  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.SpO2 with Plethysmograph.

Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. 정치망 어구어법의 개발에 관한 연구-I - 현용어구의 흐름에 대한 형상 변화 - (A Study on Improvement for Fishing Gear and Method of Pound Net - I - Net Shapes of the Commerical Net in the Flow -)

  • 본 연구는 실제 정치망 어장에서의 조류에 따른 어구 형상 변형을 추정하고, 그 변형을 최소화시키며 어구의 기본 기능을 유지시키는데 필요한 수중 현상과 어구 내부용적을 적절하게 유지할 수 있는 설계의 기본 자료를 얻기 위한 목적으로 회류 수조에서 모형실험을 실시하였으며, 흐름에 따른 대뜸의 멍줄에 작용하는 장력, 정치망 내의 유속변화와 그물형상의 변화를 측정하였다. 1. 유속이 0.0m/s에서 0.6m/s로 증가 할 때에 유속 (v)에 따른 장력(R)의 실험식은 운동장 조상의 경우 R=$19.58v^{1.98}$($r^2$=0.98) 원통 조상인 경우, R=$26.90v^{1.72}$($r^2$=0.95)이었다. 2. 유속변화는 흐름이 망내부를 통과함에 따라 감소하여 운동장이 조상인 경우, 제1원통내부에서 0.1m/s일 때 약 70%, 0.2m/s일 때 60%, 0.3m/s일 때 약 50%, 0.4~0.6m/s에서는 약 40%정도로 초기유속에 비해 감소하였다. 원통이 조상측일 경우, 제2원통을 통과한 흐름은 초기 유속의 약 30~60%로 급격히 감소하였고, 제1원통에서는 약 20~30% 비탈그물에서는 약 10~20%정도 감소하였고, 운동장내에서 다시 유속이 증가하였다. 3. 운동장이 흐름의 조상측인 경우, 운동장그물의 기울기 변화량은 0~70$^{\circ}$까지, 비탈그물은 0~63$^{\circ}$까지 변화하였고, 조하측인 제2원통은 0~47$^{\circ}$까지 변화하였다. 유속이 증가함에 따른 까래그물의 깊이 변화는 초기 깊이에 보다 제1원통과 제2원통이 약 0~45%정도, 비탈그물이 약 0~37% 정도로 감소하였다. 4. 원통이 흐름의 조상측인 경우, 제2원통의 기울기 변화량은 약 0~70$^{\circ}$까지, 비탈그물은 약 0~55$^{\circ}$까지, 조하측인 운동장은 0~50$^{\circ}$까지 였다. 까래그물의 깊이 변화는 초기 깊이와 비교해서 제1원통은 약 0~35%, 제2원통은 약 0~20%, 비탈그물은 약 0~35%까지 감소하였으며, 유속이 0.5m/s이상에서 비탈그물이 원통 입구까지 부상하여, 원통 입구를 90%이상 막았다. 5. 어구의 변형을 최소화하고 전개용적을 증가시키기 위해서는 입구 및 운동장 부분에서는 힘출의 저조시 수심 위치에 중량추를 부착하고, 운동장 바깥쪽과 제1원통의 섶장과 까래그물 제2원통의 바깥쪽, 비탈그물의 까래에 부가 중량추를 부착하는 것이 필요할 것으로 판단된다.

스텐트그라프트 장치술을 위한 대동맥류 혈류 팬텀의 유용성 (Usefulness of Pulsatile Flow Aortic Aneurysm Phantoms for Stent-graft Placement)

  • 대동맥류 질환에 사용되는 스텐트그라프트의 체외실험을 위한 대동맥류 혈류 팬텀의 유효성과 실현가능성에 대해 평가하고자 한다. 팬텀은 인체의 혈류 조건과 유사한 상황을 재현할 수 있도록 심장부분과 대동맥류 부분으로 구성되었다. 심장부분은 고압력 수중펌프와 솔레노이드 밸브를 사용하여 심장의 대동맥 혈류를 재현하였고, 대동맥류 부분은 지점토를 사용하여 동맥류 모양을 재현하고 그 틀을 투명 실리콘으로 틀을 떠내는 방법으로 제작하였다. 두부분은 실리콘 관으로 연결하였다. 제작된 팬텀에서 밸브의 개폐 시간에 따른 압력(수축기/이완기) 변화를 측정하였으며, 스텐트그라프삽입술 전, 후의 압력변화를 측정하였으며, 통계적 유의성을 알아보았다. 밸브의 개폐 시간에 따른 압력 변화는 통계적으로 유의한 결과를 보였다(P<0.05). 0.5회/초의 개폐조건에서는 팬텀의 대동맥 근위부, 대동맥류, 원위부의 압력은 각각 $157.80{\pm}1.92/130.20{\pm}1.92$, $159.40{\pm}1.14/134.00{\pm}2.92$, $147.20{\pm}1.480/129.60{\pm}2.70\;mmHg$이었으며, 1.0회/초의 개폐 조건에서는 $161.40{\pm}1.34/90.20{\pm}1.64$, $175.00{\pm}1.58/93.00{\pm}1.58$, $176.80{\pm}1.48/90.80{\pm}1.92\;mmHg$이었고, 1.5회/초의 개폐 조건에서는 $159.40{\pm}1.82/127.20{\pm}1.48$, $166.60{\pm}1.67/138.00{\pm}1.87$, and $161.00{\pm}1.22/135.40{\pm}1.67\;mmHg$이었다. 스텐트그라프삽입술 전, 후의 팬텀의 압력변화는 대동맥부분에서 측정하였으며, 각각 $143.60{\pm}1.67/90.20{\pm}1.64$, $47.20{\pm}1.92/84.60{\pm}1.82$, and $137.40{\pm}1.52/88.80{\pm}1.64\;mmHg$이었다. 결론적으로, 대동맥류 팬텀은 압력의 범위를 다양하게 적용할 수 있고, 팬텀 내에서 시술의 재현이 가능하여 동물실험 전 스텐트그라프트의 유용성을 평가하기 위한 체외실험 기구로 유용할 것으로 기대된다.

국가직무능력표준(NCS)에 근거한 조경분야 교육과정 개발 방법론 - 갭분석을 중심으로 - (A Methodology to Develop a Curriculum based on National Competency Standards - Focused on Methodology for Gap Analysis -)

  • 산업현장의 변화와 요구에 부응할 수 있는 인력을 체계적으로 양성하기 위하여 2001년에 국무조정실을 중심으로 NCS(National Competency Standards, 이하 NCS)와 국가자격체제(National Qualification Frameworks, 이하 NQF)의 도입이 결정되었다. 건설분야 내 조경 역시 2008년 "국가직무능력표준(NCS) -조경"이 시범 개발되어 2009년부터 3년간 시범운영되었다. 특히 2013년 출범한 박근혜 정부의 주요 국정과제 중 하나로 '학벌이 아닌 능력 중심의 사회 구현'이 채택되면서, 이를 실천하기 위한 구체적인 수단으로 NCS 체제 구축이 전국적으로 확산되고 있는 시점이다. 그러나 국가에서 개발한 NCS의 경우, 이상적인 직무수행능력을 명시하였기 때문에 각 대학의 학생수준의 차이, 기자재와 교수들의 확보문제, 현행교육과정의 시수 문제 등 실질적인 운영상의 문제점을 반영하지 못한 단점이 있으므로, 이를 현실적인 교육과정에 연착륙시키기 위해서는 현재의 교육과정과 NCS와의 차이 즉 갭(gap)을 명확히 분석하는 과정이 선행되어야 한다. 갭분석은 기존의 교육과정을 NCS 기반 교육과정으로 개편하기 위한 초기 단계의 방법론으로 NCS 능력단위별 능력단위요소와 수행준거를 기준으로 학과 내 기존 교육과정과의 괴리도 혹은 일치 정도를 1에서 5까지 리커트 척도를 활용하여 기입한 후 분석하는 방법이다. 이처럼 현재의 대학 내 교육과정과 NCS와의 일치 및 괴리 정도를 측정함으로써 향후 NCS 운영을 희망하는 대학에서는 NCS의 적용 가능성과 개발 운영 이후의 효과성을 검증할 수 있는 기초 도구를 확보할 수 있다. 갭분석을 통한 교육과정 개편의 장점으로는 첫째, 정부의 재정지원 사업과 연계하여 정성적인 학과별 NCS 도입률에 대한 정량적 지표를 제공할 수 있으며, 둘째, NCS 기반 교육과정 개편 시 부족한 혹은 포화상태인 부분에 대한 객관적인 기준을 제공해 준다. 즉, 해당 NCS 세분류 도입 시 부족한 능력단위 및 능력단위요소를 추출할 수 있으며, 기존 과목별 능력단위요소별로 보완 사항도 추출할 수 있는 동시에, 이를 통한 상세 강의계획서 및 기초 교과목 개설을 위한 방향성을 제시해 주는 장점이 있다. 다만, 현재까지 개발된 갭분석의 이론을 보완하여 보다 체계적으로 정비해야 하는 과제는 남아 있다. 교육부, 고용노동부는 산업현장의 요구를 교육훈련 및 자격에 체계적으로 반영하기 위해 관련 산업계 인사들이 모여 실무적인 차원에서 NCS 표준을 적극적으로 개발하고 보급하여야 하며, NCS 적용을 희망하는 대학에서는 일과 자격이 연계될 수 있는 교육과정을 NCS 기반으로 개편하여야 할 것이다. 이를 위해 대학에서는 관련 산업 전망 및 대학 내 교수자원과 지역 산업과의 관련성을 고려하여 적용하고자 하는 NCS 세분류를 명확히 선정하여야 할 것이다. 이후 NCS 기반 교육과정 개편을 위해 갭분석을 사용하여 개편의 방향과 기준을 보다 객관적이고 합리적으로 수립하여 교육과정명세서에 대한 명확한 논리적 근거를 확보하고 있어야 과정이수형 자격제도에 효율적으로 동참할 수 있을 것이다.

  • 배 경: 호흡부전의 치료에 있어 환자 체위를 prone position으로 하는 시도는 20년 전부터 보고되어 왔으며 폐산소화 및 심박출량 향상에 효과가 있다는 것이 알려져 있다. Prone position시 폐산소화의 호전은 임상 및 동물 실험에서 단락 감소에 의한 것으로 알려져 있으며 그 중요 기전은 supine position에 비해 prone position때 중력 의존부 폐의 늑막압이 더 작아 폐포 개방압이 줄어들기 때문인 바 저자 등은 prone position이 폐산소화에 미치는 효과는 supine position에서 사용한 PEEP의 폐포 고비 개방압과의 관계에 따라 달라질 것으로 추정하였다. Prone position에서의 심박출량 증가에 대하여는 양압환기시 발생하는 cardiac fossa lifting 현상이 prone position에 의해 완화될 수 있다는 주장이 제기된 바 있어 prone position이 또한 supine position에서 PEEP에 의해 초래된 심박출량 저하를 완화시킬 수 있는지를 잡종견 급성 폐손상 모델을 통해 알아보고자 하였다. 방 법 : 잡종견 7 마리 ($20.0{\pm}3.9$ kg)를 정맥 마취 후 기관내관을 삽관하고 인공호흡기를 Vt 15 ml/kg, f 20/min, I : E=1 : 3, pause 10%, PEEP 0 cm $H_2O$, $F_1O_2$ 1.0으로 설정하였다. 혈압, 분당맥박수, 폐동맥쐐기압, 심박출량 등의 측정과 동맥혈가스분석을 위해 서혜동맥과 폐동맥 천자술을 시행하였다. 대조기에 supine position 및 prone position 30분에 각각 호흡 지표 ($PaO_2/F_1O_2$[P/F], 총호흡기계탄성 [Cst]와 supine position, prone position 5분 및 prone position 30분에 혈류역학적 지표(평균동맥압, 분당 맥박수, 심박출량, 박출용적)을 측정하고 섭씨 38도의 생리식염수 (30~50 ml/kg)를 기관내관을 통하여 주입하여 급성 폐손상을 유도한 뒤 constant flow 법에 의해 inflection point(Pflex)를 측정하였다. 급성 폐손상에서의 supine position과 prone position실험은 Pflex보다 2 cm $H_2O$ 낮은 PEEP(Low PEEP)과 2 cm $H_2O$ 높은 PEEP (Optimal PEEP)에서 순차적으로 시행하고 각각 통일 시간대에 상기 지표들을 측정하였으며 Optimal PEEP 실험 마지막에 다시 supin position으로 체위를 바꾸고 5분 뒤 혈류역학적 지표들을 측정하였다. 결 과: 1. Prone position 시행시 Low PEEP 및 Optimal PEEP에서의 호흡 효과의 차이 Low PEEP하에서 P/F 비는 supine position에서 $195{\pm}112$ mm Hg, prone position 30분에서 $400{\pm}33$ mm Hg였고 (p<0.001) Optimal PEEP하에서 P/F 비는 supine position에서 $466{\pm}63$ mm Hg, prone position 30분에서 $499{\pm}63$ mm Hg였다 (p=0.016). Prone position에 의한 P/F 비 싱승량은 Low PEEP하에서 $205{\pm}90$ mm Hg로 Optimal PEEP($33{\pm}33$ mm Hg) 하에서 보다 유의하게 높았다(각각 p<0.05). 2. Prone position의 혈류역학적 효과 Low PEEP하에서 심박출량은 supine position($3.0{\pm}0.7$ L/min) 과 비교하여 prone position 5분 $3.3{\pm}0.7$ L/min(p=0.0180)로 증가하였고 prone position 30분 $3.7{\pm}0.8$ L/min(p=0.0630)로 차이가 없었다. 분당맥박수는 각각 $141{\pm}22\;min^{-1}$, $141{\pm}22\;min^{-1}$(p=0.8658) 및 $176{\pm}28\;min^{-1}$(p=0.0280)이었고 폐동맥쐐기압은 차이가 없었다. Optimal PEEP하에서 평균동맥압은 각각 $87{\pm}19$ mm Hg, $107{\pm}18$ mm Hg(p=0.0180) 및 $108{\pm}16$ mm Hg(p=0.0180), 심박출량은 각각 $2.4{\pm}0.5$ L/min, $3.3{\pm}0.6$ L/min(p=0.0180) 및 $3.6{\pm}0.7$ L/min(p=0.0180), 그리고 박출용적은 각각 $14{\pm}2$ml, $20{\pm}2$ ml(p=0.0180) 및 $21{\pm}2$ ml(p=0.0180)였다. 분당맥박수는 체위 변경에 따른 차이가 없었고 폐동맥쐐기압은 각각 $10.1{\pm}2.4$ mm Hg, $9.1{\pm}2.7$ mm Hg(p=0.0180) 및 $9.0{\pm}3.1$ mm Hg(p=0.0679) 이었다. Optimal PEEP하 prone position에서 다시 supine position으로 체위를 바꾸고 5분 후 평균동맥압은 $92{\pm}23$ mm Hg, 심박출량은 $2.4{\pm}0.5$ L/min, 그리고 박출용적은 $14{\pm}1$ ml로 모두 감소하였다(모두 p<0.05). 결 론 : 잡종견 급성폐손상 모델에서 prone position은 비교적 낮은 수준 PEEP의 폐산소화 호전 효과를 중대시켰고, 비교적 놓은 수준 PEEP에 의한 심박출량 저하를 완화시켰다. 이러한 결과들은 심박출량을 유지하면서 폐산소화를 호전시키고자 하는 ARDS 환자에서의 기계환기의 목표를 달성하는데 있어 prone position이 supine position보다 유리하다는 것을 시사한다.
