Characteristic Effects of Dangnyo-hwan for Diabetes Control Studied Using LC-MS/MS and ICP (LC-MS/MS 및 ICP를 이용한 당뇨환의 화학적 특성 분석)

  • Objectives: The primary aim of this study was to identify bioactive compounds in Dangnyo-hwan, a Korean herbal medicine, through instrumental analysis using LC-MS/MS and ICP, and investigate its potential use in diabetes treatment. Methods: The extract of Dangnyo-hwan has 12 medicinal herbs; these were compared with 18 marker substances selected from literature survey. Results: LC-MS/MS analysis could detect 9 of the 18 bioactive compounds: citruline, catalpol, berberine, ginsenoside Rb1, ginsenoside Rg1, oleanolic acid, β-sitosterol, mangiferin, and schizandrin. While harmful heavy metals such as As, Pb, Cd, Hg, Ni, and Cu were not present in high concentrations, Zn concentration was 4.2 mg in 100 g Dangnyo-hwan. Conclusions: Instrumental analysis such as LC-MS/MS and ICP was successfully used to identify bioactive compounds in Dangnyo-hwan. Detection of 9 bioactive substances and Zn from the herb medicine is a valuable finding, and suggests that Dangnyo-hwan is a candidate medicine for diabetes. Further investigations like in vitro assay, percent GPR 119 activity, and percent human DGAT-1 inhibition are underway.

A Review of Studies for Treatment of Essential Tremor Using the China National Knowledge Infrastructure Database (본태성 떨림의 한방 치료에 대한 임상 연구 동향 - China National Knowledge Infrastructure를 중심으로)

  • Objectives: The purpose of this research is to investigate clinical studies of essential tremor in China. Methods: We investigated clinical studies of Traditional Chinese Medicine therapies in traditional Chinese medical journals for essential tremor through the China National Knowledge Infrastructure (CNKI) database. In total, 15 clinical articles were analyzed. This study examined the authors; published years; types of study; treatment methods for the treatment and control groups; periods; outcome assessment; and results. Results: Nine articles used acupuncture, 6 articles used herbal therapy. The most commonly used acupoints were GV20, GB20, LR3, LI4, GB34, SP6, and EX-HN1. The most commonly used herbs were Paeoniae Radix Alba, Angelicae Gigantis Radix, Cnidii Rhizoma, Gastrodiae Rhizome and Uncariae Ramulus et Uncus. Total effective rate was 66.67-96.7%; the treatment group values were significantly higher than the control group values in all papers. Tremor score, HAMD, curative effect of TCM syndrome, and incidence of adverse effects were significantly more affected in the treatment group than in the control group. Conclusion: More active clinical studies are needed on essential tremor, and the results of this study can be used as a basis for future research.

Effects of Cold and Hot Drugs on the Activity of Monoamine Oxidase (한성 및 열성한약재가 모노아민 산화효소의 활성에 미치는 영향)

  • To explain the theory of KIMI which is the theory of therapeutics in oriental medicine, monoamine oxidase(MAO) activities were measured in the brain and liver of mice which were orally administered oriental medicinal herbs which were classified into cold and hot drugs. Rheum palmatum, Anemarrhena asphodeloides, Gardenia jasminoides, Scutellaria baicalensis and Coptis japonica were considered as the cold drugs and Zingiber officinale, Aconitum carmichaeli, Asiasarum sieboldi, Evodia officinalis and Cinnamomum cassia were included in the hot drugs. The effects of cold and hot drugs on in vitro enzyme activities were measured and compared with the in vivo effects. Serotonin is important neurotransmetter involved in the control of body temperature. The MAO plays a central role in the metabolism of many neurotransmetter monoamines including serotonin. MAO is a flavoprotein found exclusively in the mitochondrial outer membrane, occuring in the MAO-A and MAO-B subtypes. MAO-A deaminates serotonin and noradrenaline, whereas MAO-B prefers phenylethylamine and benzylamine as substrates. Coptis japonica and Aconitum carmichaeli elevated the in vivo MAO activities and especialy, in vivo MAO-B activities were significantly increased. In vitro MAO-A activities were increased by hot drugs, whereas the in vitro MAO-B activities were inhibited. Cold drugs inhibited both enzyme activities in vitro.

Thirteen-week repeated-dose oral toxicity study of the Modified Wenpitang-Hab-Wulingsan (WHW$^{(R)}$) in Sprague-Dawley rats (WHW$^{(R)}$의 랫드에서의 반복경구투여 독성에 관한 연구)

  • Objectives : We investigated the repeated-dose toxicity of Wenpitang-Hab-Wulingsan(WHW), a Korean traditional medicine prescribed with twelve herbs, which has been used for the treatment of renal disease. Methods : WHW extract prepared by GLP company. WHW was supplemented by gavage at 0, 100, 500 and 1000 mg/kg/day for 13-week consecutive days. We recorded the clinical signs of toxicity, body weight, organ weights, hematology, gross and histological changes in target organs rats and clinical chemistry analysis for all rats. Results : WHW extract at all doses was shown no mortality or abnormal clinical signs in rats during at the observation period. Furthermore, there was no difference in body weight and food-take consumption, organ weight, gross pathological findings, and urine analysis among the groups of rats treated with different doses of WHW extract. The hematological analysis and clinical blood chemistry data were revealed no toxic effects from WHW-treated rats. Conclusions : The results suggest that WHW extract in rats is a wide margin of safety on a acute toxicity.

The Trends in Foreign Clinical Trials for Pediatric Asthma in Complementary and Alternative Medicine (소아 천식 치료의 국외 보완대체의학 임상 연구 동향)

  • Objectives : The purpose of this study is to analyze the trends in foreign clinical trials for pediatric asthma in complementary and alternative medicine (CAM). Methods : We reviewed 31 randomized controlled trials (RCT) for pediatric asthma treatment in CAM published from 2005 to 2015 in PubMed and analyzed their treatments, compounds of herbal medicines, acupoints, methods of assessment, and treatment results. Results : The treatments used in the studies were herbal medicines, acupuncture, acupoint application, breathing training, supplement, normocaloric diet, yoga, massage therapy, and art therapy. The frequently used herbs were Asari Herba Cum Radix (細辛). Ephedrae Herba (麻黃), Astragali Radix. The most common acupoints were Feishu (肺兪, BL13). The methods of assessment in pediatric asthma were symptom scale, questionnaire of Quality of Life, lung function test, laboratory studies, and medication usage. In 28 out of 31 studies, treatment group gets better than non-treated group. Conclusions : This study shows that CAM is effective in treating pediatric asthma. More clinical studies using objective outcome measures are needed to prove its efficacy clearly.

  • This study has been carried out to investigate the cause symptom and treatment of wind-coughing ( 風嗽 ) by referring to 38 literatures. The results were obtained as follows; 1.   • In pathologically analyzing, 'Qi(氣)' is fall downed 'Zheng-Qi(正氣; Base energy of human body)' or 'Wai-Gan-Xie-Qi(外感邪氣; poisoned energy from outside of human body)'. And all extroverted symptoms from this 'Qi(氣)' is 'Shang-Chong(上衝)'. Also this symptom's basic mechanism is deficiency of 'Zhong-yang(中陽; basic active energy acting pivot)' by non-proper 'Xia-fa(下法; treatment way getting out focus to down side)' at early days of 'Tai-Yang-bing (太陽病; Primary stage symptoms when the cold energy of the outside tresspasses the external layer of body)'. And comparing herbs in medicines for treating 'Shang-Chong(上衝)' in Shang han lun with Japanese in Shang-han medical practitioner's view, there is 'Ping-Chong(平衝; Supress out bursting energy)''s effect when use 'Gui-zhi(桂枝; Cinnamomum cassia loureirii zeylanuicum)' for mass dosage. Based on these, 'Qi-Shang-Chong(氣上衝)' means all of the symptom's aspects which called 'Yang(陽)''s aspect that is extrovesity, dynamic and imminent by results of the experiment about Cinnamaldehyde which are the main ingredients of 'Gui-zhi(桂枝; Cinnamomum cassia loureirii zeylanuicum)' and clinical data of 'Gui-zhi-tang(桂枝湯)'.

  • Purpose : Nowadays excrement-related disease that repeats constipation and diarrhea is on the increase due to the change of dietary and lack of exercise, etc. We analyzed Bangyakhappyeon in order to find out the materia medica which is used for the excrement patterns. Methods : The database used in present thesisis consist of disease pattern, nature of medicinals and materia medica from Bangyakhappyeon was constructed. We analyzed the nature of medicinals of excrement patterns(or symptom) by frequency analysis and network analysis, and also searched main materia medica of excrement patterns(or symptom) by frequency analysis and rule mining. Results : We analyzed the nature of medicinals of excrement patterns(or symptom) in Bangyakhappyeon. And we researched the high frequency materia medica, high specificity materia medica and high frequent paired-drugs as main materia medica of excrement patterns(or symptom). Conclusion : This study found the information about frequency relationship between excrement patterns(or symptoms) and materia medica.

Apoptosis and Anti-proliferaction by Saussurea lappa and Pharbitis nil in AGS Human Gastric Cancer Cell Line

  • Objectives : We performed this study to understand the molecular basis of the antitumor effect of Saussurea lappa, Pharbitis nil, Plantago asiatica and Taraxacum mongolicum, which have been used for cancer treatment in Korean traditional medicine. Design: We analyzed, the effect of these medicinal herbs on proliferation and apoptosis of tumor cells and its association with gene expression, We performed semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR) analysis of cell cycle- and apoptosis-related genes using a gastric cancer cell line AGS. Results : Cell counting assay and $[^3H]thymidine$ uptake analysis showed that Saussurea lappa and Pharbitis nil strongly inhibit cell proliferation of AGS in a dose-dependent manner. Interestingly, gene espression assay revealed that mRNA espression levels of c-Jun, c-Fos, c-Myc, and Cyclin D1 were markedly decreased by Saussurea lappa and Pharbitis nil. Furthermore, Saussurea lappa was identified to activate expression of the p53 tumor suppressor and its downstream effector $p21^{Wafl}$, which leads to $G_1$ cell cycle arrest and apoptosis. These observations suggest that the anticancer effect of Saussurea lappa and Pharbitis nil might be associated with their regulatory capability of tumor-related gene expression.
