일부 대학생들의 스트레스 증상과 구강악습관 및 턱관절 기능장애의 관련성 (Relationship between stress, oral parafunctional habits, and temporomandibular joint disorders among college students)

  • Purpose: This study aimed to investigate the effect of stress and oral parafunctional habits on temporomandibular joint disorders (TMD) in college students. Methods: An online survey comprising 354 questionnaires was conducted from May 23, 2020, to June 21, 2020. The collected data were analyzed using Student's t-test, one-way analysis of variance, correlation tests, and multiple regression analysis. Results: The stress symptom score was higher in females than in males. The symptoms of physical stress were significantly higher among students from the Department of Public Health, whereas those of psychological stress were higher among students from the Department of Physical Education. Both TMDs and oral parafunctional habits were higher among females, students of the Departments of Arts and Physical Education, and seniors. The higher the stress symptom level, the higher the scores for TMD and oral parafunctional habits. TMD and oral parafunctional habits were found to be most affected by physical stress. Conclusion: These findings indicate that a reduction in physical stress is required to decrease the prevalence of oral parafunctional habits and TMDs among college students. This can be achieved by the incorporation of physical activity programs, which may aid in reducing the symptoms of physical stress.

  • Introduction: Sialolithes are initiated by localized deposition of calcified material in the salivary glands. And that may even cause various symptom especially swelling and pain. This study purposes to collect statistical data of sialolithiasis for clinical analysis. Materials and Methods: Among forty seven patients who have visited Seoul National University Dental Hospital during 2004-2009, patients' age, sex, location and size of stone, radiodensity of stone, symptom, surgical procedure were investigated. Statistical correlation between size, location, symptom was evaluated. Chemical composition was analyzed for 3 sialolithes. Results: The average age was 41.4 years. Sialolithiasis had slight female predilection (57.4%). Most cases occurred in the submandibular glands (91.5%). And most cases had radiopaque features (95.8%). The average size was 7.17 mm. The most frequent location of the stones were the duct orifice and the submandibular gland hilum (16 cases in each), followed by the middle part of the duct (n=8), the intraglandular area (n=4), and the proximal part of the duct (n=3). Eleven cases were asymptomatic. Thirty six cases had complaints of pain, swelling, hardness, and decrease in saliva flow (multiple symptoms). Various methods of surgery was performed. Two cases were self-removed. Thirty seven cases underwent procedure involving stone removal alone. Six cases underwent gland extirpation, and two cases underwent ductoplasty. Conclusion: There was no statistical correlation between size, location, and symptoms. Sialolith was composed of Ca (58.5-69.3%), P (30.7-35.7%), organic material, and trace inorganic material.

Prognosis in case of nerve disturbance after mandibular implant surgery in relation to computed tomography findings and symptoms

  • Purpose: The aim of this study was to evaluate the computed tomography (CT) imaging findings and clinical symptoms of patients who complained of neurosensory disturbances after mandibular implant surgery, and to investigate the relationships of these parameters with the prognosis for recovery. Methods: CT scans were reviewed in 56 patients with nerve disturbance after mandibular implant surgery. Two oral radiologists classified the imaging findings into intrusion, contact, close, and separate groups according to the distance from the inferior border of the implant to the roof of the mandibular canal (MC). The symptoms of 56 patients were classified into 8 groups and the frequency of each group was investigated. Patients were categorized according to symptom improvement into no recovery and recovery groups, and the relationships of recovery with the CT classification and specific symptom groups were analyzed. Results: Thirty-eight of the 56 nerve disturbance cases showed improvement. The close and separate groups in the CT classification had a strong tendency for recovery (90.9% and 81.8%, respectively) (P<0.05). Although the lowest recovery rate was found in the intrusion group, it was non-negligible, at 50%. The 6 patients with a worm crawling feeling all improved, while the 8 cases with a tightening sensation showed the lowest recovery rate, at 12.5%, and the symptom of a tightening sensation occurred only in the intrusion and contact groups. Conclusions: The closer the implant fixture was to the MC on CT images, the less likely the patient was to recover. Regarding paresthesia symptoms, while a worm crawling feeling is thought to be a predictor of recovery, a tightening sensation appeared to be associated with a lower recovery rate.

스트레스에 의한 구강안면증상의 발혀에 관한 역학적 연구 (An Epidemiologic Study on the Frequency of Stress Symptoms in the Orofacial Region)

  • Stress is recognized as a major predisposing and/ or precipitating factor in long-lastig intractable chronic pain, such as temporomandibular disorders, headache, and other psychophysiological disorders. So it is necessary to detect physical and psychological changes induced by stress as soon as possible for positive treatment outcome. This study was performed to investigate the occurrence rate of stress symptoms according to anatomic region, type of symptom, and other personal and social factors. 859 subjects from general population answered the stress symptom questionnaire devised by the author and composed of 50 items. Data from the questionnaire were analyzed statistically with SPSS program and the results obtained were as follows : 1. Oral symptom which showed the highest frequency rate of 38.8% was vesicular lesion of the lip and cheek. The other symptoms with more 20% occurrence rate were ulcerative leion of lip and cheek, toothache, paresthesia of teeth, eruption of tongue, tongue coating and taste change in descending order. 2. In extraoral symptoms, ?데 disturbance was the item which showed the highest frequency rate of 62.0%, and the items for stiffness of suboccipital region and neck, headache, facial swelling, furuncle of face were answered more than 45% of the subjects whereas only 14.0% of the subjucts complained jaw pain under stress. 3. The better one who thought his or her health status was, the fewer items were answered and the difference of symptom frequency by dwelling place and by having hobby were shown in extraoral symptoms only. 4. For relief of stress symptoms, 79.2% of the subjects replied only to take a rest whereas not more than 13, 5% of the subjects visited dental clinic. Correlationship between symptom sites were very high.

대구광역시 일부 초등학교 교사들의 주관적인 구강보건실태 (A study of subjective oral health actual condition in elementary school teachers, Daegu area)

  • Objective : This study was performed in order to figure out Oral Health Actual Condition in Elementary School Teachers in Deagu area. This study was conducted from March through May 2008. Method : A total of three hundred and ten Elementary School Teachers were surveyed. The collected data were analyzed by Oral Health Actual Condition and cognition, Diet habit or living and one's own intellect health state, Oral disease sign symptoms of percent and 2-test and One-way ANOVA test by using SPSS12.0 Program. Results : 1. The most of result Frequencies of tooth brushing per one day were 3 over 91.0% and Oral Examination, Oral Health Education need. 2. The result of oral disease sign and symptom were hypersensitivity due to cold food(39.0%), halitosis(21.6%), gingival bleeding tendency(21.3), clicking sound on TMJ(18.7%), hypersensitivity due to tooth burshing(17.1%), easy crown fracture and to be fine(10.0%), pain on TMJ or limitation of mouth opening(7.1%). 3. The most of result age a group oral hygiene assistance article age 20 for interdental tooth brushing(46.4%), age 50 over not used interdental tooth brushing 38.5%. 4. The result of sign and symptom and snack following was statistically significant(P<0.05), health of own cognition and Oral health of own cognition was statistically significant(P<0.001). Conclusion : The study of understanded the Subjective Elementary School Teachers Oral Health Actual Condition and Promotion of Oral Health follow up Oral examination and Oral Health Education have to system groping.

Age Differences in Signs and Symptoms of Patients with Temporomandibular Disorders

  • Purpose: The aims of this study were to evaluate the differences in subjective symptoms, clinical characteristics, distribution according to Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) subgroup, psychological profile of TMD patients, and to identify the prevalence and trend according to age. Methods: A total of 1,052 patients (261 men and 791 women; mean age, $34.40{\pm}15.73$ years) who visited the Orofacial Pain Clinic of the Department of Oral Medicine, Seoul National University Dental Hospital complaining of TMD symptoms of were evaluated. All patients were questioned for medical history, clinical symptoms and contributing factors. Clinical examination and patient grouping based on RDC/TMD was conducted. Radiographies were taken. The Korean version of RDC/TMD axis II and Symptom Checklist-90-Revision (SCL-90-R) were administered to evaluate pain-related disability level and psychological status of the patients. Results: Prevalence peaked in the 20-year-old age group. There were more women than men in all groups. The highest T-score among SCL-90-R dimensions was somatization in each group, except for teenagers who showed the highest T-score in interpersonal sensitivity. The 30-year-old age group showed the highest distribution of high disability based on the graded chronic pain scale. Age was positively associated with pain intensity (r=0.100), number of positive muscles on palpation (r=0.137) and negatively associated with maximum mouth opening (r=-0.168). Conclusions: Subjective symptoms and clinical characteristics of TMD patients show distinct tendencies according to different age groups. Treatment should be customized and personalized according to age for efficient symptom resolution and patient satisfaction.

악교정 수술시 술후 환자의 증상변화와 항생제 투여와의 관계 (THE RELATIONSHIP BETWEEN THE SYMPTOMS OF THE PATIENTS AND POST-SURGICAL APPLICATION OF ANTIBIOTICS IN ORTHOGNATHIC SURGERY)

  • This study compared and evaluated the variation of patients' symptoms depending on whether or not antibiotics had been used in the recovery period after the orthognathic surgery. 28 patients, with no particular medical history, were divided into two groups on the basis of whether antibiotics had been applied after the orthognathic surgery. All patients had been operated on maxilla and mandible simultaneously and were investigated with any symptom changes within the postoperative 4 days. Among the symptoms that could follow after the orthognathic surgery, 6 symptoms were checked every 24 hours on 5 points basis. Phases of each symptom improvements were checked and analyzed by ANOVA statistical method. Aspects of symptom changes according to application of antibiotics showed no statistical significance. Symptoms, except for swelling, showed continuous improvements after the surgery. Swelling increased until postoperative $2^{nd}\;day$, and then started to decrease. This study shows that post-surgical use of antibiotics has no significance provided that post-surgical infection had not occurred.

20-30대 일부 성인의 스트레스, 구강악습관 및 턱관절 증상의 연관성 (Relationship of Stress, Oral Habits and TMJ Symptoms in 20-30 ages Adults)

  • Objectives : The purpose of this study was to examine the relationship of stress symptoms, oral habits and temporomanbibular joint symptom among 20-30 ages adults. And it's also meant to investigate the direct and indirect influence of these factors by using a path model to determine their causal relationship. Methods : The subjects in this study were 287 selected 20-30 ages adults, on whom a survey was conducted from June 15 to July 10, 2014. The data were analyzed using SPSS 18.0(SPSS 18.0 K for window, SPSS Inc USA) and IBM SPSS Amos 18.0(SPSS Inc, Chicago, IL, USA). Results : There were significant differences in oral habits, TMJ symptoms and stress according to gender. There were significant differences in oral habits, TMJ symptoms, physical and psycho-emotional symptoms according to Systemic disease. And stress, physical, psycho-emotional symptoms, oral habits and TMJ symptoms were correlated to one another. Stress exerted a direct influence on physical, psycho-emotional symptoms, and psycho-emotional symptoms had a direct impact on physical symptoms, oral habits and TMJ symptoms. Physical symptoms exercised a direct influence on oral habits and TMJ symptoms, and oral habits affected TMJ symptoms in a direct way. physical, psycho-emotional symptoms and oral habits served as parameters that produced partial mediation effects, and the two factors had an indirect impact on TMJ symptoms. Conclusions : It's found that stress exerted direct and indirect influence on oral parafunction and TMJ symptoms. Like other diseases, oral habits and TMJ symptom that stem from stress is likely to lead to chronic diseases if the two are not noticed at the right time. Therefore individual people should try to get rid of stress in a manner to be appropriate for their own characteristics in order to maintain their oral health.

근로자의 구강자각증상과 구강건강 관련 삶의 질 (A Study on the Oral Symptoms and Oral Health-Related Quality of Life (OHIP-14) of Industrial Workers)

  • 본 연구의 목적은 근로자의 구강건조증상과 다양한 구강자각증상에 대하여 알아보고 근로자의 구강건강관련 삶의 질에 미치는 요인들에 대해 조사 분석하여 근로자의 구강건강증진을 도모하는 프로그램개발을 위한 기초자료를 제공하고자 2012년 8월 1일부터 9월 20일까지 전라북도 소재의 5개의 산업장 근로자를 대상으로 분석한 결과 다음과 같은 결론을 얻었다. 1. 구강건조증상은 사무직 근로자가 2.59점으로 영업직 근로자 1.71점보다 유의하게 구강건조증상 정도가 높았다 (p<0.001). 2. OHIP-14는 비정규직이 2.05점, 정규직이 1.82점으로 나타났다(p<0.00 1). 3. 구강건조증상은 OHIP-14와 양의 상관관계를 보였다 (r=0.456). OHIP-14는 연령과는 양의 상관관계를 보였으며 (r=0.209), 최종학력과 음의 상관관계를 보였다(r=-0.136). 4. 구강건조증상이 심할수록(${\beta}=0.383$, p<0.001), 연령이 높을수록(${\beta}=0.221$, p<0.001), 치과질환을 갖고 있다고 느꼈을 때(${\beta}=0.146$, p<0.01) OHIP-14에 유의하게 영향을 미치는 요인으로 나타났다. 결론적으로 위와 같이 근로자들의 구강건강관련 삶의 질에 영향을 미치는 요인을 참고하여 산업장 근로자의 구강건강증진을 도모하는 프로그램을 개발하고 국가보건정책 부분에서 산업구강보건 개선을 위해 적극 고려해야 할 것으로 생각된다.

Effectiveness of Computed Tomography for Blow-out Fracture

  • Purpose: This study assessed the association between eye symptoms (enophthalmos or diplopia) and site of damage, volume, deviated inferior rectus muscle (IRM) and type of fracture with computed tomography (CT). The intent is to anticipate the prognosis of orbital trauma at initial diagnosis. Methods: Forty-five patients were diagnosed with fractures of the inferior wall of one orbit. Fracture area, volume of displaced tissue, deviated IRM, and type of fracture were evaluated from coronal CT by one investigator. The association of those variables with the occurrence of eye symptoms (diplopia and enophthalmos) was assessed. Results: Of 45 patients, 27 were symptom-free (Group A) and 18 had symptoms (Group B) of enophthalmos and/or diplopia. In Group B, 12 had diplopia, one was enophthalmos, and five had both. By CT measurement, group A mean area was $192.6mm^2$ and the mean volume was $673.2mm^3$. Group B area was $316.2mm^2$ and volume was $1,710.6mm^3$. The volume was more influential on symptom occurrence. Each patient was categorized into four grades depending on the location of IRM. Symptom occurrence and higher grade were associated. Twenty-six patients had trap-door fracture (one side, attached to the fracture), and 19 had punched-out fracture (both sides detached). The punched-out fracture was more strongly associated with symptoms and had statistically significantly higher area and volume. Conclusion: In orbital trauma, measurement of fracture area and volume, evaluation of the deviated IRM and classification of the fracture type by coronal CT can effectively predict prognosis and surgical indication.
