Correction of Retracted Ala Using Spacer Graft in Secondary Cleft Lip and Nose Deformity (이차 구순비 변형 환자에서 Spacer Graft를 이용한 콧방울뒤당김(retracted ala)의 교정)

  • Purpose: In patients with unilateral cleft lip and nose deformity, alar retraction is commonly seen on the non-cleft side after cleft side is corrected. Spacer graft was used to drag down the inferior border of the alar cartilage of the non-cleft side so as to match the cleft side. By performing spacer graft and septal extension graft together, symmetry and cosmetic improvements were achieved. Methods: Seven unilateral cleft lip and nose deformity patients underwent surgery for alar retraction correction. The median age was 24 years (ranged from 15 to 34 years), and the median follow-up period was 7.4 months (ranged from 6 to 12 months). The perpendicular length from the longitudinal axis of the nostril to the alar rim, the nasolabial angle and the ala-labial angle were measured in the lateral view photo. The longest perpendicular length from the cephalic border of the alar rim to the parallel line of the alar base was measured in the frontal view photo. Results: Improvement in alar retraction was seen after the surgery. There were no specific complications during the follow-up and the symmetry of both nostrils was satisfactory. No increase in the nasolabial angle or exposure of the nostrils was seen after the tip projection via tip plasty. Conclusion: The fundamental factor in correcting alar retraction with secondary cleft lip and nose deformity is repositioning the alar rim with spacer graft, which seems to be more physiologic than other methods. The method combining spacer graft with septal extension graft will bring symmetry as well as more cosmetic improvement in correction of alar retraction with secondary cleft lip and nose deformity.

Case Study about the Ground Characteristics Analysis of Tunnel Face Fault Fractured Zone (터널막장 단층파쇄대의 지반특성 분석에 대한 사례연구)

  • The area of investigation belongs to Okchon metamorphic zone and the fault fractured zone runs parallel to the tunnel direction. It causes the independent decline of tunnel face and the slackness of the tunnel surrounding base so, after all, the severe displacement has occurred within the tunnel. Accordingly, the TSP(Tunnel Seismic Prediction) survey has been performed to investigate the extent of fault fractured zone and to analize its characteristics. Also, we have analized the behavior causes by performing the tunnel face mapping and drilling investigation, and confirmed the position and scale of geological anomaly area and front fractured zone which influences tunnel excavation and supporting. Collected data analyzed ground layer condition through 3 dimensional modeling. Several variables included in the modeling were analyzed by geostastistics. The analysis of the modeling data shows that the belt of weathering by fault fractured zone is developing on the basis of the right side of tunnel and that is decreasing to the left side. The fault fractured zone was confirmed that it has strike, $N0\~5^{\circ}E$ dip NW, and it is consisted of large-scale fractured zone including several anomalies. The severe displacement in tunnel is probably caused by asymmetrical load that n generated by the crossing of discontinuity and the rock strength imbalance of tunnel's both side by fault fractured zone, and judge that need tunnel reinforcement method of grouting etc.

A Numerical Study on the Progressive Brittle Failure of Rock Mass Due to Overstress (과지압으로 인한 암반의 점진적 취성파괴 과정의 수치해석적 연구)

  • In rock mass subject to high in-situ stresses, the failure process of rock is dominated by the stress-induced fractures growing parallel to the excavation boundary. When the ratio of in situ stresses compared to rock strength is greater than a certain value, progressive brittle failure which is characterized by popping and spatting of rock debris occurs due to stress concentration. Traditional constitutive model like Mohr-Coulomb usually assume that the normal stress dependent frictional strength component and the cohesion strength component are constant, therefore modelling progressive brittle failure will be very difficult. In this study, a series of numerical analyses were conducted for surrounding rock mass near crude oil storage cavern using CW-FS model which was known to be efficient for modelling brittle failure and the results were compared with those of linear Mohr-Coulomb model. Further analyses were performed by varying plastic shear strain limits on cohesion and internal friction angle to find the proper values which yield the matching result with the observed failure in the oil storage caverns. The obtained results showed that CW-FS model could be a proper method to characterize essential behavior of progressive brittle failure in competent rock mass.

Condenser Characteristics of Dielectric Soil Moisture Sensor (유전율 토양 수분 쎈서의 콘덴서 특성)

  • RC oscillation method was applied to study the condenser characteristics of two metal sticks insulated by vinyl tube and used in the dielectric constant determinations of most soils. Its capacitance as influenced by the contacted ambient materials was measured as relative capacitance of the sensor sticks compared with the standard one on the RC oscillation circuit. According to the equivalent circuit of the sensor stick set, the measured capacitance was composed of a basic capacitance connected in parallel with sensor stick capacitance, which was composed of lineally connected vinyl tube capacitances and the sensing part capacitance. The dielectric constant (U) of the contacted ambient moist soil located in the sensing part around the sticks interrelated with the other parameters as following equation. $$\frac{1}{C-B}=\frac{k}{U}+Z$$ where C is the output total relative capacitance, B is the hidden and fixed basic relative capacitance, k is a constant related with U, and Z is a constant for the insulating vinyl tube capacitances determined by its thickness and dielectric constant. The constant k is determined by the spacing and length of sensor sticks. The Z value is theoretically an invariable constant, but it may become considerably bigger than the determined in lab if air tube is formed on the surface of sensor sticks by some shocks on them after their installation in soil. Due to the unstability of lab Z value, it may be better to revise it after sensor stick's installation in soil and no shaking shocks should be applied on them.

Comparison of the Dose of the Normal Tissues among Various Conventional Techniques for Whole Brain Radiotherapy (여러 통상적인 전뇌방사선치료 기법에서의 정상조직의 조사선량 비교)

  • Purpose: To compare radiation dose of the brain and lens among various conventional whole brain radiotherapy (WBRT) techniques. Materials and Methods: Treatment plans for WBRT were generated with planning computed tomography scans of 11 patients. A traditional plan with an isocenter located at the field center and a parallel anterior margin at the lateral bony canthus was generated (P1). Blocks were automatically generated with a 1 cm margin on the brain (5 mm for the lens). Subsequently, the isocenter was moved to the lateral bony canthus (P2), and the blocks were replaced into the multileaf collimator (MLC) with a 5 mm leaf width in the craniocaudal direction (P3). For each patient plan, 30 Gy was prescribed at the isocenter of P1. Dose volume histogram (DVH) parameters of the brain and lens were compared by way of a paired t-test. Results: Mean values of $D_{max}$ and $V_{105}$ of the brain in P1 were 111.9% and 23.6%, respectively. In P2 and P3, $D_{max}$ and $V_{105}$ of the brain were significantly reduced to 107.2% and 4.5~4.6%, respectively (p<0.001). The mean value of $D_{mean}$ of the lens was 3.1 Gy in P1 and 2.4~2.9 Gy in P2 and P3 (p<0.001). Conclusion: WBRT treatment plans with an isocenter located at the lateral bony canthus have dosimetric advantages for both the brain and lens without any complex method changes.

The Effect of the Circuit Exercise and Conventional Exercise on Walking Ability in Chronic Stroke (순환운동과 전통적 운동이 만성 뇌졸중환자의 보행능력에 미치는 효과)

  • Purpose : This study achieved to search the effect of the circuit exercise and conventional exercise on walking ability(walking speed, endurance, dynamic balance, speed, endurance and pedestrian crossing) in chronic stroke. Methods : Since is diagnosed by stroke, to 30 chronic stroke patients who more than 1 year past the 15 circuit exercise group, the 15 conventional exercise group random the circuit exercise group applied circuit exercise 3th 8 weeks each week after neurological treatment because assigning and the conventional exercise group executed round trip walk exercise in parallel bar 3th 8 weeks each week after neurological treatment. The data of 25 patients who complete experimental course were statistically analysed. Results : The results of this dissertation were as following : 1) There were significantly increased after experimental of 10 meter walk test, 6 minutes walk test and Timed "Up and Go" test in circuit exercise group (p<.001). 2) There were significantly increased after experimental of 2, 4 and 6 lane road crossing mobility in Walking circuit exercise group(p<.01). 3) There were significantly differences after experimental of 10 meter walk test, 6 minutes walk test and Timed "Up and Go" test change quantity between circuit exercise group and conventional exercise group(p<.05). 4) There were correlations were found between the TUG test and 2, 4 and 6 lane road (2 lane road; r=.463, p<.01., 4 lane road; r=515, p<.01., 6lane road; r=.710, p<.01), and there were correlations were found between the 10 meter walk test and 6 minutes walk test(r=.595, p<.01), TUG test(r=.662, p<.01) and 6 lane road(r=.527, p<.01). Conclusion : Even if improvement of walk function through training consists in room, transfer of actuality pedestrian crossing is no change outside the room. Because it is much variable of the weather, seasonal factor, temperature, pedestrian number, state of underneath etc. outside the room. Then, in room after direction promotion of walk function to be promotion of walk function in actuality life and need development of connectable training method consider.

Study on the Modeling Technique for Prediction about Pressure Drop of an Intravenous Lung Assist Device (혈관 내 폐 보조장치의 압력손실 예측을 위한 모델링기법에 관한 연구)

  • In this paper, the correlation of Pressure drop about the Newtonian and non-Newtonian fluid was investigated experimentally for vibrating intravascular lung assist device (VIVLAD) and we determined correlation equation to make a prediction about Pressure drop for designing VIVLAD. Design conditions to predict the pressure drop of the modules were studied through an experimental modeling before inserting the artificial lung assist device into as venous. Experiments were performed by distilled water, glycerol/water mixed solution(40% glycerol) of Newtonian fluids. and the bovine blood of non-Newtonian fluids. These fluids were flowed outside and parallel of hollow fiber membranes. Also we measured pressure drop according to the number of the fiber membranes which ware inserted into the inside diameter of shell of 3 cm, and developed the prediction equations by curve fitting method based on correlation between the experimental pressure drop and the frontal area or the packing density of device. The result showed that the Pressure drop and the friction factor of the water/glycerol mixed solution were similar to that of bovine blood. It was showed that the water/glycerol mixed solution (40% glycerol) could be used for measuring the pressure drop and the friction factor instead of the bovine blood. Also, we could estimate the prediction equation of pressure drop and friction factor as the function of Packing density at the number of hollow fibers. We obtained the reliance of the prediction equations because the pressure drop and the friction factor measured from the experiments were similar to that from the prediction equation. These results may be used to further usefulness for the design of VIVLAD.

Data Mining Algorithm Based on Fuzzy Decision Tree for Pattern Classification (퍼지 결정트리를 이용한 패턴분류를 위한 데이터 마이닝 알고리즘)

  • 컴퓨터의 사용이 일반화됨에 따라 데이타를 생성하고 수집하는 것이 용이해졌다. 이에 따라 데이타로부터 자동적으로 유용한 지식을 얻는 기술이 필요하게 되었다. 데이타 마이닝에서 얻어진 지식은 정확성과 이해성을 충족해야 한다. 본 논문에서는 데이타 마이닝을 위하여 퍼지 결정트리에 기반한 효율적인 퍼지 규칙을 생성하는 알고리즘을 제안한다. 퍼지 결정트리는 ID3와 C4.5의 이해성과 퍼지이론의 추론과 표현력을 결합한 방법이다. 특히, 퍼지 규칙은 속성 축에 평행하게 판단 경계선을 결정하는 방법으로는 어려운 속성 축에 평행하지 않는 경계선을 갖는 패턴을 효율적으로 분류한다. 제안된 알고리즘은 첫째, 각 속성 데이타의 히스토그램 분석을 통해 적절한 소속함수를 생성한다. 둘째, 주어진 소속함수를 바탕으로 ID3와 C4.5와 유사한 방법으로 퍼지 결정트리를 생성한다. 또한, 유전자 알고리즘을 이용하여 소속함수를 조율한다. IRIS 데이타, Wisconsin breast cancer 데이타, credit screening 데이타 등 벤치마크 데이타들에 대한 실험 결과 제안된 방법이 C4.5 방법을 포함한 다른 방법보다 성능과 규칙의 이해성에서 보다 효율적임을 보인다.Abstract With an extended use of computers, we can easily generate and collect data. There is a need to acquire useful knowledge from data automatically. In data mining the acquired knowledge needs to be both accurate and comprehensible. In this paper, we propose an efficient fuzzy rule generation algorithm based on fuzzy decision tree for data mining. We combine the comprehensibility of rules generated based on decision tree such as ID3 and C4.5 and the expressive power of fuzzy sets. Particularly, fuzzy rules allow us to effectively classify patterns of non-axis-parallel decision boundaries, which are difficult to do using attribute-based classification methods.In our algorithm we first determine an appropriate set of membership functions for each attribute of data using histogram analysis. Given a set of membership functions then we construct a fuzzy decision tree in a similar way to that of ID3 and C4.5. We also apply genetic algorithm to tune the initial set of membership functions. We have experimented our algorithm with several benchmark data sets including the IRIS data, the Wisconsin breast cancer data, and the credit screening data. The experiment results show that our method is more efficient in performance and comprehensibility of rules compared with other methods including C4.5.

  • In the relational database systems the join operation is one of the most time-consuming query operations. Many parallel join algorithms have been developed 개 reduce the execution time Multiple hash join algorithm using allocation tree is one of the most efficient ones. However, it may have some delay on the processing each node of allocation tree, which is occurred in tuple-probing phase by the difference between one page reading time of outer relation and the processing time of already read one. This delay problem was solved by using the concept of synchronization of page execution time with we had proposed In this paper the effects of the performance improvements in each node of the allocation tree are extended to the whole allocation tree and the performance evaluation about that is processed. In addition we propose an efficient algorithm for multiple hash joins in limited number of processor environments according to the relationship between the number of input relations in the allocation tree and the number of processors allocated to the tree. Finally. we analyze the performance by building the analytical cost model and verify the validity of it by various performance comparison with previous method.

Effect of Chlorhexidine Mouthrinse on Prevention of Microbial Contamination during EBUS-TBNA: A Study Protocol for a Randomized Controlled Trial

  • Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a standard diagnostic method for mediastinal and hilar lymphadenopathy. Although rare, fatal infectious complications can occur following EBUS-TBNA. However, to date, there is a lack of effective preventive strategies to reduce these complications. We started a trial to investigate the effect of chlorhexidine mouthrinse on the prevention of microbial contamination during EBUS-TBNA. Methods: This study is a single-center, parallel-group, assessor-blinded randomized controlled trial (RCT). We will enroll 112 adult participants undergoing EBUS-TBNA using a convex probe, and randomly assign them to two groups at a 1:1 ratio. The intervention group will gargle for 1 minute with 100 mL of 0.12% chlorhexidine gluconate before EBUS-TBNA, while the control group will have no mouthrinse before the procedure. Immediately after completion of EBUS-TBNA on all targeted lesions with an aspiration needle, a needle wash sample will be taken by instilling 5 mL of sterile saline into the used needle. The primary outcome is colony forming unit (CFU) counts in aerobic cultures of the needle wash samples. Secondary outcomes are CFU counts in anaerobic cultures, fever within 24 hours after EBUS-TBNA, and infectious complications within 4 weeks after EBUS-TBNA. Conclusion: This trial was designed as the first RCT to investigate the effect of chlorhexidine mouthrinse on the prevention of microbial contamination during EBUS-TBNA. Results from this trial can provide clinical evidence for a simple, safe, and cost-effective strategy to prevent infectious complications following EBUS-TBNA (ClinicalTrials.gov ID: NCT04718922, registered on 22 January 2021).
