Palatal vault configuration and its influence on intraoral scan time and accuracy in completely edentulous arches: a prospective clinical study

  • PURPOSE. The aim of this prospective clinical study was to compare the influence of palatal vault forms on accuracy and speed of intraoral (IO) scans in completely edentulous cases. MATERIALS AND METHODS. Based on the palatal vault form, participants were divided into three equal groups (n = 10 each); Class I: moderate; Class II: deep; Class III: flat palatal vault. A reference model was created for each patient using polyvinylsiloxane impression material. The poured models were digitized using an extraoral scanner. The resultant data were imported as a solid CAD file into 3D analysis software (GOM Inspect 2018; Gom GmbH, Braunschweig, Germany) and aligned using the software's coordinate system to determine its X, Y, and Z axes. Five digital impressions (DIs) of maxilla were captured for each patient using an intraoral scanner (TRIOS; 3Shape A/S, Copenhagen, Denmark) and the resultant Standard Tessellation Language (STL) scan files served as test models. Trueness was evaluated by calculating arithmetic mean deviation (AMD) of the vault area between reference and test files while precision was evaluated by calculating AMD between captured scans to measure repeatability of scan acquisition. The scan time taken for each participant was also recorded. RESULTS. There was no significant difference in trueness and precision among the groups (P = .806 and .950, respectively). Average scan time for Class I and III palatal vaults was 1 min 13 seconds and 1 min 37 seconds, respectively, while class II deep palatal vaults showed the highest scan time of 5 mins. CONCLUSION. Palatal vault form in edentulous cases has an influence on scan time. However, it does not have a substantial impact on the accuracy of the acquired scans.

  • The aim of present investigation was to clinically measure the thickness of palatal masticatory mucosa in the hard palate as potential donor site for mucogingival surgery, to determine the relation to shape of palatal vault, form of tooth, gender, and to serve the clinical criteria t o choose the proper surgical technique. 84(mean age:25yrs) systemically and periodontally healthy volunteers participated in this study and 18 standard measurement points were defined in the hard palate, located on 3 lies which ran at different distances parallel to the gingival margin. 6 positions were designated on each of these 3 lines between the level of canine and 2nd molar and a bone sounding technique using a periodontal probe with minimal local anesthesia was utilized to assess the thickness of palatal masticatory mucosa. Student t-test was used to determine the difference in mucosal thickness between 2 groups gender, shape of palatal vault (high palatal vault vs. low palatal vault), tooth form (short-wide vs. long narrow) The result of this study were as follows: 1. Soft tissue thickness progressively increased in sites further away from the gingival margin (p<0.01). 2. Depending on position, in line a and line c the masticatory mucosal thickness increased from Ca to M2(p<0.01), but in line b the thickness increased from Ca to P2, and decreased to M1 and increased again to M2. 3. Gender did not influence the thickness of masticatory mucosa. 4. Palatal vault shape was associated with the thickness of masticatory mucosa. Thickness of low palatal vault group was thicker than high palatal group between P2 and M2 position. 5. Form of tooth did not influence the thickness of masticatory mucosa. In conclusion, palatal vault shape was associated with the thickness of masticatory mucosa. So, mucogingival surgery can be considered as a treatment modality in high palatal vault group. But, Gender and tooth form did not influence the thickness of masticatory mucosa.

Does the palatal vault form have an influence on the scan time and accuracy of intraoral scans of completely edentulous arches? An in-vitro study

  • PURPOSE. The purpose of this study was to evaluate the influence of different palatal vault configurations on the accuracy and scan speed of intraoral scans (IO) of completely edentulous arches. MATERIALS AND METHODS. Three different virtual models of a completely edentulous maxillary arch with different palatal vault heights- Cl I moderate (U-shaped), Cl II deep (steep) and Cl III shallow (flat)-were digitally designed using CAD software (Meshmixer; Autodesk, USA) and 3D-printed using SLA-based 3D-printer (XFAB; DWS, Italy) (n = 30; 10 specimens per group). Each model was scanned using intraoral scanner (Trios 3; 3ShapeTM, Denmark). Scanning time was recorded for all samples. Scanning accuracy (trueness and precision) were evaluated using digital subtraction technique using Geomagic Control X v2020 (Geomagic; 3DSystems, USA). One-way analysis of variance (ANOVA) test was used to detect differences in scanning time, trueness and precision among the test groups. Statistical significance was set at α = .05. RESULTS. The scan process could not be completed for Class II group and manufacturer's recommended technique had to be modified. ANOVA revealed no statistically significant difference in trueness and precision values among the test groups (P=.959 and P=.658, respectively). Deep palatal vault (Cl II) showed significantly longer scan time compared to Cl I and III. CONCLUSION. The selection of scan protocol in complex cases such as deep palatal vault is of utmost importance. The modified, adopted longer path scan protocol of deep vault cases resulted in increased scan time when compared to the other two groups.

전산화단층촬영법을 통한 한국인의 구개 저작 점막 두께에 대한 연구 (Measurement of soft tissue thickness on posterior palatal area by using computerized tomography in Korean population)

  • The purpose of this study was to measure the thickness of masticatory mucosa in the hard palate as a donor site for mucogingival surgery by using computerized tomography(CT), Thickness measurements were performed in 84 adult patients who took CT on maxilla for implant surgery and 24 standard measurement points were defined in the hard palate according to the gingival margin and mid palatal suture. Radiographic measurements were utilized after calibration for standardization. Data were analyzed to determine the differences in mucosal thickness by gender, age, tooth positions and depth of palatal vault. The results of this study were as follows: 1. Mean thickness of palatal masticatory mucosa was $3.93{\pm}0.6mm$ and females had significantly thinner mean masticatory mucosa($3.76{\pm}0.56mm$) than males($4.04{\pm}0.6mm$)(p<0.05). 2. The thickness of palatal masticatory mucosa increased by aging. 3. Depending on position, masticatory mucosa thickness increased from canine to premeolar, but decreased at the first molar, and increased again in the second molar region(p<0.0001). 4. No significant difference in mean thickness of palatal masticatory mucosa were indentified between low palatal vault group and high palatal vault group(p>0.05). The results suggest that canine and premolar area appears to be the most appropriate donor site for soft tissue grafting procedure. The measurement of the thickness of palatal masticatory mucosa by using computerized tomography can offer useful information clinically but further studies in as-sessing the validity and reliability of the method using computerized tomography is needed.

Does hyrax expansion therapy affect maxillary sinus volume? A cone-beam computed tomography report

  • Purpose : The aim of this study was to investigate the initial effects of maxillary expansion therapy with Hyrax appliance and to evaluate the related changes in maxillary sinus volume. Materials and Methods : Thirty patients (20 females, 10 males; 13.8 years) requiring maxillary expansion therapy, as part of their comprehensive orthodontic treatment, were examined. Each patient had cone-beam computed tomography (CBCT) images taken before (T1) and after (T2) maxillary expansion therapy with a banded Hyrax appliance. Multiplanar slices were used to measure linear dimensions and palatal vault angle. Volumetric analysis was used to measure maxillary sinus volumes. Student t tests were used to compare the pre- and post-treatment measurements. Additionally, differences between two age groups were compared with Mann-Whitney U test. The level of significance was set at p=0.05. Results : Comparison of pre-treatment to post-treatment variables revealed significant changes in the transverse dimension related to both maxillary skeletal and dental structures and palatal vault angle, resulting in a widened palatal vault (p<0.05). Hard palate showed no significant movement in the vertical and anteroposterior planes. Nasal cavity width increased on a mean value of 0.93mm(SD=0.23, p<0.05). Maxillary sinus volume remained virtually stable. No significant age differences were observed in the sample. Conclusion : Hyrax expansion therapy did not have a significant impact on maxillary sinus volume.

안면골 골절에서 상하악 치열궁 복원을 위한 양측 대구치간 철사견인술의 유용성 (The usefulness of intermolar traction wiring for restoration of maxillary & mandibular dental arch in facial bone fracture)

  • Purpose: Palatal fracture and mandible fracture result in instability of dental arch. Because they divide the maxillary and mandibular alveolus sagittally and / or transversely and comminute the dentition, they permit rotation of dental alveolar segments and significantly increase the potential for fracture malalignment, complicating fracture treatment. Previous treatment of palatal fracture consisted of palatal splint application and rigid palatal vault stabilization. This procedure result in patient's oral discomfort and removal of palate and screw. Mandible fracture often results in malocclusion due to widening of posterior aspect of dental arch. So we introduce more simple method using intermolar traction wiring, which can protect the widening of dental arch and rotation of dental alveolar segment. Methods: Arch bar and intermolar traction wiring with wire 1 - 0, or 2 - 0 was applied. After exposure of fracture line, neutrooclusion was maintained with intermaxillary fixation. And then open reduction & internal fixation on maxillary fracture line, commonly maxillary buttress, alveolar ridge, pyriform aperture except palatal vault or mandibular fracture line. After 1 week, intermolar traction wiring was removed. We checked occlusion and postoperative radiologic finding. Results: From June of 2007 to October of 2007, 10 patient, who have maxillary fracture with palatal fracture and mandible fracture, underwent open reduction & internal fixation with intermolar traction wiring. All have satisfactory occlusion and there were no complication, like gingiva disease, mouth opening impairment and nonunion. Conclusion: The intermolar traction wiring accompany open reduction and internal fixation can be alternative method for restoration of dental arch in facial bone fracture.

A COMPARISON STUDY OF THE EFFECTS OF NASAL BREATHING DYSFUNCTION DUE TO ADENOID OBSTRUCTION ON DENTITION BY FACIAL TYPE

  • In discussing the effects of adenoids on the development of the face and dentition, it is important to note their influence on the mode of breathing and to relate this to specific facial types and dentition. This study, therefore, assumed that the ability to adapt to individual's neuromuscular complex is various. And tried to investigate the effects of reduced nasal respiratory function on the development of dentition by facial type. This paper is based on children patients with enlarged adenoids and comparing them to data taken from a control group with normal respiratory function. Among the three facial types, the most statistical significant difference was observed from dolichofacial type between experimental and control group. In dolichofacial type, the experimental group showed labioversion of upper incisor, decrease in the width of upper arch, increase in overjet, increase in the rate of cross-bite, and increase in the height of palatal vault. No significant difference was observed between the two groups in the inclination of upper and lower incisors in mesofacial type, but the experimental group was observed to show decrease in the width of upper arch and increase in the height of palatal vault. On the other hand, in brachyfacial type, no significant difference was observed between the two groups in dentition variables except showing linguoversion of upper incisor. The results, which were observed in dolichofacial type, consist with Nordlund's theory of Compression.

부정교합자의 상악치열궁 및 구개에 관한 연구 (A STUDY ON THE MAXILLARY DENIAL ARCH AND PALATAL VAULT WITH MALOCCULSIONS)

  • This study was based on the study models of 32 subjects with normal occlusion, 40 with Class I malocclusion, 32 with Class II, Division 1 malocclusion and 38 with Class III malocclusion, aged 12 to 20 years (mean age 16.4 years). The purpose of present study was to define the difference between normal and malocclusion groups in maxillary dental arch and palate. On the basis of findings of this study, the following results were obtained. 1. The intermolar widths and the intercanine widths in Class II, Div. 1 malocclusion group were smaller than in normal occlusion group significantly. 2. The arch lengths measured in both Class I and Class II, Div.1 malocclusion groups were larger than in normal occlusion group. 3. The palates in Class I and Class II, Div. 1 malocclusion groups were longer and narrower than in normal occlusion, but the palates in Class III malocclusion group were shorter than in normal occlusion group significantly. 4. The palatal depths measured at level 1 in Class III malocclusion group were significantly higher than in normal occlusion and in Class II, Div. 1 group they were significantly higher than in normal occlusion at level 2 and 3. 5. The measurements of palatal areas at various levels showed no significant difference between malocclusion and normal occlusion groups. 6. The palatal indies 1 (palatal length / palatal width) measured in both Class I and Class II, Div. 1 malocclusion groups were significantly greater than in normal occlusion and the palatal indice 2 (palatal depth at level 1/palatal width) measured in all malocclusion groups are greater than in normal occlusion. 7. It was determined from findings of this study that the measurements of maxillary dental arch and palate were influenced to a considerable extent by the molar relationship.

구개 증대 보철물과 임플란트 피개의치를 이용한 설암 환자의 보철수복 증례 (Prosthetic rehabilitation of a patient with tongue cancer using palatal augmentation prosthesis and mandibular implant-retained overdenture: A case report)

  • 혀는 구강 내에서 가장 흔하게 종양이 발생하는 부위이며, 특히 측면에서 호발한다. 설암으로 인해 혀의 절제를 시행한 환자의 경우 발음, 저작, 연하 그리고 구강 위생을 위해 적절한 움직임 등의 기능에 장애를 갖게 되므로 적절한 혀의 기능을 회복하기 위해 구개 증대 보철물을 이용할 수 있다. 본 증례는 설암으로 인해 혀의 절제를 시행한 환자에서 치조설열구 및 치조제 소실 등의 불리한 조건을 임플란트를 이용한 피개의치를 이용하여 개선하였다. 또한 금속 하부구조와 지지 및 유지부를 갖는 형태의 구개 증대 보철물을 제작하여 구개 높이를 낮추어 줌으로써 제한된 혀의 운동으로도 적절한 혀구개 접촉을 얻을 수 있었다. 최종보철물 장착 후 안정적인 저작기능을 회복하였고 연하 및 발음의 문제가 개선되어 이를 보고하고자 한다.
