  • Objectives: To research Korean oriental medical doctors' use of uninsured herbal extracts and how to bring about the insurance coverage of such herbal extracts. Methods: We surveyed Korean oriental doctors about the following issues from October 17th to November 15th, 2008: (1) Korean oriental medical doctors' knowledge about herbal extract insurance and the circumstances of oriental medicine in Korea, (2) their opinion on the coverage of currently uninsured herbal extracts and the dispensing of herbal extracts from pharmacies rather than from their clinics, (3) their use of herbal extracts, and (4) how to bring about insurance coverage of uninsured herbal extracts. Results: Over 70% of the respondents said that herbal medicine prescriptions have been reduced recently and that the existence of herbal medicine is in danger. In addition, 63.64% respondents agreed with expanding insurance coverage to include currently uninsured herbal extracts in spite of the fact that patients might have to obtain herbal extracts from pharmacies rather than from Korean oriental medical clinics. The average patient number per month of uninsured herbal extracts was 13.64 people, the average dosage was 5.64g, the average cost per day was 3,859 won, and the average prescription period was 2.65 days. Korean oriental medical doctors asked an average of 12,486 won for the medical examination-prescription fee and 3,292 won in fees for prescriptions obtained outside the hospital. If insurance coverage expands to include these herbal extracts, their usage is expected to increase 2.31 times. Conclusions: This study shows Korean oriental medical doctors' use of herbal extracts and their opinions about execution of herbal extracts' insurance. A periodic study such as this one will hopefully aid in establishing polices for uninsured herbal extracts' insurance.

  • This study was conducted to examine recognition of oriental medical doctors on the quality control of herbal medicine and to provide useful information for the establishment of effective quality control system of herbal medicine. Data were collected from 202 oriental medical doctors working in oriental medical hospitals or clinics in Daegu-Gyungbuk province. Generally, there is a tendency that most respondents have low degree of recognition, trust and satisfaction of the quality control of herbal medicine while they think standardization and complete quality control systems of herbal medicine is needed highly. Thus, in order to increase confidence of consumers in herbal medicine, and to increase the utilization of oriental medical services, both strengthening of quality control systems and standardization and establishment of Oriental KFDA as a new government agency responsible for the work should be required.

Methods The productions of nitric oxide (NO), prostaglandin (PG)$E_2$, interleukin(IL)-6 and tumor necrosis factor (TNF)-${\alpha}$ were examined in the presence of the 4 herbal formulas in RAW 264.7 cells. The cells were incubated with LPS 1 ${\mu}g/mL$ and 4 herbal formulas for 18 hrs. The anti-inflammatory activity of 4 herbal formulas were investigate by carrageenin-induced paw edema in rats. The paw volume was measured at 2 and 4 hrs following carrageenininduced paw edema in rats. 3. Results Yanggyeoksanhwa-tang and Cheongsimyeonja-tang showed inhibitory effect on $PGE_2$ production in LPS-stimulated RAW 264.7 cells and a reduction in carrageenin-induced paw edema on rats. Yanggyeoksanhwa-tang showed inhibitory effect on IL-6 in LPS-stimulated RAW 264.7 cells. 4 herbal formulas not affect on NO and TNF-${\alpha}$ inhibition in LPS-stimulated RAW 264.7 cells. 4. • Objective: To derive information on the efficacy of Sam-chul-kun-bi-tang (SKT), by evaluating its anti-inflammatory effect. SKT is a widely-used herbal formula in traditional Korean medicine. In man y studies, plant-derived anti-inflammatory efficacies have been investigated for their potential inhibitory effects on lipopolysaccharide (LPS)-stimulated macrophages. This study was performed to examine the anti-inflammatory effects of SKT extract on LPS-stimulated RAW 264.7 cells. Methods: The production of nitric oxide (NO), prostaglandin $(PG)E_2$, tumor necrosis factor (TNF)-$\alpha$ and interleukin (IL)-6 were examined in a macrophage cell line, RAW 264.7 cells, in the presence of SKT. RAW 264.7 cells were incubated with LPS 1 ${\mu}g/mL$ and SKT for 18 hrs. The anti-inflammatory activity of SKT was investigated by carrageenan-induced paw edema in rats. The paw volume was measured at 2 and 4 hrs following carrageenan-induced paw edema in rats. Results: SKT showed inhibitory effect on $PGE_2$, TNF-$\alpha$ and IL-6 in LPS-stimulated RAW 264.7 cells. But SKT was not inhibitory effect on NO by LPS-stimulated RAW 264.7 cells. Administration of SKT (1 g/kg) also showed a reduction in carrageenan-induced paw edema on rats. Conclusion: These results suggest that SKT has anti-inflammatory activities in both in vitro and in vivo models.

  • Objectives : It is well known that vegetables and fruits contain minerals, including potassium, sodium and phosphorus etc. Though most oriental herbal medications are made of natural plants, western scientists suppose that they also contain certain amounts of minerals and so are injurious to kidney disease such as chronic renal disease. However, by the reason of the limitation of western medical treatment on kidney disease, many patients depend on oriental medical treatment, which includes taking oriental herbal medicine. So, in order to find out the mineral contents in oriental herbal medicine, and to establish the oriental herbal medication's safety in kidney disease, studies were performed. Methods : In this study, we analyzed 42 oriental herbs commonly used in kidney disease by the Inductively Coupled Plasma(ICP) method. Results : 1. The potassium and sodium contents of oriental herbs were 3-10 times as much as of food. 2. The mineral contents of a daily dose of oriental herbal medicine satisfied the restriction of dietary mineral in CRF, though the amount of mineral intakes by food was considered. Conclusions : The mineral contents of oriental herbal medicine are less than the limits of mineral restriction in renal failure. The yielded results, we carefully suggest that oriental herbal medicine does not induce accumulation of minerals or damage in kidney disease patients.
