Objectives : The Medical Records(醫案) are important materials in studying the developmental process of Korean Traditional Medicine. The purpose of this study was to investigate the medical records which were described in Gupyubang(及幼方), the first Korean book that specialized in pediatrics. Methods : 85 Medical records about medical traits of diagnosis and treatments in Gupyubang were analyzed and those were translated in Korean. Results : Medical records were analyzed as follows; The number of Male was 76 and the number of Female was 9. Prepubertal period was 16 and preschool period 14. Acute febrile convulsion was 6. Measles and blood symptom was 5. Ring worm pain, vomiting & diarrhea, colic pain was 4. Most of them were self treatment except one. 77 cases were improved and 6 cases were dead or worse. The medical records used oral medication, external medication, acupuncture, moxibustion and surgical manners. Conclusion : This study showed that Gupyubang is a experiential book and alse the medical records in Gupyubang was usefulness and had practical value.
Purpose: To provide baseline data for prevention and education of diabetic foot in Korea. Materials and Methods: 216 diabetic patients are included in this survey. The average age is 58.64 year old with average 9.92 years of diabetes mellitus periods. Among them, 41% used insulin injection, 42% oral medication, 14% both and 3% diet were used for their blood sugar control. The diabetic foot survey was done. Results: The survey of patients symptom and disease of their feet results: 49% had sensory decrease in their feet. Associated disease in foot were callus(15%), heel pain(10%), bunion(4%), toe deformity(2%), Tinea pedis were 9%, tingle sensation were 17%, and swelling on feet were 2%, The survey of patients privious history of foot problems are: 8% had previous operation history on their feet and 17% had history of infection on their feet, 23% had bullae history, The survey of patients prevention and education are: Those who have had more than one diabetic foot education were 45.7%, and among them 24% had routine regular foot check up. Only 5% wear special diabetic shoe and 3% had custom orthotics. Conclusion: Interests to diabetic foot care is increasing in diabetic patients in Korea. However still we need more education, and efforts for prevention. Special education and diabetic shoe is still low, The survey data could be good baseline for diabetic prevention and education.
Brovincamine is a selective cerebral vasodilator that was apparently produced via a slow calcium blockade. Brovincamine has been shown to increase ATP production and glucose and oxygen consumption in brain, improving energy metabolism. Also brovincamine inhibited platelet aggregation induced by ADP and collagen in vivo and in vitro via an increase of cAMP concentration, promoting therapeutic effects on cerebral circulatory disorders. So we investigated and represented the clinical effects and safety of brovincamine in patients with cerebral stroke. Thirty patients of cerebral stroke that was older than 2 months, who were 22 cases of cerebral infarction, 6 of cerebral embolism that originated from carebral infarction, 6 of cerebral embolism that originated from cardiac diseases, and 2 of cerebral embolism that originated from cardiac diseases, and 2 of cerebral hemorrhage, were administered of 20 mg of oral brovincamine three times daily for 8 weeks. Improvement rates of each symptom after 8 week administration were 30.8% for subjective symptoms, 76% for psychiatric symptoms and 65.6% for neurologic symptoms. In final global improvement rates, much improvement was 10%, improvement was 23.3% slight improvement is 36.7%, and no change was 30%. So global improvement rate including slight improvement was 70%. As for side effects, there were 3 cases of mild gastrointestinal symptoms, but there were no other subjective side effects and significant fluctuation in laboratory examination. Conclusively throughout the present study, brovincamine is judged to be well tolerated and effective in patients with cerebral stroke.
Objectives: The purpose of this study was to evaluate the association of smoking and drinking status with gingival symptoms among the adolescents in Korea. Methods:We used the data from the web-based survey of the National Korean Youth Risk Behavior of Korean Center for Disease Control in 2016. The study subjects were 65,528 adolescents from middle- and high schools. Data were analyzed by SPSS Ver. 19.0. Multiple regression analysis was used to assess the factors related to gingival symptoms. Results: We observed 1.52 of odds ratio (OR) for gingival symptoms for those who smoked only without drinking in middle schools. In addition, we had ORs of 1.29 or 1.69 for those who drank only without smoking, or who smoked and drank in the middle schools, respectively, compared to nonsmoking nondrinkers. Risk of gingival symptoms for those who drank only or who smoked and drank in high schools were 1.18 or 1.17 times higher than nonsmoking nondrinkers. Conclusions: Smoking and drinking status among the adolescents are the risk factors to develop gingival symptoms. The problem of smoking and drinking in the adolescents should be considered when establishing the policies and institutions to improve oral health targeting the adolescents.
Xerostomia is a symptom of subjective complaints due to decreased salivary secretion or various other causes, which is common in about 30% of elderly people. Parasympathetic stimulants or artificial saliva are used as remedies, but this has side effects, and there are limitations in completely resolving symptoms. In this study, an 88-year-old woman with subjective dry mouth syndrome for the previous two to three years was treated with spinal pharmacopuncture and herbal medicine (Yookmigihwang-tang). The patient received pharmacopuncture at the thoracic 3rd, 6th, and 10th level twice a day and took herbal medicine three times daily for seven days. Both the degree of oral dryness and behavior were improved after treatment of Korean medicine, especially in the case of pharmacopuncture for inner core muscles.

Peripheral facial nerve palsy is a neurological condition/disease characterized by unilateral paralysis of the facial muscles. We report the effects of complex Korean medicine treatment in 2 patients diagnosed with Bell's palsy who developed symptoms 3-4 days following vaccination with ChAdOx1 nCoV-19 and who received oral steroids and antiviral medications. Complex Korean medicine treatments including acupuncture, electroacupuncture, bee venom acupuncture, cupping therapy, herbal medicine, and physical therapy were performed. Symptom improvement was evaluated daily using the Numerical Rating Scale, House-Brackmann Grade, and Yanagihara's Score. Patient 1 was hospitalized for 22 days and Patient 2 visited the outpatient clinic 20 times. In both cases, postauricular pain caused by Bell's palsy disappeared, and symptoms of facial paralysis improved following completion of treatment. These findings could suggest that combined Korean medicine treatment may improve symptoms of peripheral facial nerve palsy.
식도암으로 인한 기관지 식도 누공 환자는 음식물을 섭취하면 누공을 통하여 기관지로 음식물이 흡입되어 폐렴과 같은 합병증을 유발할 수 있다. 특히 전신쇠약이 동반되고 면역력이 저하되어 있는 식도암 환자에게는 치명적인 결과를 초래할 수 있다. 기관지 식도 누공의 내시경적 치료는 환자에게 경구 섭취를 가능하게 하고, 합병증의 발생을 예방할 수 있을 것으로 생각된다. 하지만 아직까지 장기간의 효과 지속성이나 재발에 관하여는 만족스러운 결과가 없는 실정이다. 향후 새로운 내시경 치료법의 개발 및 치료 효과에 대한 연구가 필요하다고 생각한다.
낭종의 적출과 골 이식은 낭종 제거후 악골에서 발생하는 광범위한 골결손에 선택적인 치료이다. 자가 망상골이 최상의 이식재료이며 수년동안 임상적인 성공을 가져왔다. 그러나 이식재료를 얻기 위해서는 유병률이 증가되는 부가적인 외과적 시술이 요구되며, 다발성 결손 또는 이식이 광범위 할 때 자가골의 양이 불충분할지도 모른다. 변형된 방법으로 동종골 이식이 이용되어져 왔다. 동종골 이식의 한종류가 탈회 동결-건조 동종골(DFDBA)이다. 이 연구는 낭종 적출과 탈회 동결-건조 동종골 이식후 악골 결손에 일어나는 변화를 악골의 낭종 병소를 세집단으로 나누어 평가하였다. 낭종 적출 후 Group A는 탈회 동결-건조 동종골을 이식하였고, Group B는 자가골 이식을 하였으며 Group C은 다른 이식재를 이용하지 않고 낭종 적출만을 시행하였다. 세 집단은 술전, 수술 직후, 술후 6, 12, 24, 36개월의 임상적 검사와 방사선학적 골밀도를 조사하였다. Digital densitometer를 이용한 골밀도 검사에서 Group A과 Group B의 경우 비슷한 골밀도를 보였으며 Group C와 비교시 뚜렷한 증가를 볼 수 있었다. 그러므로 이 연구로부터 낭종 적출술과 같은 광범위한 악골 결손에 탈회 동결-건조 동종골 이식은 충분한 골형성을 보여 자가골 이식의 대체재료로서 고려되어질 수 있다.
Temporomandibular joint ankylosis is the movemental obstacle of mandible which depend on proliferation of bony or fibrous tissue in temporomandibular joint structure by various causes. In order to treat this, various surgical methods have been performed, but no operative methods have been produced consistently successful results. This research has been performed to the patients who had been operated due to temporomandibular joint ankylosis by studying classification, cause, onset, duration, anesthesia and treatment method, symptom, change of mouth opening, complication through medical record, X-ray, follow-up for being a help to proper selection of treatment method and evaluation of prognosis. The author obtained the following results by analyzing 44 cases among patients who had been operated due to temporomandibular joint ankylosis during 8 year hospitalization from 1986 to 1993 in Dept. of Oral & Maxillofacial Surgery of Seoul National University Hospital. 1. The occurrence was in the order of below 10, 20's, 10's, 30's. The average of occurrence was 12.95. Illness period was 50.0% within 10 years and 50% beyond 10 years. The average period of illness was 13.33 years. 2. Trauma occupied 54.5% of causes and inflammation occupied 45.5%. Men had more occurrences due to trauma and there was no difference in case of inflammation. 3. In nasotracheal intubations for general anesthesia, the cases of using fiberoptic laryngoscope occupied 40.9%, direct or blind nasotracheal intubation occupied 40.9% and the cases of using tracheostomy occupied 18.2%. 4. In operative approaching methods, submandibular & preauricular approach were mainly applied, and in operative methods, high condylectomy(Group I) occupied 11.4%, arthroplasty without interpositional material following condylectomy or gap ostectomy(Group II) occupied 11.4%, with interpositional material following high condylectomy (Group III) occupied 40.9%, and using condylar reconstruction following condylectomy or gap ostectomy(Group IV) occupied 36.6%. 5. In change of mouth opening reformed after surgery, Group III showed the best result of average 23.5mm, Group IV showed 16.3mm, Group I showed 14.9mm and Group II showed 10.2mm of reformation. Summarizing the results as written above, it is considered that early treatment is important as soon as possible in Temporomandibular joint ankylosis. It is recommended in surgical method what can lead to postoperative early movement maintaining anatomaical & functional form, and then the development of various surgical methods will be requested.
건강효능이 우수한 고품질 유색미 과피의 안전성 규명을 목적으로 ICR계 마우스 암수 각각 10마리를 이용하여 적진주벼, 흑진주벼 및 흑향벼 과피를 80% EtOH로 추출한 농축물을 1회 경구투여 후 14일 동안 실시한 급성독성 평가 결과는 다음과 같다. 시험물질을 1회 경구투여한 경우, 총 14일의 실험기간 동안 모든 시료투여군의 암수 동물에서 정상적인 체중증가가 관찰되었으며, 체중증가는 유의적인 차이를 나타나지 않았으며, 시료투여군의 경우 용량의존적인 체중변화도 관찰되지 않았다. 시험물질 투여 14일 후 부검결과, 암수 모든 실험군의 간장, 신장(좌, 우), 비장, 심장, 폐 및 뇌에서 시험물질 투여에 따른 어떤 육안적 이상 소견도 발견되지 않았다. 치사시킨 동물의 혈액 생화학적 검사 결과, 모든 시료 투여군의 ALT와 AST 활성도가 정상 수치를 나타냈으며, 본 실험의 최고 용량인 10.0 g/kg투여에도 정상치를 나타내는 것으로 보아, 유색미 과피 추출물을 10.0 g/kg 이하 섭취할 경우에는 어떠한 간독성도 나타내지 않음을 확인할 수 있었다. 이상의 실험결과를 통해 고품질 유색미인 적진주벼, 흑진주벼 및 혹향벼 과피 추출물의 단회 경구투여에 의한 독성시험 시, 시험물질 투여에 의한 표적 장기가 관찰되지 않았으며, 어떠한 독성학적인 변화도 관찰되지 않는 것으로 보아, 무해 용량은 암수 모두 10.0 g/kg을 상회할 것으로 추정할 수 있었다.
