Clinical Outcomes and Prognostic Factors Associated with the Response to Erlotinib in Non-Small-Cell Lung Cancer Patients with Unknown EGFR Mutational Status

  • Background: The efficacy of erlotinib is controversial in patients with unknown EGFR mutational status. The aim of this study was to identify the clinicopathological factors that are predictive of erlotinob treatment outcomes for NSCLC patients with unknown EGFR mutational status. Materials and Methods: A retrospective analysis of 109 patients with advanced NSCLC who had previously failed at least one line of chemotherapy and received subsequent treatment with erlotinib (150 mg/day orally) was performed. A Cox proportional hazard model for univariate and multivariate analyses was used to identify the baseline clinical parameters correlating with treatment outcome, expressed in terms of hazard ratios (HRs) and 95% confidence intervals. Results: The median treatment duration was 15 weeks (range, 4-184). The disease control rate was 55%, including disease stability for ${\geq}3$ months for 40% of the patients. Median progression-free survival and median overall survival (OS) were 4.2 and 8.5 months, respectively. The Cox model indicated that an Eastern Cooperative Oncology Group performance status (ECOG PS) ${\geq}2$ (HR 3.82; p<0.001), presence of intra-abdominal metastasis (HR 3.42; p=0.002), 2 or more prior chemotherapy regimens (HR 2.29; p=0.021), and weight loss >5% (HR 2.05; p=0.034) were independent adverse prognostic factors for OS in NSCLC patients treated with erlotinib. Conclusions: This study suggests that NSCLC patients should be enrolled in erlotinib treatment after a first round of unsuccessful chemotherapy to improve treatment success, during which they should be monitored for intra-abdominal metastasis and weight loss.

  • The morphological and histological changes of gonad in ridgetail prawn (Expalaemon carinicauda) were examined with the development of the external genitalia. The samples were collected monthly in Hampyung Bay on the western coast of Korea, from January to December 2003. The ovaries lie on the dorsal side of the female prawns thorax. The ovary cavity is located below the heart and above the hepatopancreas. The anterior part of the ovary is round and fused, and the posterior part has a pair of tubular structures. Testes are bilaterally symmetrical; the anterior part shows fused feature, and the posterior part consists of a pair of tubules. Based on histological examination, ripe eggs and spermatozoa were observed when mature females and males reach a length of about 14.9 and 11.5 mm CL, respectively. The immature female are almost same shape and structure with matured one in the endopod of first pleopod and appendix interna of the second pleopod, except the number of setae, and appear to be structurally complete after females reach a length of about 14.9 mm CL. The appendix masculina reaching beyond the distal end of the appendix interna of the second pleopod after males reach a length of more than 5.5 mm CL and a fully developed appendix masculina and appendix interna of the second pleopod appear up to a length of 11.5 mm CL.

  • This stud is my second trial to examine the costume of Balhae Dyansty. The subject of this study is the costume of a bronze statue excavated in the Maritime Provinces of Russia. The bronze statue is in the custody of Vladivostok Museum E.V. Shavkunov a Russian archelogist reported it a statue of-ficial of balhae dynasty in the 7-8th century. Judged his report lack of basis I suggest new opinion on sex, age, married or unmarried. social position manufactured time of the statue by comparative study on the costume hair-style and appearance of the statue with around nations' antiquities. hair-style of the statue is basically included in Ssangge and complexed style of Sage Ssangsuhuange, and Guange, Generally, Ssangge is a hair style of children, unmarried women, court ladies, dancing girls, and instrument player (lady)s'. The statue wears round-necked blouse long skirt broad fabric belt at high waist broad sleeve and confront collar coat cloud shaped shawl and risen tip shoes. The figure of statue is de-scribed plump. The statue shows straight sil-houette and high waist typical style of Tang, Uddai, The most characteristic facto is cloud shaped shawl Cloud shaped shawl was originally one of the northern race's shoulder ornaments. From the view of Comparative study the antiquities of NamDang and JunChock are simi-lar to this statue. Besides Tsukanovka river where the statue was excavated is Kraskino ruins of a castle formed single cultural layer of Balhae Dynasty and are relics of the layer of Balhae Dynasty and are relics of the latter period of Balhae Dynasty. Therefore this statue is assumed to have been manufactured at the last years of Balhae Dynasty and to be a figure of unmarried woman not official.

  • Password authenticated key exchange (PAKE) is a protocol that a client stores its password to a server, authenticates itself using its password and shares a session key with the server. In multi-server PAKE, a client splits its password and stores them to several servers separately. Unless all the servers are compromised, client's password will not be disclosed in the multi-server setting. In attribute-based encryption (ABE), a sender encrypts a message M using a set of attributes and then a receiver decrypts it using the same set of attributes. In this paper, we introduce multi-server PAKE protocol that utilizes a set of attributes of ABE as a client's password. In the protocol, the client and servers do not need to create additional public/private key pairs because the password is used as a set of public keys. Also, the client and the servers exchange only one round-trip message per server. The protocol is secure against dictionary attacks. We prove our system is secure in a proposed threat model. Finally we show feasibility through evaluating the execution time of the protocol.

  • In this paper, an enhanced initial ranging algorithm for large-cell coverage fixed wireless communication system is proposed. In typical wireless communication system such as WiBro, because a round-trip delay between a transmitter and a receiver is within one OFDM (Orthogonal Frequency Division Multiplexing) symbol duration, a frequency-domain differential correlation method is generally used. However, the conventional method cannot be applied due to an increase of a maximum time delay in large-cell system. In case of an accumulative differential method, estimation errors can occur because of frequent sign transitions. In this paper, therefore, we propose an algorithm which can estimate a total timing offset in a ranging channel structure for 15 km cell. The proposed method can improve performance by sign comparison based sign error correction rule between the estimated values and using a weighting scheme based on channel correlation, the number of accumulations, and the noise reduction effect in normalization process. Also, it can estimate the integer timing offset of symbol duration by comparing peak-powers after compensating for the fractional timing offset of symbol duration.

  • Several protocols which are based on tree topology to guarantee the important metrics such as energy efficiency in sensor networks have been proposed. However, studies on the effect of topologies in sensor networks, where base node has a high mobility, are very few. In this paper, we propose a minimum Wiener index tree as a suitable topology to the wireless sensor networks with high mobile base node. The minimum Wiener index spanning tree problem which aims to find a tree with minimum Wiener index from a given weighted graph was proved to be NP-hard. We designed a branch and bound algorithm for this problem. To evaluate the performance of proposed tree, the comparisons with minimum spanning tree in terms of transmission distance, energy consumption during one round, and network lifetime was performed by simulations. Our proposed tree outperformed in transmission distance and energy efficiency but underperformed in lifetime.

  • The MCT-Adaboost algorithm chooses an optimal set of features in each rounds. On each round, it chooses the best feature by calculate minimizing error rate using feature index and MCT kernel distribution. The involved process of weak classification executed by a hard decision. This decision occurs some problems when it chooses ambiguous kernel feature. In this paper, we propose the modified MCT-Adaboost classification using soft decision. The typical MCT-Adaboost assigns a same initial weights to each datum. This is because, they assume that all information of database is blind. We assign different initial weights with our propose new algorithm using some statistical properties of involved features. In experimental results, we confirm that our method shows better performance than the traditional one.

  • The purpose of this study is to construct interregional input-output table for multi-regions using non-survey methods. Despite of wide use of interregional input-output table, there have been continuing debates with respect to it's accuracy mostly when it is constructed by non-survey method. The best ideal one is to construct through real survey, while we must pay much time and cost. With regard to this concern, Hulu and Hewings developed a step by step method for the estimation of interregional input coefficients for the Indonesian economy which might be a challenge for the limitation of location quotient method, for which Round pointed out earlier. A five regions interregional input-output table for Korea is developed in this study by expanding Location Quotient method. Some development was made, however, according to the degree of collection of data and the assumption in treating the interregional flows. Finally, a comparison was made for the accuracy of input coefficients between the real surveyed and derived ones.

Epidermal Cyst of the Nasal Tip Presenting as Foreign Body Reaction (이물 반응으로 오인된 비첨부 표피 낭종)

  • Purpose: Epidermoid cyst may be congenital or acquired. Acquired cysts are most commonly of traumatic origin and result from an implantation or downward displacement of an epidermal fragment. Traumatic epidermoid cysts are rare tumors occurring on the nasal tip, especially resulting aesthetic procedure. So, we report a rare case of an iatrogenic epidermoid cyst in the nasal tip following rhinoplasty. Methods: A 44 - years old man had undergone rhinoplasty for several times. First time, the previous augmentation rhinoplasty and wedge osteotomy were performed nineteen months ago, lastly implant removal and unknown filler injection were performed one year ago at another local clinic. He had induration and tenderness on nasal tip and dorsum continued for 3 months. We thought that it caused by foreign body reaction with residual alloderm in nose. For removal of residual alloplastic material, open approach using transcolumellar incision was done. But, incidentally we found cystic mass on the nasal tip. Results: The findings were of an $0.8{\times}0.5{\times}0.5cm$ sized round cystic mass containing cream coloured material with a thick cheese - like consistency. The mass was completely excised and submitted for histology. This confirmed the diagnosis of an epidermoid cyst lined by keratinizing squamous epithelium. There was no induration, tenderness and sign of recurrence after excision of the mass. Conclusion: Epidermal cyst of the nasal tip region represents an unusual clinical lesion and it presents as foreign body reaction. And then, our case demonstrates that meticulous surgical approach and suture technique are the keys to prevention against iatrogenic nasal epidermoid cyst, especially in secondary rhinoplasty.

Imaging Features of the Intra-abdominal Granulomas in Two Cases (두 케이스에서 복강내 육아종의 영상학적 특징)

  • One dog and a cat with a history of diarrhea, anorexia and depression were referred. They showed normal laboratory examination results. Radiographic findings included a mid abdominal mass with ill-defined margin and serosal detail loss of peritoneal space. On abdominal ultrasonography, the round mass with heterogenous parenchymal echogenicty with irregular contour was found. It revealed as inflammation through ultrasound guide fine needle aspiration. After surgical removal of the masses in both cases, the clinical signs were resolved and focal pyogranuloma was confirmed through histopathologic examinations.
