Evaluation of strategic uprighting of the mandibular molars using an orthodontic miniplate and a nickel-titanium reverse curve arch wire: Preliminary cephalometric study

  • Objective: To evaluate the overall treatment effects in terms of the amount of uprighting with changes in the sagittal and vertical positions of mandibular molars after applying an orthodontic miniplate with a nickel-titanium (NiTi) reverse curve arch wire (biocreative reverse curve [BRC] system). Methods: A total of 30 female patients (mean age, 25.99 ± 8.96 years) were treated with the BRC system (mean BRC time, 10.3 ± 4.07 months). An I-shaped C-tube miniplate (Jin Biomed) was placed at the labial aspect for the alveolar bone of the mandibular incisors. A 0.017 × 0.025-inch NiTi reverse curve arch wire was engaged at the C-tube mini-plate anteriorly and the first and second premolars and molars posteriorly in the mandibular arch. Pre- and post-BRC lateral cephalograms were analyzed. A paired t-test was used to analyze the treatment effects of BRC. Results: The mandibular second molars were intrusively uprighted successfully by the BRC system. Distal uprighting with a controlled vertical dimension was noted on the first molars when they remained engaged in the BRC and the distal ends of the arch wire were laid on the second molars. The mandibular first and second premolars showed a slight extrusion. The changes in the mandibular incisors were unremarkable, while the mandibular molar angulation improved significantly. The lower occlusal plane rotated counterclockwise (MP-LOP: 1.13° ± 2.60°). Conclusions: The BRC system can provide very effective molar uprighting without compromising the position of the mandibular anterior teeth.

  • Use of ready-made orthodontic appliance can lead to inefficiencies in the final stages of the orthodontic treatment. Because patients' teeth have anatomic variations, brackets that have been designed to fit on average tooth surface may result in positional discrepancies when leveling and alignment is completed. As a result, additional steps such as rebonding, wire bending and use of auxiliaries may be needed. Even in patients who have normal tooth anatomy and proper tooth size relationships, precise bracket placement is crucial in order to efficiently control the tooth positions. Digital models can provide advantages in clinical orthodontics as virtual tooth setup could be performed, and clinicians can easily visualize the predicted final occlusion. Through this setup model, customized brackets with individualized prescription and archwires that optimally fit with the patients' dental arches can be produced using CAD/CAM technology. Also, the brackets can be accurately placed with an aid of 3D-printed jigs. The purpose of this article is to introduce the commonly used labial and lingual customized orthodontic appliance systems using digital technology.

A New Orthodontic Appliance for Rapid Anterior Alignment in Adults; Mini-Tube Appliance (MTA) (성인 전치부 부분교정을 위한 새로운 교정장치 MTA의 이용)

  • With an increase of public's perception on "minimally invasive dentistry", more patients are demanding orthodontic alignment of anterior teeth rather than restorative treatment which is more invasive. One draw back of orthodontic alignment has been that it takes long time. To overcome this limitation, a new orthodontic device named Mini-Tube Appliance (MTA), have been developed. With the combined use of light NiTi wire and inter-proximal stripping, rapid anterior alignment can be obtained efficiently. This article presents the use of MTA for rapid alignment of anterior teeth and its clinical guidelines with typical case samples.

A Study of shear bond strength of bonded retainer according to the bonding method and type of wires (접착방법 및 multistranded wire의 종류에 따른 접착식 보정장치의 전단접착강도에 관한 연구)

  • The bonded orthodontic retainer constructed from composite and multistrand orthodontic wire provides an esthetic and efficient system for maintained retention. This study was designed to measure shear bond strength of bonded retainers and to suggest a optimal combination of a multistrand wire and bonding method used when bonded retainer was fabricated. 160 sound maxillary and mandibular premolars were used for 80 test samples. After Uniformizing bonding area, length of wire, and thickness of composite, multistrand wire was bonded to fabricated a bonded retainer by direct or indirect bonding method. Shear bond strength and extension length of each sample were measured by a universal testing machine. The results of this study were as follows : 1. In vitro shear bond testing found 6-stranded, 0.0155 inch wires to have the largest shear bond strength and 3-stranded, 0.0195 inch wires to have the least shear bond strength. But, These difference was not statistically significant(p<0.05). 2. In vitro extension testing found 3-stranded, 0.0155 inch wires to have the largest extension length and 3-stranded, 0.0195 inch wires to have the least extension length(p<0.05). The larger diameter wire was used, the larger extension length was shown. But, the strand of wire is not related to the extension length of wire. 3. In comparison with direct bonding method, larger shear bond strength and extension length was shown in indirect bonding method(p<0.05).

Electrochemical Characteristics of Tooth Colored NiTi Wire (치아색으로 코팅된 NiTi 와이어의 전기화학적 특성)

  • NiTi alloy has been used for orthodontic wire due to good mechanical properties, such as elastic strength, friction resistance, and high corrosion resistance. Recently, these wire were coated by polymer and ceramic materials for aesthetics. The purpose of this study was to investigate electrochemical characteristics of tooth colored NiTi wire using various instruments. Wires (round type and rectangular type) were used, respectively, for experiment. Polymer coating was carried out for wire. Specimen was investigated with optical microscopy (OM), field emission scanning electron microscopy (FE-SEM) and energy dispersive x-ray spectroscopy (EDS). The corrosion properties of the specimens were examined using potentiodynamic tests (potential range of -1500 ~ 2000 mV) and electrochemical impedance spectroscopy (frequency range of 100 kHz ~ 10 mHz) in a 0.9 % NaCl solution by potentiostat. From the results of polarization behavior, the passive region of non-coated NiTi wire showed largely, whereas, the passive region of curved NiTi wire showed shortly in anodic polarization curve. In the case of coated NiTi wire, pitting and crevice corrosion occurred severely at interface between non-coated and coated region. From the results of EIS, polarization resistance(Rp) value of non-coated round and rectangular NiTi wire at curved part showed $5.10{\times}10^5{\Omega}cm^2$ and $4.43{\times}10^5{\Omega}cm^2$. lower than that of coated NiTi wire. $R_p$ of coated round and rectangular NiTi wire at curved part showed $1.31{\times}10^6{\Omega}cm^2$ and $1.19{\times}10^6{\Omega}cm^2$.

A Study on Frictioal Resistance Force of Orthodontic Resin Bracket (교정용 Resin bracket의 마찰 저항력에 관한 연구)

  • As increasing number of adult patients, the esthetic orthodontic appliances are needed. They are tooth-colored or translucent ceramic and resin brackets. Although ceramic and resin bracket have good esthetics, there are some disadvantage such as frictions. Recently, metal-reinforced resin bracket(MRBB) were introduced. The purpose of this study is to find frictional force of MRRB, ceramic bracket and resin brackets. There is few study in frictional force about metal reinforced resin bracket(MRRB). This study used 4 orthodontic wire(.016 S-S, .0l6X.022 S-S, .016 $TMA^{\circledR}$, .0l7X.025 $TMA^{\circledR}$ and 5 brackets(one metal bracket, one ceramic bracket, one resin bracket, two MRRB). The following result is obtained using metal bracket(Ormco.Co., U.S.A), ceramic brackets($Crystalline^{\circledR}$), resin bracket( Clear Medium $Siamase^{\circledR}$). Following conclusions are obtained. 1. Ceramic and resin bracket have significantly more frictional forces than metal reinforced resin bracket and metal bracket. 2. There is no significant difference in frictional force according to the slot types of metal - reinforced resin brackets. 3. There is no significant difference in frictional force between metal reinforced resin bracket and metal bracket. 4.. Frictional force is decreased in S-S wire than TMA wire.

An experimental study on the cytotoxicity of orthodontic wires (교정용 호선의 세포독성에 관한 실험적 연구)

  • This study was undertaken to investigate the cytotoxicity of orthodontic wires after doing various treatments to the wires. 018x025 inch Stainless steel(A) and Co-Cr(B) wires were used and each of them were divided into 4 groups. A-1 and B-1 groups were as received state, and A-2 and B-2 groups were heat treated. A-3 and B-3 groups were electropolished after heat treatment, and A-4 and B-4 groups were soldered with Ag-solder. Each group had 3 wires and these were sterilized with Ethylene Oxide gas. We used human gingival fibroblast cell culture and agar overlay technique to investigate the cytotoxicity of each group of wires. The cytotoxicity of wire was assessed using reaction index (zone index/lysis index). The findings of this study were as follows. 1. Both of the stainless steel wire and Co-Cr wire showed no cytotoxicity in as received state. 2. Heat treatment or electropolishing of the wires had no effect on the cytotoxicity of the wires 3. Soldered stainless steel wires showed a little wider zone of discoloration than soldered Co-Cr wires, but the zone index and cytotoxicity(reaction index) was not different. 4. Soldered wires showed moderate cytotoxicity in both of the wires.

Surface Characteristics of Polymer Coated NiTi Alloy Wire for Orthodontics (폴리머 코팅된 NiTi합금 교정선의 표면특성)

Recently, these wire were coated by polymer and ceramic materials for aesthetics. The purpose of this study was to investigate surface characteristics of polymer coated NiTi alloy wire for orthodontics using various instruments. Wires (round type and rectangular type) were used, respectively, for experiment. Polymer coating was carried out for wire. Specimen was investigated with field emission scanning electron microscopy(FE-SEM), energy dispersive x-ray spectroscopy(EDS) and atomic force microscopy(AFM). The phase transformation of non-coated NiTi wire from martensite to austenite occurred at the range of $14{\sim}15^{\circ}C$, in the case of coated wire, it occurred at the range of $16{\sim}18^{\circ}C$. Polymer coating on NiTi wire surface decreased the surface defects such as scratch which was formed at severe machined surface. From the AFM results, the average surface roughness of non-coated and coated NiTi wire was 13.1 nm, and 224.5 nm, respectively. From convetional surface roughness test, the average surface roughness of non-coated and coated NiTi wire was $0.046{\mu}m$, and $0.718{\mu}m$, respectively.

Managing Short Root Anomalies in Pediatric Cancer Survivors: Utilizing Resin Wire Splints and Miniscrews for Skeletal Anchorage

  • Patients with pediatric cancer often undergo multiple therapies, such as chemotherapy, radiation therapy, and stem cell transplantation. These treatments, while essential, can result in dental developmental issues, including hypodontia, microdontia, short roots, and delayed dental development. This report presents two cases of pediatric patients diagnosed with neuroblastoma who exhibited severe tooth mobility due to short roots as a complication of cancer treatment. Moreover, we investigated the conservative management of the patients' conditions using resin wire splints and orthodontic miniscrews for skeletal anchorage along with long-term follow-ups to evaluate their prognosis.

A STUDY ON THE MECHANICAL PROPERTIES OF ORTHODONTIC OPEN COIL SPRINGS (교정용 open coil spring의 역학적 특성에 관한 연구)

  • It was the purpose of this study to analyze and compare the mechanical properties of ortho dontic open coil springs. Four variable factors were presented - wire diameter (.008", .009", .010"), lumen size (.030", .032", .036"), arch wire size and shape (.016" round, $ .016^{{\prime}{\prime}}{\times}.022^{{\prime}{\prime}}$ rect.) and alloy type (HiT II, Elgiloy, Sentalloy). The total 104 specimens were divided into 13 groups, and compression test was performed on an Instron test machine. The load deflection curve of each open coil spring was obtained, from which, the load-deflection relations, stiffnesses, percent recoveries were computed statistically. The results were obtained as follows: 1. When the lumen size of the coil spring remained constant, stiffness and percent recovery increased as the wire diameter increased. 2. When the wire diameter of the coil spring remained constant, stiffness and percent recovery decreased as the lumen size increased. 3. The effect of size and shape of arch wire on the coil spring was not statistically significant. 4. In alloy types, stiffness was the greatest in HiT II (55.21), Elgiloy (42.61) and Sentalloy (7.74) in that order. Sentalloy exhibited superior percent recovery and long range of action.
