수근부에 발생한 유골 골종: 증례 보고 (Osteoid Osteoma in Wrist: A Case Report)

  • 유골 골종은 골격계의 모든 부위에서 발생 가능하지만 하지에서 절반 이상이 보고 되고 있고 수근부에서 발생이 드문 것으로 알려져 있다. 임상적으로는 통증이 거의 유일한 증상이며 주로 밤에 악화 되고, asprin이나 다른 NSAIDs에 의해 빠르게 호전 되는 특징이 있다. 저자들은 전형적인 임상양상을 나타냈던 수근부의 유골골종, 증례 2예를 방사선 검사로 진단 후 수술적 치료로 좋은 결과를 얻어 문헌고찰과 함께 보고하는 바이다.

  • 이비인후과 영역에서 발생하는 골종은 아주 희소하나 부비동에서 발생하는 골종은 이 부위의 양성종양중 가장 많으며 전두동에서 가장 호발한다고 한다. 골종은 30대에서 가장 호발하며 조직학적으로 골수가 경성인것, 연성인것, 혼합형인것 등으로 분류할 수 있으며 발생원인으로는 간배세포 종양설, 외상설, 염증설 등이 있다. 최근, 저자들은 좌측 전두동의 거대한 골종으로 인해 안구돌출 및 안구하강을 동반한 44세의 남자환자에서 성형적 전두동 수술로 종양을 제거하고 그 결손부위를 지방조직으로 이식하여 좋은 결과를 얻었기에 문헌과 함께 보고하는 바이다.

  • A 22-year-old man is presented with 3 months' history of dull pain on the lower thoracic area. Imaging studies suggested osteoid osteoma of 11th thoracic spine, which was verified pathologically after en bloc resection through transfacetal approach. The patient's preoperative pain was resolved immediately. We emphasize the importance of early diagnosis and complete surgical excision of tumor nidus.

  • The review of the literature determines that large osteomas of the mandible are relatively rare. We present a case of a 60-year-old man with painless swelling of the left submandibular area and mild difficulty in breathing. The patient complained that the mass have been progressing slowly for at least 20 years. Radiographic studies showed a giant osteoma that attached to the lingual surface of the left mandibular angle and extended from submandibular space to infratemporal space. After the excision of the tumor the patient made a full recovery.

  • Osteoma is a benign osteogenic tumor composed of cancellous or compact bone, classified as peripheral, central, or extraskeletal. Peripheral osteomas are uncommon. Excluding the maxillary sinuses, the maxilla is a rare site for osteomas. The purpose of this report was to describe clinicopathological and radiological features of two peripheral osteomas occurring in the jaws, one located in the mandible and another in the edentulous maxillary alveolar ridge. The tumors were asymptomatic and were fully excised without any complications or recurrence. The lesions were submitted to histopathological analysis and diagnosed as peripheral osteoma, compact type.

  • Osteoid osteomas are benign skeletal neoplasms that are commonly encountered in the bones of the lower extremities, but are exceedingly rare in jaw bones with a prevalence of less than 1%. This unique clinical entity is usually seen in younger individuals, with nocturnal pain and swelling as its characteristic clinical manifestations. The size of the lesion is rarely found to be more than 2 cm. We hereby report a rare case of osteoid osteoma originating from the neck of the mandibular condyle that grew to large enough proportions to result in conductive hearing loss in addition to pain, swelling and restricted mouth opening. In addition, an effort has been made to review all the documented cases of osteoid osteomas of the jaws that have been published in the literature thus far.

  • 골종은 1586년 Veiga에 의해 처음 기술되었는데 그 원인에 대해서는 아직 학설로 남아 있는 상태이다. 진성의 골종은 주로 유양돌기나 측두골에서 나타나지만, 그 예가 비교적 희귀하다고 하겠다. 이 골증은 양성으로서 서서히 커지며 성숙된 골세포에 의하여 구성된 경우가 대부분이다. 치료는 환자에게 증상이 나타날 때 그 적응이 된다. 저자들은 우측 측두골의 골종으로 약 18년간 이후부의 계속적인 종창, 골부 외이도의 폐쇄, 농성이루 및 청력감소를 초래한 환자를 경험하였기에 보고하는 바이다.

  • Mastoid osteomas of the temporal bone are rare, benign, and usually asymptomatic tumors. However, depending on their size and extension, mastoid osteomas may cause facial palsy, a sensation of ear fullness, pressure-related pain, hearing loss, recurrent external ear infections, and chronic discharge. The etiology of mastoid osteomas is still unknown, but congenital, infectious, and traumatic factors have been proposed. Surgical treatment may be performed with minimal postoperative morbidity. In this article, the authors report a case of a 48-year-old woman seeking orthodontic treatment with an unusual retroauricular protruding mass, including the diagnostic process and differential diagnosis. This case supports the essential role of cone-beam computed tomography to analyze and identify the lesion as a mastoid osteoma.

하악 과두부에 발생한 종양의 외과적 치험 2례

  • The occurrence of osteochondroma, osteoma in mandibular condlye is rare. Sign and symptoms of enlargement of condyle are open bite, cross bite, myofascial symptoms, facial asymmetry. They are treated via condylectomy, subcondylar ostetomy, etc. We treated osteochondroma with condylectomy, osteoma with condylectomy & arthroplasty with titanium mesh & resin ball. Followings are the results 1. Condylectomy of osteochondroma resulted in good function & esthetics except slight deviation of mandible to the operation side during mouth opening. 2. Condylectomy & arthroplasty with titanium mesh & resin ball resulted in good function & esthetics & no deviation of mandible.
