대구지역 치과환자들의 치과 의료기관 선택기 (Factors for Choice on Dental Care Facilities among Dental Patients in Dageu)

  • The purpose of this study is to identify if dental patients discriminate between dental clinic and dental hospital and the related factors for choice of dental facilities. A self-administered questionnaire survey was conducted from March 21 to April 11, 2011. Among 430 dental patients who visited dental clinic or dental hospital twice or more in Daegu, 410 dental patients were included in analysis. A total of 73.6% of the dental clinic visitors recognized correctly that the visiting facility was dental clinic and 82.3% of dental hospital visitors did correctly. Therefore, it can be said that dental hospital visitors recognize better the type of dental care facilities they visit. When it comes to choosing the dental facility, there were not much differences between the clinic and hospital patients in this study. Human resources, facilities and equipment, service are factors for choosing dental clinic and dental hospital in order. However, modern dental facility and equipment were more important factor for patients to choose dental hospital than clinic. It will be necessary for dental clinic or dental hospital to develop its own specific service to fulfill dental patients' needs through further studies on factors for choosing dental facilities.

전국민을 대상으로 한 한의원과 한방병원 외래이용환자의 이용실태 및 특성비교연구 - 2011년 한방의료이용 및 한약소비실태조사 보고서(보건복지부)중 이용환자의 질병치료방법 및 치료효과를 중심으로 - (Nationwide Study on the Usage and Characteristics of Patients Visiting Korean Medical Facilities - Based on the Treatment of Major Disorders, Effectiveness, Satisfaction and Occurrence Rate of Side Effects From the Ministry of Welfares Report on Usage and Consumption of Korean Medicine in 2011 -)

  • A survey conducted on 1,103 patients or caretakers visited Korean medical clinics and hospitals between August 25, 2011 to September 30, 2011 by the Ministry of Health and Welfares and Korea Institute for Health and Social Affairs was analyzed and obtained following results: 1. For sociodemographic distribution, female(813) outnumbered male(290) patients with majority of patients ranging from 30s to 60s. Married patients(793) outnumbered unwed(150) patients and 65.0% with higher than high school education. Statistical significance was seen in gender, age, marital status, and education level but no significant difference for status of employment, income level, and types of insurance between the clinics and hospitals. (P<0.05). 45.1% had less than \2,000,000 in monthly salary and most of coverage was provided by either regional or work insurance. 2. 67.9% of the patients rated health conditions to be better than average and 32.1% listed as poor. People in good health showed tendency to visit Korean medical facilities. Musculo-skeletal conditions such as arthritis, ankle sprain, lumbago, muscular injury, and frozen shoulder were common conditions, followed by gastric disorders, common cold and herbal tonics. No significant difference was observed between the clinics and hospitals for above conditions, but significance was seen in atopic dermatitis, stroke, and sequela from traffic accidents (P<0.05). 3. Ten most common conditions addressed at Korean medical facilities were lumbago, arthritis, muscular injury, back sprain, gastric disorders, ankle sprain, common cold, herbal tonics, frozen shoulder and stroke. Major treatment modalities rendered were herbal medicine, herbal supplements, acupuncture and moxibustion, cupping, tuina, and Korean midical physical therapy. No significant difference existed between the clinics and hospitals. 4. All modalities showed at least 85% effectiveness. No statistical significant difference between the clinics and hospitals except for herbal decoction. (P=0.0452) 5. 88.3% of responses showed treatment satisfaction with significant difference between the clinics and hospitals (P=0.002). The occurrence of side effects was at 2%, mostly corning from treating digestive, skin, kidney disorders and neurological issues. No significant difference was observed between the clinics and hospitals. From the above results, the typical population visiting Korean medical facilities can be summarized as being middle aged female with relatively higher education and moderately low income. The health condition is generally good and the purpose of visit is to receive traditional treatments of acupuncture, herbal medicine, and physical therapy. Treatment efficacy and satisfaction were high with no significant differences between the clinics and hospitals.

Prevalence of Depressive Disorder of Outpatients Visiting Two Primary Care Settings

  • Objectives: Although the prevalence of depressive disorders in South Korea's general population is known, no reports on the prevalence of depression among patients who visit primary care facilities have been published. This preliminary study was conducted to identify the prevalence of depressive disorder in patients that visit two primary care facilities. Methods: Among 231 consecutive eligible patients who visited two primary care settings, 184 patients consented to a diagnostic interview for depression by psychiatrists following the Diagnostic and Statistical Manual of Mental Disorders-IV criteria. There were no significant differences in sociodemographic characteristics such as gender, age, or level of education between the groups that consented and declined the diagnostic examination. The prevalence of depressive disorder and the proportion of newly diagnosed patients among depressive disorder patients were calculated. Results: The prevalence of depressive disorder of patients in the two primary care facilities was 14.1% (95% confidence interval [CI], 9.1 to 19.2), with major depressive disorder 5.4% (95% CI, 2.1 to 8.7), dysthymia 1.1% (95% CI, 0.0 to 2.6), and depressive disorder, not otherwise specified 7.6% (95% CI, 3.7 to 11.5). Among the 26 patients with depressive disorder, 19 patients were newly diagnosed. Conclusions: As compared to the general population, a higher prevalence of depressive disorders was observed among patients at two primary care facilities. Further study is needed with larger samples to inform the development of a primary care setting-based depression screening, management, and referral system to increase the efficiency of limited health care resources.

충남에서 발생한 중증응급환자의 응급의료기관 일차 방문, 전원 및 재전원 현황 (Primary visit, transfer, and re-transfer to emergency department in patients with severe emergency diseases in Chungnam)

  • Objective: Regionalization is one of the principal subjects for the advancement of rural emergency medical service systems in South Korea. This study shows the characteristics of interhospital transfer and status of the incidence of three major emergency disorders (acute myocardial infarction [AMI], acute stroke, and severe trauma) in one local province. Methods: A retrospective study was conducted for patients with three major emergency disorders who visited emergency medical facilities in one local province from January 2013 to December 2015, on the basis of the National Emergency Department Information System (NEDIS) data. Results: The incidence of three major emergency disorders had increased annually. Patients with each of these disorders tended to choose distinguishing methods of visiting emergency medical facilities. AMI patients tended to visited emergency medical facilities using private cars or on foot, while severe trauma patients usually visited by 119 ambulance, and acute stroke patients used 119 ambulance and private car in similar amounts. Overall, 65% of AMI patients were treated in intraregional medical facilities, but about 70% of acute stoke and severe trauma patients were transferred outside of the region. Conclusion: Because each of these disorders has an individual characteristic, it is difficult to expect a solution for the problems associated with emergency disorders just by assuring the availability of medical resources. Based on regionalization, a policy to provide the optimal treatment for those emergency disorders should be developed by planning public medical service systems based on the individual characteristics of emergency disorders, the standardized transfer plans of emergency patients and the assurance for mobilization and sharing of confined medical resources.

일개 치과대학병원 응급실 운영시간 변경에 따른 환자의 방문시기와 주요상병의 차이 (Difference of visiting time and major cause of disease by operating time changes of a university dental emergency room)

  • Objectives: The purpose of the study is to investigate of visiting time and major cause of disease by operating time changes of a university dental emergency room. Methods: This study was a retrospect study carried out by reviewing 9,172 records visiting the university emergency room from January 1997 to December 2009. Data were analyzed by frequency test, chi-square test, and logistic regression using SAS version 9.3. Results: Twenty four hours emergency room had 19 percent more patients than those in daytime emergency room. Daytime emergency room had more male patients during spring(26.6%) and winter(20.2%) than female patients(p<0.05). Female patients exceeded male patients during summer(24.4%) and autumn(36.2%)(p<0.05). Twenty-four hours emergency room had the maximal patients in autumn and the minimal patients in winter(p<0.05). The most common injury in the daytime was tooth fracture from 0 to 19 years old(p<0.05). Tooth fracture, jaw fracture, and soft tissue injury were the most common injury in dental emergency room and majority of the patients were the male(p<0.05). Conclusions: Twenty-four hours dental emergency room had a variety of causes of dental emergency thant that in the daytime. Twenty four hours dental emergency room must be prepared for the dental emergency patients at any time provided with personnel and facilities.

전국민을 대상으로 한 한방의료기관 이용환자의 성별 이용실태 및 특성비교연구 (Nationwide Study on the Characteristics of Patients Visiting and Using Korean Medical Facilities by Sex)

  • Objectives: This study aimed to analyze the utilization of Korean medical clinics by sex. Methods: Data was based on reports about usage and consumption of Korean medicine in 2011 by the Ministry of Health and Welfare and Korea Institute for Health and Social Affairs. 171 Korean medical hospitals and more than 300 Korean medical clinics were selected after 2 rounds of extraction, from the 12,250 Korean medical institutions throughout the country. The investigation was carried out between August 25 and September 30, 2011. The study group was composed of 290 men and 813 women; Korean medical institutions including clinics and hospitals were mainly used for their health care. Results: 1. For sociodemographic distribution, sex, marital status, education level, occupation status, and income level showed statistically significant differences. 2. Females had a lower subjective health status than did males. Muscle injury, hwa-byung, gastric disorder, lumbago, common cold, weight loss, and herbal tonics showed differences in prevalence rates between the sexes. 3. Arthritis, muscle injury, gastric disorder, ankle sprain, herbal tonics, and stroke showed differences in usage rates between the sexes. Likewise, major treatment methods had differences between the sexes. 4. There were no statistically significant differences between the sexes concerning acupuncture. Conclusions: Females were lower than males in several aspects of income and education level and occupation status. Males had a higher subjective health status than did females. Differences by sex of treatment methods and prevalence rates of several diseases existed.

고혈압 환자의 연간 내원일수, 처방일수 그리고 진료비 (Annual Visit Days, Prescription Days and Medical Expenses of Hypertensive Patients)

  • Objectives : To evaluate the annual visit days, the annual prescription days and the medical costs of hypertensive patients. Methods : The medical insurance records of 40,267 incident patients with the diagnostic code of hypertension from September 1998 through August 1999 in Daegu city were reviewed. Results : The proportion of the most proper medical care pattern group (Group VIII) who visited for 6-15 days with 240 prescription days or more a year was only 6.2%. The proper care group (Group IX) who visited for more than 16 days with 240 prescription days or more a year was 9.3%. The overall proper care group (Group VIII+IX) was therefore 15.5%. The proportion of the insufficient care group (Group I, IV) in both the number of visiting days and prescription days was 57.4%. The mean prescription day of the most proper group (Group VIII) was 29 days; the mean annual medical expenses,453,587won; the mean annual amount paid by patients, 218,013won; and mean medical expenses per prescription day, 1,483won. The proportion of the overall proper care group (Group VIII+IX) was significantly higher in adults aged 50-59, those who were enrolled in industrial workers health insurance as well as government employees and private school teachers health insurance, and those who made a higher contribution per month (p<0.01). According to the type of medical facilities, the proportion of the most proper medical care pattern group was highest in the general hospitals (9.3%) but the overall proper care group was higher in the public health centers (22.1%) and private clinics (17.1%). Conclusions : The management system of hypertension should be reinforced urgently. Therefore, it is necessary to develop guidelines including the number of visiting days per year and prescription days per visit day, and make the system provide medical facilities to more properly care for hypertensive patients.

대학운영병원과 기업운영병원에 대한 이미지 비교 (A Study on Image Difference between University Hospitals and Corporation Hospitals)

  • The objectives of this research are 1) to access different images held by three consumer groups (patients in university hospital, patients in corporation hospital, persons who are not currently visiting hospitals) on two types of hospitals(university hospitals and corporation hospitals), and 2) to investigate the personal factors affecting images of two types of hospitals. The data for this analysis were collected by questionnaire survey. A total of 403 interviews were conducted. Of these cases, 43 percent are male and 57 percent are female. The major statistical methods used for the analysis are paired t-test, factor analysis and multiple regression. The three consumer groups show a consensus that corporation hospitals are better than university hospitals in some aspects, such as kindness, facilities, and equipments. However, these groups disagree in certain images on two types of hospitals, such as popularity, credibility, and readiness for emergency. The images on two types of hospitals are varied by the respondents' sociodemographic characteristics such as age, sex, and by the type of hospital they are currently visiting.

의약분업이후 병원문전약국과 동네약국 이용자의 만족도 비교 (Comparison of patient's satisfaction on the pharmacy services between those using the pharmacy nearby hospital and those using the pharmacy in resident areas)

  • The purpose of the study was to compare patient's satisfaction on the pharmacy services after introducing the new system of separation of dispensing from prescribing medicines, between those filling their prescriptions from the pharmacy nearby hospitals and those from the pharmacy in their resident areas. To measure patient's satisfaction, a questionnaire survey was conducted with 354 outpatients who received prescriptions from any of the three university hospitals located in In-Cheon city. Study results showed that geographic accessibility to pharmacy was a main attribute to select pharmacy. Size of the pharmacy and availability of prescription drugs are the second major reasons for pharmacy selection for the patients from the pharmacy nearby hospital, whereas patronage is the second major reason for those from the pharmacy in resident areas. Overall satisfaction was higher among the patients from the pharmacy in resident areas than those from the pharmacy nearby hospitals, mainly due to better facilities(waiting area, public telephone, etc), kindness, cleanliness, shorter waiting time, and pharmacist's concern about patient's health. On the other hand, the patients of the pharmacy nearby hospitals showed low satisfaction because of long waiting time and the lack of pharmacist's knowledge and information about patients' health status and medication history. Patients visiting the hospital that has pharmacy-hospital cooperation system showed higher satisfaction as compared to those visiting the hospital without such system. This study provided an empirical evidence that it would be more advantageous for patients to receive pharmacy services from pharmacies located in their living areas than from pharmacies nearby hospitals. This implies that there is a strong need for adequate strategies to enhance the role of pharmacies in resident areas under the new system of separation of dispensing from prescribing medications.

재활치료 중인 뇌졸중환자의 한의약의료에 대한 태도 (Attitude towards Traditional Korean Medicine Use among Hemiplegic Patients after Cerebrovascular Accident)

  • Background : Complementary and alternative medicine has bee used to cardiovascular diseases. In recent years, many hospitals have tried to integrate complementary and alternative medicine(CAM) with conventional medical approaches for patients with chronic diseases. Recently, the prevalence of the use of traditional Korean medicine(TKM) among patients with chronic diseases, expecially, hemiplegia patients after cerebrovascular accident is increasing in Korea. To date, however, there were only few studies addressing the patients' attitudes, and utilization of TKM, compared to the well-documented escalating use of TKM among consumers in Korea. Objectives : The objective of this study was to analyze the prevalence of TKM use among hemiplegia patients after cerebrovascular accident and to determine what factors affect to use CAM among the patients. The study also aims to provide information on TKM and assist therapy selection among various CAM therapies for hemiplegia patients after cerebrovascular accident within health care system in which both practitioners of TKM and practitioners of modern Western medicine coexisted. Methods : The design of the study was descriptive cross-sectional, and data were collected using a 32-items questionnaire. The subjects were one hundred fifty nine patients with chronic diseases who visited or admitted to health care facilities in a hospital in Seoul Metropolitan city, Korea. Data were analyzed using 'SPSS Statistics 18.0 Network Version(on release 18.0.1 of PASW Statistics)' program. Various statistical methods were used to obtain a profile for participants and the therapies most frequently used by hemiplegia patients of TKM. Logistic regression analysis was employed in order to determine the predicting variables of TKM use. Results : The prevalence of the use of TKM was 51.6%. The most common TKM therapies used by the patients included acupuncture(93.2%), herbal medicine(64.8%), and cupping(37.5%). Results of logistic regression analysis revealed that the variables significantly related with TKM use were gender, marital status, job, No. of visiting health care facilities/week. Conclusions : This study shows that the use of TKM among the hemiplegia patients is relatively high in Korea, this topic should be taken into account in the development of a holistic approach for patients with chronic diseases and an efficient chronic disease management system in Korea.
