Objective: This study aimed to evaluate the differences in cephalometric characteristics and skeletal maturation in girls with central precocious puberty (CPP) via lateral and hand-wrist radiographs. We also aimed to identify the indicators that are most effective for determining skeletal maturity in these patients. Methods: The study included 70 Korean girls (mean age, 8.5 ± 0.5 years) diagnosed with CPP at the Department of Pediatrics, and 48 normal healthy age-matched girls who visited the Department of Orthodontics and had no history of hormone treatment or growth problems. Skeletal maturation was evaluated using lateral cephalometric and hand-wrist radiographs using cervical vertebrae maturation indicators (CVMI) and skeletal maturity indicators (SMI). Results: The mean mandibular plane angle was smaller in the CPP group than in the control group (35.8° ± 4.9° vs. 39.0° ± 6.5°), resulting in greater posterior facial height (p = 0.003). SMI was significantly greater in the CPP group (3.5 ± 1.4 vs. 2.0 ± 1.0) than in the control group (p = 0.001) and was significantly associated with CPP (r = 0.492; p = 0.001), whereas CVMI was not. Conclusions: In comparison with the control group, the CPP group exhibited a smaller mandibular plane angle, greater posterior facial height, and greater skeletal maturation. SMI may be more suitable than CVMI for determining skeletal maturation in CPP. Hand-wrist radiography is recommended in addition to lateral cephalogram for predicting growth in girls with CPP.
This investigation was undertaken to know how soft tissue facial profile could be changed with age. The 3 serial lateral cephalometric roentgenograms of the twenty nine boys and twenty six girls between 9 and 13 years of age were studied and the findings seemed to warrant the following conclusions. 1. The author made the tables of means, standard deviations in each item, sex, age. 2. Soft tissue facial angle, soft tissue facial convexity including the nose tended to increase, but others tended to remain relatively stable. 3. Facial soft tissue thickness increased with age and the growth of facial soft tissue in the middle region (point A, LS, LI) was greater than others in the facial region. 4. In the soft tissue vertical proportions, G1'-Sn/Sn-Me' was 1.1, Sn-St/St-Me' was 0.51:1, Sn-LI/LI-Me' was 0.82:1 and those were stable with age.
Objective: To evaluate the factors that affect torque control during anterior retraction when utilizing the C-retractor with a palatal miniplate as an exclusive source of anchorage without posterior appliances. Methods: The C-retractor was modeled using a 3-dimensional beam element (0.9-mm-diameter stainless-steel wire) attached to mesh bonding pads. Various vertical heights and 2 attachment positions for the lingual anterior retraction hooks (LARHs) were evaluated. A force of 200 g was applied from each side hook of the miniplate to the splinted segment of 6 or 8 anterior teeth. Results: During anterior retraction, an increase in the LARH vertical height increased the amount of lingual root torque and intrusion of the incisors. In particular, with increasing vertical height, the tooth displacement pattern changed from controlled tipping to bodily displacement and then to lingual root displacement. The effects were enhanced when the LARH was located between the central and lateral incisors, as compared to when the LARH was located between the lateral incisors and canines. Conclusions: Three-dimensional lingual anterior retraction of the 6 or 8 anterior teeth can be accomplished using the palatal miniplate as the only anchorage source. Using LARHs at different heights or positions affects the quality of torque and intrusion.
Objective: The objective of this study was to investigate the differences in masticatory efficiency among patients with different Angle's classes of malocclusion and to assess the correlation between masticatory efficiency and the occlusal contact area. Methods: The mixing ability index (MAI) was calculated for measuring masticatory efficiency of 61 adult patients according to Angle's classifications of malocclusion. The study included 25, 15, and 21 patients with Angle's Class I, II, and III malocclusions, respectively. Silicone interocclusal recording material was used to measure the occlusal contact area. Results: Both the MAI and occlusal contact area showed the highest average values in the Class I malocclusion group, followed by the Class II and Class III malocclusion groups. No significant difference was observed in the MAI values between the Class I and Class II malocclusion groups (p > 0.05), whereas a significant difference was observed between the Class I and Class III malocclusion groups (p < 0.01) and between the Class II and Class III malocclusion groups (p < 0.05). A weak positive correlation was also observed between the MAI and occlusal contact area (p < 0.01, $r^2=0.13$). Conclusions: The results of this study indicated that masticatory efficiency was the highest in patients with Angle's Class I malocclusion, followed by those with Angle's Class II and Angle's Class III malocclusions. Moreover, a weak positive correlation was observed between masticatory efficiency and the occlusal contact area.

Orthodontic treatment in conjunction with second-molar extraction has been a controversial issue among orthodontists over many decades. The aim of this study was to investigate the treatment effects of upper second molar extraction cases. The sample included 19 upper second molar extraction orthodontic cases(ten Angle's Class I's and nine Class II's, average age=13Y 6M) cared at Kyung-Hee University Department of Orthodontics. Lateral cephalometric radiographs were taken before and immediately after treatment. Seventy-nine points were digitized on each cephalogram and 38 cephalometric parameters were computed comprising 22 angular measurements, 13 linear measurements, and 3 facial proportions. The data obtained from each malocclusion group were analyzed by paired t-test. The statistical results disclosed that there was no significant change in skeletal pattern after treatment except for that accountable by growth while there was statistically significant change in dentoalveolar and soft tissue patterns. There were no significant changes in Bjork sum, posterior facial height /anterior facial height and lower anterior facial height /anterior facial height. No significant changes in anteroposterior position of maxilla and palatal plane were manifested. Although facial axis and lower facial height was slightly increased and the mandible was rotated backward and downward, there was no remarkable change in the mandibular plane. There were statistically significant changes in distal movement of upper first molar, molar key correction and overjet reduction while there was no change in the occlusal plane. The upper lip was slightly retracted simultaneously with slight increase in nasolabial angle. These results signify that distalization of upper dentition with the second molar extraction does change occlusal relationship without gross modifications in the craniofacial skeletal configurationson. Henceforth the second molar extracted would be recommended to treat severe anterior crowding and protrusion with minor skeletal discrepancy.
Objective: We sought to determine the predictors of midpalatal suture expansion by miniscrew-assisted rapid palatal expansion (MARPE) in young adults. Methods: The following variables were selected as possible predictors: chronological age, palate length and depth, midpalatal suture maturation (MPSM) stage, midpalatal suture density (MPSD) ratio, the sella-nasion (SN)-mandibular plane (MP) angle as an indicator of the vertical skeletal pattern, and the point A-nasion-point B (ANB) angle for anteroposterior skeletal classification. For 31 patients (mean age, 22.52 years) who underwent MARPE treatment, palate length and depth, MPSM stage and MPSD ratio from the initial cone-beam computed tomography images, and the SN-MP angle and ANB angle from lateral cephalograms were assessed. The midpalatal suture opening ratio was calculated from the midpalatal suture opening width measured in periapical radiographs and the MARPE screw expansion. Statistical analyses of correlations were performed for the entire patient group of 31 subjects and subgroups categorized by sex, vertical skeletal pattern, and anteroposterior skeletal classification. Results: In the entire patient group, the midpalatal suture opening ratio showed statistically significant negative correlations with age, palate length, and MPSM stage (r = -0.506, -0.494, and -0.746, respectively, all p < 0.01). In subgroup analyses, a strong negative correlation was observed with the palate depth in the skeletal Class II subgroup (r = -0.900, p < 0.05). Conclusions: The findings of this study indicated that age, palate length, and MPSM stage can be predictors of midpalatal suture expansion by MARPE in young adults.

Objective: Sliding mechanics using orthodontic miniscrews is widely used to stabilize the anchorage during extraction space closure. However, previous studies have reported that both posterior segment displacement and anterior segment displacement are possible, depending on the mechanical properties of the archwire. The present study aimed to investigate the effect of archwire stiffness and friction change on the displacement pattern of the maxillary posterior segment during anterior segment retraction with orthodontic miniscrews in sliding mechanics. Methods: A three-dimensional finite element model was constructed. The retraction point was set at the archwire level between the lateral incisor and canine, and the orthodontic miniscrew was located at a height of 8 mm from the archwire between the second premolar and first molar. Archwire stiffness was simulated with rectangular stainless steel wires and a rigid body was used as a control. Various friction levels were set for the surface contact model. Displacement patterns for the posterior and anterior segments were compared between the conditions. Results: Both the anterior and posterior segments exhibited backward rotation, regardless of archwire stiffness or friction. Among the conditions tested in this study, the least undesirable rotation was found with low archwire stiffness and low friction. Conclusions: Posterior segment displacement may be unavoidable but reducing the stiffness and friction of the main archwire may minimize unwanted rotations during extraction space closure.

Objective: This study aimed to identify possible risk factors for external apical root resorption (EARR) in the maxillary incisors after orthodontic treatment. Methods: The root length of 2,173 maxillary incisors was measured on periapical radiographs of 564 patients who received orthodontic treatment. The Kappa test was performed to evaluate intraexaminer and interexaminer reproducibility. Multiple binary logistic regression was used to determine the association between EARR and various factors. Odds ratios and 95% confidence intervals were reported. Results: The risk of developing EARR was 70% higher in orthodontic treatment with maxillary premolar extraction (p = 0.004), 58% higher in patients with increased overjet (p = 0.012), 41% lower in two-phase orthodontic treatment (p = 0.037), and 33% lower in patients with deep bite (p = 0.039). The lateral incisors were 54% more likely to develop EARR (p < 0.001), dilacerated roots were 2.26 times more likely to develop EARR (p < 0.001), and for each additional millimeter of root length, the risk of EARR increased by 29% (p < 0.001). Conclusions: The potential risk factors for EARR after orthodontic treatment included treatment with maxillary premolar extraction, increased overjet at the beginning of treatment, and dilacerated roots.
Objective: To identify optimal areas for the insertion of extra-alveolar miniscrews into the infrazygomatic crest (IZC) and mandibular buccal shelf (MBS), using cone beam computed tomography (CBCT) imaging in patients with different craniofacial patterns. Methods: CBCT reconstructions of untreated individuals were used to evaluate the IZC and MBS areas. The participants were divided into three groups, based on the craniofacial pattern, namely, brachyfacial (n = 15; mean age, 23.3 years), mesofacial (n = 15; mean age, 19.24 years), and dolichofacial (n = 15; mean age, 17.79 years). In the IZC, the evaluated areas were at 11, 13, and 15 mm above the buccal cusp tips of the right and left first molars. In the MBS, the evaluated areas were at the projections of the first molars' distal roots and second molars' mesial and distal roots, at a 4- and 8-mm distance from the cementoenamel junction. Intergroup comparisons were performed with analysis of variance and the Tukey test. Results: There was no statistically significant difference in the IZC bone thickness among the groups. For MBS bone availability, some comparisons revealed no difference; meanwhile, other comparisons revealed increased MBS bone thickness in the brachyfacial (first molars distal roots) and dolichofacial (second molars mesial and distal roots) patterns. Conclusions: There was no significant difference in the IZC bone thickness among the groups. The facial skeletal pattern may affect the availability of ideal bone thickness for the insertion of extra-alveolar miniscrews in the MBS region; however, this variability is unlikely to be clinically meaningful.

Objective: The purpose of this study was to evaluate the esthetic preference for various Korean silhouette profiles. Methods: The Korean average male and female profiles were modified by changing the lower facial height and anteroposterior lip position to produce nine types of profiles. In order to test intrarater reliability, the average profile was copied once more to be included for evaluation. A questionnaire containing 10 profiles for each sex, each of which had to be rated for preference on a numerical rating scale from 0 to 10, was administered to 30 adult orthodontic patients, 30 dental students, 30 orthodontists, and 30 dentists excluding orthodontists. The data were statistically analyzed using the intraclass correlation coefficient (ICC), independent t-test, and one-way ANOVA. Results: The ICC of overall intrarater reliability was 0.629. For several profiles, significantly higher scores were given to male profiles than to female profiles (p < 0.05). However, no significant differences were found in the scores for all profiles among the four rater groups. Among the short profiles, a significantly higher score was given to the retruded profile, and among the vertically average and long profiles, a significantly higher score was given to the horizontally average profile (p < 0.001). Among all the profiles, significantly lower scores were given to the protruded profile (p < 0.001). Conclusions: This study revealed good overall intrarater reliability, with several types of male profiles being esthetically preferred over female profiles. Moreover, while retruded and horizontally average profiles were generally preferred, protruded profiles were not.
