Clinical Evaluation of open Thoracotomy in Spontaneous Pneumothorax (자연기흉의 개흉례에 대한 검토)

  • Spontaneous pneumothorax is the sudden collapse of a lung usually caused by air leaking from a sub-visceral pleural bleb. Response to closed thoracotomy, needle aspiration and simple observation is usually prompt and effective. But in some cases, these are unsuccessful and open thoracotomy is indicated. Author reviewed 37 cases of open thoracotomy in spontaneous pneumothorax experienced in the Dept. of Thoracic and Cardiovascular Surgery, Pusan National University Hospital, since Jan., 1980 to Dec., 1984. The results were as follows: 1. The causes of spontaneous pneumothorax: 73.0% was primary, 27.0% was secondary origin. 2. The most frequent age group of the patient: Between 11 and 30 years old. 3. All of te patient were male. 4. The side of open thoracotomy: 58.8% was right side, 8.8% was both side. 5. The most common indication of open thoracotomy; Persistent air leakage. 6. The most frequent sites of bleb or bullae: A-P segment in the L.U.L. and apical segment in the R.U.L.

Isovitexin Is a Direct Inhibitor of Staphylococcus aureus Coagulase

  • Staphylococcus aureus (S. aureus) is a major pathogen that causes human pneumonia, leading to significant morbidity and mortality. S. aureus coagulase (Coa) triggers the polymerization of fibrin by activating host prothrombin, which then converts fibrinogen to fibrin and contributes to S. aureus pathogenesis and persistent infection. In our research, we demonstrate that isovitexin, an active traditional Chinese medicine component, can inhibit the coagulase activity of Coa but does not interfere with the growth of S. aureus. Furthermore, we show through thermal shift and fluorescence quenching assays that isovitexin directly binds to Coa. Dynamic simulation and structure-activity relationship analyses suggest that V191 and P268 are key amino acid residues responsible for the binding of isovitexin to Coa. Taken together, these data indicate that isovitexin is a direct Coa inhibitor and a promising candidate for drug development against S. aureus infection.

  • We present a case of intracranial aneurysm located in the P1 segment of left posterior cerebral artery in the context of tetralogy of Fallot. Complex variations included right aortic arch with abnormal branching. Also, the bilateral vertebral arteries were absent, with a type I persistent proatlantal intersegmental artery of the left side. The aneurysm was treated with endovascular intervention with a Tubridge flow diverter and was noted to be completely cured on 6-month follow-up. We discuss the many considerations in this patient including developmental and modern-era treatment.

Fermentation properties of beer produced from Korean two-row barley or malt (Gwangmaek) supplemented with Korean red ginseng extracts and Bokbunja (Rubus coreanus Miquel) juice

  • This study involved the production of specialty lager beers supplemented with Korean red ginseng extracts or Bokbunja (Korean black raspberry, Rubus coreanus Miquel) juice. The effects of the Korean red ginseng extracts or Bokbunja juice on the specific gravity, pH, yeast viability, free amino nitrogen content, reducing sugar content, color, alcohol content, turbidity, and sensory evaluation were evaluated. The alcohol content of the beers containing the extracts or juice were within the standard alcohol amounts (3.63-4.0%, v/v). The pH values of the three samples containing Bokbunja juice were lower than that of the control values. The sensory evaluation showed that the addition of Bokbunja juice was superior to the ginseng extracts, and the optimal addition time was before or after the secondary fermentation. These data indicate that the flavor and odor of the Bokbunja juice are more persistent than that of the ginseng extracts.

  • Tris (2-methyl-1-aziridinyl) phosphine oxide (MAPO) is extremely poisonous and persistent in aqueous media. An efficient UV/nano$TiO_2$ process was employed for its mineralization in a high duty falling film photo-reactor based on an experimental design scheme that considers interactions between the main variables. The influencing variables and their range were determined with preliminary studies. The results show substrate mineralization to some extent under mild conditions of: T = $30^{\circ}C$, pH = 8.5, $[MAPO]_0=60\;mg\;L^{-1}$ and $[TiO_2]=110\;mg\;L^{-1}$. The relative importance of the influencing parameters were initial pH > temperature > $[MAPO]_0$ > [$TiO_2$]; while the interdependence of all the parameters was significant. Accordingly, a reduced quadratic expression was developed. Meanwhile, mineralization kinetic studies, based on chemical oxygen demand, revealed a power law model with order of 2.6 during process time until 150 min.

  • We note that the dynamics of realized volatilities (RVs) are near the boundary between stationarity and non-stationarity because RVs have persistent long-memory and are often subject to fairly large outlying values. To forecast realized volatility, we consider a new method that adaptively use models with and without unit root according to the abnormality of observed RV: heterogeneous autoregressive (HAR) model and the Integrated HAR (IHAR) model. The resulting method is called the IHAR-O-HAR method. In an out-of-sample forecast comparison for the realized volatility datasets of the 3 major indexes of the S&P 500, the NASDAQ, and the Nikkei 225, the new IHAR-O-HAR method is shown superior to the existing HAR and IHAR method.

  • This paper is intended as a preliminary study on phonetic and phonological differences between Polish and Korean languages. In this paper an attempt is made to examine the most conspicious difficulties encountered by Polish learners who begin to speak Korean (and in doing so, 1 would hope that it might be of help to future learners of both languages). Since the phoneme inventory and general phonetic rules for both languages are very different, teaching and learning accurate pronunciation is extremely difficult for both the Poles and Koreans without any previous phonetic training. In the case of Polish and Korean we can see how strong and persistent the influences of the mother-tongue are on the target language. As an example I would like to discuss the basic differences between Polish and Korean consonants. The most important consonantal opposition in Polish is voice-/voicelessness (f. ex.; 〔b〕 / 〔p〕, 〔g〕 / 〔k〕) while in Korean, opposition such as voice-/voicelessness is of secondary importance. Therefore Korean speakers do not perceive the difference between Polish voiced and voiceless consonants. On the other hand, Polish speakers can not distinguish Korean lenis / fortis / aspirated consonants (f. ex.; ㅂ 〔b〕 / ㅃ 〔p〕 / ㅍ〔ph〕, ㄱ 〔g〕 / ㄲ 〔k〕 / ㅋ 〔kh〕)) opposition. The other very important factor is palatalization which is of vital importance in Polish and, because of this, Polish speakers are extremely sensitive to it. In Korean palatalization is not important phonetically and Korean speakers do not distinguish between palatalized and non-palatalized consonants. The transcription used here is based on ' The principles of the International Phonetic Association and the Korean Phonetic Alphabet ' (1981) by Hyun Bok Lee.

Discourse Analysis on ICT and Home - From Electronic Cottage to Sharable Home - (정보통신기술과 주거에 대한 중요 담론 분석연구)

  • This paper draws the home into the stream of information and communications technology (ICT) development. Considering that homes and our home life are constantly affected by persistent change influenced by ICT, it is remarkable that ICT has been relatively neglected as an object of research in the field of housing studies. This study provides an overview of conceptual movements in ICT/home relationships and their design and social impact through a critical discourse analysis. The conceptual movements must be considered by professionals in the field of housing and built environment to stay attuned to changes in the 21st Century. Diverse debates took place in the discourse about ICT in relation to home. The stream of thought was started by futurists who saw the electronic cottage as a solution to the social problems caused by the separation of work and home life in industrial cities. ICT has now entered a new phase, with WiFi P2P networks and location-based social media that encourage sharable homes. Based on the analysis of the discourse on ICT and home, we drew from three issues related to the paradigm shifts in homes in the context of ICT development to provide future directions for housing studies: from space programming to time scheduling, from individual ownership to shared access, and from live+work dwelling to live+work+community.

Efficacy of proton pump inhibitors and H2 blocker in the treatment of symptomatic gastroesophageal reflux disease in infants

  • Purpose: Gastroesophageal reflux disease (GERD) occurs in pediatric patients when reflux of gastric contents presents with troublesome symptoms. The present study compared the effects of omeprazole and ranitidine for the treatment of symptomatic GERD in infants of 2-12 months. Methods: This study was a clinical randomized double-blind trial and parallel-group comparison of omeprazole and ranitidine performed at Children Training Hospital in Tabriz, Iran. Patients received a standard treatment for 2 weeks. After 2 weeks, the patients with persistent symptoms were enrolled in this randomized study. Results: We enrolled 76 patients in the present study and excluded 16 patients. Thirty patients each were included in group A (ranitidine) and in group B (omeprazole). GERD symptom score for groups A and B was $47.17{\pm}5.62$ and $51.93{\pm}5.42$, respectively, with a P value of 0.54, before the treatment and $2.47{\pm}0.58$ and $2.43{\pm}1.15$, respectively, after the treatment (P=0.98). No statistically significant differences were found between ranitidine and omeprazole in their efficacy for the treatment of GERD. Conclusion: The safety and efficacy of ranitidine and omeprazole have been demonstrated in infants. Both groups of infants showed a statistically significant decrease in the score of clinical variables after the treatment.
