소염약침으로 치료한 복합부위통증증후군(CRPS) type I 환자 1례 증례보고 (The clinical observation of 1 case of the Complex Regional Pain Syndrome)

  • Objective: The objective of this study is to observe the effect of anti-inflammatory herbal acupuncture on the Complex Regional Pain Syndrome Methods : Anti-inflammatory herbal acupuncture, A-Shi Point, Sa-am acupunture were used to treat shoulder pain & ROM(range of movement)disorder, chest pain, finger's causalgic pain. We evaluated the patient through VAS(Visual Analog Scale) daily and Physical Examinations Results & Conclusions : After 42days of treatment, shoulder pain was decreased from VAS10 to VAS3, chest pain was decreased from VAS10 to VAS1.5, finger's causalgic pain was decreased from VAS10 to VAS2.5. and the patient showed that the ROM(range of movement) of shoulder was better, oriental treatment is good method for pain relief and better movement.

  • Neurolytic splanchnic nerve block is a relatively safe and effective method for the relief of intractable pain caused by upper abdominal cancer. We have experienced a case of severe acute respiratory failure during splanchnic nerve block under control of X-ray fluoroscopy. We think that the most likely cause of the acute respiratory failure was an asthmatic attack due to anxiety and dyspnea from the injury or stimulation of the diaphragm and pleura in this case.

  • Objective: Case study of hemorrhoid improvement by ortho-cellular nutrition therapy. Methods: A Korean woman in her 40s had been experiencing hemorrhoid symptoms for two years, and recently, severe anal pain rendered her unable to carry out daily activities. Results: After implementing nutrition therapy, the patient's anal pain and edema were completely resolved. Conclusion: Applying nutrition therapy to patients with hemorrhoids can assist in symptom relief.

Successful Treatment of Severe Sympathetically Maintained Pain Following Anterior Spine Surgery

  • Sympathetic dysfunction is one of the possible complications of anterior spine surgery; however, it has been underestimated as a cause of complications. We report two successful experiences of treating severe dysesthetic pain occurring after anterior spine surgery, by performing a sympathetic block. The first patient experienced a burning and stabbing pain in the contralateral upper extremity of approach side used in anterior cervical discectomy and fusion, and underwent a stellate ganglion block with a significant relief of his pain. The second patient complained of a cold sensation and severe unexpected pain in the lower extremity of the contralateral side after anterior lumbar interbody fusion and was treated with lumbar sympathetic block. We aimed to describe sympathetically maintained pain as one of the important causes of early postoperative pain and the treatment option chosen for these cases in detail.

노인 간호 요양시설에서의 치매환자 통증관리 프로토콜 개발 및 효과 (Development and Effect of Pain Management Protocol for Nursing Home Patients with Dementia)

  • Purpose: This study was done to develop a pain management protocol for nursing home patients with dementia and to examine effects of the protocol on pain assessments and interventions by the nurses and on pain relief signs in the patients. Method: The six steps in the protocol development and the examination of effect are outlined. Three rounds using the Delphi technique and one group pretest-posttest design experiment were developed. Design issues, such as sample selection and sample size, are addressed in relation to the study protocol. Results: After implementation of the pain management protocol, there were significant changes nursing actions including frequency of number of physical examinations, utilization of pain assessment tools, and request to doctors for discomfort management and there were significant changes in frequency in the number of verbal and physical expressions of pain, and emotional patterns. Conclusion: This is the first pain management protocol for patients with dementia in Korea. However, more study will be needed to determine the methodological strength and necessary revisions for the protocol.

Peripheral nerve blocks for acute trigeminal neuralgia involving maxillary and mandibular branches: a case report

  • Trigeminal neuralgia (TN) is neuropathic pain that affects the trigeminal nerve branches. Facial pain experienced by patients with TN is typically intense and excruciating. The second and third branches (maxillary and mandibular) are commonly affected. This case report focuses on the potential treatment options for acute TN attacks involving these branches. The proposed approach involves extra-oral peripheral blocks using local anesthetics. Pain levels were measured using a visual numeric scale (VNS) with potential side effects and other relevant documented information. The patients showed responses from high pain levels to almost complete remission (from 8 to 2 and from 10 to 2 on the final VNS), with no significant side effects. This technique provides immediate pain relief and complements oral medications by offering comfort and confidence until the desired drug effect is achieved.

음악요법이 근골격계 외상환자의 통증 및 우울에 미치는 영향 (Effects of Music Therapy on Pain and Depression of Musculoskeletal Trauma Patients)

  • The purpose of this study was to determine the effects of music therapy on acute, subacute and chronic pain and depression of musculoskeletal trauma patients. The study was designed using nonequivalent control group pretest-posttest design within the framework of an adaptation model. The subjects were composed of forty patients, and twenty of them were assigned to the experimental group and twenty to the control group within the unit of patients. Data were summarized as follows : 1. There were significant changes of pain scores in an experimental and a control group measured before and after the treatment. 2. There were significant changes of pulse rates, respiration rates and systolic blood pressure, but were no significant changes of diastolic blood pressure in an experimental and a control group measured before and after the treatment. 3. There were no significant changes of the amount of ${\beta}$-endorphin in an experimental and a control group measured before and after the treatment. 4. There were no significant changes of depression scores in an experimental and a control group measured before and after the treatment. As a result, music therapy was a useful nursing intervention for relief of acute, subacute and chronic pain. Two suggestions could be made on the ground of the results of this study. 1. On the basis of endogenous pain control theory, it's necessary to research the changes of the amount of ${\beta}$-endorphin as the effect of the music therapy on patients having severe pain. 2. It's necessary to research the changes of the amount of ${\beta}$-endorphin according to the lapse of time after the music therapy for pain relief.

전산화 단층촬영 유도하 복강신경총 차단이 암성통증관리에 미치는 영향 (CT-guided Celiac Plexus Block Using Anterior Approach)

  • Backgroud: We have performed the CT-guided celiac plexus block (CPB) using anterior approach to evaluate the safety and efficacy of the procedure and to determine the role of CT. Methods: CPB were done in 10 patients (5 men and 5 women: mean age, 58.1 years) with intractable upper abdominal pain due to terminal malignancy of the stomach (n=3), pancreas (n=4), gallbladder (n=2), and liver (n=1). To permit an anterior approach, patients lay supine on the CT scan table during the procedure. One 21-guage Chiba needle was placed just anterior to the diaphragmatic crus between the celiac and superior mesenteric arteries and 10~12 ml of dehydrated alcohol was injected. Degree of pain relief following the procedure was assessed and pain was graded on a numeric rating scale (NRS) from 0 to 10. Results: The results suggest a direct relation between the degree of celiac invasion and the response to the CPB. With CT guidance, it is possible for us to direct the needle into more accurate region, allowing alcohol to be deposited in specific ganglion area. Conclusions: CT-guided CPB using an anterior approach was an easy and effective way of reducing intractable upper abdominal pain due to terminal malignancies. CT-guidance allowed precise needle placement and safe procedure. Careful classification of cases is important to predict the degree of pain relief using the grading system based on the degree of involvement of the celiac plexus.

향기요법 간호중재의 통증완화 효과 - 체계적 문헌고찰과 메타분석 - (Aromatherapy Nursing Intervention for Pain Relief A Systematic Review and Meta Analysis)

  • 본 연구는 체계적 문헌고찰과 메타분석을 이용하여 향기요법 간호중재의 통증완화에 대한 효과를 산출하고자 하였다. 연구출처는 KERIS, KISS, DBpia, Cochrane Library, Ovid-Medline와 Pubmed를 통해서 2000년부터 2014년까지 출판된 연구로 총 20개의 연구에 1029명의 대상자를 메타분석하였다. 연구대상이 되는 주요어는 PICO와 아로마, 향기, 향흡입, 향마사지, 아로마테라피, 통증, 통증관리 등이다. 메타분석결과를 살펴보면, 향기요법 간호중재의 통증완화에 대한 효과는 전체 효과뿐만 아니라 하위요인에서도 통계학적으로 유의한 차이를 나타내었다. 연구결과를 바탕으로 향기요법 간호중재가 통증완화 효과를 나타냈 수 있다는 근거자료를 확보하기 위해서는 추후연구가 필요하다고 사료된다.

발목염좌에 배혈에 따른 전침과 고주파온침의 진통효과 (Analgesia of Electroacupuncture and Radio-Frequency Warm Needling in Acupoint Combination on Ankle Sprain in Rats)

  • Objectives : The purpose of this study was to investigate the analgesic effect of electroacupuncture(EA) and radio-frequency warm needling(RFWN) stimulation in acupoint combination on ankle sprained pain in rats. Methods : The lateral ligaments of the Sprague-Dawley rats ankle were injured surgically resulting in sprain, of which was divided into EA, RFWN treatment groups and control group without treatment. The level of pain was measured through foot weight bearing force ratio followed by calculating pain relief. To stimulate proximal or distal area in ankle sprain, combination of proximal acupoints(GB34-GB39) and distal acupoints(GB39-GB42) from sprain area were applied, respectively, to either EA or RFWN stimulation. In addition, naltrexone or phentolamine was injected intraperitoneally before the stimulation to observe the pathway of analgesic effects. Results : In the proximal combination of GB34-GB39, EA and RFWN significantly increased pain relief compared to the control group (p<0.05). However, in distal combination with GB39-GB42, both EA and RFWN stimulation did not relieve pain due to ankle sprains. In the combination of GB34-GB39, the analgesia of EA was inhibited by blockade of the ${\alpha}$-adrenoceptor receptor. The analgesia of RFWN was inhibited by blockade of the ${\alpha}$-adrenoceptor receptor as well as ${\mu}$-opioid receptor. Conclusions : We observed that the proximal combination was effective in relieving pain when the treatment by acupoint combination was applied to the ankle sprain pain. Also, it was confirmed that this analgesia was also related to the pathways of ${\mu}$-opioid receptors and/or ${\alpha}$-adrenoceptors.
