A 21-year-old male visited our emergency room. He could not remember the mechanism of injury. He was found beside a motorcycle. Initial vital sign was stable. Observation and conservative treatment were planned at the intensive care unit (ICU). On the third day at ICU, he complained sudden flank pain. It was colicky and hard to control. Without the pain, he had no specific symptom, sign, or laboratory findings. On computed tomography, renal pelvis was filled with hematoma which induced hydronephrosis. Double-J catheter and percutaneous nephrostomy was implemented by an intervention radiologist. Hematome in the renal pelvis was aspirated during the procedure. Symptom of the patient was subsided after the procedure. He was discharged without specific complications.

Continuous epidural catheter insertion is common practice in postoperative analgesia. Subarachnoid migration of epidural catheter is a rare complication. Presumed delayed subarachnoid migration of an epidural catheter occured in a 58-year-old female patient after subtotal gastrectomy. Delayed respiratory depression occured 7 hrs after transfer to admission room. She was intubated and had ventilatory care. The fluid from the epidural catheter was examined and the result showed that gucose was 107 mg% and protein was 31 mg%. Immediate naloxone administration and ventilatory care for one day was done. The patient discharged without any sequalae.

A surgical patient is identified by an operating surgeon or a nurse manually. There always exists a chance that he/she misses the necessary identification process. It can bring about serious and critical demage to the surgical patient. Thus, it is necessary to prevent this kind of mistake automatically. This study proposes a business model identifying surgical patients using RFID system. The paper presents a design of the system identifying patients at the entrance of a operating room. It gives also some user interfaces of the system.

Main bronchial injury after blunt trauma is very rare in all bronchial injuries and the pathogenesis is variable and not well known in everycases. We report a case of complete transsection of right main bronchus by blunt trauma. This 24-year old patient was transferred from a local hospital with a chest tube. Because of the severe subcutaneous emphysema and tension pneumothorax, we inserted one more chest tube resulting no obvious interval change. With the impression of bronchial injury, we performed an exploratory thoracotomy. We couldn't proceed bronchoscopy in the operation room because of his unstable vital sign. After opening of the chest wall, we could identify completely transsected right main bronchus. We anastomosed the bronchus with 4~0 Vicryl interruptedly. After operation, the patient was recovered without any complication.

Vomiting in pediatric patient is frequently encountered problem in emergency room or outpatient clinic. In differential diagnosis, age of the patient or accompanying symptoms should be considered in the differential diagnosis. Accurate diagnosis is very important because surgical treatment is necessary in some of the conditions. Imaging diagnosis of conservative modality such as upper gastrointestinal series or colon study is still important radiological examination in the initial differential diagnosis, but recently ultrasonography offers accurate diagnosis in many situations. The cause of vomiting in pediatric are diverse according to the age group :neonatal sepsis, necrotizing enterocolitis, or hypertrophic pyloric stenosis in neonates : gastroesophageal reflux, viral enteritis, or intussusception in infant: midgut volvulus, appendicitis, metabolic disorders, or increased intracranial pressure also an be the cause. knowledge of radiological findings of normal gastrointestinal tract is important to recognize abnormalities. A discussion of radiological findings in variable surgical conditions to present as vomiting in pediatric patients is offered.

The objective of this study was to find out the level of patient expectation and satisfaction on their services. The questionaire reports of 781 patients, which were structured with the question about the level expectation and satisfaction of the services. Major findings of this study were as follows 1) The percentage of patients satisfied with the services as a whole is 59.1 %,where more patients are satisfied with the attitude of therapists (87.3 %), the therapeutic-skill of therapists(84.1 %) and the adequateness of treatment (63.9 %) than the expenses of services (42.0 %) and waiting time(47.4 %). 2) Fourty two percents of patients responded that they expected to be recovered completely.

Considering the rapid growth of elderly population and the increase of medical demand for aged, many of geriatric hospitals are needed in Korea. On the other hand, supplies of hospitals for acute care are considered to be excessive. So many of small and medium hospitals facing management problems have been converged to geriatric hospitals. These attempts are considered resonable for efficient utilization of health care resources in Korea. This paper aims to identify the concept of geriatric hospital and to analyze the problems and then to seek the alternatives for architectural planning on the basis of surveying the geriatric hospitals converged from hospitals for acute care.

Recently, medical technology and IT technology have been trying to converge to overcome limitations of the time and the space in medical technology application. In this study, an app was developed to support remote medical direction for emergency medical services. The developed app allows doctors in remote locations to receive real-time emergency patient situations from emergency paramedics. It can also guide the patient's condition diagnosis and emergency treatment and enable rapid response from the emergency room.

In Korea, it is urgently required to develop the health care facilities and services for People with dementia. The progression of dementia is associated with a gradual decline in functional, cognitive, emotional, and social abilities. Therefore, architectural design for People elderly with dementia should be focused to maintain and preserve this abilities. This study aims to derive the design guidelines of the ward and patient roon unit planning to maintain this abilities in the health care facilities for People with dementia. For this, investigation and analysis through the case studies of domestic and foreign facilities and P.O.E of domestic facilities were performed.

While there are reports regarding burns occurring to patients during the surgery, there are little reports concerning the incidents of the burns related to neurosurgical operations. Moreover, in Korea, even surveys and statistics on the incidents of burns in operating rooms are not known. This report explores burns occurring to a patient in an electrocautery scenario after disinfecting the surgical site with alcohol during the preparation of a neurosurgical operation in an operating room where there is much exposure to oxygen. The authors show a case of a 33-year-old male patient who undergoing evacuation of hematoma on occipital lesion, suffered second degree burns as a result of surgical fires.
