Aplastic anemia and dental implant rehabilitation: a clinical trial

  • The purpose of this study was to investigate implant-supported restoration as a technique for restoring missing teeth in patients with aplastic anemia. Recurrent bleeding from wound sites leads to persistent release of iron in the tissue. Excessive iron in tissue is related to clinical findings, including fibrosis, poor wound healing, and high level of angiogenesis, which are possible etiological factors of reduced osseointegration. A 44-year-old female patient with aplastic anemia was treated with multiple endosseous implants throughout the mandible and in the posterior region of the maxilla. After 14 implants were placed, radiological and clinical parameters were assessed during the follow-up period. Marginal bone did not change significantly during the follow-up period. The fine trabecular bone in intimate contact and enclosing the implant fixture was sufficient for successful osseointegration. None of the 14 implants were associated with compilations during the seven-year experimental period. This study suggests that dental implant procedures are a safe and reliable treatment option for restoration of missing dentition in patients with aplastic anemia.

THE EFFECT OF SOFT LASER ON SOFT TISSUE WOUND HEALING PROCESS (SOFT LASER를 이용한 연조직 창상의 치료 효과)

  • In children, they were presented pain and discomfort after surgical procedure. Some study has been suggested that Soft Laser promote healing process. So, the aim of the present study was to examine the effect of low power generating semiconductor laser on healing process after surgical procedure. DENS-BIO Laser applied to the wounds created by mesiodens extraction and lingual frenectomy. DENS-BIO Laser was irradiated on the wound with pulse 8(1000Hz) and 2mW, for 4 minutes. And then, healing process of surgical site was observed. The results from the present study can be summarized as follows : 1. In the irradiated wound, the healing process is more faster than not irradiation group. 2. Pain is less than not irradiation group.

THE EFFECTIVENESS OF POST OPERATIVE ADMINISTRATION OF ANTIBIOTICS IN ORTHOGNATHIC SURGERY: INFECTION FREQUENCY BASED ON POST-SURGICAL APPLICATION OF ANTIBIOTICS (악교정 수술후 항생제 투여의 효용성 : 술후 항생제 투여 여부에 따른 창상감염 빈도)

  • This study observed the frequency of post-surgical infection according to post-surgical application of antibiotics in order to evaluate the benefits of the use of antibiotics after the orthognathic surgery. 349 patients without any specific medical history were divided into two groups depending on whether or not antibiotics had been applied after the surgery. The mean(SD) age of the 349 patients was 22.7(${\pm}4.25$) with a male-female ratio of 168:181. 226 patients received only 1.0g of a third-generation cephalosporin(Cefpiramide) intravenously 30 minutes prior to the surgery. Likewise, 123 patients received 1.0g of Cefpiramide 30 minutes prior to the surgery and twice daily longer than the third day after surgery. The mean(SD) duration of administration was 4.75(${\pm}0.89$) day. The patients were evaluated after surgery for any postoperative infections according to the criteria: purulent drainage from a wound, spontaneous wound dehiscence accompanied by swelling, pain, and fever around the wound. However, 14 patients of 226 patients received antibiotics only prior to the surgery developed postoperative infection, 2 patients of 123 patients received antibiotics longer than postoperative 3 days developed postoperative infection. Postoperative infection frequency showed no significant difference between the two groups(p=0.094). Also, bi-maxillary operation and mandibular operation alone, showed no significant difference in the frequency of post-surgical infection when antibiotics had been continuously used after the surgery. From this study, postoperative use of antibiotics seems to be unnecessary with view of the little significance of the factors that could affect the wound infection.

Radiologic study of the healing process of the extracted socket of beagle dogs using cone beam CT (Cone beam CT를 이용한 비글견 발치창 치유과정에 대한 방사선학적 연구)

  • Purpose: To longitudinally observe the healing process of extracted socket and the alterations of the residual ridge in healthy adult dogs using cone beam CT (CBCT). Materials and Methods: The mandibular premolars of two beagle dogs were removed and the extraction sites were covered with the gingival tissue. CBCTs (3D X-ray CT scanner, Alphard vega, Asahi Co.) were taken at baseline and at 1 week interval for 12 weeks. Radiographic density of extracted wounds was measured on normalized images with a custom-made image analysis program. The amount of alveolar crestal resorption after the teeth extraction was measured with a reformatted three-dimensional image using CBCT. Bony healing pattern of extracted wound of each group was also longitudinally observed and analyzed. Results: Dimensional changes occurred during the first 6 weeks following the extraction of dogs' mandibular premolars. The reduction of the height of residual ridge was more pronounced at the buccal than at the lingual aspect of the extraction socket. Radiographic density of extracted wounds increased by week 4, but the change in density stabilized after week 6. New bone formation was observed at the floor and the peripheral side of extracted socket from week 1. The entrance of extracted socket was sealed by a hard-tissue bridge at week 5. Conclusion: The healing process of extracted wound involved a series of events including new bone formation and residual ridge resorption.

The Temporal and Spatial Expression of the Cytokeratin in Keratinocytes during Cutaneous Wound Healing on the Amphibian(Bombina orientalis) (양서류 피부 상처회복과정 중 각질화세포 cytokeratin의 분포)

  • The system of wound healing is very complex biological processing that includes inflammatory, reepithelialization, and matrix construction. For identification of the transitional pathway of the keratinocytes, we have employed immunohistochemical analysis using cytokeratin antibody after wounding. Epithelium in skin of the frog(Bombina orientalis) was examined with transmission electron microscopy. Cytokeratin was expressed in normal basal and gland cavity cells. At 3-hour basal layer cells were strong positive, however cells of the upper layer were negative reaction. Day1 and 2 after post-wounding, regenerating epithelial cell layer was positive reaction, especially basal layer cells were strong positive. At day 10 after wounding, the degree of positive reaction to basal cells of regenerating epithelial tissue was equal to day 7 wound tissue. At day of 19th, basal and spinous layer cells were strong positive reaction. Regenerating epithelial cells were positive but some basal cells were strong positive at day 27. From this result, we identified that the migration of the keratinocytes in amphibian skin wounds is initiated from basal layer fells and the keratinocytes migrate into basal and middle of the wound area.

Effects of Oral Administered Hot Water Extracts of Korean Black Ginseng on Wound Healing in Mice (피부(皮膚) 창상(創傷) 동물모델에서 흑삼(黑蔘) 열수 추출물 경구 투여의 효과)

  • Objectives This study aims to evaluate the wound healing effects of oral administered hot water extracts of Korean black ginseng (KBG). Methods 40 C57BL/6 mice were divided into five groups; normal, control, vitamin E 200 mg/kg, KBG 100 mg/kg, KBG 200 mg/kg, each n=8. Skin wounds were made in the back of all mice except normal group using biopsy punches. Wounds were observed on days 7 and 14 after injury. The anti-oxidant and inflammatory protein levels were evaluated using western blotting. Skin tissue was analyzed by hematoxylin & eosin and Masson's trichrome staining method. Results KBG significantly accelerated reducing wound area. KBG significantly decreased myeloperoxidase activity. KBG significantly decreased oxidative stress factors such as NADPH oxidase-4 and p22phox and increased antioxidant enzymes including nuclear factor erythroid 2-related factor2, kelch-like ECH-associated protein-1, heme oxygenase-1, superoxide dismutase, catalase and glutathione peroxidase-1/2. Moreover, KBG significantly decreased inflammation factors including nuclear factor-κB, phosphorylated inhibitor of κBα, cyclooxygenase-2, inducible nitric oxide synthase, tumor necrosis factor-α and interleukin (IL)-6 and increased anti-inflammation cytokine such as IL-4 and IL-10. In addition, KBG significantly increased tight junction proteins including claudin-1, claudin-3, claudin-4. In histopathologic, KBG made the epithelium thin and uniform, and accelerated the remodeling of collagen. Conclusions The results suggest that KBG has healing effects on skin wound in mice by anti-inflammatory and antioxidant activity.
