Role of Probiotics in the Treatment and Prevention of Common Gastrointestinal Conditions in Children

  • Probiotics are live microorganisms that confer health benefits to the host when administered in adequate amounts. Although recommendations for probiotic use should be strain-specific, many systematic reviews, including recommendations from different societies, recommend probiotic use in general, providing no relevant information for healthcare professionals regarding which probiotic to recommend for which clinical indication, at what dose, and for how long. This narrative review aimed to present the available evidence on the use of probiotics in the prevention and treatment of common gastrointestinal diseases in children, considering the strain and dose used. Furthermore, this study summarizes the evidence on the possible side effects and quality of products containing probiotics.

Clostridium difficile in Children: To Treat or Not to Treat?

  • Clostridium difficile infection has been increasing since 2000 in children and in adults. Frequent antibiotics use, comorbidity, and the development of hypervirulent strains have increased the risk of infection. Despite the high carriage rates of C. difficile, infants rarely develop clinical infection. Discontinuing antibiotics and supportive management usually leads to resolution of disease. Antibiotics use should be stratified depending on the patient's age and severity of the disease.

Diffuse alveolar hemorrhage and recombinant factor VIIa treatment in pediatric patients

  • Diffuse alveolar hemorrhage (DAH) is a life-threatening pulmonary complication in patients with hematologic malignancies or autoimmune disorders. The current treatment options, which include corticosteroids, transfusions, extracorporeal membrane oxygenation (ECMO), and immunosuppressants, have been limited and largely unsuccessful. Recombinant activated factor VII (rFVIIa) has been successfully administered, either systemically or bronchoscopically, to adults for the treatment of DAH, but there are few data on its use in pediatric patients. The current literature in the PubMed database was reviewed to evaluate the efficacy and risk of rFVIIa treatment for DAH in pediatric patients. This review discusses the diagnosis and treatment of DAH, as well as a new treatment paradigm that includes rFVIIa. Additionally, the risks and benefits of off-label use of rFVIIa in pediatric patients are discussed.

연세대학교 치과대학병원 소아치과의 진정법의 변화 양상 (The Changes of Sedation in the Department of Pediatric Dentistry, Yonsei University Dental Hospital)

  • 진정작용은 행동조절이 어렵고 광범위한 치료가 필요하거나 의학적 문제가 있는 어린이의 치과 치료에 유용한 행동조절 방식이다. 본 연구의 주된 목적은 연세대학교 치과대학병원 소아치과에서 사용하는 진정제와 진정치료의 경향을 분석하여 진정법의 사용의 변화를 살펴보는데 있다. 본 연구는 2011년 - 2016년까지 연세대학교 치과대학병원 소아치과의 전자의무기록(EMR)을 바탕으로 환자 및 진정법을 검토하였다. 연구 결과에 따르면, 진정법으로 치료받은 환자들의 수가 일반적으로 증가했다. 남자가 여자보다 진정법을 이용한 치과 치료를 더 많이 받았으나 2015년부터는 유사한 비율로 적용되고 있다. 전신질환(ASA II) 환자의 치료 비율은 2013년에 감소했지만, 이후에는 일정하게 유지되었다. 4 - 5세 및 6 - 10세의 진정 치료 비율은 일반적으로 증가하는 반면, 0 - 2세 및 11세 이상은 감소했다. 3세는 비교적 일정한 비율로 유지되고 있다. 가장 자주 사용되는 진정법은 아산화질소 흡입진정법이었으며, chloral hydrate의 사용이 감소하는 동안 benzodiazepine계열의 사용이 증가했다. 최근에는 행동관리 기법으로 진정법이 확대되고 있다. 그러나 안전한 사용을 위하여 다차원적 접근이 요구되고 있으며, 진정제 사용의 안전성과 효능에 대한 추가 연구 역시 동반 수행되어야 한다.

진정요법사용에 관한 설문조사 (SEDATION PRACTICES IN DENTAL OFFICE : A SURVEY OF MEMBERS OF THE KOREAN ACADEMY OF PEDIATRIC DENTISTRY)

  • 치과에서 행해지는 진정요법이 안전하게 수행되기 위해서는 환자에 대한 모니터가 필수적이다. 우리나라에서 소아치과의사에 의해 이루어지는 진정요법에 관해 알아보고자 대한소아치과학회 회원 245명을 대상으로 1998년 10월 1일부터 1998년 12월 20일까지 진정요법의 사용여부 및 사용하는 진정요법의 빈도, 방법, 사용하는 약물, 용량 결정, 모니터 여부, 응급장비 등에 관한 설문조사를 실시하여 다음과 같은 결론을 얻었다. 1. 설문지 응답자의 29%에서 진정요법을 이용하여 치료를 하는 것으로 조사되었다. 설문지 응답자의 71%에서는 진정요법을 하지 않는 것으로 조사되었다. 2. 진정요법을 사용하는 사람 중 61%에서만 진정요법 치료시 모니터를 사용하는 것으로 조사되었다. 진정요법을 사용하는 사람의 39%에서는 진정요법 치료시 모니터를 사용하지 않는 것으로 조사되었다. 3. 가장 흔하게 사용되는 진정요법은 아산화질소-산소를 이용한 흡입진정요법이었다. 그 외에 경구 투여시 가장 흔하게 사용되는 약물은 chloral hydrate였다. 4. 진정요법을 사용하는 사람의 28%에서만 동의서를 작성하는 것으로 조사되었다. 5. 진정요법을 이용하여 치료를 한다고 응답한 사람의 60%에서만 응급장비를 구비하고 있으며, 응답자의 91%에서 응급처치에 관한 교육이 필요하다고 응답하였다.

Recent Achievements in Stem Cell Therapy for Pediatric Gastrointestinal Tract Disease

  • The field of stem cell research has been rapidly expanding. Although the clinical usefulness of research remains to be ascertained through human trials, the use of stem cells as a therapeutic option for currently disabling diseases holds fascinating potential. Many pediatric gastrointestinal tract diseases have defect in enterocytes, enteric nervous system cells, smooth muscles, and interstitial cells of Cajal. Various kinds of therapeutic trials using stem cells could be applied to these diseases. This review article focuses on the recent achievements in stem cell applications for pediatric gastrointestinal tract diseases.

소아환자 보호자의 소아 대상 약물사용 관련 지식, 태도, 행동 분석 연구 (Analysis of Knowledge, Attitudes, and Practices for Safe Medication Use in Caregivers for Pediatric Patients)

  • Background: The safe use of medication in children is important, as it can have significant implications for their health and wellbeing. Parents or caregivers play a crucial role in the medication administration process. Therefore, this study would evaluate the needs and current status (knowledge, attitude, and practice) of caregivers for children on the safe use of medication. Methods: We conducted questionnaires on 150 caregivers with children under nine years old and asked about their knowledge, attitude, and practice regarding the safe use of medication and their needs. Each question was conducted on a 5-point scale. Results: Of the 150 respondents, 128 were women, 75 were in their 30s, and 63 were in their 40s. Among them, 29 parents answered that they had been educated about the safe use of medicines, and 26 (84%) were satisfied with the education. Knowledge, attitude, and practice assessments on drug safety use (mean±SD) were found to be 3.83±0.44, 3.67±0.38, and 3.53±0.45, respectively. There were significant differences between the educated group and the uneducated group in knowledge and practice, not attitude. Conclusion:To provide appropriate education to caregivers of pediatric patients is necessary to ensure the safe and effective use of medication in children. Moreover, appropriate education can improve caregivers' understanding of medication use and lead to proper administration. Therefore, it is imperative to develop educational materials and strategies for medications that incorporate both specialized knowledge and practical applications for pediatric patients.

Use of Anti-TNF Alpha Blockers Can Reduce Operation Rate and Lead to Growth Gain in Pediatric Crohn's Disease

  • Purpose: Pediatric Crohn's disease (CD) is directly related to growth and has a high probability of requiring surgical intervention(s); therefore, more active treatment for CD is required for children. This study investigated the impact of biologics on growth and disease course associated with surgery. Methods: This was a retrospective cohort study involving patients diagnosed with CD at the Seoul National University Children's Hospital (Seoul, Korea) between January 2006 and October 2017. The aim was to determine the characteristics of pediatric patients with CD and whether biologics affected growth and the surgical disease course. Results: Among patients who underwent surgery for CD, the mean number of operations per patient was 1.89. The mean time from initial diagnosis to surgery was 19.3 months. The most common procedure was fistulectomy (34%), followed by incision and drainage (25%). In all patients, the use of biologics increased the height (p=0.002) and body mass index (BMI) (p=0.005). Among patients who underwent surgery, height (p=0.004) and BMI (p=0.048) were increased in the group using biologics. Patients who used biologics exhibited a low operation rate only within 2 years after diagnosis, with no differences thereafter (p=0.027). Conclusion: Although biologics could not mitigate the operation rate in pediatric patients who underwent surgery for CD, biological therapy delayed disease progression within 2 years of disease onset. Additionally, biologics conferred growth and BMI benefits in this window period. Therefore, it may be helpful to use biologics for optimal growth in pediatric patients with a high probability of undergoing future surgery.

일본에서 소아질환에 적용하는 한약치료에 대한 문헌고찰 (A Literature Study of Kampo Drug Treatment for Children in Japan)

  • Objectives : This research aimed to analyze studies on pediatric disease treated by kampo drug, kind of kampo drug used in children, treatment period, and the result of kampo drug treatment for children in Japan. Methods : We got 263 search result with searching word 'kampo medicine' and '小兒', 'children', '乳兒' in J-stage. We selected 34 articles among them which were related to objective of research to analyze studies by type of pediatric disease treated with kampo drug, kinds of kampo drug for each disease, treatment period and result of kampo drug treatment for children. We considered frequency of kampo drug use & pediatric disease treated with kampo drug and significance of research. Results : According to analyzed results, respiratory diseases are the most frequent diseases that are healed by kampo drug. Next sequenced diseases are skin disease. In Kind of kampo drug for pediatric disease. Goreisan and Shosaikoto (柴胡桂枝湯), Shokenchuto (小建中湯) are used frequently. Also, various disease treated with kampo drug were improved. Conclusions : Japanese Doctors consider the Kampo drug is safe and has a lot of merit compared to modern medication. Especially for symptom with unknown origin & immune diseases such as upper respiratory tract infections. Referring to clinical cases of kampo drug in Japan, we will use kampo drug for various pediatric diseases in future.

한국 소아치과 전문의 수련과정의 진정법 실태조사 (A Survey of Sedation Practices in the Korean Pediatric Dentistry Residency Program)

  • Background: Recently the use of sedation by pediatric dentists in Korea is increased. This study describes training programs about sedation practices in Korean pediatric dentistry residency program. Methods: A questionnaire was filled in by participants of Korean Academy of Pediatric Dentistry on 17th-18th August, 2008. Also the data about sedation practices of the training institution is collected by phone call. Results: Seventy two percent of respondents used sedation. Most of them used sedation with agents under 25% of their patients. Distribution of ages in patients sedated with agents was 3 years, 4-5 years, under 2 years, 6-10 years, and more than 10 years. Determinative factors of using sedation were behavior management, number of visiting, amount of treatment and general condition, and oral route was the most favorable route. Sixty six percent of them have failed on sedation, and thity percent of them have rarely failed on sedation. Only fifty percent of dentists using sedation completed the cardiopulmonary resuscitation course. Conclusions: For safety, dentists using sedation need to be educated about emergency equipment and management. Especially medication dose, use frequency and the detail related to treatment procedure should be discussed carefully. Also putting a patient under general anesthesia and taking emergency measures should be discussed with Korean Dental Society of Anesthesiology.
