Clinical Application of Dermatic Care by Laser Therapy in Korean Oriental Medicine (레이저를 이용한 피부치료의 한의학 임상활용-모세혈관 확장증을 중심으로)

  • Phototherapy have perpetual history, we can see an instance of it in Yellow Emperor's Conon of Medicine(黃帝內經) and Laser therapy is a field of Phototherapy being used frequently in the latest time. We treated patients with telangiectasia by laser therapy and experimented it's efficacy and studied the means of clinical application in Korean medicine. We report the capabilities of application of laser therapy, based on Korean medical theories by using resemblance between laser therapy with kyung-rak(經絡) control technique in the line that connects with the past.

  • Research papers must be expression given by letter that had fixed types, rules and universal sentence languages to inform many persons about study results until present when these research paper was considered review, oriental medical papers had some problems, as like difficult contents not to inform absolutely, usage of past language, not considerable in logic history and continuity, and have used not fixed types and rules, error of statistical analysis and research construction. Also centered clinic that had trends important treat tools by drugs ,acupuncture and moxa studies. In briefly, papers of korea traditional medicine had some several problems. Oriental medicine has very concerns not only korean but other countries in present and then it will increases much more study in future. For the purpose cope with this concerns korea traditional medicine researchers need much educations of relation department and must recognized much concerns.

Clinical Study for Tremor in 30 Admission Cases (진전(震顫)을 주(主) 증상(症狀)으로 입원(入院)한 환자(患者) 30례(例)에 대(對)한 임상적(臨床的) 고찰(考察))

  • The clinical study was carried out the 30 patients with tremor who were treated in Daejeon University Oriental Hospital from 1 April 1997 to 31 March 2001. The results were summarized as follows. 1. The ratio of female was higher, especially in the psychosomatic tremor, the ratio of female was higher and in the age distribution the aged over 50 were higher frequence. 2. The case without past history was most, the most ordinary preceding disease was hypertention and the next was diabetes, drinking history had no concern with tremor, first visit was most, in the psychosomatic tremor complication by relation was the most inducing factor and both parkinson tremor and essential tremor were no inducing factor. 3. Tremor appeared to be busy in extremities, parkinson tremor appeared to be accompanied with musculoskeletal system symptoms and they were in descending order lower limb weakness, extremities numbness, general body weakness e.t.c. both essential tremor and psychosomatic tremor appeared to be accompanied with psychosomatic symptoms and they were in descending order dizziness, headache e.t.c. 4. In classification of Four Human coporeal constitution the number of patients, Tae-Eum-In(太陰人) was most, the prescription drugs of tranquillizing the liver and relieving anxiety such as GYEJIYONGGOLMORYETANG(桂枝加龍骨牡蠣湯) and nourishing Yin, blood and relieving anxiety such as SAMULANSINTANG(四物安神湯) were used to be busy. 5. The rate of treatment was collectively improved and was higher in the yaung age than in the old age, the period of the clinical history was the shorter within one month, the rate of improvement was the better.

  • Hymenoptera stings can cause serious injury to humans. We report the clinical findings of 6 cases of Hymenoptera stings. All patients developed painful erythematous papules at the sting sites and had a past history of parasitoid wasp sting. This is the first clinical report of the parasitoid wasp, Cephalonomia gallicola, causing human stings in Korea.

  • Insomnia is a temporary or continuous condition in which one has difficulty sleeping, due to physical impediments or physical problems. Because insomnia does harms to a labor power or imposes mental and physical burden on a patient, we should pay attention to this disease. This case is about a patient who was 62 years old man with a chronic insomnia since 1996, inspite of no past history about psychical disease. He had sufferred from a cerebral infarction and hypertension since March. 2002. and was hospitalized in Sangji University korean medicine hospital, 25. June. 2002. This patient had been treated by herb-med, Yanggyuksanhwatang for 4 weeks and showed an improvement. This paper describes the process and contents about the patient was cured.

Clinical Analysis of the 991 Outpatients with Gastrointe stinal Symptoms and Extra-gastrointestinal Symptoms (소화기 증상을 주소로 내원한 외래환자 991명에 대한 위장관 증상 및 동반 증상 분석)

  • Objectives: Clinically, patients with dyspepsia often complain of several gastrointestinal (GI) and extra-GI symptoms. However, there have been few studies investigating the relationship between GI symptoms and extra-GI symptoms. The aim of this study was to characterize the clinical features of the 991 outpatients with GI and extra-GI symptom and the relationships between GI and extra-GI symptoms. Methods: 991 patients newly visiting Weedahm Oriental Hospital for GI symptoms were included. Those who had disorders caused by the hepatobiliary-pancreatic system were excluded. The 991 outpatients were interviewed with a standardized questionnaire inquiring about demography, past medical history, past institutional care, GI symptoms and extra-GI symptoms. Results: Among 991 patients, 780 (78.7%) had more than three GI symptoms and 451 (45.5%) had more than five. Among 991 patients, 545 (55.0%) had more than three extra-GI symptoms and 285 (28.8%) had more than five. There was a statistically significant correlation between GI symptoms and extra-GI symptoms such as headache, fatigue, forgetfulness, eyeball pain, unrest, dizziness, muscle pain, chest pain, and dyspnea. Conclusions: We found that there was statistically significant correlation between GI symptoms and extra-GI symptoms, which suggests the possibility of main common pathophysiology between GI symptoms and extra-GI symptoms. Further studies will be required to elucidate the main common pathophysiology between GI symptoms and extra-GI symptoms.

Glioma Mimicking a Hypertensive Intracerebral Hemorrhage

  • Here, we report a rare case of an anaplastic astrocytoma masquerading as a hypertensive basal ganglia hemorrhage. A 69-year-old woman who had been under medical management for hypertension during the past 3 years suddenly developed right hemiparesis with dysarthria. Brain computed tomography (CT) scans with contrast and CT angiograms revealed an intracerebral hemorrhage (ICH) in the left basal ganglia, without an underlying lesion. She was treated conservatively, but underwent a ventriculoperitoneal shunt operation 3 months after the initial attack due to deteriorated mental status and chronic hydrocephalus. Three months later, her mental status deteriorated further. Magnetic resonance imaging (MRI) with gadolinium demonstrated an irregular enhanced mass in which the previous hemorrhage occurred. The final histological diagnosis which made by stereotactic biopsy was an anaplastic astrocytoma. In the present case, the diagnosis of a high grade glioma was delayed due to tumor bleeding mimicking hypertensive ICH. Thus, a careful review of neuroradiological images including MRI with a suspicion of tumor bleeding is needed even in the patients with past medical history of hypertension.

A Study of the Case Records of a Herbalist En Su-ryong in the late Chosun dynasty (19세기 고창지방 의원 은수룡이 남긴 경험의안)

  • This paper analyzes the case records of a herbalist En Su-ryong who lived at the Kochang area of Chollapukdo province in the 19th century. The records, which were included in his collection of works, Tantojip(呑吐集), were consisted of 11 clinical diagnosis and prescriptions. The result of the analysis is as follows. First, En Su-ryong's records are estimated to be valuable enough to contribute to the development of the Korean clinical medicine, in light of the fact that the present established prescriptions or medical theory came from the repetition of trial and error by many herb doctors. Second, his case records are unique in the style of writing, because they were consisted of only his own clinical diagnosis and prescriptions case by case, while those of ordinary herbalists were classified by the types of the symptoms of a disease, with their prescriptions modified from the past established. Third, in the records he minutely wrote not only the names and the addresses of the patients under his care, but also the names of the diseases, the progress and the contents of his treatment, and even the perfect cure or not. Therefore, his case records are appreciated to be very important from the standpoint of the history of the society.

Recurrent onycholysis in a patient with Behcet's disease (베체트병에서 발생한 재발성 손발톱박리증)

  • Onycholysis is defined as a distal or distal lateral separation of the nail plate from the underlying or lateral supporting structures including nail bed, hyponychium, and lateral nail fold. Trauma, infection, psoriasis, thyrotoxicosis, and numerous drugs are common causes of onycholysis. However, there are few specific data on nail findings in Behcet's disease (BD). In this paper, we report on a 60-year-old man with BD, with no past history except BD, who developed recurrent onycholysis. The symptoms of onycholysis are considered to be recurrent depending on the activity of BD. The nail lesion showed improvement after classic treatment of BD and topical steroid ointment.

  • Unresolved major pulmonary embolism (UMPE) is an uncommon condition which causes pulmonary hypertension, cor pulmonale and death. An accurate and prompt diagnosis of UMPE is very important in the management of such patients with pulmonary embolectomy. Follow-up lung scans can lead to earlier diagnosis of UMPE especially on patients who have a history of acute pulmonary embolism in the past and. present with pulmonary hypertension, respiratory insufficiency and cor pulmonale. We report a case of UMPE strongly suggested by follow-up lung scans and subsequently confirmed by pulmonary angiography and postmortem examination.
