Preoperative Identification of Perforator Using CT Angiography in Fibular Osteocutaneous Free Flap Head and Neck Reconstruction (유리비골피부피판을 이용한 두경부 재건 시 CT Angiography를 통한 천공지의 확인)

  • Purpose: Fibular osteocutaneous free flap is the procedure of choice for mandibular reconstruction. However, the anatomic consistency and the reliability of the skin paddle have been considered to be questionable and the utilization of the fibular osteocutaneous free flap can be challenging for the inexperienced surgeon. Preoperative computed tomography (CT) angiography can support revolutionary help with the operator design of the fibular osteocutaneous flap. The purpose of this article is to share the valuable experience of support with preoperative CT angiography. Methods: Three consecutive patients, who needed mandibular reconstruction, were treated with fibular osteocutaneous free flap. Each of the patients had undergone lower extremity CT angiography before the surgery. The CT angiographies were scrupulously investigated to calculate the locations and the tracts of the peroneal artery perforators. We compared the findings of the CT angiography with those of the real operation. Results: The information about the perforators was sufficiently matched with the findings of the operation. With the use of preoperative CT angiography, we were able to achieve confident performance during operation, shortening of operation time, and fine outcomes with a no flap failure. Conclusion: The CT angiography of lower extremity can provide reliable information of the perforators of the fibular osteocutaneous free flap.

Formulation of Rectal Supopositories of Omeprazole (오메푸라졸 함유 직장좌제의 제제설계)

  • In order for formulation of rectal containing OMZ, the OMZ suppositories were prepared using water-soluble base, PEG 4000 base and oil-soluble base, Witepsol H 15. Chemical stability of OMZ in suppositories was increased when Witepsol H 15 was used as a suppository base and arginine was added as a stabilizer. The decomposition of OMZ in suppository bases followed the first-order kinetics and their rate constants were 0.11 day $^{1}(t_{1/2}$=/6.25 days) for Witepsol H 15 suppository and 0.48 day $^{1}(t_{1/2}$=/1.43 days) for PEG 4000 suppository, respectively. On the other hand, the decomposition rate constants of Witepsol suppository and PEG suppository stabilized with arginine were 3.89$\times$10$^{-3}$(t$_{1/2}$=171.1 days) and 8.76$\times$10$^{-3}$ day $^{1}(t_{1/2}=79.9 days), respectively. Shelf-lives of the Witepsol and PEG suppositories stabilized with arginine were t$_{90%}$=291.8 days and t$_{90%}$=282.1 days at $35^{\circ}C$ and 75% RH, respectively. The dissolution test of OMZ suppositories was performed by rotating dialysis cell(RDC) method and the release rate constant was calculated by the simplified Higuchi's equation, Q'=K' t$^{1/2}$. Dissolution of OMZ from suppositories was augmented as arginine was added, particle size of OMZ was reduced and a suitable surfactant such as SLS was added. RDC method was more appropriate and available than Paddle method to evaluate the dissolution rate of lipophilic-base suppositoies. Arginine was found to be a very useful exipient for the enhancement of stability and dissolution of OMZ in suppositories.

  • To develop a sustained-release tablet which had the similar dissolution to commercial ibudilast-loaded sustained-release capsule, the tablets were prepared using hydroxypropylmethylcellulose phthalate (HPMCP), ethylcellulose (EC) and hydroxypropylcellulose (HPC), and dissolution test were carried out with paddle method in KP. The tablet prepared only with HPMCP and EC showed sno similar dissolution pattern to the commercial product. As the ratio of HPMCP/HPC in tablet decreased, the dissolution rate of drug decreased in pH 1.2 but increased in pH 6.8. Furthermore, an ibudilast-loaded sustained-release tablet composed of [ibudilast/EC/HPMCP/HPC (10/10/170/10 mg/tab)] gave similar dissolution to commercial product in pH 1.2 for 3 h and in pH 6.8 for 10 h. Thus, it could be a potential candidate for the substitute of commercial capsule.

Sensing Technologies for Grain Crop Yield Monitoring Systems: A Review

  • Purpose: Yield monitoring systems are an essential component of precision agriculture. They indicate the spatial variability of crop yield in fields, and have become an important factor in modern harvesters. The objective of this paper was to review research trends related to yield monitoring sensors for grain crops. Methods: The literature was reviewed for research on the major sensing components of grain yield monitoring systems. These major components included grain flow sensors, moisture content sensors, and cutting width sensors. Sensors were classified by sensing principle and type, and their performance was also reviewed. Results: The main targeted harvesting grain crops were rice, wheat, corn, barley, and grain sorghum. Grain flow sensors were classified into mass flow and volume flow methods. Mass flow sensors were mounted primarily at the clean grain elevator head or under the grain tank, and volume flow sensors were mounted at the head or in the middle of the elevator. Mass flow methods used weighing, force impact, and radiometric approaches, some of which resulted in measurement error levels lower than 5% ($R^2=0.99$). Volume flow methods included paddle wheel type and optical type, and in the best cases produced error levels lower than 3%. Grain moisture content sensing was in many cases achieved using capacitive modules. In some cases, errors were lower than 1%. Cutting width was measured by ultrasonic distance sensors mounted at both sides of the header dividers, and the errors were in some cases lower than 5%. Conclusions: The design and fabrication of an integrated yield monitoring system for a target crop would be affected by the selection of a sensing approach, as well as the layout and mounting of the sensors. For accurate estimation of yield, signal processing and correction measures should be also implemented.

Usefulness of the Purse-string Suture Technique for Aesthetic Breast Reconstruction Surrounding the Nipple-areolar Complex (유두 유륜 복합체 주변의 미용적 유방 재건을 위한 주머니끈 봉합의 유용성)

  • Purpose: In cases of breast reconstruction with a free transverse rectus abdominis musculocutaneous (TRAM) flap after skin-sparing mastectomy with nipple-areolar complex (NAC) removal, the flat contour of the flap's skin paddle can easily look unnatural and dissatisfying. Reconstructed NAC on the flap surface lacks the elevated contour that the normal areola possesses, resulting in an unnatural final result. Therefore, we would like to introduce a novel method to improve this problem and report the satisfactory results we obtained. Methods: Operations were conducted on 19 patients who underwent immediate breast reconstruction with a free TRAM flap and skin-sparing mastectomy from January 2009 to January 2010, with a mean follow-up of nine months. While the TRAM free flap was being inset, a purse-string suture was carried out on the dermal layer of skin flap to create a slight protrusion with Gore-$Tex^{(R)}$ sutures. Results: The elevated mound surrounding the NAC was well maintained for an average follow-up period of nine months. Nipple projection was also well maintained. There was no complaint about breast contour or nipple height reduction. In addition, there was no reported incidence of other complications. Conclusion: The purse-string suture technique presents a more natural breast silhouette around the NAC and helps to maintain nipple projection. Furthermore, it does not require any supplementary incisions or complicated skills. There has been no report of additional complications using this technique.

An Experimental Investigation of the Aeroelastic Stability of Next-generation Blade for Helicopter (헬리콥터용 차세대 블레이드의 공력탄성학적 안정성에 관한 시험적 연구)

  • This paper describes the aeroelastic stability test of the small-scaled 'Next-generation Blade(NRSB)' with NRSH (next-generation hub system) and HCTH hingeless hub system in hover and forward flight conditions. Excitation tests of rotor system installed in GSRTS (general small-scale rotor test system) at KARI (Korea Aerospace Research Institute) were carried out to get lead-lag damping ratio of blades with flexures as hub flexure. MBA(moving block analysis) technique was used for the estimation of lead-lag damping ratio. First, NRSB-1F blades with HCTH hub system, then NRSB- 1F with NRSH hub system were tested. Second, NRSB-2F blades with NRSH hub system were tested. Tests were done on the ground and in the wind tunnel according to the test conditions of hover and forward flight, respectively. Non-rotating natural frequencies, non-rotating damping ratios and rotating natural frequencies were showed similar level for each cases. Estimated damping ratios of NRSB-1F, NRSB-2F with HCTH and NRSH were above 0.5%, and damping ratio increased by collective pitch angle increasement. Furthermore damping ratios of NRSB-2F were higher than damping ratios of NRSB-1F in high Pitch angle. It was confirmed that the blade design for noise reduction would give observable improvement in aeroelastic stability compared to paddle blade and NRSB-1F design.

Clinical Experience of Thoracodorsal Perforator Based Free Flap (흉배혈관 천공분지에 기초한 유리피판술의 임상적 이용)

  • Perforator flaps have become increasingly popular in microsurgery nowadays and are being used widely for many cases of reconstruction after trauma and cancer ablation. And thoracodorsal perforator based free flap is one of them having the merits of carrying a large skin paddle with leaving intact innervation and function of the remaining latissimus dorsi muscle. We made a homogeneous thin flap excluding the main muscle with a long vascular pedicle and tried to decrease the donor site morbidity. But, it needs a long learning-curve and we have met marginal flap necrosis frequently. Besides, prolonged operation time for complete perforator dissection may be a tedious job to the microsurgeon. To overcome these disadvantages, we usually included very small portion of the latissimus dorsi muscle during this flap elevation around the pedicled 2-3 thoracodorsal perforators during this flap elevation. We performed 3 cases of thoracodorsal perforator based free flap at Hallym university sacred heart hospital between May and August 2005 for the soft tissue defect of the scalp and feet. The average flap size was $8{\times}14\;cm$. Although it is not a true perforator flap, we can get the reliability for the flap survival with much better blood circulation and save the time of one or two hours to dissect the perforators completely. All cutaneous flaps survived completely without any complication except one fatty female who had the very small superficial fat necrosis due to flap bulkiness. We believe the thoracodorsal perforator based free flap can be extended its versatility and reliability by including the very small portion of the muscle around the perforators.

Yield monitoring systems for non-grain crops: A review

  • Yield monitoring systems have become integral to precision agriculture, providing insights into the spatial variability of crop yield and playing an important role in modern harvesting technology. This paper aims to review current research trends in yield monitoring systems, specifically designed for non-grain crops, including cabbages, radishes, potatoes, and tomatoes. A systematic literature survey was conducted to evaluate the performance of various monitoring methods for non-grain crop yields. This study also assesses both mass- and volume-based yield monitoring systems to provide precise evaluations of agricultural productivity. Integrating load cell technology enables precise mass flow rate measurements and cumulative weighing, offering an accurate representation of crop yields, and the incorporation of image-based analysis enhances the overall system accuracy by facilitating volumetric flow rate calculations and refined volume estimations. Mass flow methods, including weighing, force impact, and radiometric approaches, have demonstrated impressive results, with some measurement error levels below 5%. Volume flow methods, including paddle wheel and optical methodologies, yielded error levels below 3%. Signal processing and correction measures also play a crucial role in achieving accurate yield estimations. Moreover, the selection of sensing approach, sensor layout, and mounting significantly influence the performance of monitoring systems for specific crops.

Research on Improving Drying Technology For Sewage Waste Using Direct Flotation Using Heat Storage Media (축열메디아 활용 직접부상방식을 이용한 하수찌꺼기의 건조기술 향상에 관한 연구)

  • This study was conducted to improve energy efficiency and problems such as clumping and fouling in the glue zone that occur in the moisture content range of 40 to 60% when sewage dehydration residue is directly fed into the dryer. The temperature of the hot air is low at 270~300℃, and the paddle-type flotation method and dehydrated residue are applied to the circulated heat storage media to increase the contact area with the hot air, thereby reducing energy recovery and gas emissions. The water content of the dried residue is 2.7. ~7 .5%, the heat of evaporation of moisture was 608.0~690.6 kcal/kg·H2O, which confirmed an energy saving effect of about 8.8% compared to the heat of evaporation of moisture of 714.5 kcal/kg·H2O when no heat storage media was used.

Surgical Considerations of One-Stage Reconstruction of Large Extremity Defects Using a Thin Deep Inferior Epigastric Perforator Flap

  • Background One-stage reconstruction with "thin perforator flaps" has been attempted to salvage limbs and restore function. The deep inferior epigastric perforator (DIEP) flap is a commonly utilized flap in breast reconstruction (BR). The purpose of this study is to present the versatility of DIEP flaps for the reconstruction of large defects of the extremities. Methods Patients with large tissue defects on extremities who were treated with thin DIEP flaps from January 2016 to January 2018 were included. They were minimally followed up for 36 months. We analyzed the etiology and location of the soft tissue defect, flap design, anastomosis type, outcome, and complications. We also considered the technical differences in the DIEP flap between breast and extremity reconstruction. Results Overall, six free DIEP flaps were included in the study. The flap size ranged from 15 × 12 to 30 × 16 cm2. All flaps were transversely designed similar to a traditional BR design. Three flaps were elevated with two perforators. Primary closure of the donor site was possible in all cases. Five flaps survived with no complications. However, partial necrosis occurred in one flap. Conclusion A DIEP flap is not the first choice for soft tissue defects, but it should be considered for one-stage reconstruction of large defects when the circulation zone of the DIEP flap is considered. In addition, this flap has many advantages over other flaps such as provision of the largest skin paddle, low donor site morbidity with a concealed scar, versatile supercharging technique, and a long pedicle.
