A toxoplasmic uveitis case of a 60-year-old male in Korea

  • A toxoplasmic uveitis case was reported on the focus of impairment of pathological findings and serological antibody titers after chemotherapy. A chief complaint of a 60-year-old male was a decreased and blurred vision in his right eye for 2 weeks after experiencing tremendous stress and fatigue. A steroid therapy for 3 weeks was not effective and the retinal lesion became necrotic. Anti-Toxoplasma gondii antibody titer was checked to be a strong positive by both ELISA and indirect latex agglutination assay (lLA). He was treated with Fansidar F for 8 weeks. His vision improved as the necrotic lesion healed by scarring, but the antibody titers still remained very high without any signs of negative conversion. It is suggested to be a recurrent case of the past asymptomatic infection by presumed immune suppression caused by excessive stress.

  • This 32 year-old male army officer was admitted with complaints of thumb size mass on buccal musoca which onterfere with mastication. Routine laboratory test revealed W.N.L. and the results of the excisional biopsy was Plemorphic Adenoma of salivary gland origin which was on the left buccal musoca( of 21 year-old male army officer). The size of tumor mass was 1.0 x 1.2 x 0.8cm, hard, encapsulated and seperated completely from surrounding tissue when enucleated surgically. Wound was healed unventfully and the result was good.

  • Dieulafoy lesions, vascular anomalies typically found along the gastrointestinal tract, have been viewed as rare and obscure causes of sudden intestinal bleeding, especially in pediatric patients. Since their discovery in the late 19th century, the reported incidence has increased. This is due to an increased awareness of, and knowledge about, their presentation and to advanced endoscopic diagnosis and therapy. Our patient was a three-year-old male, without a complex medical history. He presented to the emergency department with acute hematemesis with blood clots and acute anemia requiring blood transfusion. Endoscopy revealed four isolated Dieulafoy lesions along the lesser curvature of the stomach, which were treated with an epinephrine injection. The Dieulafoy lesion, although thought to be rare, should be considered when investigating an acute gastrointestinal bleed. These lesions have been successfully treated endoscopically. Appropriate anticipation and preparation for diagnosis and therapy can lead to optimal outcomes for the pediatric patient.

누두흉 치험 2례 보고 (Surgical correction of funnel chest: report of 2 cases)

  • Two patients with funnel chest deformity were corrected in the department of Thoracic Surgery, N.M.C. The first case was a 12 months old male suffering from recurrent upper respiratory tract infection and symmetrical funnel chest deformity, of which hollow cavity was measured 40 ml of water. He was corrected by method of sternal turnover. The result was satisfactory. The second case was a 16 years old male suffering from exertional dyspnea, recurrent upper respiratory tract infection, wheezing sound [esp., at night], and asymmetrical funnel chest deformity, of which hollow cavity was measured 80 ml of water. He was corrected by modified Ravitch method. Until postoperative 4 months, result was satisfactory. Thereafter, respiratory wheezing, exertional dyspnea and chest wall deformity were returned to pre-operative status.

삼판막 이식수술 (2례 보고) (Triple Valve Replacement -A report of two cases-)

  • Simultaneous triple valve replacements were performed in two patients on January and April 1980 at Seoul National University Hospital. The first case was 17 years old male patient with a history of exertional dyspnea for 7 years. He was in class III by the NYHA functional classification and diagnosed as aortic insufficiency, mitral steno-insufficiency and tricuspid insufficiency. The second case was 46 years old male patient suffered from exertional dyspnea for 5 years, He was in class IV and diagnosed as aortic stenoinsufficiency, mitral stenoinsufficiency and tricuspid insufficiency. Triple valve replacements were performed under the deep hypothermia and pharmacologic cardiac arrest with aortic cross clamping for 80 minutes to 159 minutes. Total extracorporeal circulation time were 197 and 176 minutes respectively. The postoperative courses were uneventful.

누두흉의 수술교정 (Surgical Correction of Funnel Chest: A Report of 2 Cases)

  • Two patients with funnel chest deformity corrected in the Department of Thoracic Surgery, Korea University Hospital are presented. The first case was a 21 year old male suffering from exertional dyspnea and recurrent upper respiratory tract infection with asymmetrical funnel Chest deformity, which the hollow cavity was measured 250ml of water. He was corrected by Ravitch method. The result was uneventful. The second case was a 34 year old male with symmetrical and asymptomatic deformity which the hollow cavity was measured 45ml of water. He was corrected by the method of sternal turnover [Wada Method]. On postoperative 8th day, seroma was noted in subxiphoid space and followed secondary infection of the pseudomonas. So, the infected cartilages were resected and curetted. The result was uneventful.

  • Recently we experienced coronary artery bypass graft in two patients over 70years of age. Among them, one case was 74 old year male and the other case was 73 old year male. The coronary artery diseases of them were angiographically diagnosed to triple vessel diseases. We only used great saphenous vein as graft vessel in two patients. The postoperative courses were good except mediastinal bleeding in 1 case. They were discharged with good results. They were still healthy 40 months in one and 30 months in the other after coronary artery bypass grafting.

기부비동 확장증(Pneumosinus dilatans) 2례 (2 Cases of Pneumosinus Dilatans)

  • Pneumosinus Dilatans consists of an abnormal dilatation of the paranasal sinuses which contain air only and lined by normal mucosa. It is a rare condition, the etiology of which is unclear. A 16 year old male complained frontal bossing which developed slowly. Simple X-ray and CT showed abnormal distension of frontal sinus. Another 19 year old male complained slowly growing left cheek mass. Simple X-ray and CT showed abnormal distension of anteromedial wall of maxillary sinus. In view of the cosmetic appearance, operations were performed. Distended sinuses walls were removed and reconstructed using $Medpore^{(R)}$. I report 2 cases of pneumosinus dilatans which developed in frontal and maxillary sinuses and the literature reviewed.

Necrotizing Sialometaplasia: 두 증례보고 (Necrotizing sialometaplasia: Report of 2 cases)

  • Necrotizing sialometaplasia (NS) was defined by Abrams et al. in 1973 as a reactive necrotizing inflammatory process involving minor salivary glands of the hard palate. Before that recognition, many patients with this condition had been improperly treated because of its clinical and histologic resemblance to malignancy such as mucoepidermoid carcinoma and squamous cell carcinoma. We report two cases of necrotizing sialometaplasia. One case involved a 58-year-old male who had an ulcerative palatal lesion exposing underlying bone which has the typical features of the above mentioned condition. Another case involved a 59-year-old male who developed a necrotizing sialometaplasia in association with a dome-shaped palatal swelling which was proved as an adenoid cystic carcinoma after operation biopsy.

  • A 54-year-old male farmer residing in Chunchon, Korea. complaining of blood tinged discharge and tinnitus in the left ear for two days. was examined in August 16, 1996. Otoscopic examination revealed live maggots from the ear canal. The patient did not complain of any symptoms after removal of maggots. Five maggots recovered were identified as the third stage larvae of Lucilia sericata (Diptera: Calliphoridae). This is the first record of aural myiasis in Korea.
