  • Purpose: In order to clarify the correlation of mandibular setback using bilateral intraoral vertical ramus osteotomy (BIVRO) and post-surgical transverse mandibular width (TMW), this study examined the pre- and postsurgical changes in hard and soft tissues of TMW and the relationship of TMW and the amount of mandibular setback. Patients and Methods: One-hundred seven patients who had undergone BIVRO were evaluated radiographically and clinically. A comparison study of the changes in hard and soft tissue after surgery in all 107 patients was performed with preoperative, 1 month, 3 month, 6 month and 1 year postoperative posteroanterio cephalograms and clinical photographs by tracing. And this changes were evaluated in parts to amounts of mandibular setback. Results: Statistically significant increases of TMW in hard and soft tissue from preoperative to postoperative 1 month were seen. TMW in hard tissue from 1 month to 1 year postopertive were gradually decreased. TMW in soft tissue was not changed uniformly but almost equal to pre-operative width. And there was no significant correlation between TMW and amount of mandibular setback. Conclusions: The results show that mandibular setback using BIVRO did not significantly influence increasing of TMW in soft tissue.

  • Conventional surgical therapy for oral soft tissue includes the use of scalpel, diathermy, cryotherapy and electrosurgery. But, these therapies have some surgical problems. Nowadays, laser surgery can be considered as the another option for conventional surgical therapy. Compared to conventional surgical therapies, advantages of laser therapy include maintenance of sterile conditions, promotion of wound healing, reduction of bleeding, less instruments, post operative pain reduction, less scar, saving cost by using fewer materials, staff and time. Carbon dioxide ($CO_2$) laser uses gaseous medium, and has long wavelength about 10,600nm. The first advantage of $CO_2$ laser for surgical treatment of oral soft tissue is hemostasis and visibility improvement by making relatively dry field. These case reports are about cases of minor surgery of oral soft tissue using $CO_2$ laser, and emphasize advantages of laser compared to conventional surgical therapies.

  • The comparative detectability of the artificial periapical defects among Ektaspeed Plus film. digitized and digora images was evaluated. The artifical defects were made in the cancellous bone and cancellous-cortical junction with the size of 1.0×0.8mm², 1.4×1.1mm² and 2.8×2.2mm². The defects in cancellous-cortical junction extended into cortical bone with the depth of 0, 0.5 and 1.0 mm. The results were as follows : 1. In junctional defects Ektaspeed Plus film for 2.8×12.2mm² defect showed the highest detectability. But significant difference were only found between Ektaspeed Plus films and digitized images (p<0.05). 2. Almost all defects within cacellous bone were not detected except a few digitized and Digora images for the size of 2.8×2.2mm². Digora images for them showed significant differences with Ektaspeed Plus films and digitized images (p<0.05). 3. The sensitinity of all imaging modalities were 0.9 or 1.0 in junctional defects for the size of 1.4×2.2mm² and 2.8×2.2mm². For cancellous defects, Digora image showed the highest sensitivity of 0.6 for the size of 2.8×2.2mm². 4. Significant differences for change of exposure time were found in most group of Ektaspeed Plus films and digitized images (p<0.05) . But there was no significant differences in Digora images for cacellous defects.

  • There ate anteroposterior$\cdot$vertical maxillary underdevelopment, transverse maxillary deficiency and wide cleft alveolus$\cdot$oroanual fistula among cleft lip ant palate related maxillary deformities. For treatment of these deformities, ones have used conventional treatment methods, there were often unsatisfactory results to patients and operators both. Since llizarov introduced effective technique of bone lengthening and augmentation for a variety of limb defotmities, application of distraction osteogenesis on maxillofacial area has been used to solve those disadvantages of conventional methods. Authors introduced following three cases about use of distraction osteogenesis. The first case is the application of RED(rigid external distraction) II system for the treatment of the anteroposterior$\cdot$vertical maxillary hypoplasia after several times of surgery and end of development in bilateral cleft lip and palate patient. The second case is the application of the USPD(unilateral segmental palatal distraction) for the resolution of the unilateral posterior crossbite and transverse dental arch asymmetry after alveolorraphy in growing unilateral cleft lip and palate patient. The third case is the application of transport distraction osteogenesis far closure of the wide clef alveolus and oroantral fistula in growing bilateral cleft lip and palate patient. There were satisfactory results in these cases. Particularly, in comparison with the decreases of relapse rates, the reduction of the hospitalization time and post-operative discomfort owing to minimal surgical intervention.

  • Patients undergone partial maxillectomy experience post-operative masticatory, phonetic, and swallowing difficulties. They also encounter social and psychological challenges due to changes in their facial appearances. Thus, functional and esthetic recovery through maxillofacial prosthesis becomes significant for these patients. The objective of an appropriate obturator is to restore palate and improve phonetic and swallowing ability by separating the oral cavity, nasal cavity, maxillary sinus, and nasopharynx. In this case report, an obturator was fabricated for a patient who had partial resection from the maxillary posterior region to the pharynx due to squamous cell carcinoma. The purpose of this case study is to describe the results because the patient was successfully improved both functionally and esthetically.

  • The purpose of this study was to evaluate the effect of tricalcium phosphate and Vitapex on the dogs' periapical tissues. Twenty mandibular premolars from 5 healthy dogs were used for this study. After the animals were anesthetized intramuscularly, pulp chambers were open and pulp tissue was extirpated with a barbed broach and H-file. Then the working length of the root canal was measured with H-file and pulp tissue was completely removed. Before the actual canal filling, the root canals of twenty teeth have been experimentally infected with opening the pulp chamber for 5 weeks. Periapical radiographs of the experimental teeth were taken to monitor the periapical pathological condition. Each root apex of 20 premolars was perforated with engine reamer and the root canals were enlarged with No. 30-60 H-files. They were divided into treated as follows. Control group: The root canal was filled with gutta-percha. Experimental group 1: The canal was dried with sterile paper points and mixture of tricalcium phosphate and physiological saline was overfilled beyond the root apex with a lentulo spiral. Then the root canal was filled gutta-percha and lateral condensation and the pulp chamber was filled with Caviton. Experimental group 2: The root canals were overfilled with Vitapex and were treated in the same manner as those in experimental group 1 At 1,2,3, and 8 weeks after experiment, the periapical tissues including the alveolar bone were fixed with 10% formalin solution for I week and decalcified with Plank-Rycho solution for 5 weeks. The specimens were embedded in paraffin and serial sections were cut into a thickness of 6 ${\mu}m$ at the plane of the root apex. Hematoxyline-eosin and Masson's trichrome stain were made for the histo-pathological examinations. The results were as follows: 1. Ingrowth of collagen fiber was observed from 1 week in control group and experimental groups. 2. The rate of bone formation of experimental group 1 was accelerated more than that of experimental group 2. 3. Resorption of cementum was seen in control group, but apposition of cementum was seen in experimental groups.

  • Objectives: This study aimed to assess the effect of 38% carbamide peroxide on the microleakage of class V cavities restored with either a silorane-based composite or two methacrylate-based composites. Materials and Methods: A total of 96 class V cavities were prepared on the buccal surface of extracted human teeth with both enamel and dentin margins and were randomly assigned into three groups of Filtek P90 (3M-ESPE) + P90 system adhesive (3M-ESPE)(group A), Filtek Z250 (3M-ESPE) + Adper Prompt L-Pop (3M-ESPE)(group B) and Filtek Z350XT (3M-ESPE) + Adper Prompt L-Pop (group C). Half of the teeth were randomly underwent bleaching (38% carbamide peroxide, Day White, Discus Dental, applying for 15 min, twice a day for 14 day) while the remaining half (control) were not bleached. Dye penetration was measured following immersion in basic fuchsine. Data were statistically analyzed using Kruskal-Wallis and Mann-Whitney U tests at a level of 0.05. Results: No significant differences were found between composites in the control groups in enamel (p = 0.171) or dentin (p = 0.094) margins. After bleaching, microleakage of Z250 (in enamel [p = 0.867] or dentin [p = 0.590] margins) and Z350 (in enamel [p = 0.445] or dentin [p = 0.591]margins) did not change significantly, but the microleakage of P90 significantly increased in both enamel (p = 0.042) and dentin (p = 0.002) margins. Conclusions: No significant differences were noted between the bleached and control subgroups of two methacrylate-based composites in enamel or dentin margins. Microleakage of silorane-based composite significantly increased after bleaching.

  • The purpose of this study was to observe the microleakage of composite resin filling using several glass ionomer cements. The Class V cavities of eighty noncarious human molars were prepared at the cementoenamel juction on the facial and lingual surfaces of each tooth with a No.330 carbide bur in a high speed handpiece. The cavity dimensions were $3.0{\pm}0.5mm$ wide, $2.0{\pm}0.5mm$ high, and $1.5{\pm}0.5mm$ deep and all enamel cavosurface margins were beveled with a No.558 carbide bur in low speed handpiece. The bevel was approximately $45^{\circ}$ and 0.5-1.0mm in width. A total of the 160 cavities was divided into four groups, and then 144 cavities among them were three experimental groups and remaining sixteen cavities were control group. All of the prepared cavities were restored as follows: group 1 : Preparations were restored with there three glass ionomer cements. group 2 : Preparations were restored with a composite resin with three glass ionomer cement bases placed $0.2{\pm}0.1mm$ short of the cavosurface margin. group 3 : Preparations were restored with a composite resin with three glass ionomer cement bases extened to the cavosurface margin. group 4 : As control group, preparations were restored with a composite resin, PALFIQUE. The specimens were then thermocycled in a range of $6^{\circ}C-60^{\circ}C$ and immersed in a bath of 2.0% aqueous basic fuchsin solution for 24 hours. Dye penetration was read on a scale of 0 to 4 by Tani and Buonocore's method. The following conclusions were derived from the results obtained; 1. All groups showed significantly more leakage at the gingival margins than at the occlusal margins(p<0.0005). 2. At the gingival margins, group 1 showed less leakage than group 3(p<0.01) and group 4(p<0.0005), while group 3 exhibited less leakage than group 2(p<0.01) and group 4(p<0.0005). 3. At the occlusal margins, group 4 showed less leakage than group 3(p<0.1) and group 1(p<0.005), while group 3 exhibited less leakage than group 2(p>0.1) and group 1(p<0.025).

  • The purpose of this study was to examine the influence of canal preparation on the property of apical seal. Seventy canals from extracted human maxillary and mandibular teeth were randomly selected and divided into two groups: conventional group and step-back group. Thirty five canals in one group were enlarged with a, conventional preparation method and thirty five canals in the other group were instrumented in a step-back method with K-file. After instrumentation sixty canals were obturated by lateral condensation of gutta percha and zinc oxide eugenol cement, and five canals in each group were not filled and used as control. All the specimens were immersed in 2% methylene blue dye solution and the depth of dye penetration into the canals were observed by macroscope at the intervals of 1 day, 2 days, and 7 days. The following results were obtained. 1. All the canals experimented showed varying degrees of dye penetration. 2. There was no significant difference between conventional method and step-back method in the depth of dye penetration and the degree of dye penetration tended to increase with time passage in both groups. 3, In conventional preparation group the mean dye penetration was 3.6mm at 1 day, 4.8mm at 2 days. and 6.5mm at 7 days. 4. In step-back prepareation group, the specimens exposed to the dye for 1 day showed mean dye penetration of 4.1mm, and the specimens immersed in the dye for 2 days and 7 days revealed mean dye penetration of 4.6mm and 6.2mm respectively.
