Three-dimensional finite element analysis on the effects of maxillary protraction with an individual titanium plate at multiple directions and locations

  • Objective: A three-dimensional-printed individual titanium plate was applied for maxillary protraction to eliminate side effects and obtain the maximum skeletal effect. This study aimed to explore the stress distribution characteristics of sutures during maxillary protraction using individual titanium plates in various directions and locations. Methods: A protraction force of 500 g per side was applied at forward and downward angles between 0° and 60° with respect to the Frankfort horizontal plane, after which the titanium plate was moved 2 and 4 mm upward and downward, respectively. Changes in sutures with multiple protraction directions and various miniplate heights were quantified to analyze their impact on the maxillofacial bone. Results: Protraction angle of 0-30° with respect to the Frankfort horizontal plane exhibited a tendency for counterclockwise rotation in the maxilla. At a 40° protraction angle, translational motion was observed in the maxilla, whereas protraction angles of 50-60° tended to induce clockwise rotation in the maxilla. Enhanced protraction efficiency at the lower edge of the pyriform aperture was associated with increased height of individual titanium plates. Conclusions: Various protraction directions are suitable for patients with different types of vertical bone surfaces. Furthermore, when the titanium plate was positioned lower, the protraction force exhibited an increase.

  • This study was designed to get the Knowledges of the dental arch shapes of Koreans. The subjects consisted of 101 individuals with normal occlusion.   • A girl aged 18 years and 1 month, had a Angle's Class III malocclusion, characterized by .anterior crossbite, anterior crowding, and constriction from right lower 2nd premolar to right lower 2nd molar. This patient underwent sealing and (equation omitted) extraction, and multibanded system was placed. After 14 months, anterior crossbite and crowding was corrected, an bothdental arches were improved. After 6 months from debanding, band space disappeared and any relapse was not detected.

CHANCE OF CONDYLAR POSITION AFTER ORTHODONTIC TREATMENT OF ANTERIOR OPEN BITE (전치개교의 교정치료와 과두위치 변화)

  • This study was aimed to evaluate the changes in condylar position when severe anterior open bite patients were treated with MEAW. The subjects consisted of 11 patients(21 TMJs) who visited the departement of orthodontics in SNUH, having severe anterior open bite as a chief complain. They were supposed to wear the up and down elastics and MEAW after finishing the leveling. The condylar position was evaluated with individualized corrected tomography in centric occlusion taken before and after treatment. The results were as follows; 1. In the change of condylar position after treatment, there were no statistically significant differences in right and left TMJs. 2. In the change of condylar position after treatment, there were no statistically significant differences in each malocclusion groups. 3. There were no statistically significant differences in percent dispacement of condyle between before and after treatment 4. There were no statistically significant differences in the amount of change in condylar change.

  • In adult patients, the corrections of their malocclusion may be more difficult and require longer treatment time due to thicker layer of cortical bone and reduced blood supply. Recently, various methods such as surgery, implant for anchorage and corticotomy have been tried to overcome these problems. Corticotomy is a surgical technique in which a fissure is made through the cortical bone that surrounds a tooth so that the tooth is embedded within a block of bone that is connected to adjacent blocks through only the medullary bone. Technique of corticotomy has been widely used for correction of maxillary transverse deficiency, but hasn't actively in other fields of orthodontics. We applied corticotomies in many types of orthodontic treatment and had satisfactory results. We suggested clinical application of corticotomy in adult orthodontics to reduce treatment period and to achieve better stability after orthodontic treatment.

  • A 23 year-old female with skeleto-dentoalveolar protrusion of maxilla, minor broken contact points between anterior teeth, and missing of lower 1st molars, has been treated with multibanded edgewise technique. After treatment of 14 months, she has gained functional overbite-overjet relationship and facial harmony due to the retraction of upper anterior teeth. Root resorption was slight. Especially, us ing the space of missed lower 1st molars instead of extracting lower premolars, expected and favorable results were obtained.

  • Distraction osteogenesis is a well-estabilished procedure of membraneous bone formation and has been used to correct craniofacial deformities in dentofacial orthopedic-surgery area for decades. In this articale, distraction osteogenesis is used for treatment of facial asymmetry. The patient underwent procedures to lengthen the mandibular ramus and body. After distraction, orthodontic treatment was done for ooclusal settling.

  • The purpose of this study was to determine the mean value of overbite and overjet, and to find the correlation among overbite, overjet and relative maxillofacial structures using the data from orthodontic casts and cephalometric films of boys and girls aged from 9 to 18 years with normal occlusions. The results were obtained as follows: 1. Incisal overbite depth was decreased from Group I to Group III. 2. The highest value of correlation coefficient with incisal overbite was 0.368 of incisal angle. in Group III. 3. Overjet was not significantly correlated with the age and sex. 4. Ratio of mesiodistal diameters of upper and lower incisors was not significantly correlated with overbite and overjet.

  • In this study, in order to survey the effect on public recognition in malocclusion and orthodontic treatment by brochures made by Korean orthodontic association, sample were divided into control and experimental group and a questionnaire was inguired and the results were as follows: 1. In the experimental group, frequency of recognition for the need of orthodontic treatment was high, and this indicates dental health behavior and recognition can be inspired by adequate giving of information. 2. Between the control and the experimental group, patterns of the negative opinion was different, and especially the demand for specific orthodontic treatment can be influenced by the content of brochures. 3. The brochures used in this study can call attention to develope the orthodontic aspect for the malocclusion, hereafter more systemic and active measures for development of public dental behavior and recognition should be needed.
