간이정신진단검사를 이용한 구강안면동통환자의 심리학적 분석 (A Psychological Analysis of the Orofacial Pain Patients Through SCL-90-R)

  • Personality characteristics of orofacial pain patients was analyzed psychologically by means of the SCL-90-R. 36 TMD patients, 20 burning mouth syndrome(BMS) patients, 31 trigeminal neuralgia(TN) patients, 20 control I and 28 control II were subjected at Orofacial pain clinic, Department of Oral Medicine and Health Promotion Center, Pusan National University Hospital during the period from 1998 to 1999. The obtained results were as follows: 1. Mean values of T-scores on 9 basic scales in all the groups were within normal range. 2. The T-scores of SOM, O-C, ANX and HOS in TMD patient group were significantly higher than those in control I group, but there was no significant difference in all scales between TMD patient group and control II group. 3. The T-scores of SOM in BMS patient group was significantly higher than those in control I group, but there was no significant difference in all scales between BMS patient group and control II group. 4. The T-scores of SOM in TN patient group was significantly higher than those in control II group. 5. As compared with present and absent of the history of systemic diseases, there was no significant difference of the scales in TMD, BMS and TN patient groups but the T-scores of the patient groups with the history of systemic diseases tended to higher than those of the patient group without the history of systemic diseases. 6. As compared with acute and chronic groups, the T-scores of O-C, I-S, PAR, PSY in chronic BMS patient group were significantly higher than those in acute BMS patient.

치위생과 학생들의 윤리적 성향과 자아존중감이 환자 의료정보보호 인식에 미치는 영향 (The Effect of Ethical Propensity and Self-Esteem of Dental Hygiene Students on the Awareness of Patient Medical Information Protection)

  • Purpose : This study explores the effects of dental hygiene students' ethical propensity and self-esteem on their awareness of protecting patient medical information and how these factors interact to influence their professional ethical consciousness. The aim of this study is to highlight the role of university education in shaping the ethical values of dental hygienists. Methods : Between June 1 and November 30, 2023, 210 dental hygiene students in Busan Metropolitan City participated in this study. After providing consent, the participants completed a self-administered questionnaire comprising 69 questions. The questions covered general characteristics (8 questions), ethical propensity (20 questions), self-esteem (10 questions), and awareness of patient medical information protection (31 questions), all measured on a five-point Likert scale. Results : Correlation analysis indicated that idealism had a significant positive correlation with relativism (r=0.35, p<.001), self-esteem (r=0.28, p<.001), and awareness of patient medical information protection (r=0.46, p<.001). Relativism had a significant positive correlation with self-esteem (r=0.17, p<0.05) and awareness of patient medical information protection (r=0.23, p<.01). Self-esteem had a significant positive correlation with awareness of patient medical information protection (r=0.34, p<.001). Multilinear regression analysis revealed that idealism (ß=0.38) and self-esteem (ß=0.22) significantly increased the level of awareness of patient medical information protection. However, relativism did not have a significant impact on the level of awareness of patient medical information protection. The model explained 26.0 % of the variance (24.9 % when corrected). Conclusion : Higher levels of idealism and self-esteem correlate with greater awareness of patient medical information protection. In addition to appropriate program development and strategies, university curricula for dental hygiene students should include systematic and continuous training to enhance the values of ethical awareness, idealism, and self-esteem.

Patient-Centredness, Job Satisfaction and Psychological Distress: a Brief Survey Comparing Oncology Nurses and Doctors

  • Background: We aimed to explore whether levels of patient-centredness, job satisfaction and psychological distress varied between oncology nurses and doctors. Materials and Methods: In a cross-sectional study using self-administered questionnaires, a total of 24 nurses and 43 doctors were assessed for patient-centredness, psychological distress, and job satisfaction using the Patient-Practitioner Orientation Scale, Hospital Anxiety and Depression Scale, and Job Satisfaction Scale. Data were analysed using descriptive statistics, independent samples t-test and MANCOVA, with p<0.05 considered significant. Results: Overall response rate was 95.6% (43/45) for physicians and 85.7% (24/28) for nurses. Even after adjusting for known covariates, our principal finding was that doctors reported greater psychological distress compared to nurses (p=0.009). Doctors also reported lower job satisfaction compared to nurses (p = 0.017), despite higher levels of patient-centredness found in nurses (p=0.001). Findings may be explained in part by differences in job characteristics and demands. Conclusions: Mental health is an important concern not just in cancer patients but among healthcare professionals in oncology.

시계열 분석을 통한 7개질병군 포괄수가제의 환자 비용 변화 분석 -의료기관의 유형별 특성을 중심으로- (Analysis of Changes in Patient Costs in 7Diagnosis-Related Groups through Time Series Analysis - Focusing on the Characteristics of Medical Institutions -)

  • Objectives : This study analyzed the trends of patient costs in 7diagnosis-related groups(DRG) since July 2013 when the government made it mandatory for all hospitals and clinics. Methods : Data were collected from the 7DRG score chart published by the Ministry of Health and Welfare(MoHW) from July 2013 to January 2017. The average value of the weekday relative value scale was multiplied by unit price, referred to as'- "patient costs by disease group"-' and they were analyzed by time series. Results : Patient costs had increased among all patients with a comprehensive disease. Small and medium-sized hospitals (hospitals and clinics) showed a slight increase in patient costs. Conclusions : Enforcement of the Korean diagnosis-related groups has led to management crisis in small and medium-sized hospitals and deterioration medical service quality. To solve this problem, The weekday relative value scale of small and medium-sized hospitals should be increased significantly.

Validity of patient-derived xenograft mouse models for lung cancer based on exome sequencing data

  • Patient-derived xenograft (PDX) mouse models are frequently used to test the drug efficacy in diverse types of cancer. They are known to recapitulate the patient characteristics faithfully, but a systematic survey with a large number of cases is yet missing in lung cancer. Here we report the comparison of genomic characters between mouse and patient tumor tissues in lung cancer based on exome sequencing data. We established PDX mouse models for 132 lung cancer patients and performed whole exome sequencing for trio samples of tumor-normal-xenograft tissues. Then we computed the somatic mutations and copy number variations, which were used to compare the PDX and patient tumor tissues. Genomic and histological conclusions for validity of PDX models agreed in most cases, but we observed eight (~7%) discordant cases. We further examined the changes in mutations and copy number alterations in PDX model production and passage processes, which highlighted the clonal evolution in PDX mouse models. Our study shows that the genomic characterization plays complementary roles to the histological examination in cancer studies utilizing PDX mouse models.

  • The purpose of this study was to identify factors influencing patient satisfaction and to evaluate the utility of patient satisfaction as an outcome indicator. The study was conducted by mailed questionnaire. The subjects were 900 patients discharged from adult nursing units in a tertiary teaching hospital. On the discharge date, questionnaires were distributed by two trained research assistants. The questionnaire developed by the researchers was based on Larson(1996)'s study, and consisted of 71 items with the following components: overall satisfaction, domain-specific satisfaction(administration process, hospital facility and environment, nurses, and doctors), patients' loyalty(intention to use the health care service of the hospital in the future), recommendation to others, health benefits, and demographic characteristics. Each item was rated using a five point Likert scale ranging from '1=strongly disagree' to '5= strongly agree'. The response rate was 43%(387/900). The satisfaction level with the health care service was generally high. Perceived health status was the only significant factor influencing satisfaction level. Satisfaction with doctors contributed the most to explaining overall satisfaction. Overall satisfaction was significantly correlated with patient loyalty, recommendation, to other and perceived health benefit. It was found that the score of satisfaction was positively correlated with the score of loyalty, recommendation, and health benefit. Therefore, patient satisfaction seems to be a good outcome indicator.

Clinical Implementation of Precision Medicine in Gastric Cancer

  • Gastric cancer (GC) is one of the deadliest malignancies in the world. Currently, clinical treatment decisions are mostly made based on the extent of the tumor and its anatomy, such as tumor-node-metastasis staging. Recent advances in genome-wide molecular technology have enabled delineation of the molecular characteristics of GC. Based on this, efforts have been made to classify GC into molecular subtypes with distinct prognosis and therapeutic response. Simplified algorithms based on protein and RNA expressions have been proposed to reproduce the GC classification in the clinical field. Furthermore, a recent study established a single patient classifier (SPC) predicting the prognosis and chemotherapy response of resectable GC patients based on a 4-gene real-time polymerase chain reaction assay. GC patient stratification according to SPC will enable personalized therapeutic strategies in adjuvant settings. At the same time, patient-derived xenografts and patient-derived organoids are now emerging as novel preclinical models for the treatment of GC. These models recapitulate the complex features of the primary tumor, which is expected to facilitate both drug development and clinical therapeutic decision making. An integrated approach applying molecular patient stratification and patient-derived models in the clinical realm is considered a turning point in precision medicine in GC.

혈액투석 환자의 음식갈망, 환자활성화 및 가족지지가 식이 자가관리에 미치는 영향 (Effects of Food Craving, Patient Activation, and Family Support on Dietary Self-Care in Hemodialysis Patients)

  • Purpose: This study aimed to identify the effect of food craving, patient activation, and family support on dietary self-care performance in hemodialysis patients. The study will provide baseline data for developing nursing interventions to improve hemodialysis patients' dietary self-care. Methods: The data was collected from 132 patients who regularly underwent outpatient hemodialysis between October 26 and November 12, 2022. Frequency and percentage, mean and standard deviation, independent t-test, one-way ANOVA, Scheffé test, Pearson's correlation coefficient, and multiple regression analysis were used to analyze the data, and all analyses were performed using SPSS 27.0. Results: The average scores of the main variables were 39.55±7.42 for food craving, 77.37±10.50 for patient activation, 63.62±11.72 for family support, and 38.21±5.54 for dietary self-care. The factors affecting the dietary self-care were age (β=.36, p<.001), family support (β=.36, p<.001), and patient activation (β=.20 p=.007). The total explanatory power of all variables was 34.0% (F=18.20, p<.001). Conclusion: Based on these results, it is necessary to consider the characteristics of age and improve the level of family support and patient activation when developing programs to improve dietary self-care in hemodialysis patients.

여성전문병원 건축계획을 위한 환자공간만족도 평가에 관한 연구 (A Study on the Evaluation of the Patient Spatial Satisfaction for the Architectural Planning of the Women's Hospital)

  • The purpose of this study is to find out the design guideline for women's hospital focusing on the patient needs. This study is performed the literature review of the performance evaluation & spatial satisfaction. And then, the spatial characteristics of selected women's hospitals, which were recently built in Korea, were analyzed. This study is suggested in the spatial satisfaction evaluation model in order to the spatial evaluation performance elements of the patient needs. The evaluation model of space efficiency derived from this study is expected to be used for the actual guide to architecture of women's hospitals as substantial materials. Finally, the model is used to for the planning & design for the future women's hospital.

  • The articulation disorders associated with velopharyngeal insufficiency (VPI) in cleft palate patients are interested to clinicians particularly. The purpose of this study was to investigate mainly the oropharyngeal air pressure and overall air flow in cleft palate patients. The pressure-measuring catheter was positioned at the midportion of the oropharyngeal cavity with a facial mask. Test words were composed of 9 meaningless polysyllabic words and 17 meaningful words. Aerophone II and Nasometer II were used to measure peak air pressure, mean air pressure, maximum flow rate, volume, phonatory flow rate, nasalance. The data shows that airflow of the cleft palate patient group were higher than those of the control group. Intraoral air pressure of the cleft palate patient group was lower than those of the control group. The first vowel formant and first Bandwidths of the cleft palate patient group were higher than those of the control group.
