기류 가역성 평가에 있어서 최대호기유속 측정의 유용성 (Validity of Peak Expiratory Flow for Assessing Reversible Airflow Obstruction)

  • 목 적 : 최대호기유속 측정도 $FEV_1$과 함께 기도폐색의 정도를 평가할 수 있으며 기도폐색의 가역성을 평가하는 것은 기도폐쇄질환 환자들의 진단 및 치료에 중요하다. 최대호기유속은 $FEV_1$에 비해 측정이 간단하며 쉽고 빠르게 검사할 수 있는 장점이 있어 1차 진료기관에서도 쉽게 이용할 수 있다. 그러나 최대호기유속으로 기류 가역성을 평가한 연구는 매우 적다. 이에 천식 및 만성폐쇄성폐질환 환자들에게서 최대호기유속과 $FEV_1$으로 기류 가역성올 비교 평가하여 1차 진료기관에서 최대호기유속을 측정함으로써 기류 가역성을 평가하는데 기준을 마련하고자 본 연구를 시행하였다. 방 법 : 본 연구는 호흡곤란, 기침 및 천명을 주소로 계명대학교 동산병원 호흡기내과를 방문한 환자들 중 천식 및 만성폐쇄성폐질환으로 진단 받은 80명의 환자들을 대상으로 조사하였다. 베타-2 교감신경작용제(salbutamol) 흡입 직전과 15분 후 측정한 $FEV_1$의 추정정상치가 12% 이상 증가하고 노력성폐활량 또는 $FEV_1$의 절대값이 200ml 이상 증가한 경우를 기류가역성이 있는 것으로 정의하였다. 최대호기유속의 가역성은 베타-2 교감신경작용제 사용 후 절대값의 변화(1/min)로 표현하였으며 최대호기유속의 절대값의 변화에 따른 기류폐색의 가역성 평가에 대한 민감도, 특이도, 양성예측율과 음성예측율을 구하였다. 결 과 : 대상환자 80명중 36명(45%)에서 기류 가역성이 있었다. 폐활량측정법으로 기류 가역성이 증명된 환자에서 최대호기유속으로 기류 가역성을 평가하여서 최대호기유속의 민감도, 특이도, 양성예측을 그리고 음성예측율을 구하였을때 기저치보다 30 l/min 이상 증가한 경우에 가장 높게 관찰되었고 민감도 72.2%, 특이도 72.7%, 양성예측율 68.4% 그리고 음성예측율은 76.2%로 관찰되었다. 결 론 : 이상의 결과로 베타-2 교감신경작용제 사용 후 최대호기유속의 절대값의 변화는 기류 가역성을 평가하는데 간편하고 효율적인 방법으로 생각된다.

스퍼터링 코팅기법을 이용한 중성자 검출용 B4C 박막 개발 (Development of B4C Thin Films for Neutron Detection)

  • 헬륨-3는 높은 반응효율, 장시간 사용가능성, 감마선에 대한 낮은 반응확률 등과 같은 장점들을 가지고 있기 때문에 대부분의 중성자 검출기의 반응물질로 사용되어 왔다. 그러나 지난 몇 년 사이 전세계적인 헬륨-3의 부족으로 인해 기체의 수급이 어려워지고 있고 이에 따라 가격이 급격히 증가하게 되었다. 이러한 이유로 헬륨-3 대체 물질들을 이용한 고효율의 중성자 검출기의 개발에 대한 연구가 많은 연구그룹에 의해 활발히 진행되기 시작하였다. 이러한 연구에서는 다양한 물질들을 이용하고 있으며, 이 중에서 붕소-10은 다른 대체물질과 비교할 때 상대적으로 높은 중성자 반응확률, 낮은 감마반응효율, 물질의 안정성, 가격적 이점 그리고 기존 헬륨-3를 이용한 검출기의 계측회로의 재활용 가능성 등과 같은 장점들 때문에 많은 연구그룹에서 붕소-10을 이용한 중성자 검출기 개발을 진행하고 있다. 본 논문에서는 중성자 검출기에 사용될 수 있는 붕소-10 박막을 개발하고 이에 대한 성능평가를 수행하였다. 중성자 검출기의 반응물질로 붕소-10을 사용하기 위해서는 중성자와 붕소-10이 반응하여 생성되는 이차방사선을 측정할 수 있어야 한다. 본 연구에서 활용한 기체충진형 중성자 검출기의 경우 붕소-10을 얇은 박막 형태로 제작하여 중성자와 반응하여 생성된 이차방사선이 기체를 이온화 시켜서 생성되는 이온쌍을 측정하는 방법을 이용한다. 그러므로 중성자 반응효율과 이차방사선의 재흡수율을 고려한 붕소-10(탄화붕소)의 적절한 두께를 선정할 필요가 있다. 이를 위해서 본 논문에서는 몬테칼로 기법을 이용하는 MCNP6를 이용하여 다양한 두께에 따른 중성자신호수집효율의 변화를 계산하였다. 또한, 스퍼터링 기법을 이용하여 다양한 두께의 박막을 제작하고 이를 이용하여 중성자 반응신호를 측정하였다. 그리고 제작된 박막의 2차원 모니터링을 위한 다중선 비례계수기의 적용가능성을 타진하기 위해 제작된 붕소박막이 설치된 2차원 다중선 비례계수기를 제작하고 중성자 응답 특성을 평가하였다.

경사제 피복재의 누적피해를 이산시간 확률과정으로 고려한 조건기반 유지관리의 할인비용모형 (Discounted Cost Model of Condition-Based Maintenance Regarding Cumulative Damage of Armor Units of Rubble-Mound Breakwaters as a Discrete-Time Stochastic Process)

  • 경사제 피복재를 예방적으로 유지관리할 수 있는 조건기반 할인비용모형을 제안하였다. 하중발생 사상을 이산시간 확률과정으로 고려하는 추계학적 누적 피해모형과 보수보강 비용에 대한 경제성 모형을 결합하여 수학적으로 유도하였다. 특히 본 논문에서 유도된 조건기반 유지관리의 할인비용모형은 시간에 따른 비용의 가치 뿐만 아니라 누적피해의 비선형성도 고려할 수 있다. 본 연구의 결과는 기존 모형들의 결과와 비교하여 만족스럽게 검증되었다. 또한 구조물의 중요도와 이자율 변화에 대한 민감도 분석도 수행하여, 구조물의 중요도가 높아질수록 예방적 보수보강의 최적시기는 빨라지나 이자율은 커질수록 반대의 경향이 나타난다는 것을 알았다. 한편 본 연구에서 유도된 추계학적 기대비용모형을 이용하여 여러 조건에 대하여 임의의 경사제 피복재 단면을 해석하였다. 표본경로기법을 적용하여 임의의 태풍 내습에 따른 경사제 피복재의 기대 누적피해수준을 예측하여 피해강도함수의 계수들을 추정할 수 있었다. 특히 하중발생 과정을 HPP(Homogeneous Poisson Process) 뿐만 아니라 DSPP(Doubly Stochastic Poisson Process)로도 해석하여 기대 누적피해수준에 미치는 하중발생의 불확실성에 대한 영향을 분석하여 하중발생사상을 이산시간 확률과정으로 고려해도 된다는 것을 확인하였다. 조건기반 할인비용모형의 해석 결과에 의하면 경사제 피복재의 설계조건에 따라 기대 누적피해수준의 거동특성이 크게 달라지고 이에 따라 예방적 보수보강을 수행하는 최적시기도 변한다는 것을 알 수 있었다. 마지막으로 파괴한계, 구조물의 중요도 그리고 이자율을 변화시키면서 예방적 유지관리를 가장 경제적으로 수행할 수 있는 최적시점과 피해규모를 결정할 수 있었다.

심근관류 SPECT의 정량적 분석에서 관류정량값 정상변이의 고려: 생존심근 평가에서의 유용성 (Consideration of Normal Variation of Perfusion Measurements in the Quantitative Analysis of Myocardial Perfusion SPECT: Usefulness in Assessment of Viable Myocardium)

  • 목적: 심근관류 SPECT에서 자동정량화 소프트웨어를 이용해 관류 및 기능에 대한 객관적이고 재현성 높은 정량 값을 얻을 수 있지만, 이러한 정량값들을 이용할 때는 분절별로 적지 않은 정상변이가 있음을 고려해야 한다. 이 연구에서는 관류에 대한 정량값에서 정상변이를 고려한 새로운 지표들을 유도하고, 이러한 지표들이 임상적 유용성을 가지는지 생존심근 평가를 통해 알아보았다. 대상 및 방법: 심근관류 SPECT에서 관류의 정상변이를 구하기 위해 55명(남:여=28:27)의 관상동맥질환 저확률군을 선정하였다. 이들에게서 $^{201}Tl$ 휴식기/$^{99m}Tc-MIBI$ 부하기 심근관류 SPECT를 실시하고, 20분절 모델을 이용하여 $^{201}Tl$ 휴식기 관류정량값에 대한 각 분절의 평균(m)과 표준편차(SD)를 구하였다. 생존심근 평가를 위해서는 관상 동맥질환을 진단 받고 관상동맥우회로이식술(CABG)을 시행한 환자 48명을 연구대상에 포함하였다. 이들은 수술전 $^{201}Tl$ 휴식기/$^{99m}Tc$-MIBI 부하기 게이트/$^{201}Tl$ 24시간 지연 SPECT을 실시하였고, 수술 3개월 후 추적 게이트 SPECT를 실시하여 생존심근을 판정하였다. 수술 전 $^{201}Tl$ 24시간 지연 SPECT에서 분절별 $Q_{delay}$ (관류정량값), ${\Delta}_{delay}$ ($Q_{delay}$ - m), $Z_{delay}$ (($Q_{delay}$ - m)/SD)가 정의되었고 이들의 생존심근에 대한 진단성적은 수신자특성곡선 상의 곡선하면적(AUC)을 통해 평가하였다. 결과: 관류정량값은 분절 사이에 상당한 변이를 보여, 남자의 경우 최저치 분절에서 $51.8{\pm}6.5$, 최고치 분절에서 $87.0{\pm}5.9$였고, 여성의 경우 최저치 분절 $58.7{\pm}8.1$, 최고치 분절 $87.3{\pm}6.0$으로 나타났다. 생존심근에 대한 진단성능 평가에서 $Q_{delay}$의 AUC는 0.633인데 반해 ${\Delta}_{delay}$$Z_{delay}$의 AUC는 각각 0.735와 0.716으로 나타나 $Q_{delay}$에 비해 유의하게 높은 값을 보였다(각각 p=0.001, 0.018). 가장 높은 AUC를 보인 ${\Delta}_{delay}$는, 최적분리점 -24.7에서 85%의 예민도와 53%의 특이도를 가지는 것으로 나타났다. 결론: 심근관류 SPECT의 자동 정량화 분석에서 관류정량값의 정상변이는 분절에 따라 상당한 것으로 나타났다. 이들을 고려하여 유도된 정량적 지표들은 직접적인 관류정량값에 비해 생존심근 진단에서 더 나은 진단성적을 보였다. 이 연구는 심근관류 SPECT의 정량적 분석에서 정상변이의 고려가 중요함을 시사한다.

분석조건별 담수어류의 환경 DNA 메타바코딩 효율 비교: 필터, 추출 키트, 프라이머 조합 및 PCR 방법 (Efficiency Comparison of Environmental DNA Metabarcoding of Freshwater Fishes according to Filters, Extraction Kits, Primer Sets and PCR Methods)

  • 메타바코딩을 이용한 환경 DNA 분석은 검출 감도가 높아 어류의 생물다양성 평가 및 멸종위기종의 검출에 유용한 기술이다. 이번 연구는 메타바코딩을 이용해 우리나라 담수어류를 대상으로 높은 검출 효율을 보일 수 있는 적합한 분석방법을 확인하기 위해 4가지 분석조건별, 즉 필터(cellulose nitrate filter, glass fiber filter), 추출 키트(DNeasy® Blood & Tissue Kit, DNeasy® PowerWater Kit), 프라이머 조합(12S rDNA, 16S rDNA) 그리고 PCR 방법(conventional PCR, touchdown PCR)로 나타나는 Operational Taxonomic Units(OTUs) 수와 종 조성을 비교하였다. Glass fiber filter와 DNeasy® Tissue & Blood Kit를 이용해 추출한 시료는 12S rDNA와 16S rDNA 프라이머 조합에서 담수어류 OTUs가 가장 많이 검출되었다. 모든 분석조건 중 프라이머 조합에서만 조기어강(Class Actinopterygii) 평균 OTUs 수에서 통계적으로 유의한 차이를 보였고(Non-parametric Wilcoxon Signed Ranks Test, p=0.005), 담수어류 평균 OTUs 수는 유의하지 않았다. 종 조성 비교 결과 역시 프라이머 조합에서 유의한 차이를 보였고(PERMANOVA, Pseudo-F=6.9489, p=0.006), 나머지 분석조건에서는 유의한 차이를 보이지 않았다. NMDS 분석 결과 종 조성은 유사도 65% 기준에서 프라이머 조합에 따라 묶였고, 16S rDNA 프라이머 세트는 주로 멸종위기종인 모래주사(Microphysogobio koreensis), 꼬치동자개(Pseudogobio brevicorpus)가 기여하였고, 12S rDNA 프라이머 세트는 주로 일반종인 피라미(Zacco platypus), 꺽지(Coreoperca herzi) 등이 기여한 것으로 나타났다. 본 연구는 국내 하천에서 채취한 시료에 대한 메타바코딩을 이용한 종 다양성 분석의 기초정보를 제공한다.

첨단 전자산업 폐수처리시설의 Water Digital Twin(II): e-ASM 모델 보정, 수질 예측, 공정 선택과 설계 (Water Digital Twin for High-tech Electronics Industrial Wastewater Treatment System (II): e-ASM Calibration, Effluent Prediction, Process selection, and Design)

  • 본 연구에서는 Part I에서 제안한 첨단 전자산업 폐수처리시설 특화 Water Digital Twin모델인 e-ASM을 이용하여 랩-파일럿 처리장 데이터를 바탕으로 모델 보정(Calibration), 유입 성상에 따른 제거 효율, 유출수 예측 및 최적 공법 선정을 수행하였다. 첨단 전자산업 폐수처리시설의 특화 모델링을 위하여, 민감도 분석을 통해 e-ASM 모델의 정합성과 상관성이 높은 동역학적 파라미터를 선정하였고, 다중반응표면분석법 (Multiple response surface methodology, MRS)을 이용하여 동역학적 파라미터를 보정하였다. e-ASM 모델의 보정 결과, Lab-scale, Pilot-scale 단위의 실험데이터와 90% 이상의 높은 정합성을 보였다. 그리고 4가지 유기폐수 처리처리공법인 MLE, A2/O, 4-stage MLE-MBR, Bardenpho-MBR을 제안한 Water Digital Twin으로 구현하여 유입 폐수의 성상별 운전조건에 따라 제거효율을 분석하였으며, Bardenpho-MBR이 C/N ratio 변화에서도 안정적으로 COD (Chemical oxygen demand)를 90% 이상 제거하며 높은 총 질소 제거 효율을 보였다. 그리고 유입 폐수의 조건별 Bardenpho-MBR공정의 수리학적 체류시간(Hydraulic retention time, HRT)이 3일 이상일 때 1,800 mg L-1의 고농도 TMAH 폐수를 98% 이상 제거할 수 있음을 확인할 수 있었다. 이와 같이, 본 연구에서 개발한 e-ASM은 전자산업 제조시설별, 유입 폐수의 성상별 특화 모델링을 통해 높은 정합성을 가진 전자산업 폐수처리공정의 Water Digital Twin를 구현할 수 있고, 최적운전, Water AI, 최적가용기법 선정 등의 응용 가능성을 바탕으로 지속 가능한 첨단전자 산업을 위해 활용될 수 있을 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
