Three-dimensional comparison of 2 digital models obtained from cone-beam computed tomographic scans of polyvinyl siloxane impressions and plaster models

  • Purpose: This study was performed to evaluate the dimensional accuracy of digital dental models constructed from cone-beam computed tomographic (CBCT) scans of polyvinyl siloxane (PVS) impressions and cast scan models. Materials and Methods: A pair of PVS impressions was obtained from 20 subjects and scanned using CBCT (resolution, 0.1 mm). A cast scan model was constructed by scanning the gypsum model using a model scanner. After reconstruction of the digital models, the mesio-distal width of each tooth, inter-canine width, and inter-molar width were measured, and the Bolton ratios were calculated and compared. The 2 models were superimposed and the difference between the models was measured using 3-dimensional analysis. Results: The range of mean error between the cast scan model and the CBCT scan model was -0.15 mm to 0.13 mm in the mesio-distal width of the teeth and 0.03 mm to 0.42 mm in the width analysis. The differences in the Bolton ratios between the cast scan models and CBCT scan models were 0.87 (anterior ratio) and 0.72 (overall ratio), with no significant difference (P>0.05). The mean maxillary and mandibular difference when the cast scan model and the CBCT scan model were superimposed was 53 ㎛. Conclusion: There was no statistically significant difference in most of the measurements. The maximum tooth size difference was 0.15mm, and the average difference in model overlap was 53 ㎛. Digital models produced by scanning impressions at a high resolution using CBCT can be used in clinical practice.

Determination of Scattered Radiation to the Thyroid Gland in Dental Cone Beam Computed Tomography

  • Background: Cone beam computed tomography (CBCT) is a specialized medical equipment and plays a significant role in the diagnosis of oral and maxillofacial diseases and abnormalities; however, it is attributed to risk of exposure of ionizing radiation. The aim of the study was to estimate and determine the amount of scattered radiation dose to the thyroid gland in dental CBCT during maxilla and mandible scan. Materials and Methods: The average scattered radiation dose for i-CAT 17-19 Platinum CBCT (Imaging Sciences International) was measured using a Multi-O-Meter (Unfors Instruments), placed at the patient's neck on the skin surface of the thyroid cartilage, with an exposure parameter of 120 kVp and 37.07 mAs. The surface entrance dose was noted using the Multi-O-Meter, which was placed at the time of the scan at the level of the thyroid gland on the anterior surface of the neck. Results and Discussion: The surface entrance dose to the thyroid from both jaws scans was 191.491±78.486 µGy for 0.25 mm voxel and 26.9 seconds, and 153.670±74.041 µGy from the mandible scan, whereas from the maxilla scan the surface entrance dose was 5.259±10.691 µGy. Conclusion: The surface entrance doses to the thyroid gland from imaging of both the jaws, and also from imaging of the maxilla and mandible alone were within the threshold limit. The surface entrance dose and effective dose in CBCT were dependent on the exposure parameters (kVp and mAs), scan length, and field of view. To further reduce the radiation dose, care should be taken in selecting an appropriate protocol as well as the provision of providing shielding to the thyroid gland.

  • Clinical assessment of bone-graft healing in the maxillofacial region is generally limited to clinical evaluation, radiographs, and biopsy. Sequential interpretation of osseous repair, more sensitive than with conventional radiography is possible with a non-invasive, non-destructive radionuclide method. Technetium-99m radionuclide bone scan was used in the evaluation of the progress of osteogenic activity in autologous iliac bone graft and freeze-dried bone allograft of dog's mandible. Bone scan was performed at 1wk, 2wk, 4wk, 6wk, and 8wk after grafting. In autologous graft the activity ratio for the graft bone remained greater than that of the host since 2자 after grafting; however, in lyophilized allograft the activity ratio for graft bone was greater than that of the host at 6자 after grafting.

임플랜트 전산화단층사진에서 하악의 위치와 gantry각의 변화가 임플랜트 매식로 평가에 미치는 영향에 관한 연구 (The effect of mandibular position and gantry angle on the evaluation of implant site with implant CT)

  • Purpose: The altered gantry angle during scanning for some multiplanar reconstruction CT program (CT/MPR) may cause distortion of the image. The aim of this study was to ascertain whether there is a image distortion in a reformatted image when the gantry and the object are equally inclined using ToothPix and DentaScan program. Materials and Methods: A resin block model with four cylindrical holes and a human dry mandible were used. Two MPR software packages, ToothPix and DentaScan program, were used for reformatted panoramic images. The block and the gantry were equally inclined at 0°, 15°, and 30°. Results: With ToothPix program, a resin block model with empty holes and a dry mandible showed inclined images in the reformatted panoramic image. Increasing the gantry angle, the depth and inclination of the holes were increased in the reformatted central panoramic images. However, a resin block model with gutta perch a in its holes and a dry mandible with a wire in its mandibular canal didn't show image distortion. With DentaScan program, image distortion was not seen in any situation. Conclusion: ToothPix program may distort the reformatted image when the gantry angle is not at zero degrees. However, with DentaScan program, the patient may be positioned comfortably and the gantry can be adjusted to the patient positioning.

임시 의치와 이중 디지털 스캐닝 기법을 활용한 전악 고정성 임플란트 수복 증례 (Full mouth rehabilitation with fixed implant-supported prosthesis using temporary denture and double digital scanning technique: a case report)

  • 임플란트 고정성 보철물 제작 시 디지털 시스템을 이용하여 수복할 때, 3차원적인 임플란트 위치의 구내 스캔과 임시 의치 혹은 임시 보철물 장착 후 구내 스캔을 이용한 이중 디지털 스캐닝(double digital scanning) 기법이 필요하다. 구내 스캔 시, 스캔 바디를 안정적인 랜드마크로 이용하면 다지털 인상의 정확도 향상과 기공과정이 효율적이고 단순화될 수 있다. 본 증례는 완전 디지털 시스템을 활용하여 맞춤형 치과용 임플란트 지대주 및 임시 보철물, 최종 보철물을 계획하고 제작하였다. 임플란트 식립 후, 임플란트에 체결한 구내 스캔 바디와 임시 의치 조직면에 인기 된 구내 스캔 바디의 스캔 영역을 중첩하였다. 중첩된 파일에서 임시 의치의 수직 고경에 맞는 맞춤형 치과용 임플란트 지대주와 임시 보철물을 제작하여 환자에게 시적 하였고, 임시 보철물을 토대로 최종 보철물을 제작하였다. 임시 의치 제작 시 설정한 수직 고경, 중심위 등을 최종 보철물까지 활용하여 기능 및 심미적으로 만족할 만한 결과를 얻어 이를 보고하고자 한다.

구강 내 스캐너와 구강 외 스캐너를 사용하여 취득된 스캔 데이터 정확도 비교 (Comparison of accuracy of digital data obtained by intra-oral scanner and extra-oral scanner)

  • Purpose: The purpose of this study was to compare the accuracy of the scan data acquired by the extra-oral and intra-oral scanner. Methods: The maxillary right first molar was made of polymethyl methacrylate(PMMA) specimen. This PMMA specimen was scanned with a engineering scanner and intra-oral scanner. Meanwhile, extra-oral scanner scanned stone die duplicated from PMMA master die. Trueness and precision of scan datas was measured by 3-dimensinal inspection. Independent t-test was conduct to analysis the significant difference(a=0.05). Results: In the trueness analysis, mean of discrepancies were 13.82um for intra oral scanner and 16.84 um for extra-oral scanner. In the precision analysis, mean of discrepancies were 11.72 for inta-oral scanner and 9.2 for extra-oral scanner. Both trueness and precision showed a statistically significant difference (Table 1, p<0.05). Conclusion: Intra-oral scanner can show higher trueness than extra-oral scanner, it has lower precision.

두경부 전산화 단층촬영시의 주요 장기선량, 유효선량 및 위험도 (ORGAN DOSE, EFFECTIVE DOSE AND RISK ASSESSMENT FROM COMPUTED TOMOGRAPHY TO HEAD AND NECK REGION)

  • The organ or tissue doses were determined with head and neck phantom measurement for multiple axial scans (36 slices), multiple coronal scans (13 slices), 3 types of single axial scans(orbit, maxillary sinus and mandibular canal) and single coronal scan (maxillary sinus). For each scan sequence 30 TLDs were placed in selected sites(16 internal sites and 14 external sites) in a tissue-equivalent phantom. The exposure was made at 120kVp, 500mAs with 5 mm slice width. The results were as follows : 1. In multiple axial scans, the greatest effective dose recorded was that delivered to the thyroid glands(2.77 mSv) and the least was that received by the skin(0.05 mSv). From these data, stochastic effects were 202.2x10/sup -6/ and 3.7×10/sup -6/, respectively. 2. In multiple coronal scans, the greatest effective dose recorded was that delivered to the salivary glands(0.58 mSv) and the least was that received by the skin(0.01 mSv). From these data, stochastic effects were 42.2×10/sup -6/ and 0.7×10/sup -6/, repectively. 3. Among single axial scans, the greatest effective dose recorded was that delivered to the salivary gland(0.38 mSv) in maxillary sinus scan. From this data, stochastic effect was 27.7×10/sup -6/. 4. In single coronal scan, the greatest effective dose recorded was that delivered to the salivary gland(0.01 mSv). From this data, stochastic effect was 1.0×10/sup -6/. 5. The equivalent dose measured that delivered to the lens of the eyes was 69.64 mSv in multiple axial scan, 39.32 mSv in multiple coronal scan and 36.77 mSv in single axial scan(orbit).

가족성 거대 백악종 (Familial gigantiform cementoma)

  • Familial gigantiform cementoma is a rare fibre-cemento-osseous disease of the jaws which appears to be transmitted as an autosomal dominant trait with variable expressivity of the phenotype. A 7-year-old girl visited DKUDH complaining of the painless facial deformity. Clinically, significant facio-lingual expansion was observed at the left maxilla, left mandibular body and symphysis portion. Malposition of lower anterior teeth was found. Panoramic radiograph and CT scan showed the extensive expansile mixed lesion at maxilla and mandible. Bone scan revealed hot spot at the maxilla and left side of mandible. Histologic examination revealed moderately dense fibrous connective tissue with scattered masses resembling cementum. The patient's mother had a history of the mandibular resection due to benign tumor. Her younger brother had buccal expansion of right mandible. We report our finding of a family that has exhibited clinical, radiographic and histologic findings consistent with the familial gigantiform cementoma.

초음파를 이용한 교근의 단면두께 측정의 재현성에 대한 연구 (The reproducibility of ultrasonographic assessment of local cross-sectional dimensions of masseter muscle)

  • Purpose : To determine if ultrasonography is a reliable technique to assess masseter muscle sites within intra- and interobserver and the scanning level and/or the muscle condition affect local cross-sectional dismension (LCSD) measurements of masseter muscle. Materials and Methods : 10 subjects without sign and symptom of temporomandibular disorders and missing posterior teeth were examined by ultrasonography. Bilateral ultrasonographic examinations were performed with a linear (B-scan) 7.5 MHz small-part transducer to register LCSDS of the masseter muscle on three different levels. Scans were made on relaxed and clenching condition. Three oral and maxillofacial radiologists measured at two sessions with a time interval of at least 5 minutes. Results : Variables such as 'condition' and 'level' had a significant effect on muscle measurements (p<0.05). There was no significant difference between 'sessions'(p>0.05) and 'observers' (p>0.05). LCSDS on lower scan level were significantly thinner than those on upper and middle level. Those on clenching condition were significantly thicker than those on relaxed condition (p<0.05). The scanning level with the highest reproducibility was middle with clenching condition (ICC=0.90, $MSE=0.55\%$). Conclusion : The data suggested that ultrasonography was a reliable method for measuring LSCD of masseter muscle in intra- and Interobserver and middle scan level showed the most reliable data.

The influence of different scan resolutions on the detection of proximal caries lesions

  • Purpose: This study was conducted to evaluate the effect of different spatial resolutions of a photostimulable phosphor plate (PSP) radiography system on the detection of proximal caries lesions. Materials and Methods: Forty-five extracted human permanent teeth were radiographed using a PSP system (VistaScan Perio Plus) and scanned at the 4 resolutions (10 lp/mm, 20 lp/mm, 25 lp/mm, and 40 lp/mm) available in the system. Three independent examiners scored the images for the presence and absence of proximal caries lesions using a 5-point scale. The presence or absence of caries was confirmed by histological sections of the examined teeth (defined as the gold standard). Intra- and inter-observer reproducibility was calculated by the weighted kappa test. One-way analysis of variance with the post hoc Tukey test was used to compare the area under the receiver operating characteristic curve for the classifications made with each resolution. Results: For the detection of enamel lesions, the spatial resolution of 10 lp/mm was significantly superior to the other resolutions. However, the spatial resolution did not affect the detection of caries lesions in dentin (P>0.05). Conclusion: Spatial resolution may influence the accuracy of the detection of incipient caries lesions in radiographs with PSP plates. Images with low spatial resolution seem to be more appropriate for this purpose.
