Diagnosis of Neospora caninum Infection by PCR using Formalin-fixed, paraffin-embedded tissues

  • N. caninum infections have been associated with neonatal paresis as well as abortion around the world. Bovine abortion induced by N. caninum was first reported in 1997 in Korea. Diagnosis of N. caninum infection is usually based on histopathology and immunohistochemical detection of organism. However, often the tissues having lesion suggestive of N. caninum infection were negative on immunohistochemistry. (omitted)

포유자돈(哺乳仔豚)에 발생(發生)한 Viral Encephalomyelitis의 병리조직학적관찰(病理組織學的觀察) (Histopathological Observations on Viral Encephalomyelitis in Suckling Piglets Raised in Korea)

  • The outbreak of viral encephalomyelitis (Talfan disease) in suckling piglets was diagnosed pathologically for the first time in Korea. The clinical signs of affected sucklings were very high morbidity without death, and fever ($39^{\circ}C$ or higher) with incoordination of the rear limbs and paresis, while no specific gross lesions were observed at autopsy. Histopathological changes were confined to the nervous system, and were of the nature of a nonsuppurative and nondemyelinating encephalomyelitis which is usual for the viral encephalitis. It was characterized by perivascular cuffing, meningitis, neuronal degeneration, neuronophagia and glial nodules in the brain and spinal cord.

Cerebellar encephalopathy from diminazene aceturate (beneril) toxicity in a dog

  • A 2-year old castrated male Alaskan malamute was referred with primary complaints of marked anemia, hemeglobinuria and depression. Laboratory tests revealed canine babesiois with severe anemia. The dog was treated by blood transfusion and beneril (diminazene aceturate, 3.5 mg/kg IM). Two days after Beneril injection, the dog suddenly showed ataxia progressing to paresis. MRI revealed irregularly diffused lesions in the cerebellum. The case was tentatively diagnosed as cerebellar encephalopathy caused by diminazene toxicity. The dog successfully recovered following steroid therapy.

  • Intracranial lipomas are rare lesions, which are believed to be congenital malformations. They are usually asymptomatic incidental findings and localized in the midline. However, they may occasionally produce neurological symptoms such as seizure, headache, mental changes, paresis. Currently, diagnosis of intracranial lipomas is made on based of imaging modalities, particularly Magnetic resonance imaging(MRI). Because Lipomas are strongly adherent to the surroundings and typically enclose both vessels and nerves, Surgical approach is rarely indicated.

  • Voice therapy after post-thyroidectomy dysphonia is designed to improve glottal closure without supraglottic hyperfunction by development of abdominal support for breathing and intrinsic muscle strengthening exercises. Regarding voice therapy of unilateral vocal fold paresis/paralysis, several techniques such as head positioning, digital manipulation, pushing and pulling method are used for trial or temporary therapy. And the holistic voice therapy such as LSVT, Accent method, VFE are useful for hypofunctional voice disorders. These may be effective interventions as a temporary improvement until normal voice recovers or may provide the essentials for long-term rehabilitation.

개에서 Discospondylitis의 진단과 치료 일례 (A case of Discospondylitis in a Dog)

  • Discospondylitis is an infection of an intervertebral disk with concurrent osteomyelitis of contiguous vertebrae. Clinical signs are variable and include pain, fever, anorexia, weight loss, depression, stilted gait and paresis or paralysis. A seven-year-old, intact female Yurkshire terrier dog was refered to the Vfterinary Medicine Teaching Hospital of Seoul National University because of intermitent pain of unknown cause for 2 weeks. On physical examination, pain was evident when spines were manipulated, especially thoracolumbar junction part. No neurologic deficits were deteced in the general neurologic test. Spinal radiography demonstrated the bony lysis of the vertebral end plates and sclerosis of the $T_{12-13}$ vertebral body. Based on the results of examinations, the dog was diagnosed as discospondylitis and recoverd following antibiotic therapy.

  • A castrated male, 9-year-old Yorkshire terrier was presented with a depression and bilateral hind limbs lameness. On physical examinations, upper motor neuron signs and stiffness of the hind limbs, back pain and progressive paresis were identified. Marked periosteal new bone formations and lysis include the first lumbar vertebra to the sacrum, bilateral iliums acetabulums and bilateral femoral heads were observed in survey radiographs. After death with septicemia suspected, renal infarction and the 5th vertebral osteomyelitis include pelvic periostitis were diagnosed in histological examination.

  • A 34-year-old man presented with a severe headache, fever, and cervical lymphadenopathy followed by generalized tonic-clonic seizure. Evaluations showed splenomegaly, elevated liver enzymes, and 380 white blood $cells/mm^3$ in the cerebrospinal fluid. Two weeks after admission, he developed sudden vertigo. Examination revealed spontaneous horizontal-torsional nystagmus to the right and bithermal caloric tests documented left canal paresis. A cervical lymph node biopsy disclosed subacute necrotizing lymphadenitis. We report a case of aseptic meningitis and unilateral vestibulopathy associated with histiocytic necrotizing lymphadenitis (Kikuchi's disease).

  • A 69-year-old woman presented with right abducent nerve paresis caused by an ectopic pituitary adenoma invading the posterior wall of the sphenoid sinus. The tumor was removed via transsphenoidal approach. The histological diagnosis was invasive pituitary adenoma with bony destruction. The symptom was improved without complication. The authors present a rare case of ectopic pituitary adenoma with a literature review.

Anterior canal-sparing bilateral vestibulopathy in MELAS syndrome

  • Vestibular dysfunction has rarely been reported in MELAS syndrome. A 40-year-old male with long-term diabetes and hearing loss experienced a stroke-like episode with hemisensory disturbance and lactic acidosis. Brain MRI showed temporo-parieto-occipital cortical lesions, and a final diagnosis was made of MELAS syndrome with the mitochondrial 3243A>G mutation. Neuro-otologic evaluations revealed anterior-canal-sparing bilateral impairments of the vestibulo-ocular reflex in the video head impulse test and no caloric paresis. This unique pattern of vestibular dysfunction may aid in diagnosing MELAS syndrome.
