. The calculated suspended sediment concentrations for regular waves show reasonable agreement with measurements at Deltaflume. The model results for random waves show that the suspended sediment concentration is higher than those for regular waves and that the sediment diffuses higher than for regular waves with the significant wave height and the peak wave period of the irregular waves.
Dyslipidemia is a component of the metabolic syndrome and a risk factor for cardiovascular diseases. Nutrition counseling is important to improve dyslipidemia. The purpose of this study was to evaluate the effectiveness of nutrition counseling in adults with risk factors for dyslipidemia diagnosed by the national health screening program. The nutrition counseling for adults with risk factors for dyslipidemia was carried out at a public health center in Gyeonggi-do. Thirty four patients out of forty five participants in the program completed the nutrition counseling program. The nutrition counseling was provided 3 times during a 12-week period. Individualized nutrition counseling to improve dietary habits was conducted after examining participants' dietary intake through questionnaires about dietary habits and whether they practice dietary guidelines. Data about serum lipid profiles, body composition, nutrition knowledge, the practice of dietary guidelines, and dietary behavior were collected before and after nutrition counseling to evaluate the effectiveness of nutrition counseling. All data were statistically analyzed by SPSS program (Korea ver.18.0) and significant difference was evaluated by paired t-test and ${\chi}^2$-test. Body weight, body fat and WHR were significantly decreased after nutrition counseling. Total-cholesterol, TG, and LDL-cholesterol were significantly decreased but HDL-cholesterol did not show significant changes. Both scores of nutrition knowledge and the practice of dietary guidelines improved significantly (p < 0.001). This study shows that nutrition counseling helps to encourage healthy eating practices and to improve serum lipid profiles of adults with risk factors for dyslipidemia. Overall, results indicated that nutrition counseling resulted in positive changes to lower the reliance on medications. Therefore, nutrition counseling should be considered for the initial treatment of dyslipidemia.

Temperatures of engine head and liner depend on many factors such as spray and combustion process, coolant passage flow and engine related structures. To estimate the temperature distribution of engine structure, multi-dimensional computational fluid dynamics (CFD) codes have been mainly adopted. In this case, it is of great importance to obtain the realistic wall temperature distribution of entire engine structure. In the present work, a CFD-FEM coupling methodology was presented to address this demand. This approach was applied to a real large-size marine diesel engine. CFD combustion and coolant flow simulations were coupled to FEM temperature analysis. Wall heat flux and wall temperature data were interfaced between combustion simulation and solid component temperature analysis via translator by a commercial CFD package named FIRE by AVL. Heat transfer coefficient and surface temperature data were exchanged and mapped between coolant flow simulation and FEM temperature analysis. Results indicate that there exists the optimum cell thickness near combustion chamber wall to reasonably predict the wall heat flux during combustion period. The present study also shows that the effect of cell refining on predicting in-cylinder pressure during combustion is negligible. Hence, the basic guidance on obtaining the wall heat flux needed for the reasonable CFD-FEM coupling analysis has been established. It is expected that this coupling methodology is a robust tool for practical engine design and can be applied to further assessment of the temperature distribution of other engine components.
In the present study, we made measurements of PM-bound metal concentrations from seven different urbanized locations in Seoul for the period covering March 2001 through May 2002. The measurement data were analyzed to explore the possible influences of spatial factors on metal distribution characteristics. To check for the importance of such aspects on metal distribution characteristics, the measured data were compared between different metals and between different sites by several criteria including (1) coefficient of variation (CV) values; (2) temporal variability; and (3) the abundance of strongly correlated pairs. The overall results of our study indicate strong diversity in the distribution characteristics of different metals. It is found that some metals (like Fe, Mn, and Pb) tend to exhibit strong compatibility among different study sites. However, no such compatibility appears to exist for certain metals like Cu. To account for the importance of spatial factors, complex relationships between source/sink processes and geochemical characteristics of a given metallic component may have to be examined in a systematic manner.
Chronic postsurgical pain (CPSP) is an unwanted adverse event in any operation. It leads to functional limitations and psychological trauma for patients, and leaves the operative team with feelings of failure and humiliation. Therefore, it is crucial that preventive strategies for CPSP are considered in high-risk operations. Various techniques have been implemented to reduce the risk with variable success. Identifying the risk factors for each patient and applying a timely preventive strategy may help patients avoid the distress of chronic pain. The preventive strategies include modification of the surgical technique, good pain control throughout the perioperative period, and preoperative psychological intervention focusing on the psychosocial and cognitive risk factors. Appropriate management of CPSP patients is also necessary to reduce their suffering. CPSP usually has a neuropathic pain component; therefore, the current recommendations are based on data on chronic neuropathic pain. Hence, voltage-dependent calcium channel antagonists, antidepressants, topical lidocaine and topical capsaicin are the main pharmacological treatments. Paracetamol, NSAIDs and weak opioids can be used according to symptom severity, but strong opioids should be used with great caution and are not recommended. Other drugs that may be helpful are ketamine, clonidine, and intravenous lidocaine infusion. For patients with failed pharmacological treatment, consideration should be given to pain interventions; examples include transcutaneous electrical nerve stimulation, botulinum toxin injections, pulsed radiofrequency, nerve blocks, nerve ablation, neuromodulation and surgical management. Physical therapy, cognitive behavioral therapy and lifestyle modifications are also useful for relieving the pain and distress experienced by CPSP patients.
Background: Hemiplegic patients usually have difficulty maintaining balance. Balance training is a major component of there habilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of PLS(Posterior Leaf Spring), FES(Functional Electrical stimulation), treadmill training, and neurodevelopmental treatment on the improvement of balance in patients with hemiplegia. Methods: We looked into published studies from Dankook University’s electronic library databases of RISS4U, KMbase, NCBI, and MEDLIS concerning the effectiveness of any form of intervention leading to improvement of balance. All types of studies relevant to the topic that were published in English during the time period of 1986 to 2005 were included. Results: 1. There were significant differences in standing balance, dynamic activity balance, and gait speed between barefoot subjects and subjects who wore SPAFO and HPAFO(p<.05). 2. The changes in ROM and FRT related to sex, age, height, and weight part of the diagnosis, as well as experience relapse, was of meaningless value. Changes in ROM related to the duration of pain and experiences of falling down were also meaningless. However, FRT showed significant static differences(p<.05). 3. The body-weight-support treadmill training scoring of standing balance, step length, and a timed 10m walking test showed definite improvement. 4. The proprioceptive control approach improved dynamic balance in patients with hemiplegia. Conclusion: Consequently, further study is needed to verify methods when physical therapists are researching balance ability in hemiplegic patient.
