Upper canine is important because it protects and maintains the stability of the dental arch and also, joins the anterior with the posterior teeth. The incidence of impaction of upper canine is the second most frequent next to the third molar because it takes a long period of time to develop, and has a complicated path of eruption, and erupts lately. After the age of 10, clinical and radioglaphic examination can be used in revealing the possibility of impaction and efforts should be put to reduce the side effects. To prevent impaction, selective extraction of primary canine at the age of 8 to 9 could be considered and prolonged retention of primary canine in oral cavity should be avoided at this time. Once the impaction is iden, the first stage of the treatment is to lcocalize the lesion by radiographic examination and According to the severity, orthodontic traction or autotransplantation should be considered and comprehensive diagnosis and treatment plan of malocclusion should be established. Generally, labial impaction is due to arch length discrepancy and palatal impaction is due to malposition or morphologic pathosis of lateral incisors rather than arch length discrepancy. In surgical procedure, peridontal problems should be considered and the minimum amount of bone and soft tissue should be reduced and direct bonding method of many attachment methods should be recommended. Especially in traction of labially impacted canine, it should be guided to erupt through the keratinized zone and proper forced magnitude should be applied. The importance of periodontal condition should always be in mind following the patient education to mintain the good oral hygiene at each stage of treatment. Properly managed impacted canine can provide function and esthetic by proper diagnosis and treatment if extraction of canine is not indicated.

Management options for impacted maxillary canines can include (1) continued observation, (2) extraction of the primary canine to aid spontaneous eruption, (3) uncovering and bonding of the impacted tooth and its eruption using orthodontic traction, (4) autotransplantation, and (5) extraction followed by prosthetic replacement. Autotransplantation should be considered when the degree of malposition is too severe to correct by orthodontic alignment. The present report describes the management of an ectopic eruption of the left maxillary canine in an 10-year-old girl. The treatment included the extraction of primary maxillary left canine and the autotransplantation using a Rapid Prototyping model. By using RP model to contour the recipient bone and check for fitting in the prepared socket, the extra-oral time can reduce. The autotransplanted canine showed mobility within normal limit, negative response to percussion and positive to electric pulp test after 6 months.
Purpose: An impacted tooth is defined as a tooth that shows delayed eruption and is expected to erupt incompletely by clinical and radiograph examination despite it reaching its expected time of eruption. The aims of this study were to investigate the clinical and radiological characteristics and treatment duration and method of impacted teeth in Korean patients. Materials and Methods: For this study we used clinical records, study models, panoramic radiographs and intraoral photographs of patients who attended Gachon University Gil Hospital Orthodontic Department between 2005 and 2008. There were 164 patients with a total number of 202 impacted teeth. Results: Male patients shows a little more prevalence than female patients (1.13:1). The under 12 age group had the highest prevalence of tooth impaction, and the over 19 age group showed the least prevalence of tooth impaction. The ratio of tooth impaction between the left to right ratio was 1.73:1 and maxilla and mandible was 1.84:1. The impacted teeth were most commonly positioned buccally (76 cases, 41.5%). Full nap closure technique (108 cases, 81.2%)was most frequently used for attachment of surgical traction hooks. Maxillary canine impaction was most commonly encountered both in male and female patients. The mean treatment period was 12.2 months and the success rate of treatment was 90.3%. The canine tooth shows the longest treatment time and highest failure rate. The ankylosis was the major cause of failure. Conclusion: Impacted teeth most commonly show in left side maxilla in the under 12 age group. And it is most commonly positioned buccally. The mean treatment period was 12.2 months, and the success rate of treatment was 90.3%.
Preface: Dental implant is important method that may solve the mastication, occlusion, esthetic, temporomandibular joint, and psychologic problem in oral and maxillofacial surgery. It is ideal that all of the implant are well positioned by adequate technique. By the way it‘s not always possible because of some anatomic, physiologic factor. In this case, If the implant can be moved to adequate position, it may be possible more esthetically and implanted patients more satisfied, but the majority of Implantists and orthodontists have thought that it is not possible. However, Implant, in fact, can be moved. and thus we can overcome the limit of implantation more. The aim of the present study was to evaluate the possibility of implant movement after corticotomy. Case report: Patient missed the upper right first molar. and implantation was done after completion of socket healing. We wait six months for osseointegration. Then, corticotomy was done under local anesthesia and close coil was used for orthodontic force. After traction during 3 weeks, we find the change of implant position at horizontal plane. we can not see the degenerative change on adjacent structure and tracted implant. there is a clinical mobility on upper right second premolar that used for anchorage but it subside spontaneously at the timing of prosthetic restoration without additional treatment. Discussion: As we could have some knowledge with this experiment, we report the case of implant movement after corticotomy and suggest a method about more esthetic implant treatment with a review of literature.

Intrusive luxation that takes approximately three percent of permanent teeth commonly occures at anterior teeth. This intrusion frequently leads to pulp necrosis, root resorption, marginal bone loss and these complications are influenced by depth of intrusion and stage of root development. Various treatment approaches have been suggested to manage of intrusive luxation. Techniques aiming to reposition the intruded tooth include an observation for spontaneous re-eruption, surgical or orthodontic repositioning. We report two cases with clinically satisfactory results for traumatically intruded maxillary central incisor. In one case which has a large open apex and mild intrusion depth, we observed for spontaneous eruption and then repositioning by forced eruption method. In other case, which has been completely intruded, was repositioned by surgical extrusion and followed by apexification.

Objective: The aim of this study was to evaluate the amount of tooth movement and histologic changes with different corticotomy designs and micro-osteoperforation in rabbits. Methods: The sample consisted of 24 rabbits divided into three experimental groups (triangular corticotomy [TC] and indentation corticotomy [IC] with flap, and flapless micro-osteoperforations [MP]) and a control. A traction force of 100 cN was applied by connecting the first premolars to the incisors. The amount of tooth movement was measured. Kruskal-Wallis test was used to assess differences in tooth movement between the groups. Micro-computed tomography, hematoxylin and eosin staining, and tartrate-resistant acidic phosphatase (TRAP) analysis were performed. Analysis of variance was applied to assess differences in TRAP-positive osteoclast count between the groups. Results: The amount of tooth movement increased by 46.5% and 32.0% in the IC and MP groups, respectively, while the bone fraction analysis showed 69.7% and 8.5% less mineralization compared to the control. There were no significant intergroup differences in the number of TRAP-positive osteoclasts. Conclusions: The micro-osteoperforation group showed no significant differences in the amount of tooth movement compared to the corticotomy groups, nor in the TRAP-positive osteoclast count compared to both corticotomy groups and control.
Orthodontic treatment is more complicated when both soft and hard tissues must be considered because an impacted maxillary canine has important effects on function and esthetics. Compared with extraction of impacted maxillary canines, exposure followed by orthodontic traction can improve esthetics and better protect the patient's teeth and alveolar bone. Therefore, in order to achieve desirable tooth movement with minimal unexpected complications, a precise diagnosis is indispensable to establish an effective and efficient force system. In this report, we describe the case of a 31-year-old patient who had a labio-palatal horizontally impacted maxillary left canine with a severe occlusal alveolar bone defect and a missing maxillary left first premolar. Herein, with the aid of three-dimensional imaging, sequential traction was performed with a three-directional force device that finally achieved acceptable occlusion by bringing the horizontally impacted maxillary left canine into alignment. The maxillary left canine had normal gingival contours and was surrounded by a substantial amount of regenerated alveolar bone. The 1-year follow-up stability assessment demonstrated that the esthetic and functional outcomes were successful.
Objective: The purpose of this study was to investigate the maxillary protraction effects of the Tandem Traction Bow Appliance (TTBA), a new appliance devised several years ago for the treatment of growing skeletal Class III patients. Methods: Participants were 88 Korean children (42 boys, 46 girls) with skeletal Class III malocclusion treated with TTBA at the orthodontic clinic of Ewha Womans University Mokdong Hospital. Mean age at the start of treatment was 7.5 years${\pm}$1.5 years. Mean treatment periods were $13{\pm}3$ months. Pretreatment and posttreatment lateral cephalograms were traced and superimposed by the same investigator and analyzed by modified McNamara analysis and pitchfork analysis. Changes were evaluated with paired t-tests at a significance level of p<0.05. Results: The maxilla and maxillary dentition moved forward. The mandible moved backward, although not significantly; and the mandibular dentition moved forward. The net dental changes combined with the apical base change resulted in a favorable total molar relationship correction. Net dental movement was 26% and the apical base change 74% of the total molar relationship correction. Conclusion: These results suggest that TTBA has a maxillary protraction effect that can be useful in the treatment of growing skeletal Class III malocclusion with maxillary deficiency.
Objective: The purposes of this study were to evaluate the differences between sand blasted, large grit and acid-etched (SLA) treated mini-implants and smooth surface orthodontic mini-implants in relation to the removal torque as well as the histologic analysis. Methods: Custom-made, screw-shaped, titanium implants with a length of 9.5 mm and an outer diameter of 1.8 mm were divided into 2 groups; the SLA group (20 SLA treated orthodontic mini-implants) and the smooth surface group (20 smooth surface mini-implants), and placed In the tibia metaphysis of 10 rabbits. Each rabbit had 4 mini-implants placed, 2 in each tibia. The right tibia were implanted with the SLA group mini-implants and the left tibia had the smooth group mini-implants placed. Each mini-implant group were immediately applied with a continuous traction force of 150 g using a Ni-Ti coil spring. The rabbits were sacrificed 6 weeks post-surgically. Subsequently, the legs were stabilized, the Ni-Ti coil springs were removed and the mini-implants were removed under reverse torque rotation with a digital torque gauge. Results: 6 weeks after placement, the SIA group presented a higher mean removal torque value (8.29 Ncm) than the smooth group (3.34 Ncm) and histologic analysis revealed a higher new bone formation aspect along the screw in the SLA group. Conclusion: Results of this study indicates that SLA treated mini-implants may endure higher orthodontic forces without loosening.

We describe our experience with conservative condylectomy for the correction of facial asymmetry in five patients with osteochondroma of the mandibular condyle. All five patients presented with malocclusion and facial asymmetry, which are common clinical findings of osteochondroma involving the mandibular condyle. We performed conservative condylectomy without additional orthognathic surgery for all five patients, preserving the vertical height of the condylar process as much as possible. Following surgery, intermaxillary traction using a skeletal anchorage system with rubber elastics was performed on all patients to improve occlusion, and, when necessary, additional minimal orthodontic treatment was performed. The mean follow-up period was 42 months. At the last follow-up visit, all patients exhibited satisfactory facial symmetry and remodeling of the remaining condyle, with stable health and no signs of recurrence. In conclusion, conservative condylectomy alone, without subsequent orthognathic surgery, is adequate for the restoration of facial symmetry and the preservation of vertical condylar height in select patients with condylar osteochondroma.
