Heparin-induced Thrombocytopenia Type II after Free Flap Operation

  • After radical excision of a tumor in the maxillofacial area, functional and esthetic reconstruction is needed, including flap surgery. Among the many etiologies of flap failure, venous thrombosis is one of the most frequent. Heparin is used routinely in the effort to avoid development of venous thrombosis. In rare cases, heparin-induced thrombocytopenia (HIT) type II occurs due to exposure to heparin. Heparin attached to platelet factor 4 forms a PF4/heparin-immunoglobulin G immune complex on platelet surfaces. This complex activates platelets, which leads to multiple coagulation in venous and arterial blood. We report here on a rare occurrence of HIT type II following fibula free flap surgery.

  • Distraction osteogenesis was pioneered by Ilizarov in the treatment of injured extremities. Its subsequent application to Oral and Maxillofacial Surgery has opened a new chapter in the treatment of facial deformity. Careful monitoring of the period of distraction and consolidation of the callus is important and has been well described. Complications, such as infection, haematoma and premature ossification, are difficult to diagnose and can compromise the outcome after the surgery. Too slow rate of distraction results in premature fusion of cortices and too rapid information of bone 'cyst' within the callus distraction. I experienced 2 patients of congenital unilateral mandibular hypoplasia, so called Hemifacial Microsomia. After distraction surgery, I evaluated callus formation of mandibular distraction with Ultrasound Sonography during distraction and consolidation period. Plain radiography, although it enables accurate measurement of the distraction gap, did not give sufficient detail to allow assessment of early stages of bone formation, But, ultrasound monitoring could enable continuous monitoring of the distraction gap without exposure to ionizing radiation and allow detection of fine detail, which may influence manipulation of the callus.

Postantibiotic Effects and Postantibiotic Sub-MIC Effects of Antibiotics on Treponema Denticola

  • Postantibiotic effect (PAE) is defined as the length of time that bacterial growth is suppressed following brief exposure to an antibiotic. In this study, the in vitro PAE, postantibiotic sub-MIC effect (PA SME) and sub-MIC effect (SME) of antibiotics on Treponema denticola ATCC 35405 were investigated. The PAE of doxycycline and metronidazole were 20.3 h and 25.0 h, respectively. The PA SMEs examined by addition of 0.1, 0.2 and 0.3X MICs during the postantibiotic phase of the bacteria for metronidazole were longer than those for doxycycline. In contrast, the SMEs for doxycycline were longer than those for metronidazole. The PA-SME and SME values increased as the concentration of antibiotics increased. The present study illustrates the existence of PAE, PA-SME and SME for several antibiotics against T. denticola, thereby extending the pharmacodynamic advantages of these antibiotics.

디지털구내방사선사진의 구리당량화상에 대한 평가 (An Assessment on Cu-Equivalent Image of Digital Intraoral Radiography)

  • Geometrically standardized dental radiographs were taken. We prepared Digital Cu-Equivalent Image Analyzing System for quantitative assessment of mandible bone. Images of radiographs were digitized by means of Quick scanner and personal Mcquintosh computer. NIH image as software was used for analyzing images. A stepwedge composed of 10 steps of 0.1mm copper foil in thickness was used for reference material. This study evaluated the effects of step numbers of copper wedge adopted for calculating equation. kVp and exposure time on the coefficient of determination(r²)of the equation for conversion to Cu-equivalent image and the coefficient of variation and Cu-Eq value(mm) measured at each copper step and alveolar bone of the mandible. The results were as follows: 1. The coefficients of determination(r²) of 10 conversion equations ranged from 0.9996 to 0.9973(mean=0.9988) under 70kVp and 0.16 sec. exposure. The equation showed the highest r was Y=4.75614612-0.06300524x +0.00032367x² -0.00000060x³. 2. The value of r² became lower when the equation was calculated from the copper stepwedge including 1.0mm step. In case of including 0mm step for calculation. the value of r showed variability. 3. The coefficient of variation showed 0.11, 0.20 respectively at each copper step of 0.2, 0.1mm in thickness. Those of the other steps to 0.9 mm ranged from 0.06 to 0.09 in mean value. 4. The mean Cu-Eq value of alveolar bone was 0.14±0.02mm under optimal exposure. The values were lower than the mean under the exposures over 0.20sec. in 60kVp and over 0.16sec. in 70kVp. 5. Under the exposure condition of 60kVp 0.16sec.. the coefficient of variation showed 0.03. 0.05 respectively at each copper-step of 0.3, 0.2mm in thickness. The value of r² showed over 0.9991 from both 9 and 10 steps of copper. The Cu-Eq value and the coefficient of variation was 0.14±0.01mm and 0.07 at alveolar bone respectively. In summary. A clinical application of this system seemed to be useful for assessment of quantitative assessment of alveolar provided high coefficient of determination is obtained by the modified adoption of copper step numbers and the low coefficient of variation for the range of Cu-Equivalent value of alveolar bone from optimal kVp and exposure time for each x-ray machine.

치조병소에 대한 X-선학적 비교연구 (A COMPARATIVE STUDY OF ROENTGENOGRAPHIC APPEARANCE OF PERIODONTAL OSSEOUS LESIONS)

  • The purpose of this article is to re-examine the roentgenographic appearance of alveolar osseous in an effort to determine the value of the orthopantomogram in the diagnosis of periodontal disease. A total of 158 osseous lesions from 13 human dry skulls were studied. 14 Ultra-speed periapical films, 2 bitewing films and Kodak panoramic film are used to obtain radiographs of all defects. The bisecting technic was used, with a target-film distance of. 8 inches and exposure factors of 70 kVp, 10MA and 0.4 sec. at anterior teeth, 0.6 see at posterior teeth in exposure time. For orthopantomogram, Panoura Eight-C was used with a exposure factors of 90kVp, 10MA. and 15sec. exposure time. All films were developed in a light. tight darkroom at 68°F for 4½ minutes. Comparison of orthopantomogram and intraoral films on the view-box was carefully studied in relation to the types of osseous defect visually evident; Proximal intraosseous defects, Interproximal craters, Interproximal hemisepta, Furcal defects on multirooted teeth, and Facial or Lingual one-walled defects. The results obtained were as follows; 1. Proximal osseous defects throughout the dental arches and furcal defects on facial and lingual surfaces of multirooted teeth can be identified with a high degree of accuracy from their orthopantomographic appearances. 2. Lesions on facial or lingual surface of the alveolar arches are rather difficult to locate or recognize on the dental radiographs. 3. In determining whether the proximal and furcal lesions are located facially or lingually, Orthopantomogram is superior to the conventional film Orthopantomogram obtained with standardization of head in proper position revealed the complete visualization of alveolar bone without showing occlusal surface of molars and proximal superimposition of teeth. Thus, on the standardized orthopantomograms, The roentgenographic characteristics of each defect were determined.

A Proposal for a Predictive Model for the Number of Patients with Periodontitis Exposed to Particulate Matter and Atmospheric Factors Using Deep Learning

  • Background: Particulate matter (PM) has been extensively observed due to its negative association with human health. Previous research revealed the possible negative effect of air pollutant exposure on oral health. However, the predictive model between air pollutant exposure and the prevalence of periodontitis has not been observed yet. Therefore, this study aims to propose a predictive model for the number of patients with periodontitis exposed to PM and atmospheric factors in South Korea using deep learning. Methods: This study is a retrospective cohort study utilizing secondary data from the Korean Statistical Information Service and the Health Insurance Review and Assessment database for air pollution and the number of patients with periodontitis, respectively. Data from 2015 to 2022 were collected and consolidated every month, organized by region. Following data matching and management, the deep neural networks (DNN) model was applied, and the mean absolute percentage error (MAPE) value was calculated to ensure the accuracy of the model. Results: As we evaluated the DNN model with MAPE, the multivariate model of air pollution including exposure to PM2.5, PM10, and other atmospheric factors predict approximately 85% of the number of patients with periodontitis. The MAPE value ranged from 12.85 to 17.10 (mean±standard deviation=14.12±1.30), indicating a commendable level of accuracy. Conclusion: In this study, the predictive model for the number of patients with periodontitis is developed based on air pollution, including exposure to PM2.5, PM10, and other atmospheric factors. Additionally, various relevant factors are incorporated into the developed predictive model to elucidate specific causal relationships. It is anticipated that future research will lead to the development of a more accurate model for predicting the number of patients with periodontitis.

Survival Analysis of Oral Squamous Cell Carcinoma in a Subgroup of Young Patients

  • Oral squamous cell carcinoma (OSCC) is predominantly a disease of middle-aged men with long-term exposure to tobacco and alcohol. An increasing trend has been reported at a younger age worldwide. Clinical records of 100 patients under the age of 45 years treated specifically for oral cavity SCC in our hospital during a 10-year period were retrospectively analyzed to calculate the survival rates. An obvious male predominance coincided with smoking trend among Chinese young individuals and female patients were more likely to have no traditional risk factors such as smoking or drinking. The 5-year overall survival rate and disease-free survival rate were 61.0% and 75.5%, respectively, consistent with other published series over the decade showing a relatively better survival among the young. No significant differences clearly correlated with outcome when comparing non-smokers non-drinkers to ever-smokers and ever drinkers (P>0.05). Overall survival rate and disease free survival rate was found to be significantly higher in patients with early-stage disease than with advanced stage disease (P=0.001, P=0.009 respectively). The strong influence of clinical stage on prognosis emphasizes the importance of early diagnosis and treatment of oral malignancies for this unique clinical subgroup.

음용수중 휘발성 유기오염물질의 노출경로에 따른 위해도 추정치 비교연구 (A Study on Comparison of Risk Estimates Among Various Exposure Scenario of Several Volatile Organic Compounds in Tap Water)

  • Risk assessment processes, which include processes for the estimation of human cancer potency using animal bioassay data and calculation of human exposure, entail uncertainties. In the exposure assessment process, exposure scenarios with various assumptions could affect the exposure amount and excess cancer risk. We compared risk estimates among various exposure scenarios of vinyl chloride, trichloroethylene and tetrachloroethylene in tap water. The contaminant concentrations were analyzed from tap water samples in Seoul from 1993 to 1994. The oral and inhalation cancer potencies of the contaminants were estimated using multistage, Weibull, lognormal, and Mantel-Bryan model in TOX-RISK computer software. In the first case, human excess cancer risk was estimated by the US EPA method used to set the MCL(maximum contaminant level). In the second and third case, the risk was estimated for multi-route exposure with and without adopting Monte-Carlo simulation, respectively. In the second case, exposure input parameters and cancer potencies used probability distributions, and in the third case, those values used point estimates(mean, and maximum or 95% upper-bound value). As a result, while the excess cancer risk estimated by US EPA method considering only direct ingestion tended to be underestimated, the risk which was estimated by considering multi-route exposure without Monte-Carlo simulation and then using the maximum or 95% upper-bound value as input parameters tended to be overestimated. In risk assessment for volatile organic compounds, considering multi-route exposure with adopting Monte-Carlo analysis seems to provide the most reasonable estimations.

콘빔전산화단층촬영에서 노출 조건에 따른 화질 유지 및 선량 감소에 대한 평가 (Evaluation of Dose Reduction and Maintaining Image Quality according to Exposure Factors of Cone Beam Computed Tomography)

  • 본 연구는 CBCT 장비의 표준 노출 조건(80 kV, 7 mA)을 기준으로 관전압과 관전류를 낮추어 촬영하면서 표준 노출 조건의 물리적 화질 요소값을 유지할 수 있는 노출 조건의 제시를 목표로 두고자한다. 영상의 물리적 화질 요소값의 측정을 위해 변조전달함수(MTF)가 분석되었고 선량 측정을 위해 선량-면적 곱(DAP)을 이용하였다. 관전압(80, 78, 76 kV)과 관전류(7, 6, 5, 4, 3 mA)의 15가지 조합의 노출 조건에서 Sedent ex IQ 팬텀 (Leeds Test Objects Ltd., Boroughbridge, UK)의 CBCT 영상을 얻었고 MTF 10이 각 조건에서 계산 되었다. 표준 노출 조건과 비교시 80 kV-6 mA, 80 kV-5 mA 노출 조건은 MTF 10에 있어 유의한 차이를 보이지 않았기에 본 연구에서 사용된 CBCT 장비의 경우, 80 kV-5 mA로 낮춘 노출 조건에서 물리적 화질요소값을 유지하면서 선량을 감소시킬 수 있는 것으로 판단되어진다.

Conversion coefficients for the estimation of effective dose in cone-beam CT

  • Purpose: To determine the conversion coefficients (CCs) from the dose-area product (DAP) value to effective dose in cone-beam CT. Materials and Methods: A CBCT scanner with four fields of view (FOV) was used. Using two exposure settings of the adult standard and low dose exposure, DAP values were measured with a DAP meter in C mode ($200mm{\times}179mm$), P mode ($154mm{\times}154mm$), I mode ($102mm{\times}102mm$), and D mode ($51mm{\times}51mm$). The effective doses were also investigated at each mode using an adult male head and neck phantom and thermoluminescent chips. Linear regressive analysis of the DAP and effective dose values was used to calculate the CCs for each CBCT examination. Results: For the C mode, the P mode at the maxilla, and the P mode at the mandible, the CCs were 0.049 ${\mu}Sv/mGycm^2$, 0.067 ${\mu}Sv/mGycm^2$, and 0.064 ${\mu}Sv/mGycm^2$, respectively. For the I mode, the CCs at the maxilla and mandible were 0.076 ${\mu}Sv/mGycm^2$ and 0.095 ${\mu}Sv/mGycm^2$, respectively. For the D mode at the maxillary incisors, molars, and mandibular molars, the CCs were 0.038 ${\mu}Sv/mGycm^2$, 0.041 ${\mu}Sv/mGycm^2$, and 0.146 ${\mu}Sv/mGycm^2$, respectively. Conclusion: The CCs in one CBCT device with fixed 80 kV ranged from 0.038 ${\mu}Sv/mGycm^2$ to 0.146 ${\mu}Sv/mGycm^2$ according to the imaging modes and irradiated region and were highest for the D mode at the mandibular molar.
