The purpose of this study is related to the entrepreneurial activation of middle-aged and the elderly people for identifying the influence of entrepreneurial competency and entrepreneurial motivation on the entrepreneurial intentions and the mediating effects of opportunity search. In order to achieve the purpose of this study, a survey was conducted targeting the middle-aged and the elderly people in the ages of 40~60 years, and among 250 copies of collected questionnaires, 230 copies were finally used. SPSS 24.0 program was used for analysis. Results are as follows. First, the entrepreneurial competency had a positive effect on entrepreneurial intentions. Specifically, it has been proved that information competency, experience competency, network competency had a significant influence on entrepreneurial intentions. Second, the entrepreneurial motivation had a positive effect on entrepreneurial intentions. Furthermore, it has been proved that independence maintenance, wealth creation had a positive effect on entrepreneurial intentions, but cost of living were rejected. Third, the results of verifying the mediating relationship of opportunity search showed that the opportunity search has performed a mediating role in the effect of experience competency, network competency, and independence maintenance on entrepreneurial intentions.

Background: Transcatheter aortic valve replacement (TAVR) has been suggested as a less invasive treatment for high-risk patients with aortic valve disease. I n this study, we compared the outcomes of conventional surgical aortic valve replacement (AVR) and TAVR in elderly patients aged over 80. Methods: A total of 108 patients aged 80 years or older who underwent isolated AVR (n=35) or TAVR (n=73) from 2010 through 2015 at Asan Medical Center were identified. Early and late clinical outcomes, including echocardiographic findings, were evaluated in both groups. The mean follow-up duration was $766.4{\pm}528.7days$ in the AVR group and $755.2{\pm}546.6days$ in the TAVR group, and the average timing of the last follow-up echocardiography was at $492.6{\pm}512.5days$ in the AVR group and $515.7{\pm}526.8days$ in the TAVR group. Results: The overall early mortality was 2.8% (0 of 35, 0% in the AVR group vs. 3 of 73, 4.1% in the TAVR group). Permanent pacemaker insertion was significantly more common in the TAVR group (p=0.010). Renal failure requiring dialysis and new-onset atrial fibrillation was more frequent and the length of hospital stay was longer in the AVR group; however, this difference did not reach statistical significance. In the TAVR group, 14 patients (19.2%) were rehospitalized due to cardiac problems, and 13 patients (17.8%) had developed significant paravalvular leakage by the time of the last follow-up echocardiography. Conclusion: TAVR could be a good alternative to conventional surgical AVR in elderly patients. However, TAVR has several shortcomings, such as frequent significant paravalvular leakage or readmission, which should be considered in decision-making.

The objective of this study is to examine the structural relations between physical self-efficacy, depression, and ego-integrity among the elderly (60 years and older) currently participating in leisure sports. In addition, since the variables affecting ego integrity are presented in a multidimensional manner, it is very important to examine the cause and effect of the variables from an integrated perspective. To achieve this objective, 427 senior citizens attending the leisure sports program of a seniors center were assigned as the research subjects. For the structural equation modeling performed in the study, confirmatory factor analysis and reliability analysis were conducted for each variable, and direct, indirect, and total effects were estimated. The model fit is based on TLI, CFI, and RMSEA, and both the fit of the measurement model and the fit of the research model satisfy the criterion. The results of the analysis are as follows. First, in the results of verifying the validity and reliability of the questionnaire, all the variables of physical self-efficacy, depression, and ego-integrity were suitable. Second, the perceived physical ability had positive effects on depression, current life satisfaction, wise life, and attitude toward life, whereas the physical self-expression confidence had positive effects on current life satisfaction and attitude toward life, but negative effects on acceptance of the past life. Lastly, depression had negative effects on current life satisfaction. The results showed that the elderly identified the integrated relationship of physical self - efficacy, depression, and self-integration.

Objective: The purpose of the study is to evaluate appropriate weight for aged farmers in manually handling livestock feed in bags using ergonomic methods. Background: In the livestock industry in South Korea, despite the trend of aging of labor manpower, heavy items are still manually handled in many farms. In particular, among stockbreeding works, the handling of feed in bags weighing 25~30kg is reported as a cause of frequent injuries and musculoskeletal system diseases. However, studies on the standard for recommended weight allowed considering the physical characteristics of aged farmers older than 60 years with greatly decreased physical strength and muscle strength are insufficient. Method: To evaluate appropriate weight for handling of heavy livestock feed in bags, physical techniques for measuring recognized levels of physical work loads, the NLE (NIOSH lifting equation) a method that is an observation type technique, and an ergonomic modeling technique to predict compressive force imposed on L5/S1 were used. Subjects who participated in the experiment were organized into two groups of males/females with mean age exceeding 60 years, and lifting tasks were evaluated for nine weight levels. Results: Based on the results of psychophysical measurement, females showed a tendency of more drastic increases compared to males when weight was over 19kg. The results of estimation of regression models for the weight, 18.0 kg ($r^2=0.97$) and 15.3kg ($r^2=0.97$) were evaluated as stable load for males and females, respectively. In addition, both the observation type evaluation and ergonomic model evaluation showed stable loads in a range of 15~18kg. Conclusion: Given the results of the study, the weight of the feed in bags currently distributed to farms can become a cause of not only overexertion but also farm work related disasters such as musculoskeletal disorders and safety accidents. Providing livestock feed in bags weighing not more than 19kg for aged farmers is judged desirable, and managerial improvement for this matter is considered necessary. Application: The results of the present study can be utilized as useful data for institutional improvement of the weight of livestock feed in bags.
Stable Compression Fractures(SCF hereafter) are most often caused by trauma such as traffic accidents. These SCFs usually occur in the thoracic and lumbar regions of the spinal cord. Human life spans have increased as a result of medical advances, which in turn has led to an increase in the elderly population. SCFs are divided into the categories of stable and unstable. The categories are determined through X-rays and P/Ex tests. The D.I.T.I. is then used to diagnose the severity and prognosis of the fracture injuries, and it allows an objective evaluation of the symptoms. This author researched 40 patients who were treated at Wonkwang University Oriental Medical Center from October 1995 to December 1996. The patients were diagnosed by X-ray as having SCFs and treated primarily with acupuncture. Both the changes in D.I.T.I. and patients' self-diagnoses of their conditions before and after treatment yielded the meaningful results which have been presented here. 1. 75% of those who suffer from Stable Compression Fractures are 60 years of older. Of those, the ratio of male to female is 1 to 4. This is due in part to the fact that many elderly women seek Oriental medicine treatment for conditions associated with advanced age. 2. 53.3% of these injuries occurred between T11 and L2. 3. 65% of patients were hospitalized for 10 to 29 days, and then able to be treated on an outpatiens basis. 4. The D.I.T.I. results showed 50% below $0.2^{\circ}C$, 30% between 0.3 and $0.5^{\circ}C$, 10% between 0.5 and $0.7^{\circ}C$, 10% between 0.7 and $0.9^{\circ}C$, and 0% over $0.9^{\circ}C$. 5. The results of treatment using Modified Evaluation System in Thoracolumbar Vertebral Fractures Method showed that 55% of patients achieved excellent recovery and 40% achived good recovery. After comparing the D.I.T.I. results before and after treatment, we found 50% of patients showed excellent recovery $({\Delta}T{\leq}0.2)$ and 40% showed good recovery$(0.2<{\Delta}T{\leq}0.5)$.

Objectives: This study was performed to investigate the association between hopelessness, social support and community integration, and depression. Methods: We analyzed data from a questionnaire administered to subjects aged 65 years or older living alone for a 2015 community mental health project in Gyeongnam S city. We used a correlation analysis and a hierarchical regression analysis to identify factors that affect depression. Results: When controlling for socio-demographic factors, hopelessness and community integration had significant effects on depression. That is, the higher the hopelessness (B=0.768, p<.001) and the lower the community integration (B=-0.163, p=.012), the higher the depression in elderly living alone. Conclusions: There is a need for approaches at both the individual and social levels that consider and improve measures to prevent and reduce depression. Moreover, it is especially necessary to enhance community integration.

Purpose: It is unclear whether adding concurrent chemotherapy (CT) to definitive radiotherapy (RT) following induction CT is a tolerable and cost effective treatment for non-small-cell lung cancer (NSCLC) patients aged 70 years or older with comorbidities. This study evaluated the actual clinical outcomes between concurrent chemoradiotherapy (CCRT) and RT alone following induction CT or not in patients (≥70 years) in a single institution's clinical practice. Materials and Methods: A total of 82 patients with unresectable stage III NSCLC between 2004 and 2016 were retrospectively analyzed. Their treatment tolerance and clinical outcomes such as overall survival (OS), locoregional recurrence (LRR), treatment toxicities and distant metastasis (DM) were evaluated. Early mortality rates were also evaluated as 4-month mortality after RT. Results: Fifty-four patients received CCRT and 28 patients received RT alone. Induction CT before RT was performed for 68.5% and 50.0% in CCRT and RT alone groups. Treatment tolerance was significantly worse in CCRT (p = 0.046). The median survival was 21.1 and 18.1 months for CCRT and RT alone, which was not statistically significant. LRR and DM were also not different. Most early deaths after CCRT were attributed to non-cancer-related mortality. Acute esophagitis of grade ≥2 occurred more following CCRT (p = 0.017). In multivariate analysis, a Charlson Comorbidity Index (CCI) of ≥5 and a weight loss of ≥5% after RT were associated with poor OS. The factors adversely affecting 4-month survival were a CCI of ≥5 and CCRT. Conclusion: There were no significant differences in OS, LRR, and DM between CCRT and RT alone treatment in elderly patients. However, there was a poorer tolerance and higher incidence of acute esophagitis in the CCRT group. Specifically, when the patients had a CCI of ≥5, RT alone seems to be reasonable with a low probability of early death.

In recent years, the substantial increase in the population's average age leads to an exceeded number of older persons comparing with the number of any other age group. As a result, both industry and academia are focused on the development of several solutions aimed to guarantee a healthy and safe lifestyle to the elderly. Ambient Assisted Living (AAL) approach is the way to guarantee better life conditions for the aged and for monitoring their health conditions by the development of innovative technologies and services. AAL technologies can also provide more safety for the elderly, offering emergency response mechanisms, fall detection solutions, and video surveillance systems. Unfortunately, due to the sensitive nature of AAL data, AAL systems should satisfy security requirements such as integrity, confidentiality, availability, anonymity, and others. In this paper, we propose an adaptive authentication protocol for the AAL systems. The proposed authentication protocol not only supports several important security requirements needed by the AAL systems, but can also withstand various types of attacks. In addition, the security analysis results show that the proposed authentication protocol is more efficient and secure than the existing authentication protocols.
This study was conducted to identify and compare the needs of the integrated by local government, and to provide basic data for the provision of integrated care services. This is a secondary data analysis study using data from 10,299 elderly people living in 17 local governments who participated in the '2017 National Survey on the Aged People in Korea'. The difference in medical needs, daily living support needs, and social activity support needs were 2.4, 6.0, and 2.0 times higher in the highest regions than in the lowest regions. In addition, the size of the medical needs group, welfare needs group, and integrated care needs group varied by region. Through this study, it was confirmed that the level of integrated care needs varied by region. Therefore, in order to efficiently provide the services at the national level, it is necessary to accurately assess the needs of the aged in each local government, and accordingly, prioritize and identify available resources within the care service and appropriate allocation.

Objectives: To compare the self-rated health in chronic disease patients with depression, chronic disease patients, and depression patients, and to observe the related factors to the self-rated health of people age 65 and older. Methods: The subjects were 2,549 elderly people, over 65 years old in Busan Metropolitan City who participated in 2009 community health survey. Association between self-rated health and general characteristics, life style and disease status were observed. Depressive symptoms were measured using the Centre for Epidemiologic Studies Depression Scale (CES-D). Analysis of complex sample was done with SAS (ver. 9.2), using ${\chi}^2$-test and multiple logistic regression. Results: Among total 2,549, there were 740 normal people (29.8%), 50 people with depression (1.8%), 1,495 people with other chronic diseases (58.2%), and 264 people with the comobidity of depression and other chronic diseases (10.1%). Good self-related health accounted for 20.3% for the whole, 33.4% for normal, 16.7% for chronic disease, 16.1% for depression, and 3.2% for chronic diseases with depression. Disease, gender, education, income, alcohol drinking, regular exercise and regular walking were independent factors associated with the good self-related health. Conclusions: It is suggested that when the chronic disease control program for the elderly is developed, depression care should be considered along with the program. This program should be given priority to the women and the vulnerable classes and should also be related to the regular walking.
