Spontaneous Disappearance of a Pericardial Cyst: Case Report and Literature Review

  • Pericardial cysts are rare benign anomalies generally discovered as incidental findings on radiographic images. Rarely, pericardial cysts cause symptoms and may lead to complications. A 56-year-old woman presented to the emergency department for mild chest pain. A cardiovascular and respiratory examination revealed no abnormalities, while a chest X-ray and subsequent thoracic computed tomography (CT) showed a pericardial cyst. The patient refused both percutaneous treatment and thoracic surgery. Three years later, a thoracic CT scan showed that the pericardial cyst had disappeared. Although the spontaneous resolution of these lesions is rare, this article highlights the possibility of conservative management in select cases.

  • Tuberculosis developed in the pericardium usually occurs as diffuse constrictive pericarditis or effusive pericarditis which contains much pericardial effusion. But types such as localized abscess or tuberculoma are very rare. Myocardial tuberculosis is also very rare and mainly extended directly from hilar lymph node or spreaded hematogenously in miliary tuberculosis. It is known to be able to make arrhythmia or heart failure by invasion of conduction system or myocardial muscle mass, but it is usually discovered by incidental postmortem autopsy and rarely concerned by clinical basis. Recently we have experienced a case of localized intrapericardial tuberculous abscess which extended to myocardium and operated that successfully. So we would report them with reference study.

심내막염 환자에서의 Aorto-mitral fibrous skeleton의 재건술 -치험 1례- (Reconstruction of Aorto-mitral Fibrous Skeleton in Complicated Native Valve Endocarditis -A Case Report-)

  • This is a report of successful management of a patient with complicated native valvular endocarditis. Initially stable patient showed sudden collapse at the end of 4th week of antibiotics coverage. Echocardiography revealed that previous vegetation at the Aorto-mitral Fibrous Skeleton[AMFS developed into a false aneurysm, perforated to left atrium and caused fistulous communication between left ventricle and left atrium. Extensive debridement was performed including part of the ascending aorta, aortic cusps, the AMFS, anterior mitral cusp and roof of the left atrium. Reconstruction of the AMFS with tailored single piece of autologous pericardium enabled the implantation of mechanical valves at the aortic and the mitral position. Ascending aorta and roof of the left atrium were repaired with autologous pulmonary artery patch graft and another autologous pericardial patch. The patient was discharged on postoperative 16th day and followed - up till now without any residuae or sequelae.

Surgical Reconstruction for High-Output Chylothorax Associated with Thrombo-Occlusion of Superior Vena Cava and Left Innominate vein in a Neonate

  • We report a case of high-output chylothorax associated with thrombo-occlusion of the superior vena cava (SVC) and left innominate vein (LIV) following an arterial switch operation in a neonate. The chylothorax was resolved by 3 weeks after surgical reconstruction of the SVC and LIV using fresh autologous pericardium. We confirmed the patency of the SVC and LIV with a 1-year follow-up computed tomographic scan at our outpatient clinic.

  • To develop the supplementary heart valve which could reduce the complications of prosthetic heart valve, in vitro experiments were performed to analyze the movement of the supplementary valves. The autologous pericardium was attached to deficient portion of the porcine valve which was dissected completely and partially. The instantaneous motion of the leaflet was pictured by the 35mm camera and the pressure drop through the valve and cardiac output were measured in mock circulatory system. The durability of the valves were tested in severe hemodynamic conditions.

  • Pericardial rupture due to blunt trauma is very rare, but can lead to serious complications. It occurs mainly on the left, is found incidentally during surgery, and is seldom discovered radiologically unless accompanied by cardiac herniation. The following case describes a 53-year-old traffic-accident victim who received emergency pericardial repair and bleeding control via an exploratory thoracotomy and an exploratory laparatomy. The patient was discharged without any complication and remained healthy at six month after injury.

  • This presents a case of anomalous pulmonary venous drainage from the right lung to the inferior vena cava [scimitar syndrome], which was corrected with an intracardiac approach. Autogenous pericardium was used to create an intracardiac tunnel redirecting flow from the anomalous vein to the left atrium through a surgically enlarged atrial septal defect. The postoperative results have veen satisfactory.

  • 12년 전 23mm 기계판막과 소심낭편으로 작은 대동맥륜과 심실중격 및 우심실유출로를 확장하는 Konno씨 수술을 받은 28세된 남자가 갑작스런 심부전증으로 전원되었다. 대동맥판막 상부와 하부의 대동맥과 심실 중격을 복원한 소심낭편에 천공들이 있었는데, 그 심낭편은 심하게 석회화 되고 변성되어 있었다. 그 천공들은 석회화 되고 변성되어 딱딱해진 소심낭편과 기계판막 자체의 유동성에서 생긴 균열에 의한 것으로 보였다. PTFE 이식편을 이용하여 Konno씨 수술을 다시 시행하였다.

  • Norwood 시술에 있어서 체순환 혈관을 넓히는 데 동종 혈관을 이용한 방법이 많이 알려져 있으나, 동종 혈관은 숫자적으로 제한적이고 구하기 어려운 점이 있다. 본원에서는 생후 23일로 여러 부위의 좌심실 유출로 협착이 있는 남아(2.2 kg)와 생후 9일로 좌심실형성부전증 남아(3.0 kg)에서 우심낭으로 체순환 유출로 모양을 재단하여 사용하였다. 이 방법은 술식이 쉽고 재현 가능성이 높으며 동종혈관처 럼 매우 효과적이다.

  • 데스모이드는 재발빈도가 높은 양성 종양이다. 본 저자들은 흉강 및 복강에 걸쳐 있는 거대한 데스모이드를 충분한 절단면을 확보한 후 절제하고 간좌엽절제술, 좌폐하엽 쐐기절제술, 부분적 심막절제술 후 우심막을 이용하여 심막재건술을 시행하였고 횡격막을 부분 절제한 후 마렉스 메쉬로 횡격막 재건술을 시행하였다.
